Development of a Home-based Nursing Intervention, Mothering Program for Low-Birth-Weight Infants (저체중출생아를 위한 가정간호형 모성역할중재 프로그램 개발과 그 효과에 대한 연구)

  • The purpose of this study was to develop a parenting intervention program and determine the efficacy of the program with low-birth weight infants and their mothers. Nine dyads for the experimental group and twelve dyads for the control group discharged from the Neonatal Intensive Care Unit of a University Hospital in Seoul were recruited for the study. For the intervention group, programmed education and support which focused on the maternal sensitivity of the infant's behavior. rearing environment. motherinfant interaction and infant care were given to each subject. Individual counseling and home visits were provided at discharge, one week after discharge. and one and three months of corrected age in every infant. Structured questionaires were administered and feeding interactions were videotaped and coded by a blinded certified observer. A Quasi-experimental design was conducted for this study. Postpartum depression, maternal self esteem. infant care burden, HOME. mother-infant interaction, and infant development were measured. Results were in favor of the intervention versus the control group. On the Beck depression inventory, intervention mothers showed decreasing trends in depressive symptom vs control mothers although, there were statistically no significant differences between the two groups at each time. The mean score of experimental group was 11.55(mild depression state) at discharge and became 8,6(normal state) at 1 month of corrected age. On the other hand, the mean score of the control group was 13.92(mild depression state) at discharge and became 14.0. Maternal self esteem in both groups improved over time. Infant care burden in both groups was also shown to increase over time. There was a significant difference between the two groups in HOME(p=.0340) at 3 months of corrected age. HOME scores of the experimental group and the control's were 31.10 and 25.58, respectively. Mothers' emotional and language responses were significantly high in the intervention group compared with the control group(p=.0155). Intervention group (53.33) showed a significantly high quality of motherinfant interaction compared with the in control group (42.80)(p =.0340). Intervention group mothers appeared have a better quality of mother-infant interaction behaviors. On the other hand, there was no statistical difference in the infant part between groups. Intervention group infants had higher trends in a general developmental quotient: although, there was no statistical difference between groups. The general developmental quotient of intervention infants was 102.56 and control's was 91.28. However, the developmental quotient of the domain of 'individuality-sociality' was higher in the intervention group infants compared with the control's(p=.0155). The concerns identified by parents revealed two domains of an infants' health management -knowledge and skills in caregiving of lowbirthweight-infants, characteristics of lowbirthweight infants, identifying a developmental milestone, coping with emergency situations and relaxation strategies of mothers from the infant care burden. Interview data with the mothers of low-birth weight infants can be used to develop intervention program contents. Limited intervention time and frequency due to time and cost limitations of this study should be modified. The intervention should be continuously implemented when low-birth weight infants become three years old. An NNNS demonstration appeared to be a very effective intervention for the mothers to improve the quality of mother-infant interactions. Therefore intervening in the mothers of low-birth weight infants as early after delivery as possible is desirable. This study has shown that home visit interventions are worthwhile for mothers only beyond the approach as an essential factor in ability of facilitating a growth fostering environment. In conclusion. the intervention program of this study was very effective in enhancing the parenting for the mothers of low-birth weight infants, resulting in health promotion of low-birth weight infants. The home-visit outreach intervention program of this study will contribute to the health delivery system in this country where there is a lack of continuous follow-up programs for low-birth weight infants after discharge from NICU, if it is activated as part of the home visit programs in community health systems.

Primipara's maternal Identity & Self Confidence for Caring the Baby During the Immediate Postpartum Period (산욕초기 초산모의 모성 정체성과 영아돌보기 활동 자신감에 관한 연구)

