Prevalence of osteoporosis according to nutrient and food group intake levels in Korean postmenopausal women: using the 2010 Korea National Health and Nutrition Examination Survey Data

  • BACKGROUND/OBJECTIVES: Women's bone health status is closely related with environmental factors and lifestyle factors. The purpose of this study is to evaluate the dietary risks of osteoporosis and osteopenia for Korean postmenopausal women. SUBJECTS/METHODS: The data from 1,433 subjects from the 2010 KNHANES were used and divided into three groups: normal, osteopenia, and osteoporosis group using bone mineral density (BMD). Nutrient intakes and food intake frequency were evaluated. Logistic regression analysis was applied to determine the odds ratios for osteoporosis and osteopenia. RESULTS: The RNI percentage of each nutrient and food intake frequency from 12 food groups decreased as bone mineral density status deteriorated. Risk for osteoporosis of low calcium (Ca) intake, under the EAR, showed an odds ratio of 2.13(95% CI; 1.26-3.61, P < 0.05). Higher intake frequency showed preventive effect from osteoporosis compared to lower intake frequency in such food group as dairy products (ORs 0.40, CI 0.21-0.75), beans (ORs 0.49, CI 0.29-0.83), seaweeds (ORs 0.55, CI 0.32-0.94), fish (ORs 0.56, CI 0.32-0.98), and fruits (ORs 0.42, CI 0.23-0.79) after adjusting for age. CONCLUSION: To prevent osteoporosis in later life, sufficient Ca intake and more frequent intakes of foods containing Ca such as dairy products, beans, fish, seaweeds, and fruits, which help in Ca absorption, should be stressed for Korean postmenopausal women.

Changes in the prevalence of anemia in Korean adolescents, 1998-2018

  • Background: Anemia is an important health problem affecting approximately 25% of the global population. Although its prevalence is decreasing worldwide, few studies have examined the prevalence of anemia in Korean adolescents. Purpose: This study aimed to determine the prevalence of anemia in Korean adolescents over the past 21 years using data from the Korea National Health and Nutrition Examination Survey. Methods: We investigated the prevalence of anemia in adolescents aged 10-18 years for the period 1998-2018 according to sex, age, residential area, and household income. The effects of menarche age and menstruation were examined in female adolescents for the period 2001-2018. Results: Among the total 11,782 participants, the weighted prevalence of anemia was 4.0%. The prevalence of anemia in male participants significantly decreased (from 3.0% to 0.5%, P<0.001), whereas that in female participants did not change significantly over time (from 7.9% to 8.5%, P=0.054). The average age at menarche was 12.4±0.0 years; the prevalence of anemia in females increased from age 13 years onward. Multivariate analysis revealed that anemia was more common in female than male participants (odds ratio [OR], 9.88; 95% confidence interval [CI], 7.19-13.57; P<0.001). In female adolescents, the prevalence of anemia increased with age (OR, 1.26; 95% CI, 1.15-1.38, P<0.001) and was 3.6 times higher after than before menarche (OR, 3.65; 95% CI, 2.21-6.05, P<0.001). Conclusion: Over the 21-year study period, the prevalence of anemia decreased among male adolescents but did not change in female adolescents. In female adolescents, age and menarche were identified as significant risk factors that require continued attention.

Correlation between Total Sleep Time and Weekend Catch-up Sleep and Obesity based on Body Mass Index : A nationwide cohort study in Korea

