Effect of High heel on Lumbar and sacral curve (하이힐이 요부 및 천골경사각에 미치는 영향)

  • Purpose : This study is to know how position change in high-heels affects sacral tilt angle. 15 healthy women aged 21.87(standard deviation=3.54) were tested. Method : Lumbar and sacral tilt angle was measured by radiography barefooted, and after 15 mins of application time, they were measured in the same way in high-heels. Result : There was not notable difference in lumbar lordosis, lumbosacral angle, and sacral tilt angle. However, there was differences in change degree, which was measured by estimation data of [post-pre)/pre]${\times}$100. Conclusion : There was no notable difference in lumbar lordosis, lumbosacral angle, and sacral tilt angle, but as there were differences in change degree, research about how women's lumbar change when heel height increases is needed.

Effects of Lumbar Stabilization Exercise on Motor Neuron Excitability and Pain in Patients with Lumbar Disc Herniation

  • Background: Lumbar disc herniation (LDH) causes neurological symptoms by compression of the dura mater and nerve roots. Due to the changed in proprioception inputs that can result in abnormal postural pattern, delayed reaction time, and changed in deep tendon reflex. Objective: To investigate the effects of lumbar stabilization exercises on motor neuron excitability and neurological symptoms in patients with LDH. Design: Randomized Controlled Trial (single blind) Methods: Thirty patients with LDH were recruited; they were randomly divided into the balance center stabilization resistance exercise group (n=15) and the Nordic walking group (n=15). Each group underwent their corresponding 20-minute intervention once a day, four times a week, for four weeks. Participants' motor neuron excitability and low back pain were assessed before and after the four-week intervention. Results: There were significant differences in all variables within each group (p<.05). There were significant differences between the experimental and control groups in the changes of upper motor neuron excitability and pain (p<.05), but not in the changes of lower motor neuron excitability and Korean Oswestry Disability Index. Conclusion: Lumbar stabilization exercises utilizing concurrent contraction of deep and superficial muscles improved low back function in patients with LDH by lowering upper motor neuron excitability than compared to exercises actively moving the limbs. Lumbar stabilization exercises without pain have a positive impact on improving motor neuron excitability.

  • Objectives This study is planned to classify effectiveness of Korean medicine treatment on lumbar intervertebral Disc herniation patients' fear-avoidance belief. Methods We measured 84 patients with lumbar intervertebral disc herniation in Jaseng Korean Medicine Hospital. We used numeric rating scale, oswestry disability index, fear-avoidance belief questionaire for evaluating effectiveness of oriental medicine treatment on lumbar intervertebral disc herniation. And We used Magentic Resonance Imaging(MRI) for classifying the patient who has lumbar intervertebral disc or not. Results 1. There was statistical effectiveness of korean medicine treatment on lumbar intervertebral disc herniation patients' fear-avoidance belief (p<0.01). ${\Delta}NRS$ and ${\Delta}ODI$ (r=0.587) was Most statistically correlative. ${\Delta}ODI$ and ${\Delta}FABQ$ had a little of statistically correlation (r=0.313). ${\Delta}NRS$ and ${\Delta}FABQ$ has no statistically correlation (r=0.145). Conclusions This study suggests that screening for the korean medicine treatment reduces fears and negative belief of patients with the lumbar disc herniation and prevents chronic physical symptoms that come from them so that it can be a great help to controlling patients' pain clinically.

