아동 친화적인 도시에 대한 아동.청소년의 인식 (Children and Adolescents' Assessments of Child Friendly Cities)

  • Child Friendly Cities (CFC) is a framework for action with the aim of implementing the UN CRC led by local government in an urban context. A number of surveys with 755 children and adolescents were conducted in order to assess the community's child-friendliness level of Seoul. The results of this study were as follows : 1) Comprehensive assessment of the city's child-friendliness was overall seen to be negative. 2) In regards to the city's child-friendliness, the assessments of Children were more positive than those of the adolescents. 3) Boys assessed the city's child-friendliness more positively than girls. 4) The children and adolescents living in more wealthy communities assessed the city's child-friendliness more positively than those living in poorer areas. This study suggested that a local action plan for the Child Friendly Cities must be implemented to fulfill the CFC standards.

Impact of the Coronavirus Disease Pandemic on Mental Health Among School Students in Korea During the COVID-19 Pandemic

  • Objectives: The coronavirus disease (COVID-19) pandemic has had various effects on mankind, especially children and adolescents. Because children and adolescents spend a lot of time at school, COVID-19 has had a great impact on school mental health. In this study, we investigated the effect of prolonged COVID-19 on school mental health. Methods: We prepared self-report questionnaires for depression (Children's Depression Inventory, CDI), anxiety (Korean version of the Penn State Worry Questionnaire for Children; Generalized Anxiety Disorder-7, GAD-7), and post-traumatic stress (Primary Care Post-traumatic Stress Disorder, PC-PTSD) for administering to students aged between 7 and 18 years, recruited by a COVID-19 psychological prevention support group in the Gwangmyeong Mental Health Welfare Center for 2 years, in 2020 and 2021. Results: For children aged 7-12 years, there was no significant difference between the years 2020 and 2021 in the assessment of depression, anxiety, and post-traumatic stress. Conversely, for adolescents aged 13-18 years, there was a significant increase in the scale scores (CDI, PC-PTSD, and GAD-7). Conclusion: Prolonged COVID-19 might have had a significant impact on the mental health of adolescents who spent a lot of time at school. When comparing the years 2020 and 2021, middle and high school students were more affected by COVID-19 than elementary school students.

  • The Committee on Dyslipidemia of Korean Pediatric and Adolescents of the Korean Society of Pediatric Endocrinology has newly developed evidence-based clinical practice guidelines for dyslipidemia in Korean children and adolescents. These guidelines were formulated with the Grading of Recommendations, which include both the strength of recommendations and the quality of evidence. In the absence of sufficient evidence, conclusions were based on expert opinion. These guidelines are based on the 2011 National Heart, Lung, and Blood Institute Guidelines, which focus on the prevention of cardiovascular disease in children and draw from a comprehensive review of evidence. These guidelines contain the definition of and screening process for dyslipidemia and introduce new dietary methods: the Cardiovascular Health Integrated Lifestyle Diet (CHILD)-1, the CHILD-2-low-density lipoprotein cholesterol, and the CHILD-2-triglyceride. Potential drug therapies for dyslipidemia along with their main effects and doses were also included.

아동과 어른 그리고 청소년에 대한 사회적 표상: 성격적, 관계적 및 과업적 특성을 중심으로 (Social Representations on Children, Adults and Adolescents: With Focus on Personality, Relationships and Tasks)