  • Purpose : Obesity is a major public health burden in developed countries and a well-known risk factor for cardiovascular disease. Short sleep duration is associated with obesity, as well as diabetes, heart disease and death. In modern society, habitual sleep restrictions seem unavoidable due to social obligations and work schedules along with a tendency toward decreased sleep time. Therefore, the purpose of this study is to examine the effect of differences in sleep time between weekdays and weekends on body mass index (BMI). Methods : This study involved 4,234 Korean adults aged 20 to 64 years based on data obtained from the 7th national health and nutrition examination survey (2016). All subjects were classified into the weekend catch-up sleep group (weekend CUS group). and the non catch-up sleep group (non-CUS group). Results : The longer the average sleep time, the lower was the BMI, and the larger the difference in sleep time between weekdays and weekends, the lower was the BMI. Compared with those with an average sleep time of 8 hours or more, obesity was 1.6-fold higher when the average sleep time was less than 6 hours, and 1.2-fold higher in the case of sleep time of 7 hours or more and less than 8 hours. When the difference in sleep time between weekdays and weekends was 0 or less, more than 0 hours but less than 1 hour, and more than 1 hour and less than 2 hours, the risk of obesity was 1.2-fold, 1.1-fold and 1.1-fold higher, respectively, compared with the risk associated with a sleep time difference of 2 hours or greater between weekdays and weekends. However, the difference was not statistically significant. Conclusion : Short sleep duration is positively associated with obesity. In addition, weekend catch-up sleep affects BMI.

30~49세 한국 성인의 당뇨병 유병 및 당뇨병 인지 관련 요인 (Factors Associated with Diabetes Prevalence and Diabetes Awareness in Korean Adults Aged 30-49)

  • 본 연구는 제8기 국민건강영양조사 자료를 이차 분석한 서술적 조사연구이다. 한국의 30~49세 성인 중 당뇨병 유병자 총 300명의 당뇨병 유병률과 인지율을 분석하고 이에 영향을 미치는 요인을 확인하고자 시도되었다. 자료 분석은 SPSS 25.0 program을 이용하여 복합표본 빈도와 백분율, χ2-test, 로지스틱 회귀분석을 실시하였다. 당뇨병 유병자 중 당뇨병임을 인지한 자는 171명(57.0%)이었으며 인지에 영향을 미치는 요인은 성별, 교육수준, BMI, 주관적 건강상태, 심혈관질환, 부계 및 모계 가족력이었다. 본 연구의 결과를 토대로 비교적 젊은 30~49세 당뇨 유병률을 감소시키고 유병자들의 인지율을 높이는 정책 및 중재 프로그램을 수립하는데 기초자료로 활용될 수 있을 것이다.

Food sources of zinc and nutritional status with usual dietary zinc intake in Korean toddlers and preschool children

  • BACKGROUND/OBJECTIVES: This study aimed to evaluate the food sources of zinc and the usual intake of dietary zinc among Korean toddlers and preschool children. SUBJECTS/METHODS: A total of 2,679 children aged 1-5 years was selected from the 2009-2013 Korea National Health and Nutrition Examination Survey (KNHANES) data. Dietary data collected from a single 24-h recall were used to evaluate the food sources of zinc. To estimate usual zinc intake, the distribution obtained from single 24-h recall data in the total sample was adjusted using the ratio of within-to-between-person variance in zinc intake obtained from 2-day 24-h recall sub-sample data of the 2009 KNHANES. The proportion of children with usual zinc intake below the estimated average requirement (EAR) and above the tolerable upper intake level (UL) was assessed. RESULTS: The main sources of zinc in Korean children were grains, dairy products, and meat. The mean usual intakes of zinc among all individuals, those aged 1-2 yrs, and those aged 3-5 yrs were 5.50, 5.01, and 5.83 mg/d, respectively. In all participants, 1.1% of the children consumed zinc below the EAR, whereas 10.7% exceeded the UL. The proportion of children with excessive zinc intake was 25.6% in the 1-2 yrs age group and 0.6% in the 3-5 yrs age group. CONCLUSIONS: According to the current UL, the risk of excessive zinc intake appears to be high among Korean toddlers. Future studies that monitor the health effects of excessive zinc intake are needed to appropriately guide zinc intake in children.

