의료용 화상환자 압박복의 제조 국가별 비교 (Comparison of Medical Compression Garments by Manufacturing Country)

  • A special medical compression garment has been developed to be worn after surgery or treatment using an elastic fiber in which the amount of compression can be adjusted to a specified targeted area to reduce the formation of hypertrophic scarring. In order to develop this medical compression garments, specialized technical skill in fiber, compression class and body measurements are needed. This study provides product survey data for the development of medical compression garments. An investigation analyzed medical compression garments of 16 brands from 6 countries sold in the Korean market & online, such as Make Me Heal, Jobst, Bio Concepts, Design Veronique$^{(R)}$ Nouvelle and Leonisa$^{(R)}$ (USA), Respire(Germany), Malcom$^{(R)}$, Holistic garments and Jobskin(UK), Technomed, kanav Healthcare and Sindhoori surgicals(India), Soo medical and C&C medical(Korea), Secondskin(Australia). The main fibers of compression garments were nylon and spandex: in addition, the fiber content was different by country and brand. The number of compression classes of products was USA(4), UK(4), and India(4). For body measurements, USA and many brands used (bust, under bust, waist, hip, thigh, knee, calf, ankle, upper arm, elbow, wrist, armhole bicep) circumference and length.

Two-dimensional and volumetric airway changes after bimaxillary surgery for class III malocclusion

  • Objectives: Any change in maxilla and mandible position can alter the upper airway, and any decrease in the upper airway can cause sleep disorders. Thus, it is necessary to assess airway changes after repositioning of the maxilla and mandible during orthognathic surgery. The purpose of this study was to evaluate linear and volumetric changes in the upper airway after bimaxillary surgery to correct class III malocclusion via cone-beam computed tomography (CBCT) and to identify correlations between linear and volumetric changes. Materials and Methods: This was a prospective cohort study. CBCTs from 10 class III patients were evaluated before surgery and three months after. The Wilcoxon one-sample test was used to evaluate the differences in measurements before and after surgery. Spearman's rank correlation coefficient was used to test the correlation between linear and volumetric changes. Results: The results show that the nasopharyngeal space increased significantly, and that this increase correlated with degree of maxillary advancement. No significant changes were found in volumes before and after surgery. A correlation was found between linear and volumetric oropharyngeal changes. Conclusion: Bimaxillary surgical correction of class III malocclusion did not cause statistically significant changes in the posterior airway space.

연세대학생 2,378명을 대상으로 한 부정교합빈도에 관한 연구 (A STUDY ON THE PREVALENCE OF MALOCCLUSION IN 2,378 YONSEI UNIVERSITY STUDENTS)

  • Since malocclusion affects a large segment of the population, it is by definition a public health problem. The etiology ana treatment of malocclusions have been studied by clinicians; however epidemioloic aspect of tile problem have been neglected. This study was undertaken using Angle's classification to obtain and to evaluate epidemiologic data on the prevalence of malocclusion in a group of 2,378 Yonsei University students, 17 to 23 years of age. All freshmen were selected, except for those students receiving orthodontic treatment and those few with too many missing teeth which prohibits classification by Angle's method. The following results were obtained: 1) Almost $91\%$ of students had malocclusion of the teeth severe enough to require correction. 2) There was a statistically significant difference in malocclusion between males and females($93.66\%$ malocclusion in males, $79.13\%$ malocclusioa in females). 3) Crowding was most pravalent in class I malocclusion. 4) There appeared to be a specific association between the number of lost first molars and Angle's classification. 5) In this study, more class II, Div.2 malocclusion appeared than in Massier's and Frankel's study of Caucasians, which used similar criteria. Class III malocclusion was more prevalent than normal occlusion in the Korean students studied, but in Caucasians' normal occlusion was more prevalent.

DEA분석을 통한 국내 수도권 주요 대학병원의 효율성 분석 (An Analysis of Efficiency in Major University Hospitals in Domestically Capital Area Through DEA Analysis)