  • 본 연구는 개방형 질문을 이용한 두 개의 경험적 연구를 통해서 아동(혹은 아이), 어른(혹은 성인) 그리고 청소년에 대한 한국대학생의 사회적 표상을 살펴보았다. 연구 1은 한국대학생 97명(남: 28명, 여: 69명)을 대상으로 아동과 어른에 대한 사회적 표상을 비교하였다. 그 결과, (어른다운) 어른과 대비되는 아이의 특성은 대부분 성격적인 측면과 관련되었으며, 특히 아이는 어른에 비해 외향성이 높은 반면, 호감성, 성실성 및 교양에서의 수준은 낮았다. 또한, 아이와 어른 모두 그들의 행복과 불행은 거의 대부분 대인관계적 특성과 건강, 가정의 경제적 환경이나 사회적 성취수준에 의해 결정되었다. 반면, 좋은 아이는 성격적으로 타인에 대한 배려나 독립성 등 높은 호감성과 성실성에 의해 거의 전적으로 규정되었는데, 이러한 특성은 아이다운 아이의 특성보다는 어른다운 어른의 특성과 매우 유사했다. 연구 2에서는 한국대학생 57명(남: 16명, 여: 41명)을 대상으로 청소년에 대한 사회적 표상을 알아보았다. 그 결과, 전반적으로 청소년은 성격이나 자아개념, 신체적 및 과업적 특성에서 아이와 어른의 중간 단계에 위치하고 있었다. 또한, 행복하거나 불행한 청소년은 아이 및 어른의 경우와 마찬가지로, 관계적 특성과 과업적 특성 혹은 환경적 특성에 의해 크게 결정되었다. 그러나, 좋은 혹은 나쁜 청소년은 아이의 경우와는 달리 성격에서의 호감성과 성실성 이외에도 자아 관련 특성, 관계적 특성 및 과업적 특성에 의해서도 규정되었다. 이러한 결과를 한국사회의 문화적 특성 및 인간발달의 입장에서 논의하였다.

아동과 청소년의 수치심과 부모와의 의사소통이 또래압력에 미치는 영향 (The Effects of Children's Shame and Communication with Their Parents on Peer Pressure)

  • The purpose of this study was to investigate the interaction and effects of shame and parent-child communication on children and early adolescents' peer pressure. The subjects of this study were 359 children and early adolescents from 5th grade in elementary school to 2nd grade in middle school (187 boys, and 172 girls) in Daejeon city. The results of this research indicated that gender and age played a significant role on the effects of shame and parent-child communication on peer pressure. Open communication with mothers showed a positive relationship with boys' peer pressure, whereas open communication with father showed a negative relationship with boys' peer pressure. The interaction effects of shame and open-communication with fathers and mothers worked differently on peer pressure across gender and age. Interaction effects of shame and open communication with fathers and mothers were significant for boys, but not for girls. Interaction effect of shame and open communication with mothers was significant for children and adolescents.

소아 및 청소년의 신체형장애 (Somatoform Disorders of Children and Adolescents)

  • Somatoform disorders do occur among children and adolescents. Among the seven disorders under the grouping of somatoform disorders of DSM-IV, three disorders, namely somatization disorder, pain disorder and conversion disorder are seen relatively more often than one can expect in childhood and adolescence. Pain disorders are more prevalent among children before adolescence, whereas conversion disorder and somatoform disorder are seen more often during adolescence and early adulthood. Diagnoses of somatofram disorders should not be made by the process of exclusion, but based on positive findings that positive evidence that normal functioning is possible and that a positive history of psychosocial stress and or intrapsychic conflict exists. Treatment strategy should be mindful of including collaboration with primary care health professionals and family therapy staff in addition to all the basic treatment modalities essential for the treatment of children and adolescents.

Pulmonary Artery Angioplasty for Improving Ipsilateral Lung Perfusion in Adolescent and Adult Patients: An Analysis Based on Cardiac Magnetic Resonance Imaging and Lung Perfusion Scanning

  • Background: The left pulmonary artery (LPA) may be kinked and stenotic, especially in tetralogy of Fallot, because of ductal tissue and anterior deviation of the conal septum. If LPA stenosis is not effectively treated during total correction, surgical angioplasty is occasionally performed. However, whether pulmonary artery (PA) angioplasty in adolescents or adults improves perfusion in the ipsilateral lung remains unclear. Methods: This retrospective review enrolled patients who underwent PA angioplasty for LPA stenosis between 2004 and 2019. Among patients who underwent a lung perfusion scan (LPS) or cardiac magnetic resonance imaging (cMRI) pre- and post-pulmonary angioplasty, those aged >13 years with <40% left lung perfusion (p-left) in the pre-angioplasty study were included. Preoperative and postoperative computed tomography, LPS, and cMRI data were collected. The perfusion ratio was analyzed according to the LPA's anatomical characteristics. Results: Seventeen adolescents and 16 adults (≥18 years old) were finally included (median age, 17 years). The most common primary diagnosis was tetralogy of Fallot (87.9%). In all patients, LPA angioplasty was performed concomitantly with right ventricular outflow tract reconstruction. No patients died. Preoperative p-left was not significantly different between adolescents and adults; however, adolescents had significantly higher postoperative p-left than adults. P-left significantly increased in adolescents, but not in adults. Seven patients had significant stenosis (z-score <-2.0) confined only to the proximal LPA and demonstrated significantly increased p-left. Conclusion: PA angioplasty significantly increased ipsilateral lung perfusion in adolescents. If focal stenosis is confined to the proximal LPA, PA angioplasty may improve ipsilateral lung perfusion, regardless of age.

