Constant score in asymptomatic shoulders varies with different demographic populations: derivation of adjusted score equation

  • Background: In the present study, the age- and sex-adjusted Constant score (CS) in a normal Indian population was calculated and any differences with other population cohorts assessed. Methods: The study participants were patients who visited the outpatient department for problems other than shoulder and healthy volunteers from the local population. Patients without shoulder pain/discomfort during activity were included in the study. Subjects with any problem that might affect shoulder function (e.g., cervical, thoracic spine, rib cage deformity, inflammatory arthritis) were excluded. Constant scoring of all participants was performed by trained senior residents under the supervision of the senior faculty. Shoulder range of movement and strength were measured following recommendations given by the research and Development Committee of the European Society for Shoulder and Elbow Surgery (2008). A fixed spring balance was used for strength measurement; one end was fixed on the floor and the other end tied with a strap to the wrist of the participant, arm in 90° abduction in scapular plane with palm facing down. Results: Among the 248 subjects (496 shoulders), the average age was 37 years (range, 18-78 years), 65.7% were males (326 shoulders) and 34.3% females (170 shoulders). The mean CS was 84.6±2.9 (males, 86.1±3.0; females, 81.8±2.9). CS decreased significantly after 50 years of age in males and 40 years of age in females (p<0.05). The mean CS was lower than in previous studies for both males and females. Heavy occupation workers had higher mean CS (p<0.05). A linear standardized equation was estimated for calculating the adjusted CS for any age. Conclusions: Mean CS and its change with age differed from previous studies among various population cohorts.

Effect of propofol on salivary secretion from the submandibular, sublingual, and labial glands during intravenous sedation

  • Background: Recent animal studies have suggested the role of GABA type A (GABA-A) receptors in salivation, showing that GABA-A receptor agonists inhibit salivary secretion. This study aimed to evaluate the effects of propofol (a GABA-A agonist) on salivary secretions from the submandibular, sublingual, and labial glands during intravenous sedation in healthy volunteers. Methods: Twenty healthy male volunteers participated in the study. They received a loading dose of propofol 6 mg/kg/h for 10 min, followed by 3 mg/kg/h for 15 min. Salivary flow rates in the submandibular, sublingual, and labial glands were measured before, during, and after propofol infusion, and amylase activity was measured in the saliva from the submandibular and sublingual glands. Results: We found that the salivary flow rates in the submandibular, sublingual, and labial glands significantly decreased during intravenous sedation with propofol (P < 0.01). Similarly, amylase activity in the saliva from the submandibular and sublingual glands was significantly decreased (P < 0.01). Conclusion: It can be concluded that intravenous sedation with propofol decreases salivary secretion in the submandibular, sublingual, and labial glands via the GABA-A receptor. These results may be useful for dental treatment when desalivation is necessary.

Dynamic Cardiac Magnetic Resonance Fingerprinting During Vasoactive Breathing Maneuvers: First Results

  • BACKGROUND: Cardiac magnetic resonance fingerprinting (cMRF) enables simultaneous mapping of myocardial T1 and T2 with very short acquisition times. Breathing maneuvers have been utilized as a vasoactive stress test to dynamically characterize myocardial tissue in vivo. We tested the feasibility of sequential, rapid cMRF acquisitions during breathing maneuvers to quantify myocardial T1 and T2 changes. METHODS: We measured T1 and T2 values using conventional T1 and T2-mapping techniques (modified look locker inversion [MOLLI] and T2-prepared balanced-steady state free precession), and a 15 heartbeat (15-hb) and rapid 5-hb cMRF sequence in a phantom and in 9 healthy volunteers. The cMRF5-hb sequence was also used to dynamically assess T1 and T2 changes over the course of a vasoactive combined breathing maneuver. RESULTS: In healthy volunteers, the mean myocardial T1 of the different mapping methodologies were: MOLLI 1,224 ± 81 ms, cMRF15-hb 1,359 ± 97 ms, and cMRF5-hb 1,357 ± 76 ms. The mean myocardial T2 measured with the conventional mapping technique was 41.7 ± 6.7 ms, while for cMRF15-hb 29.6 ± 5.8 ms and cMRF5-hb 30.5 ± 5.8 ms. T2 was reduced with vasoconstriction (post-hyperventilation compared to a baseline resting state) (30.15 ± 1.53 ms vs. 27.99 ± 2.07 ms, p = 0.02), while T1 did not change with hyperventilation. During the vasodilatory breath-hold, no significant change of myocardial T1 and T2 was observed. CONCLUSIONS: cMRF5-hb enables simultaneous mapping of myocardial T1 and T2, and may be used to track dynamic changes of myocardial T1 and T2 during vasoactive combined breathing maneuvers.

