Background: The purpose of our study was to evaluate the usefulness of hip arthroscopy including extensive capsulectomy for synovial chondromatosis of the hip. Methods: From 2008 to 2016, 13 patients with synovial chondromatosis of the hip were treated with arthroscopic removal of loose bodies and synovectomy using three arthroscopic portals. An extensive capsulectomy was performed to allow the remaining loose bodies to be out of the extracapsular space, and the excised capsule was not repaired. All patients were assessed by clinical scores and the radiographs were reviewed to determine whether the remaining loose bodies disappeared at the last follow-up. Results: Eight men and two women were followed up for a minimum of 1 year (mean, 3.8 years; range, 1 to 6.8 years) after hip arthroscopy. Clinical outcomes such as modified Harris hip score, University of California Los Angeles score, and Western Ontario and McMaster Universities Osteoarthritis Index score improved at the last follow-up. Although seven hips had remaining loose bodies after arthroscopic surgery, the remaining loose bodies disappeared in five hips (71.4%) at the last follow-up. Conclusions: Arthroscopic surgery was useful to treat synovial chondromatosis of the hip. In spite of limited removal of loose bodies, arthroscopic procedures including extensive capsulectomy could be effective for the treatment of synovial chondromatosis of the hip.
Objective: Degenerative knee arthritis is the most common disease that occurs in older people. Constriction-induced movement therapy (CIMT) has been reported to be as an effective treatment for the impairments, such as asymmetric weight-bearing and reduced balance that occurs after receiving a total knee replacement (TKR). Game-based rehabilitation training for persons with TKR is interesting and provides a variety of feedback. Design: Randomized controlled trial. Methods: Thirty-six subjects with TKR were randomly assigned to either the CIMT game training (n=12), general game training (n=12), or the control (n=12) group. Each group underwent twelve sessions (30 min/d, 3 d/wk for 4 weeks). In the CIMT game training group, the application of CIMT adjusted the weight of the pressure delivered from the two boards used in Wii games. In the general game training group, the game was played without adjusting the weight of pressure. The game training used the Wii Fit's Ski Slalom application. Subjects were assessed with the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and the Activities-specific Balance Confidence (ABC) Scale. Results: All three groups showed significant improvements in pain, stiffness and physical function, total WOMAC scores, and ABC scores after the intervention (p<0.05). Significant differences were observed in physical function, total WOMAC scores, and ABC scores of the CIMT game training group compared with the other groups (p<0.05). Conclusions: The CIMT game training and general game training exhibited improvements on stiffness, but the CIMT game training exhibited a larger effect on lower extremity function and balance confidence levels.

Objective: The purpose of this case study was to investigate selective region high-frequency diathermy at trigger points with therapeutic exercises on pain, function, balance and gait in older patients with degenerative knee osteoarthritis (DKO). Design: A case report. Methods: The patient who participated in this study was a 71-year-old woman, who had been diagnosed with moderate osteoarthritis with grade II Kellgren & Lawrence grading scale. The intervention consisted of selective region high-frequency diathermy at trigger points, with hip and knee stretching and strengthening exercises. The participant was given assessments before and after every intervention session using the Visual Analogue Scale (VAS), Western Ontario and McMaster Universities Arthritis Index (WOMAC), the Timed Up and Go test (TUG) and the 10 Meter Walk Test (10MWT). The participant performed the intervention 18 times for a total of 30 minutes each. Results: As a result of this study, the patient VAS decreased to 3 points, and the WOMAC decreased to 53 points. In addition, the TUG decreased to 3.25 s and the 10MWT decreased to 1.14 s. Conclusions: The results of this study suggest that selective region high-frequency diathermy at trigger points with therapeutic exercises may be an effective intervention to decrease pain, improve knee function, balance and gait in patients with DKO. The selective region high-frequency diathermy with therapeutic exercises may be feasible and provide potential benefits for rehabilitation of DKO.