도시 및 농어촌 거주 노인의 초가공식품 섭취 상태와 당뇨 및 공복혈당장애에 대한 단면연구 (Association of ultra-processed food with diabetes and impaired fasting glucose in elderly populations (urban and rural): a cross-sectional study)

  • Objectives: This study examined the association between ultra-processed food (UPF) consumption and chronic diseases in elderly Koreans. Methods: Data from the 2019-2021 Korea National Health and Nutrition Examination Survey were analyzed. Dietary intake and UPF consumption were assessed using the NOVA food classification based on 24-hour recall data from 3,790 participants (aged 65+ years). Participants were divided into 4 groups based on the quartile of energy intake from UPFs. Regions were classified as urban or rural. Multivariable logistic regression was employed to estimate the adjusted odds ratios (AORs) with 95% confidence intervals (CIs) after controlling for potential confounders. Results: Among the participants, 71.3% resided in urban and 28.7% in rural areas. Compared to the urban elderly, rural participants tended to be older, have lower education and income levels, be more likely to live in single-person households, and have a higher smoking rate (P < 0.05). Urban elderly consumed more UPFs daily (146.1 g) compared to rural residents (126.6 g; P < 0.05). "Sugar-sweetened beverages" were the most consumed category in both regions. "Sweetened milk and its products" and "traditional sauces" were prominent in urban areas, while rural elderly consumed more "traditional sauces" and "distilled alcoholic beverages." Rural areas also had a higher carbohydrate-to-calorie ratio than urban areas. Compared to the lowest quartile of UPF intake, the highest quartile was significantly associated with impaired fasting glucose only in rural areas (AOR, 1.48; 95% CI, 1.00-2.19; P for trend = 0.0014). No significant associations were observed for diabetes in either urban or rural areas. Conclusions: This study suggests that high intake of UPFs is associated with increased odds of impaired fasting glucose in rural elderly. Further research is needed to elucidate the specific negative health effects of UPFs in different populations, and targeted efforts should promote healthy diets in both urban and rural areas.

한국 성인 인구집단의 정신건강 상담과 우울증 유병과의 관계 (Relationship between Mental Health Counseling and Depression Prevalence in the Korean Adult Population)

  • 본 연구는 한국 성인 인구집단의 지난 1년간 정신건강 상담 이행 여부와 우울증 현재 유병 여부와의 관계를 확인하기 위한 2차 자료 분석연구이다. 연구자료는 질병관리청에서 수행한 국민건강영양조사 제 7기 3차 연도(2018년) 원시자료로부터 추출되었다. 연구대상자는 2018년 1월부터 12월까지 수행된 국민건강영양조사에 참여한 대상자 중 만 19세 이상의 성인 인구 6,238명이었다. 자료 분석은 SPSS/WIN 27.0 version 프로그램을 이용하였다. 일반적 특성은 복합표본 빈도 분석을, 일반적 특성과 지난 1년간 정신건강 상담 이행 여부와의 관련성, 일반적 특성과 우울증 현재 유병 여부와의 관련성, 지난 1년간 정신건강 상담과 우울증 현재 유병과의 관련성 등은 복합표본 로지스틱 회귀분석으로 분석하였다. 지난 1년간 정신건강 상담 이행 여부는 성별, 결혼상태, 경제활동 여부, 소득수준 등과 유의한 연관성이 있었다(p<0.05). 우울증 현재 유병 여부는 연령 변수와 유의한 연관성이 있었다(p<0.0001). 지난 1년간 정신건강 상담을 이행한 대상자가 그렇지 않은 대상자에 비해 우울증 현재 유병의 오즈비가 5.09배(OR, 5.085; p<0.0001)가 더 낮게 나타났다. 이러한 결과는 한국 성인인구집단의 정신건강에 관한 취약한 요인을 예측함으로써 우울증 예방 및 관리를 위한 기초자료로 활용될 수 있을 것이다.