  • In this study the levels and influencing characteristics of maternal identity and self confidence for caring the baby were identified during the immediate postpartum period. 114 primiparous women who delivered vaginally normal baby participated in the survey from August 1 to October 31, 1996. SD scale was to measure maternal identity which consisted of 11 items for mother and 6 items for baby. Likert scale was to measure self confidence for caring the baby (38 items). Cronbach's alphas for evaluating internal consistency as follows : .86 for maternal identity and .96 for self confidence scale for caring the baby. The study showed these results : 1. Mean score of maternal identity(82.03 : 52.65 for mother, 29.38 for baby) was considered relatively low. 2. Mean score of self confidence for caring the baby(113.91) was considered relatively low. 3. There were significant influencing characteristics of primiparous to maternal identity : age(total ; F=3.53, p=.0329, for mother ; F=2.60, p=.0719, for baby ; F=3.12, p=.0481), prenatal infant care preparation(total ; t=2.31, p=.0306, for mother ; F=2.62, p=.0160), knowledge about infant care during the hospitalization(for baby ; F=3.94, p=.02222), colostrum feeding(total ; t=1.95, p=.0541, for baby ; t=2.71, p=.0080), frequency of breast feeding during the hospitalization(for baby ; F=3.91, p=.0228)and feeding type after discharge(for baby ; F=3.18, p=.0456). 4. There were significant influencing characteristics of primiparous to self confidence for caring the baby : routine husband support(F=6.09, p=.0031), prenatal infant care preparation(t=2.04, p=.0574), knowledge about infant care during the hospitalization(F=3.15, p=.0467), education of breast feeding during the hospitalization(t=-1.79, p=.0850). 5. Correlation between maternal identity and self confidence for caring the baby was r=.37608(p=.0001). This study implies that special education programs for primipara are needed. For the future, this study suggest that maternal role variables need to be monitored through the home visiting follow up. Also intervention programs related to prenatal care, discharge education, home visiting follow up need to be developed and then evaluated their effectiveness.

A Study on the Mothers' Knowledge and Practice about the Infant Weaning Diet (영유아 어머니의 이유식에 대한 지식정도 및 실태조사)

  • The main purpose of this study was to present the basic materials that help the nursing of the infants and consultation of childcare by examinating the knowledge and practice of the mothers about the weaning diet. The data for this study was collected from 205 mothers with infants(6-36 months) who visited two hospitals or one university hospital in Seoul, who were living in one apartment Suwon area between July. 7. and September 10, 2000. The 57 questionnaires were used for collecting the data and SAS program was used to analyse the data. The results are as follows. 1. The items of low score in the result of the knowledge data were as follows. [Initial weaning time, the important nutritional problem in infant, considering factor during feeding, the relation of proper weaning food & age, the time of used by cup, to apply of proper spicies in weaning food, the time of completing weaning, the time of stopping weaning at abnormal sign, and the inappopriate reason of dry mixed powdered food (Sunsik) as a weaning food.] 2. Higher educated mothers presented more high score than lower educated mothers in the knowledge about the weaning. (p<0.05) 3. Before the weaning, more mothers were using the commercial milk (51.2%) than the breast milk (13.2%). 4. Mothers get the knowledge about the weaning from the infants care book of cook books(26.4%). 68% of mothers had not received any consulting service with regard to the weaning, 10.2% of them consulted Pediatrician about the weaning and 0.5% of them with nurse. 5. Most mothers began supplymentary food, from four to six months (65.4%). the ratio was about the same as the ratio of mothers who knEw the appropriate time for the introduction of supplymentary food(83.2%) 6. The main solid food was commercial food, not home-prepared food. 7. Higher educated mothers used home-prepared weaning food more often than lower educated mothers(P<0.05). 8. The used rate by spoon was 57.4%, but the rate of mother's knowledge data was 95.0%. 9. The time of completing the weaning was in 12-18 months(53.8%). It is similar to the mother's knowledge data(52.7%) Conclusion ; According to knowledge data, it turns out that most mothers know moderatly about the weaning (everage 60/100). However their knowledge and practice on the methods & procedures on the weaning were inadquate. Especially, most mothers did not consult with those qualification about the weaning. Therefore, the role of the nurse as a provider of information on the weaning should be emphasized with varied educational programs in many health care center.

Correlation of Childcare Teachers' Knowledge of Infectious Diseases, Health Locus of Control, Self-Efficacy, and Practice of Disease Prevention during the COVID-19 Pandemic (코로나 19 팬데믹 상황에서 보육교사의 감염병 지식, 건강통제소재, 자기효능감과 감염병 예방행위 실천의 관계)

  • Objective: This study aims to provide empirical data on epidemic management measures and childcare teacher education in centers. It examines the relationship between infectious disease knowledge, health control knowledge, and self-efficacy in relation to the practice of preventive behaviors among infant and toddler teachers during the COVID-19 pandemic. Methods: The study involved 300 teachers from Seoul and Gyeonggi Province. Data were collected through questionnaires and analyzed using SPSS 23.0. Results: The study found that there is a positive relationship between childcare teachers' knowledge of infectious diseases and their engagement in preventive behavior. Additionally, internal control among the health care factors of childcare teachers also showed a positive relationship. Furthermore, the study revealed that the self-efficacy of childcare teachers is positively associated with their practice of preventive behavior. Overall, infectious disease knowledge, internal control measures, and self-efficacy were all found to have positive relationships with preventive behavior. Conclusion/Implications: This study proposes a new direction for future teacher education by highlighting the effectiveness of psychological factors, specifically health control materials and self-efficacy, in addition to focusing on improving infectious disease knowledge.