Background: Osteoarthritis is a common condition with an increasing prevalence and is a common cause of disability. Osteoarthritic pain decreases the quality of life, and simple gait training is used to alleviate it. Knee osteoarthritis limits joint motion in the sagittal and lateral directions. Although many recent studies have activated orthotic research to increase knee joint stabilization, no study has used patellar tendon straps to treat knee osteoarthritis. Objects: This study aimed to determine the effects of patellar tendon straps on kinematic, mechanical, and electromyographic activation in patients with knee osteoarthritis. Methods: Patients with knee osteoarthritis were selected. After creating the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), leg length difference, Q-angle, and thumb side flexion angle of the foot were measured. Kinematic, kinetic, and muscle activation data during walking before and after wearing the orthosis were viewed. Results: After wearing the patellar tendon straps, hip adduction from the terminal stance phase, knee flexion from the terminal swing phase, and ankle plantar flexion angle increased during the pre-swing and initial swing phases. The cadence of spatiotemporal parameters and velocity increased, and step time, stride time, and foot force duration decreased. Conclusion: Based on the results of this study, the increase in plantar flexion after strap wearing is inferred by an increase due to neurological mechanisms, and adduction at the hip joint is inferred by an increase in adduction due to increased velocity. The increase in cadence and velocity and the decrease in gait speed and foot pressure duration may be due to joint stabilization. It can be inferred that joint stabilization is increased by wearing knee straps. Thus, wearing a patellar tendon strap during gait in patients with knee osteoarthritis influences kinematic changes in the sagittal plane of the joint.
목적: 관절경적 세척술 및 변연 절제술을 시행하고 배양 검사로 확진된 화농성 슬관절염을 가진 환자들에서, 단기적 임상 경과와 중장기적 기능적 결과를 확인하고, 이에 영향을 미치는 인자를 분석하였다. 대상 및 방법: 관절경적 변연 절제술을 시행하고, 배양검사상 양성으로 판명된 32예의 환자를 대상으로 하였으며, 평균 추시 기간은 41.6개월이었다. 감염에 대한 임상적 평가로 사망, 재발, 적혈구 침강 속도 및 C-반응성 단백 수치의 정상화 기간, 정주용 항생제 투여 기간, 재입원 여부를 조사하였다. 방사선학적 평가로 인공 슬관절 치환술, 관절염 진행 정도를, 기능적 평가로 modified Lysholm, Tegner activity 및 Korean version of the Western Ontario and McMaster Universities(K-WOMAC) score를 조사하였다. 결과: 황색 포도상 구균이 21예에서 동정되었고, 적혈구 침강 속도 및 C-반응성 단백 수치의 정상화 기간은 평균 78.0 및 67.6일이었다. 2예에서 사망하였고, 6예에서 재발하였다. 입원 중 재발은 만성 신부전이 동반된 환자에서 빈발하였다(P=0.034). 5례의 환자에서 재입원이 관찰되었고, 최초 내원시 방사선 검사상 관절염 정도가 심하고(P=0.032), 입원 중 수술 횟수가 많을 수록 재입원의 가능성이 높았다(P=0.006). 21례에서 관절염이 진행하였고, 최종 Kellgren-Lawrence 등급은 최초 내원시의 등급과 관련이 있었다(P=0.007). 5례에서 인공 슬관절 치환술을 시행받았으며, Lysholm score는 평균 53.5점, Tegner activity score는 평균 2.7점, K-WOMAC score는 평균 44.2점이었다. 결론: 관절경적 치료 후 최종 관절염 정도는 기왕의 관절염 정도에 의해 결정되었다. 기존 퇴행성 변화가 심한 환자에서 재입원이 많았고, 만성 신부전 환자에서는 재발이 많아 이들 환자들에서는 주의를 요한다.