Analysis of Medical and Korean Medical Services Utilization after Lumbar Surgery Patients: Using Health Insurance Review and Assessment Service's Patients Sample Data (요추수술 후 환자의 의과 및 한의과 의료기관 이용 행태 분석: 건강보험심사평가원 표본데이터를 이용하여)

  • Objectives We are going to analyze patient's medical and Korean medicine use trends after lumbar surgery, and examine the percentage of use of Korean medicine after surgery and its relevance to the medical care outcome after lumbar surgery. Methods Using 3% patients' sample data of the Health Insurance Review and Assessment Service, two groups were compared the treatment progress of the Korean Medicine treatment group and the untreated group after lumbar surgery by hierarchical logistic regression analysis. After hierarchical logistic regression analysis(including propensity scores), two groups were compared after lumbar surgery, the Korean Medicine treatment group within 50days and untreated group within 50days. Results Lumbar surgery was performed in 2750 patients in 2015. It was 3.72 that the risk(odds ratio) of finished treatment of patients treated without Korean Medicine, compared to patients with Korean Medicine. It was 0.12 that the risk of continuing treatment(odds ratio) of patients treated with Korean Medicine within 50 days, compared to patients treatment more than 50 days. Conclusions The ratio of Korean Medicine treatment after lumbar surgery was 14.8%. The group that did not have Korean Medicine showed a higher possibility of treatment termination than the group who did not. Among the groups treated with Korean Medicine, the early treatment group was more likely to end treatment than the late treatment group. Considering various situations in the medical environment, further studies such as prospective studies and long-term data analysis are considered to be necessary.

  • Background: We developed a novel integrative lumbar stabilization technique that combines lumbar extension (LE) exercise with abdominal drawing-in maneuver (ADIM) to ameliorate low back pain (LBP) associated with neuromuscular imbalance and instability, based on the collective evidence of contemporary spinal rehabilitation. Objects: The specific aim of the present study was to investigate the effects of LE exercise with and without ADIM on core muscle strength, lumbar spinal instability, and pain, as well as functional characteristics in individuals with LBP using advanced radiographic imaging techniques. Methods: patients with mechanical LBP (N = 40, 6 males; $35.1{\pm}7.6years$) were recruited and randomly assigned either to the combined LE and ADIM (experimental group) or the LE alone (control group). Outcome measures included the visual analog scale, the modified Oswestry Disability Index, muscle strength imbalance (MSI), and radiographic imaging. The lumbar intervertebral displacement (LID), intervertebral (IV) and total lumbar extension (TLE) angles were calculated to evaluate the lumbar segmental instability. Results: The experimental group showed significant differences in the L3-L4, L5-S1 LIDs, L4-L5 and L5-S1 IV angles, and TLE angle as compared to the controls (p<.05). Immediate pain reduction and muscle strength imbalance ratio were significantly different between the groups (p<.05). Conclusion: These results suggest that the addition of ADIM significantly increased lumbar spinal stabilization in individuals with LBP, thereby reducing pain associated with functional lumbar flexion during daily activities.

Clinical Analysis of Anterior Lumbar Interbody Fusion for Failed Back Surgery Syndrome (Failed Back Surgery Syndrome에서 전방 요추체간 유합술의 치료성적분석)

  • Objective : To evaluate the role of anterior lumbar interbody fusion in treatment of failed back surgery syndrome, the authors retrospectively analyzed the result of anterior lumbar interbody fusion performed in our institute. Methods : Fifteen FBSS patients due to variable causes have been treated with anterior lumbar interbody fusion in our institute from April 1994 to June 1999. We analyzed clinical changes in 15 patients who were followed up for an average of 23 months. Results : The etiologies of FBSS were post operative discitis(6 cases), post operative instability(3 cases), post operative adhesion(5 cases), and recurrence(1 case). These fifteen FBSS patients were treated with anterior lumbar interbody fusion. The overall treatment outcome was satisfactory(excellent and good) in 11 cases. Three patients were slightly improved, but post operative low back pain was remained. One patient who had underwent nerve root injury due to pedicle screw insertion showed no improvement. Conclusion : We conclude that the anterior lumbar interbody fusion for FBSS seems to be safe and favorable treatment in selective patients, because low incidence of nerve injury risk and post-operative infection.