영상기기를 활용한 경혈의 인체 해부구조에 대한 중국 내 연구 동향 (Research Trends on the Human Anatomy Structure of Acupoints Using Imaging Devices in China)

  • Objectives : Acupuncture is recognized as a significant therapeutic method in traditional medicine, underscoring the importance of precise understanding of acupoint locations and anatomical structures. The aim of this study is to investigate trends in anatomical studies of acupoint using imaging devices in China through literature search. Methods : This study utilized Chinese databases CNKI, WANFANG, and WEIPU to search for articles. The analysis focused on the publication status of articles by year, types of imaging devices, research subjects and methods, and criteria for acupoint locations. Results : A total of 15 articles were selected in this study. The first article was published in 1996. Articles were published at a rate of less than one per year until 2020, but rapidly increased to three and six in 2021 and beyond. Studies using ultrasonography were the most prevalent, followed by Magnetic Resonance Imaging(MRI) and Computed Tomography(CT) scans. There were eight studies conducted on healthy volunteers, six studies observing both healthy volunteers and cadavers, and also studies involving patients with specific medical conditions. Various criteria were employed to determine acupoint locations, including the WHO standard, national administration of traditional Chinese medicine nomenclature and location of acupoints, as well as references from literature. Conclusions : In China, researches are actively being conducted to observe the anatomical structure of acupoints using various imaging devices. Recent trends indicate an increase in studies utilizing ultrasound and there is a growing focus on exploring the locations that elicit the sensation of "obtaining qi" rather than just the anatomical locations of acupoints. Through this study, it is hoped that the appropriate utilization of imaging devices will lead to various studies aimed at elucidating the characteristics of the anatomical tissues actually stimulated during acupuncture, thereby increasing the safety and efficacy of acupuncture treatments.

InGaAlP 레이저 경피혈액조사가 정상성인의 심박변이도에 미치는 영향 (The Effect of InGaAlP Laser Transcutaneous Blood Irradiation on Heart Rate Variability in Healthy Adults)

  • Objective : The effects of Indium-Gallium-Aluminium-Phosphide(InGaAlP) laser transcutaneous irradiation on heart rate variability(HRV) in healthy adults are investigated with power spectrum analysis(PSA) of HRV. Methods : The control group consisted of 20 healthy volunteers (10 men, 10 women). The experiment was divided into 5 different periods, the pre-1st laser period(10 minutes), the 1st laser period(30 minutes), the post-1st laser period(5 minutes), the 2nd laser period( 30 minutes) and the post-2nd laser period(30 minutes). HRV was measured for 5 minutes at the pre-1 st laser period, the post-l st laser period and the post-2nd laser period. The laser period is the period in which InGaAlP laser transcutaneous Irradiation treatment occurs. Results : 1. SDNN of volunteers at post-1st laser period and post-2nd laser period significantly increased compared with that of the pre- 1 st laser period. 2, Ln(VLF) at post-I st laser period significantly increased compared with that of pre-1st laser period, while Ln(HF) at post-2nd laser period significantly decreased compared with those of pre- I st laser period and post-1st laser period, 3. Ln(TP) at post-1st laser period and post-2nd laser period significantly increased compared with that of pre-1st laser period, 4, LF/HF Ratio at post-2nd laser period significantly increased compared with those of pre-1st laser period and post-1st laser period. But the other variables did not significantly change. Conclusions : The results suggest that InGaAlp laser transcutaneous Irradiation in healthy adults is associated with the autonomic nervous systems. Further study is needed for investigating the effects of laser irradiation on autonomic nervous systems.

정신적 스트레스와 신문(神門)(HT7) 자침(刺鍼)이 정상성인의 심박변이도(HRV)에 미치는 영향 (The Effect of Mental Stress Stimulation and Acupuncture at Shinmun(HT7) on Heart Rate Variability)

  • Objectives : The purpose of this study was to assess the effects of mental stress stimulation and acupucnture applied at HT7 point on heart rate variability (HRV) in normal subjects. Methods : Thirty seven healthy volunteers participated in this study. The volunteers were randomly allocated into two groups. Both groups performed a mental arithmetic stress test and stroop color-word test for 10 minute. Then, acupuncture needles were inserted on HT7 for the study group and on non-acupucnture point for the control group. HRV was recorded three times at test time and before and after acupuncture stimulation. Results : After giving mental stress stimulation to both groups, LF and LF!HF ratio were increased and HF was decreased significantly. After giving acupuncture to study group, LF and LF/HF ratio were decreased and HF was increased, but it was not significant. After giving acupuncture to control group, LF and LF/HF ratio were increased and HF was decreased, but it was not significant. Conclusions : This results show that mental arithmetic stress test and stroop color-word test contribute to activation of the sympathetic nervous system Acupuncture at HT7 could be relatively useful to decrease sympathetic activity and balance autonomic nervous system for those who are under stress.