한국 노인의 노쇠와 지역박탈지수의 관련성 연구 (The Relationship between Frailty and Area Deprivation Index among Older Adults in South Korea)

  • 연구배경: 본 연구는 지역박탈지수(Area Deprivation Index, ADI)와 우리나라 노인의 노쇠와의 관련성을 분석하는 것을 목적으로 하였다. 방법: 2014년부터 2019년까지 국민건강영양조사 자료 중 65세 이상 노인을 대상으로하였다. 노쇠는 Fried phenotype을 기준으로 평가하였다. ADI와 노쇠의 관련성을 검증하기 위하여 다중 로지스틱 회귀분석을 실시하였다. 결과: 연구대상자 9,825명 중 ADI가 낮은 지역의 노인보다 높은 지역의 노인이 1.23배 더 (odds ratio [OR], 1.23; 95% confidence interval [CI], 1.09-1.37). 기혼자의 경우 ADI가 낮은 지역에 거주하는 노인보다 높은 지역에 거주하는 노인이 1.35배(OR, 1.35; 95% CI, 1.16-1.57) 더 노쇠한 것으로 나타났다. 결론: 본 연구는 65세 이상의 노인을 대상으로 ADI와 노쇠의 관련성을 파악하였다. 특히 기혼자의 경우 ADI가 높은 지역에 거주할 수록 더 노쇠한 것으로 나타났다. 따라서 건강불평등 해소를 위해 박탈지수가 높은 지역과 낮은 지역 간 격차를 감소시키기 위한 정책적 개입의 필요성을 제언한다.

국가표준식품성분표 제9개정판의 구축 과정 및 주요 특징 (Development of 9th Revision Korean Food Composition Table and Its Major Changes)

  • Objectives: The Korean Food Composition Table (KFCT) was first published in 1970, and has since been updated every five years by the Rural Development Administration (RDA). This study was conducted to introduce the development strategies, features, and challenges of the $9^{th}$ revision of the KFCT. Methods: Due to the increasing demands of nutrient database users and generators, the RDA started a new research project in 2013 to improve the quantity and quality of data for the $9^{th}$ revision of the KFCT. Over 1,000 food items frequently consumed in Korea were selected as key foods using the results of the Korean National Health and Nutrition Examination Survey. About 200 raw materials and processed food items were collected and analyzed every year. Target nutrients that were analyzed by collaborative labs, such as, sugars, selenium, iodine, and biotin, were increased from 22 to 43. Analytical sample handling procedures and data quality evaluation systems were also established in collaboration with 10 contracted labs. Data were evaluated for data quality according to the FAO/INFOODS, CODEX, and AOAC guidelines. Results: The $9^{th}$ revision contains data on 3,000 food items and up to 43 and 140 food nutrients for the printed table and the excel database file, respectively. Overall, 1,485 data items were newly added, 973 of which were provided by the RDA and 512 were cited from foreign nutrient databases. The remaining 1,515 food items were maintained as in the $8^{th}$ revision. Conclusions: The KFCT provides the basic infrastructure for food and nutrition policy, research, and dietary practice in South Korea. The use of the KFCT has increased exponentially in the past few years in both public and private sectors; accordingly, increased efforts should be paid to the preparation, improvement, and maintenance of KFCT.

채소류 섭취에 따른 연령별 잔류농약 노출평가 (Dietary Exposure Assessment by Ages due to Pesticide Residues in Vegetables)

  • 2008년 국민건강영양조사에서 서울시민의 연령별 채소섭취량을 산정하여 2007년부터 2009년까지 서울지역에서 유통 중인 채소류에 대한 잔류 농약을 모니터링 하여 채소류섭취에 따른 연령별 농약성분의 노출량을 평가하였다. 서울시민에 대해 연령별 평균 채소류 섭취량을 분석한 결과 통계적으로 유의한 차이가 있었다(p<0.001). 연령그룹 중 40~59세 그룹의 채소류 섭취량이 가장 높게 나타났고 5세 이하 그룹의 채소류 섭취량이 가장 낮게 나타났다. 2007년부터 2009년까지 채소류 섭취에 따른 연령별 잔류농약의 %ADI(Acceptable Daily Intake)는 모두 1 이하로 나타나 잔류농약으로 인한 위해 가능성은 낮을 것으로 판단된다.