목적: 슬관절 내측 구획 골관절염으로 근위 경골 절골술 및 미세 골절술을 함께 시행한 환자 군에서 이차적 관절경 검사를 시행하여 연골 재생 정도를 평가하고 연골 재생에 영향을 미치는 요인을 규명하고자 한다. 또한 연골 재생의 정도와 기능적 결과가 연관성이 있는지 분석하고자 한다. 대상 및 방법: 2007년부터 2015년까지 근위 경골 절골술 및 미세 골절술을 함께 시행하였고, 최소 2년이 지나 금속판 제거술 및 이차 관절경 검사까지 시행 받은 81예를 대상으로 하였다. 술 전 대퇴부 연골 손상의 정도는 ICRS (International Cartilage Research Society) 분류를 이용하였고, 모두 grade III, IV 였다. 이차적 관절경 검사 이후에는 연골 재생이 양호한 그룹(grade I, II)과 불량한 그룹(grade III, IV)으로 분류하였다. 다변량 로지스틱 회기 분석을 통하여 연골 재생에 영향을 미치는 독립적인 요인을 파악하였다. 또한 두 그룹간의 수술 전후에 Knee Society score (KSS)와 Western Ontario and McMaster Universities Osteoarthritis Index score (WOMAC)를 이용하여 기능적 결과를 비교하였다. 결과: 연령, 성별, 체질량 지수, 술 전후 방사선학적 요인, 술 전 관절의 상태는 연골 재생의 정도에 유의한 영향을 끼치지 않았으며, 큰 연골 병변 범위(≥2.0 cm2) (p=0.011)와 키스 병변(kissing lesion)의 존재(p=0.027)는 불량한 연골 재생과 연관성이 있었다. KSS와 WOMAC 점수상에서는 연골의 재생이 양호한 그룹과 불량한 그룹 간의 유의한 차이는 없었다. 결론: 큰 연골 병변과 키스 병변의 존재는 근위 경골 절골술 및 미세 골절술 후 불량한 연골 재생과 관계가 있었다. 하지만 재생된 연골의 상태는 기능적 결과와 연관성을 보이지 않았다.
목적: 무릎 내측 구획 골관절염 환자에서 초음파 유도하 무릎 관절강내 경 반월상 연골 주사를 소개하고 그 임상적 결과를 분석하고자 하였다. 대상 및 방법: 2019년 3월부터 2019년 7월까지 3개월 이상 무릎 통증이 지속되었던 무릎 내측 구획 골관절염 환자 중 초음파 유도하 경 반월상 연골 주사를 시행한 36명의 환자에 대해서 후향적으로 분석하였다. 초음파 유도하 척추 바늘을 이용하여 무릎 관절강내 내측 구획에 corticosteroid와 국소마취제를 주사하였다. 시술 전, 1주, 4주, 8주째 신체진찰을 하였고 numeric pain rating scale (NRS), Lequesne index, Western Ontario and McMaster Universities Osteoarthritis (WOMAC) 점수를 측정하였다. 시간에 따른 NRS, Lequesne index, WOMAC 점수를 분석하였으며 상당한 호전이 있었던 환자들의 비율을 분석하였다. 또한 내측 구획 골관절염의 단계에 따른 NRS, Lequesne index, 상당한 호전이 있었던 환자들의 비율을 분석하였다. 결과: 시술 전과 비교하여 1주, 4주, 8주째 NRS와 Lequesne index는 감소하였고 8주째까지 통증 감소 효과는 차이 없이 지속되었다. 상당한 호전이 있었던 환자들의 비율은 1주, 4주, 8주째 각각 50.0%, 47.2%, 52.8%였으며 8주째까지 통증 감소 효과는 차이없이 지속되었다. 시술 전과 비교하여 1주, 4주, 8주째 WOMAC 점수는 감소하였고 8주째까지 차이 없이 감소가 지속되었다. 골관절염 1, 2단계 환자들에서 상당한 호전이 있었던 환자들의 비율이 골관절염 3, 4단계 환자들보다 1주, 4주, 8주째 유의하게 많았다(p<0.05). 결론: 무릎 내측 구획 골관절염 환자에서 초음파 유도하 무릎 관절강내 경 반월상 연골 주사는 평균 8주 이상의 통증 감소 및 기능 개선 효과를 보였다. 특히 내측 구획 골관절염 1, 2단계 환자에서는 통증 감소 효과가 더 컸다.