  • This study analyzed efficiency by utilizing DEA analytical technique centering on materials for 2009 of 20 major university hospitals in capital area. Input variables were utilized professor & full-time doctor, resident, nurse & number of bed hospitals. Output variables were analyzed by dividing number of annual outpatients & number of annual inpatients, and annually total outpatient profit & inpatient profit into a model of the standard for number of patients and the standard for medical profit. DEA analysis was elicited efficiency score by applying CCR, BCC, BFG, scale profit, and SE model. Through t-test after eliciting efficiency score, the implications were suggested by comparing efficiency between DMU in Seoul and DMU in capital area, by comparing between high-class general hospitals and general hospitals, and by comparing between high-class general hospitals in Seoul and 5 big hospitals. As a result of analysis, the major university hospitals in capital area showed high efficiency as a whole close to "1," but indicated low efficiency relatively in CCR field. Thus, the expansion in scale within capital area was indicated to reach the limit. Second, in a model of analyzing the standard for number of patients, the medical institutions, which are being operated efficiently, were indicated to be 10 DMUs. In the standard for medical profit, 12 DMUs were analyzed to be operated efficiently. Third, the efficiency in general hospital was higher than high-class general hospital. Thus, the efficiency of operation was indicated to be more important than scale. Also, large high-class hospitals(big 5) where are located in downtown Seoul showed the higher efficiency than other general high-class general hospitals, but were indicating very low efficiency in some DMUs. Fourth, as a result of generalizing and evaluating the number of patients and the medical profit, the efficient DMU was indicated to be more when analyzing on the basis of medical profit than the standard for number of patients. Thus, major university hospitals in capital area were indicated to make more effort for section in medical profit. Based on the analytical results of efficiency, a strategy for reinforcing efficiency in inefficient DMU was indicated to be needed a strategy of creating customers for promoting number of patients and a strategy for making operation efficient for increasing profitability.

사물안신탕가미방과 침 치료로 임상 증상에 호전을 보인 완전방실차단 환자 치험 1례 (A Case Report of Complete Atrioventricular Block Treatment with Samulanshin-tang-gamibang and Acupuncture)

  • Introduction: This study reports the effect of herbal medicine (Samulanshin-tang-gamibang) and acupuncture on complete atrioventricular (AV) block. Case presentation: A 63-year-old female with complete AV block was experiencing dyspnea, palpitation, dizziness, headache, bradycardia, and insomnia, and she was treated with Samulanshin-tang-gamibang and acupuncture for 12 days. To evaluate the treatment, a numeric rating scale (NRS) and the New York Heart Association (NYHA) functional classification was used. The patient's NRS scores decreased from 6 to 2 for dyspnea and palpitation and from 5 to 1 for dizziness and headache. Her NYHA Class improved from Class II to Class I. No side effects were observed during treatment. Conclusion: This study suggests that herbal medicine and acupuncture may be effective in relieving symptoms caused by complete AV block. However, the long-term effects of the treatment were not observed, and so further studies are still needed.

인문사회의학 교육과정 개선을 위한 제안 (Suggestions for the Improvement of Medical Humanities Education)

  • Medical humanities has become a third area of medical education following basic and clinical medicine. Also, in the national evaluation of medical schools, medical humanities education is an important factor. However, there are many difficulties in teaching medical humanities in medical schools. First, it is still an unfamiliar education area to medical schools and professors. Second, still, there is no consensus on the definition and contents of this education. Third, it is usually very difficult to find professors who have interest and the ability to teach medical humanities. Fourth, even medical students do not understand why they should study medical humanities and sometimes do not eagerly participate in class. This paper suggests some solutions for these problems. First, medical humanities need to be divided into sections according to how easily the contents can be accepted by existing medical education system and apply these sections in the introduction of this education gradually and in stage. One example of the division can be as follows: Group 1) medical ethics and medical law which can be most easily accepted. Group 2) medical communication skills which can be relatively easily accepted. Group 3) medical history and medical professionalism which is relatively difficult to accept, and Group 4) medical philosophy, medicine and music, medicine and literature, medicine and art, medicine and religion, etc. which is the most difficult to accept. In this paper, four things are suggested. Second, divide the contents into mendatory courses and elective courses. Third, allocate the contents throughout the four years from the first year though the fourth year according to the spiral curriculum model. This paper reports some new ideas and methods for medical humanities education. First, to stimulate students' participation, several methods were applied in a large size lecture and student projects. Second, the emphasis of writing in class and evaluation were discussed. Third, the provision of hands on experience is more emphasized than lectures. Fourth, inviting some doctors who work in non-medical areas such as journalism, pharmaceutical industry, etc is suggested. Trial and error is inevitable in this education, but it is essential in molding a good doctor, so medical professors who are interested or in charge of this medical humanities education need to share their ideas and experiences.

The Outcome of Corpus Callosotomy for Intractable Epilepsy : 10 Years Experience of Corpus Callosotomy

  • Objective : The purpose of this study is to evaluate the effect of the corpus callosotomy and to elucidate possible prognostic factors. Methods : The cases of 39 patients who underwent corpus callosotomy were reviewed retrospectively. Clinical outcomes were analyzed using Engel's classification, with consideration of various presurgical conditions and the extent of the callosal resection during follow-up more than one year. Results : Satisfactory outcome [Engel's class I, II] was obtained in 20 patients [51%] of 39 patients. In 36 cases with drop attack seizures, the class I, II outcomes were 22 patients [61%]. When the patients were grouped according to the extent of callosal resection, the class I, II outcomes were 50% of the patients with anterior 1/2 or 2/3, 50% of those with anterior 4/5 callosotomy, and 57% of those with total callosotomy, respectively. The mean follow-up period was 34 months [24 to 58 months]. Conclusion : Although it is not statistically significant, the patients who had underwent total callosotomy show better outcomes than those with partial callosotomy. Corpus callosotomy is efficacious in controlling medically intractable epilepsy in appropriately selected patients.

