  • Complex regional pain syndrome (CRPS) is rare, characterized by pain from diverse causes, and presents as extreme pain even with minor irritation. General anesthesia may be required for dental treatment because the pain may not be controlled with local anesthesia. However, treatment under general anesthesia is also challenging. A 38-year-old woman with CRPS arrived for outpatient dental treatment under general anesthesia. At the fourth general anesthesia induction, she experienced severe pain resulting from her right toe touching the dental chair. Anesthesia was induced to calm her and continue the treatment. After 55 minutes of general anesthesia, the patient still complained of extreme toe pain. Subsequently, two administrations for intravenous sedation were performed, and discharge was possible in the recovery room approximately 5 h after the pain onset. The pain was not located at the dental treatment site. Although the major factor causing pain relief was unknown, ketamine may have played a role.

  • Rotator cuff tear is a common cause of shoulder pain and disability in adults. Due to the various nature of progression of rotator cuff tears and the complex biomechanics of the shoulder joint, repair and treatment of large-to-massive tears are challenging for many surgeons. Despite the recent popularity of reverse total shoulder arthroplasty as a treatment option for large-to-massive irreparable rotator cuff tears, biological and mechanical repair augmentation has also shown promise as a viable treatment option. The purpose of this study was to briefly summarize and review current studies on the assessment and arthroscopic treatment of large-to-massive rotator cuff tears, whether repairable or irreparable, to aid in developing a consensus on future treatment directions.

  • In order to treat the complex wastewater containing organic compound and solids, pre-treatment system associated with molecular separation process were investigated. The reductions of COD and turbidity were obtained after coagulation processes using Alum (Aluminium sulfate, $Al_2(SO_4)_2{\cdot}18H_{2}O$) and PACl (poly aluminium chloride as 17% $Al_{2}O_{3}$). The results of study were as follows: using variable dosage of Alum, COD removal was highest at 4,000 mg/l, and the reduction of COD and turbidity was 42% and 92%, respectively. The optimum coagulation would be effective at pH 7.3 than pH 9.0 by the addition of alum at a concentration of 6,000 mg/l and PACl was add at 4.25% in raw complex wastewater with 2,000 mg/l alum at pH 7.3, the reduction of COD was reduced by 32%. But coagulation aid experiments indicated that PACl would be more effective in sludge separation ability than COD removal efficiency.

  • Complex regional pain syndrome (CRPS) is a painful and disabling disorder that can affect one or more extremities. Unfortunately, the knowledge concerning its natural history and mechanism is very limited and many current rationales in treatment of CRPS are mainly dependent on efficacy originated in other common conditions of neuropathic pain. Therefore, in this study, we present a case using a total spinal block (TSB) for the refractory pain management of a 16-year-old male CRPS patient, who suffered from constant stabbing and squeezing pain, with severe touch allodynia in the left upper extremity following an operation of chondroblastoma. After the TSB, the patient’s continuous and spontaneous pain became mild and the allodynia disappeared and maintained decreased for 1 month.