목적: 고관절 전치환술 후 발생한 장요건 충돌 환자에서 관절경적 장요건 절단술이 유용한 방법인지 알아보기 위하여 임상 결과에 대하여 조사하였다. 대상 및 방법: 2013년 9월부터 2019년 12월까지 고관절 전치환술 후 지속되는 서혜부 통증 및 굴곡 시 통증을 호소하는 18명(19예 고관절)의 환자를 대상으로 하였다. 평균 연령 60세(범위, 50-69세)의 환자들에서 고관절 전치환술 후 증상발현까지는 평균 4개월(범위, 1-9개월)이 소요되었으며, 18명 환자 중 13명에는 직접전방접근법, 5명에는 측방접근법을 통한 고관절 전치환술이 시행되었다. 병력청취, 신체 검사, 혈액검사, 단순 방사선촬영과 컴퓨터 단층촬영을 이용한 영상 검사, 그리고 국소주사치료를 통하여 장요건 충돌을 진단하였다. 이들 모든 환자는 관절경적 처치를 시행받았고, 역동적 신체 검사를 통하여 술장 내에서 충돌이 이루어짐을 확인하였으며, 근육부위까지 도달하도록 관찰된 건 부위 모두에 대하여 건절단술을 시행하였다. 수술 전, 후, 추시기간의 증상 및 통증 정도에 대하여 조사하여 비교하였다. 결과: Western Ontario and McMaster Universities Osteoarthritis Index 점수는 술 전 평균 58.4점(범위, 40-88점)에서 술 후 평균 35.0점(범위, 15-76점)으로 감소하였다. 시각통증등급도 마찬가지로 술 전 평균 4.0 (범위, 2-6)에서 술 후 평균 1.4 (범위, 0-4)로 감소하였다. 16명(88.9%)에서 통증 경감 및 하지거상 시 통증의 호전을 보였으며, 2명에서는 통증 잔존과 술 후 근력 약화를 보였다. 이후 추시기간에서 근력 약화는 호전되었다. 1명의 경우 소전자부에서 장요건 건절단술을 시행하였으나 증상이 지속되었으며, 관절부에서 한 차례 더 건절단술 시행 후 임상증상의 호전을 보였다. 결론: 고관절 전치환술 후 발생한 장요건 충돌에서 고관절 관절경을 이용한 장요건 건절단술은 양호한 임상 결과를 보였다.
Objectives : The aim of this study is to determine whether Chinemys reevesii Gray pharmacopuncture is a clinically effective treatment for women with knee osteoarthritis. Methods : Female patients with knee osteoarthritis were recruited from May $1^{st}$, 2014 to April $15^{th}$, 2015 at the Department of Acupuncture & Moxibustion Medicine of Dong-Eui University Korean Medical Hospital. Patients were randomly assigned to two groups. 4.0 ml of Chinemys reevesii Gray pharmacopuncture was injected at acupuncture points of the experimental group(n = 24) twice a week for three weeks. The control group(n = 24) received 5 % dextrose injections in order to identically stimulate the same acupuncture points of $ST_{35}$, $ST_{34}$, $SP_9$, $SP_{10}$, $GB_{34}$, $LR_8$, $EX-LE_4$, and $EX-LE_2$. The Short-Form McGill Pain Questionnaire(SF-MPQ), the Korean Western Ontario and McMaster Universities Osteoarthritis Index(KWOMAC), and the European Quality of Life-5 Dimensions(EQ-5D) were recorded to evaluate treatment efficacy at first visit, after two weeks and four weeks from the initial visit. Results : 40 patients completed the trial. The experimental group showed more significant improvement than the control group in the scores of SF-MPQ, KWOMAC, and EQ-5D. However, there was no significant difference between groups in a few of the assessment scales. Conclusions : Chinemys reevesii Gray pharmacopuncture can be an effective treatment in control of pain, improvement in function and health-related quality of life for women with knee osteoarthritis.

Objectives: We investigated the efficacy and safety of miniscalpel acupuncture (MA) for knee osteoarthritis (KOA) in an assessor-blinded randomized controlled pilot trial; this would provide information for a large-scale randomized controlled trial. Methods: Participants (n = 24) were recruited and randomly allocated to the MA group (experimental) or acupuncture group (control). The MA group received treatment once a week for 3 weeks (total of 3 treatments), while the acupuncture group received treatment two times per week for 3 weeks (total of 6 treatments). The primary outcome was pain as assessed by a visual analogue scale (VAS). The secondary outcomes (intensity of current pain, stiffness, and physical function) were assessed using the short-form McGill Pain Questionnaire (SF-MPQ) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Assessments were performed at baseline, 1, 2, and 3 during treatment and at week 5 (2 weeks after the end of treatment). Results: Of the 24 participants, 23 completed the study. Both groups showed significant improvements in VAS, SF-MPQ, and WOMAC. However, there were no significant differences between the MA and acupuncture groups. No serious adverse event occurred and blood test results were within normal limits. Conclusion: Our results suggest that although both MA and acupuncture provide similar effects with regard to pain control in patients with KOA, MA may be more effective in providing pain relief because the same relief was obtained with fewer treatments. A large-scale clinical study is warranted to further clarify these findings.

제 5 장 계약 해지 및 이용 제한

② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