소아 청소년에서의 신경이완제 악성증후군에 대한 종설 (Neuroleptic Malignant Syndrome in Children and Adolescents : A Review)

  • Neuroleptic malignant syndrome (NMS) is a severe iatrogenic complication of treatment with antipsychotic medication. The aim of this review is to provide the clinical characteristics and treatments of children and adolescents with NMS. Searches were conducted in Medline, Korean studies Information Service System (KISS), and Research Information Service System (RISS). Sixteen case reports and two review articles were selected in Medline, and two Korean cases reported in department of emergency medicine and pediatrics were selected from RISS. Heterogeneous and atypical presentations of NMS were observed in children and adolescents. Some noticeable differences were observed between adult patients and child patients with NMS. In addition, symptom presentations related to atypical antipsychotic agents differed from those of typical ones. In treatment, bromocriptine and benzodiazepine were recommended for management of symptoms. In particular, electroconvulsive therapy (ECT) was a useful treatment option. For prevention and early detection of NMS in children and adolescents, evaluation of risk factors and understanding of diagnostic features of NMS are very important.

한의 치료를 받은 저체중 소아청소년의 특성 및 치료 효과에 대한 고찰 (A Study on the Characteristics and Treatment Effects of Underweight Children and Adolescents Treated by Korean Medicine)

  • Objectives This study aimed to provide evidence for the use of Korean medicine in underweight patients by analyzing the medical records of underweight children and adolescents. Methods Children and adolescents who visited Korean pediatrics at Kyung Hee University Hospital from June 1, 2019 to May 31, 2022, who had an age-specific weight percentile which was less than 5 and diagnosed with E639.005 or R628.001 were selected for the study. A retrospective chart review was conducted by examining the medical records of subjects who received Korean medical treatment. The treatment effect was analyzed in all subjects and subgroups by comparing changes in height, weight, and body mass index (BMI) percentiles before and after Korean medicine treatment. Results A total of 55 underweight subjects were included in the study. General characteristics such as distribution by sex, age, number of visits, and weight percentile, and clinical characteristics such as diagnosis, past history, family history, drug history, current or past symptoms, and symptom differentiation were collected. The composition of herbal medicines and the contents of other Korean medical treatments were analyzed. In the case of treatment effect, the weight and BMI percentile of all subjects increased significantly. Conclusions This study was conducted to suggest Korean medicine as a method of proper weight management in children and adolescents. In the future, follow-up studies with a large number of subjects and controlled variables, such as the period of taking herbal medicine, are needed.

Development of the Computerized Mathematics Test in Korean Children and Adolescents

  • Objectives: This study was conducted in order to develop a computerized test to measure the level of mathematic achievement and related cognitive functions in children and adolescents in South Korea. Methods: The computerized Comprehensive Learning Test-Mathematic (CLT-M) consists of the whole number computation test, enumeration of dot group test, number line estimation test, numeral comparing test (magnitude/distance), rapid automatized naming test, digit span test, and working memory test. To obtain the necessary data and to investigate the reliability and validity of this test, 399 children and adolescents from kindergarten to middle school were recruited. Results: The internal consistency reliability of the CLT-M was high (Cronbach's alpha=0.76). Four factors explained 66.4% of the cumulative variances. In addition, the data for all of the CLT-M subtests were obtained. Conclusion: The computerized CLT-M can be used as a reliable and valid tool to evaluate the level of mathematical achievement and associated cognitive functions in Korean children and adolescents. This test can also be helpful to detect mathematical learning disabilities, including specific learning disorder with impairment in mathematics, in Korea.
