기존치료법과 침도침 시술을 병행한 요추관협착증의 증례보고 및 분석 (Study and three Cases Report for Lumbar Spinal Stenosis Treatment Using a Combination of Acupotomy and Existing Treatments)

  • This study reports the effect of acupotomy for patients diagnosed with lumbar spinal stenosis(LSS). We treated three patients diagnosed with LSS using acupotomy. We measured the Numerical Rating Scale(NRS), Oswestry Disability Index(ODI) scores and Odom's criteria before and after treatment to assess improvements in symptoms and functions. In all patients, NRS, ODI scores and Odom's criteria were improved. We found a beneficial effect with acupotomy for three patients diagnosed with LSS. Thus, these data suggest LSS may be treated successfully with acupotomy.

복합부위통증증후군 환자에서 삼상 골스캔의 유용성 평가 (The Effectiveness of a Three Phase Bone Scan for Making the Diagnosis of Complex Regional Pain Syndrome)

  • Background: Complex regional pain syndrome (CRPS) is still difficult to diagnose in the field of chronic pain management. CRPS is diagnosed by purely clinical criteria based on the characteristic signs and symptoms, which have to be differentiated from similar pain conditions like posttraumatic neuropathic pain. Until now, there has been a lack of objective diagnostic tools to confirm the diagnosis of CRPS. The aim of this study was to evaluate the usefulness of a three phase bone scan (TBS) for making the diagnosis of CRPS. Methods: A total of 121 patients who had been diagnosed with CRPS were evaluated. All the patients were examined by performing a TBS as a part of the diagnostic work-up. A diffuse increased tracer uptake on the delayed image (phase III) was defined as a positive finding for CRPS. Results: Forty-one patients (33.9%) out of 121 showed the positive results on the TBS. The patients with a duration of pain of less than 24 months had a significantly higher positive result (43.4%) on the TBS than did the patients with duration of pain longer than 24 months (12.1%). Conclusions: A TBS could give a better objective result for diagnosing CRPS for patients with a shorter duration of pain and a TBS gives little information for the diagnosis of CRPS in patients with a duration of pain longer than 24 months.

가감청영탕(加減淸營湯)에 효과를 보인 열성 아토피 피부염 환자 6례 증례 보고 (6 Case of Atopic Dermatitis Diagnosed as the Febrile Tendency Treated with Gagamchengyoung-tang(Jiājiǎnqīngyíng-tāng))

  • Objective : The purpose of this study is to investigate the effect of Gagamchengyoung-tang($Ji\bar{a}ji\check{a}nq\bar{i}ngy\acute{i}ng-t\bar{a}ng$) for atopic dermatitis patients diagnosed as the febrile tendency. Method : This case study was done on 6 atopic patients with febrile tendency who have visited Korean Medicine Clinic from 2013.08.01. to 2014.04.30.. Gagamchengyoung-tang($Ji\bar{a}ji\check{a}nq\bar{i}ngy\acute{i}ng-t\bar{a}ng$) was prescribed to all 6 patients and evaluated the symptom change through photographs and questionnaire. Objective SCORAD Index(OSI) was used to evaluate the objective symptom and VAS was used to evaluate the subjective change. Results & Conclusion : The Objective SCORAD index and VAS were decreased in all patients. The average OSI reduction score was 26.3. Most symptoms of 6 patients were improved. Gagamchengyoung-tang ($Ji\bar{a}ji\check{a}nq\bar{i}ngy\acute{i}ng-t\bar{a}ng$) can be effective in patients who were diagnosed as febrile tendency.

한의학적 치료를 활용한 손톱을 침범한 사마귀 환자 11례 증례보고 (11 Cases of Periungual Warts Treated by Korean Medicine)

  • Objectives : The purpose of this study is to report the effect of the Korean medical treatment for periungual warts. Methods : From December 2016 to March 2019, all patients were treated by herbal medicine, acupuncture, sweet bee venom acupuncture and moxibustion. Photographs of lesions were used to evaluate the changes in symptoms. Results : After Korean medical treatment, the periungual warts of all patients were disappeared remarkably and there were no adverse effects such as nail dystropy, nail loss and no recurrence at all. Conclusions : This study shows that Korean Medicine such as herbal medicine, acupuncture and moxibustion was effective to treat periungual warts.

더러브렛 말에서 부비동염의 외과적 치료 (Surgical Treatment of Paranasal Sinusitis in Thoroughbred Horse)

  • A 11-year-old gelding Thoroughbred horse was presented with a history of unilateral nasal discharge of right side. The horse was diagnosed as maxillary sinusitis based on blood examination and radiography. The horse was treated with bone flap of right maxillary sinuses. Abscess of maxillary sinus was drained and irrigated with antibiotic-saline and povidone iodine solution. After surgical procedure, nebulization and medication applied by the result of antibiotic sensitivity test. After treatment for two weeks, the patient was recovered completely.

한의 의료기관 비급여 진료 실태조사 (A Survey on Uncovered Services in National Health Insurance of Traditional Korean Medicine Institution)

  • Objectives : This study was conducted to survey on uncovered services in National Health Insurance(NHI) of Traditional Korean Medicine(TKM) Institution. Methods : For TKM doctors working in hospital, it was surveyed to professors working in university-affiliated hospital. A total of 40 professors were participated online survey and all of them was included. For TKM doctors working in clinic, an e-mail survey was conducted for members of the association of Korean medicine. A total of 436 TKM doctors, 279 TKM doctors were included study and 157 were excluded because of duplication clinic or not working in clinic. It was conducted general status and uncovered services in NHI status. Results : The proportion of uncovered services in NHI was 54.7% for hospitals, 39.0% for clinics and there was a significant difference between hospital and clinic. Decoction and herbal(bee venom) acupuncture were most commonly used in both institutions. For decoction treated patients, It was commonly treated Sibjeondaebo-tang, Bojungykki-tang, Gwibi-Tang and patients chief complaints was thoraco-lumbar pain, functional dyspepsia, fatigue. Conclusions : It is necessary to expansion of benefits range of the NHI on TKM services.

일개 보건소의 양약-한약 병용투여 현황 보고 : 2019년 의무기록에 대한 후향적 분석 (Current Status of Combination Western and Korean medicine in Public Health Center in a Rural Area)

  • Objectives : This study is retrospective chart review research on the combined-prescription of Western and Korean medicine in Public Health Center in rural area. Methods : Researchers reviewed medical records of patients who was prescribed Korean medicine and Western medicine from 1st, Jan, 2019 to 31st, Dec, 2019 in Public Health Center. 50 patients' medical records were included. Results : Total number of treatment is 3,808 cases and 1.3% of them is prescribed Korean medicine and Western medicine simultaneously. Prescription of Korean medicine is 153 cases and Western medicine is 160. Jowiseunggi-tang and Diroba tablet were the most frequently prescribed Korean medicine and Western medicine. Furthermore, Korean Medicine Doctors of Public Health Center use muscular skeletal disease system code(M code among KCD code) for prescription and Western Medicine Doctors use diseases of the circulatory system(I code among KCD code) frequently. Conclusions : We analyze 50 patients who were prescribed both Korean medicine and Western medicine. Mostly, patients were prescribed medicine for different diseases in each clinic. In western medicine clinic, drugs about circulatory or endocrine disease were prescribed frequently and in Korean Medicine clinic, drugs about muscular skeletal disease were most frequently prescribed. This result imply the real world's combination of prescription status that was different from result of National health insurance corporation database. Senior patients in rural area take medicine long period and have various underlying disease. We call for some attention about senior and rural area patients' prescription status in interaction studies of Korean medicine and Western medicine.