진성 골격성 III급 부정교합에서 두개저, 상악, 하악의 위치 및 크기에 관한 연구 (A STUDY OF POSITION AND SIZE OF CRANIAL BASE, MAXILLA, AND MANDIBLE IN TRUE SKELETAL CLASS III PATIENTS)

  • The facial patterns were expressed by the interrelation of variable factors such as heredity, function and environment. Such variable factors have an effect on the growth and development of maxillofacial bones. The malocclusions with skeletal discrepancies are caused by abnormal forms, sizes and positions of cranial base, maxilla and mandible. For the proper diagnosis and treatment planning, the analysis of such structures is necessary. Lateral cephalograms of 54 adults with class III malocclusion patients (test group) and 61 adults with normal occlusion (control group) were analyzed. Anteroposterior relations and sizes of cranial base, maxilla, mandible were estimated to compare with those of normal ones. In test group, the anterior cranial base length was within normal range, but posterior cranial base, maxilla and mandibular body were longer than those in control group, significantly. Based on the cranial base, the location of maxilla in test group was normal, but the location of mandible was more anterior than that in control. Based on the maxilla, the location of mandible was more anterior in test group than that in control. Both mandibular body and ramus anteroposterior lengths in test group were larger than those in control. Both mandibular plane angle and upper gonial angle were within normal range, but lower gonial angle was significantly high in test group.

Maxillary protraction using customized mini-plates for anchorage in an adolescent girl with skeletal Class III malocclusion

  • The treatment of skeletal Class III malocclusion in adolescents is challenging. Maxillary protraction, particularly that using bone anchorage, has been proven to be an effective method for the stimulation of maxillary growth. However, the conventional procedure, which involves the surgical implantation of mini-plates, is traumatic and associated with a high risk. Three-dimensional (3D) digital technology offers the possibility of individualized treatment. Customized mini-plates can be designed according to the shape of the maxillary surface and the positions of the roots on cone-beam computed tomography scans; this reduces both the surgical risk and patient trauma. Here we report a case involving a 12-year-old adolescent girl with skeletal Class III malocclusion and midface deficiency that was treated in two phases. In phase 1, rapid maxillary expansion and protraction were performed using 3D-printed mini-plates for anchorage. The mini-plates exhibited better adaptation to the bone contour, and titanium screw implantation was safer because of the customized design. The orthopedic force applied to each mini-plate was approximately 400-500 g, and the plates remained stable during the maxillary protraction process, which exhibited efficacious orthopedic effects and significantly improved the facial profile and esthetics. In phase 2, fixed appliances were used for alignment and leveling of the maxillary and mandibular dentitions. The complete two-phase treatment lasted for 24 months. After 48 months of retention, the treatment outcomes remained stable.

Long-Term Management of Seizures after Surgical Treatment of Supratentorial Cavernous Malformations : A Retrospective Single Centre Study

  • Objective : Seizure recurrence after the first-ever seizure in patients with a supratentorial cerebral cavernous malformation (CCM) is almost certain, so the diagnosis and treatment of epilepsy is justified. The optimal method of management of these patients is still a matter of debate. The aim of our study was to identify factors associated with postoperative seizure control and assess the surgical morbidity rate. Methods : We retrospectively analysed 45 consecutive patients with a supratentorial CCM and symptomatic epilepsy in a single centre. Pre- and postoperative epidemiological data, seizure-related patient histories, neuroimaging results, surgery details and outcomes were obtained from hospital medical records. Seizure outcomes were assessed at least 12 months after surgery. Results : Thirty-five patients (77.8%) were seizure free at the long-term follow-up (Engel class I); six (13,3%) had rare, nocturnal seizures (Engel class II); and four (8.9%) showed meaningful improvement (Engel class III). In 15 patients (33%) in the Engel I group; it was possible to discontinue antiepileptic medication. Although there was not statistical significance, our results suggest that patients can benefit from early surgery. No deaths occurred in our study, and mild postoperative neurologic deficits were observed in two patients (4%) at the long-term follow-up. Conclusion : Surgical resection of CCMs should be considered in all patients with a supratentorial malformation and epilepsy due to the favourable surgical results in terms of the epileptic seizure control rate and low postoperative morbidity risk, despite the use of different predictors for the seizure outcome.