  • Purpose : The purpose of this study was to examine the effects of the trunk stabilization exercise in the musical tempo on lumbar lordosis angle, muscle activity and pain. Methods : For the 30 people with lumbar lordosis angle legion and back pain, a random selection was made with MLSE (15) and LSE (15) to measure VAS, lumbar lordosis angle and Muscle Activity. Result : There were significant decreases in intra group comparisons to lumbar lordosis angle were seen in MLSE and LSE groups, and significant decreases in inter group comparisons in MLSE groups. significant decreases in intra group comparisons to VAS were seen in MLSE and LSE groups, and significant decreases in inter group comparisons in MLSE groups. Significant intra-group comparison of muscle activity, MLSE groups increases were rectus obdominis(right/left) and erector spinae muscle(right/left), LSE groups increases were erector spinae muscle(right/left), and significant increases in inter group comparisons rectus obdominis(right) and erector spinae muscle(left) in MLSE groups Conclusion : Based on the above findings, a program to restore the lumbar lordosis angle, and increase muscle strength should be developed at by applying the combine existing trunk stabilization physical therapy technique and musical tempo.

The Effects of Lumbar Stabilization Exercise for Spinal Function in Patients with Low Back Pain (요부 안정화운동이 요통환자의 요추부 기능개선에 미치는 영향)

  • The purpose of this study was to investigate the effects of lumbar stabilization exercise therapy on low back pain patients' lumbar spinal function. Identify the effect of stabilization exercise therapy, this study attempted to determine lumbar spinal functions, using spinoscopy, for 20 patients with low back pain This study applied lumbar stabilization exercise to 20 low back pain patients without a control group for 8 weeks and 4 times a week, and examined their spinal functions before and after the application. Data collected from the test were analyzed using Wilcoxon signed ranked test, a nonparametric test. Absolute index, functionality and performance increased significantly compared to them before treatment. FE loads and velocity control while conducting exercise tasks increased significantly compared to them before treatment. Test item ROL and ROM, which indicate the change of angle, both showed significant differences. Of stiff spine, stiff pelvic score and sprain score, which indicate the effects of the conduct of exercise tasks on the movement of the spine, stiff spine score and sprain score showed significant differences. According to the results as presented above, lumbar stabilization exercise may be greatly helpful in improving low back pain patients' lumbar spinal functions.

  • The purpose of this study were to measured peak torque of lumbar extensor at various degrees and to compare with the choronic low back pain patients and healthy subjects back extensor peak torque. Thirty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<.05). 4. Forty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<;.05). 5. Fifty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<.05). 6. Sixty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<.05).

The effects of Hamstring Stretching Exercise on Angle of Lumbar Flexion and Pain Reduction with Chronic Low Back Patients (슬괵근 스트레칭운동이 만성요통 환자의 요부 굴곡각도와 통증감소에 미치는 효과)

  • The purpose of this research is to study how the stretching exercise along with heat and electric therapy can effect on Angle of Lumbar Flexion and Reduction of Pain against the Hamstring Construction Symptom that can be found in the Chronic Low Back Patients. The target of research was 28 person selected out of the patients diagnosed as Lumbar Sprain and Lumbar Syndrome, who have positive from the active test of Hamstring construction and passive test. Their age range is 20 to 60 years old, 14 male and 14 female. As a research method, simple heat and electric therapy was applied to counter part while Hamstring Stretching Exercise including simple heat and electric therapy was applied to the experimental part. After 6 weeks treatment for each part, the results of effect on Angle of Lumbar Flexion and Pain Reduction was compared and analysed on the basis of the gravity angle meter and Visual Analogue Scale (VAS). The outcome of research is as follows: 1. Angle of Lumbar Flextion increased effectively not in the conventional counter group but the experimental group to which Hamstring Stretching was applied simultaneously. 2. remarkable effect was shown in the experimental part applied Hamstring Stretching. Consequently from the results of this study, we could find out the fact that the therapy combined preserve treatment and hamstring stretching not only produced increasing Lumbar Flexion Angle but also was more effective on reducing pain of the patients according to it than the conventional or preserve therapy.