GLP Perspectives of Bioequivalence Studies

  • Bioequivalence is a term in pharmacokinetics used to access the expected in vivo biological equivalence of two proprietary preparations of a drug. Bioequivalence studies are usually performed for generic drugs. Two pharmaceutical products are bioequivalent if they are pharmaceutically equivalent and their bioavailabilioes after administration in the same molar dose are similar. Bioequivalence is usually accessed by single dose in vivo studies in healthy volunteers and the reference product is usually the innovator product that is marketed. Regulatory definition of bioequivalence is based on the statistical analysis of thebioavailability of the reference and test product. In general, two products are evaluated as bioequivalent if the 90% confidence interval of the relative mean Cmaxand AUC of the test to reference product are within 80.00% to 125.00% in the fasting state. Key process in bioequivalence study is development and validation of bioanalytical method, determination of the drug concentration in the biosamples (usually plasma or serum) obtained from volunteers, calculation of the pharmacokinetic parameters and statistical analysis of the pharmacokinetic parameters. Although current guidelines and regulations do not require the bioequivalence studies to be done under good laboratory practice (CLP), the issues to perform the bioequivalence studies under GLP environment is emerged both from the regulatory and industry side. GLP perspectives of bioequivalence studiesare needed to be discussed in respect to achieve quality assurance in bioequivalence studies.

호흡 유형에 따른 건강한 성인의 정적균형능력 비교 (Comparison of Static Balance Abilities on Respiratory Types in Healthy Adults)

  • Purpose: To investigate breathing-related changes in the balance ability of healthy adults. Methods: The participants were 36 healthy adults in their 20s and 30s. All participants were trained in three breathing (neutral, thoracic, and abdominal) methods one week before measurements. We used AccuSway to measure each participant's balance ability, using each breathing technique, in two postures (bipedal and unipedal). Results: During the bipedal balance task, abdominal breathing produced significant increases in path length and sway velocity. Abdominal breathing resulted in significant changes in sample entropy at the anteroposterior location compared with neutral breathing (p<.05). In the normalized anteroposterior location, there were significant changes in backward movement during thoracic and abdominal breathing compared with those during neutral breathing (p<.05). During the unipedal balance task, path length and sway velocity increased significantly during voluntary breathing compared with those during neutral breathing (p<.05). There was a significant change in backward movement when abdominal breathing-compared with neutral breathing-was used in the normalized anteroposterior location (p<.05). In the normalized left-right location, there was a significant shift to the right during thoracic breathing compared with that during neutral breathing (p<.05). Conclusion: Compared with neutral breathing, altered voluntary breathing patterns affect balance in healthy adults. Our results indicated that that static balance was more affected by abdominal breathing than by neutral breathing. Future studies should examine variables such as the breathing volume, rhythm, and method.

Isoprostane Characteristics in Sick House Syndrome, Atopy and Asthma Patient

  • An imbalance between oxidants and antioxidants, in favor of oxidants leading to oxidative stress, is known to play an important role in the pathogenesis of various diseases. Isoprostanes are structurally stable isomers of the conventional enzymatically derived prostaglandins, which are produced in vivo primarily by a free radical catalyzed peroxidation of polyunsaturated fatty acids. In asthmatics, disease severity can occur from environmental exposure to air pollution. Some surveys suggested that air pollutants, especially diesel-exhaust particulates, could trigger allergic sensitization and development of atopic diseases. Sick house syndrome (SHS) presents healthy damage owing to the indoor environment of a building. The aim of this study was to examine isoprostane as a parameter fur oxidative stress in environments related diseases such as sick house syndrome, atopy and asthma. We measured plasma and urinary levels of isoprostane from health volunteers, sick house syndrome, atopy and asthma patients. Plasma isoprostane concentrations in asthma and sick house syndrome group were significantly higher than in control. Urinary isoprostane levels were significantly higher in volunteers with sick house syndrome and asthma compared with health volunteers. These findings suggest that plasma and urinary isoprostane measurement may have useful clinical implications for investigating sick house syndrome and asthma. The interventions that decrease exposure to environmental reactive oxygen species might be beneficial in these diseases.

정상 성인에서 구혈대에 의한 신경기능의 변화 (The Effect of Tourniquet Inflation on Neural Functions: A Volunteer Study)

  • Background: Tourniquet pain has important impacts on anesthesia. Tourniquet pain and accompanying cardiovascular changes are important factors that make patients in distress during anesthesia. As tourniquet pain may be modified by anesthesia, a study on the changes in the neural functions by tourniquet inflation in normal volunteers is important. Methods: Time-dependent changes in tourniquet pain, heart rate, phantom limb sensation, motor function, pain to pressure on upper extremity of 10 healthy and unpremedied volunteers were measured. Each parameter were measured every 5 minutes starting from 10 minutes before inflation to 15 minutes after deflation of tourniquet. Tourniquet was deflated when the subject felt unbearable pain (score 100 with visual analog scale). Results: Subjects manifested time-dependent pain responses to tourniquet inflation, characterized by increase in VAS, systolic and diastolic blood pressure. Mean duration of tourniquet inflation was 36.4 minutes, volunteers experienced motor paralysis at 27.6 minutes and sensory loss at 33.1 minutes. Pain to pressure decreased over time in both arms. The degree of decrease was greater in the arm on which tourniquet was applied than that in the non-applied arm. Phantom limb sensation occurred in 3 subjects. Conclusions: This study demonstrated dynamic changes in the neural functions during tourniquet inflation period. Tourniquet-induced pain and resultant hypertension occurred in all subjects. Appropriate anesthetic management is needed for the surgery using tourniquet.