Effects of a Newborn Care Education Program using infant model (아기모형을 활용한 신생아 돌봄 교육 프로그램 적용 효과)

  • The aims of this study were to examine the effects of a newborn care program (NCP) using an infant model. The subjects of this study were 48 mothers in the G city postpartum center. The NCP educational program was provided orally and via demonstration three times in two weeks. Pre and post questionnaires were given and the data was collected in July 2013 to September 2013. The findings suggest that knowledge of the newborn levels were increased significantly after the NCP (Mean=2.27 vs. 2.81, t=-7.049, p<.001). Parenting efficacy levels were increased significantly after the NCP (Mean=2.69 vs 2.89, t=-5.545, p<.001). The satisfaction with the education levels was 4.12. These findings suggest that providing a structured NCP educational program will be useful for adapting and obtaining a new mother role by improving the level of parenting efficacy and newborn care knowledge.

Development of Copyright Education Program for the Infant and Child Teachers (영유아 교원을 위한 저작권 교육 프로그램 개발)

  • The purpose of this study is to develop a copyright education program for the directors and teachers in kindergartens and day care centers. In this study, these programs were developed based on the actual situation of knowledge, copyright education requirements, and professional opinions. In this way, a copyright education programs were designed. Through the program, the directors and teachers were trained on the subject. Then, the overall evaluation was carried out through an open questionnaire and in-depth interview. Through this evaluation, the program was modified. These programs were made up of education purposes, education objectives, education contents, education methods, operating methods, and education evaluation. Education content was intended to be educated through examples from kindergartens and day care centers.

A Pilot Study on Korean Version Development of the Rearing Knowledge and Practice for Infant Parents (부모의 양육지식과 실천 도구 개발 예비연구)

  • Rearing knowledge is a significant factor of relevance for rearing practice, and assessment of rearing knowledge and practice is important for infant parents. The author adopted the Child Rearing Knowledge Scale (CRKS) and Child Rearing Practice Scale (CRPS) that were developed and validated by Saramma & Thomas. According to the international linguistic validation process, pilot testing was done based on 20 infant's mothers in one Oketani massage center and one public health center. Descriptive statistic methods and Wilcoxon Rank Sum Test were employed to evaluate the level and compare the mean score according to general characteristics of subjects of the tools. Reliability and validity were tested by Cronbach's alpha and Spearman Correlations. The mean age of the subjects was 33.94 (${\pm}2.99$) years and the mean age of babies was 3.35 (${\pm}0.58$) months. The understanding level of the Korean version of the CRKS and CRPS was 1.80 (${\pm}0.65$) and 1.33 (${\pm}0.54$), respectively. In addition, the CRKS and CRPS were relatively easy to use. The mean score of the CRKS was 22.50 (${\pm}4.89$), which was a moderate score, while the mean score of the CRPS was 30.75 (${\pm}2.04$), which was high. The Cronbach's alpha values of the CRPS were as follows: feeding, 0.71; growth and development, 0.64; cleaning and protection 0.68; infant stimulation, 0.77. There was a significant correlation between infant stimulation of the CRPS and growth and development of the CRKS (r=0.530, p=0.016). The CRKS score of medical staff was significantly higher than that of non-medical staff (p=0.04). The CRKS and CRPS are expected to be used in clinical or community care practice as easy-to-use tools that are easy to respond to.

A Survey on the Experience of Accident of Infant, Toddler and Preschooler, and the Education about Emergency Care for Parents (영 유아 및 학령전 아동의 안전사고 및 어머니의 응급처치 정보 접촉 경험에 관한 조사 연구)

  • The purpose of this study was to investigate the experiences of accident and visiting emergency center, and the experiences of education about emergency care for parents. The subjects of this study were 117 mothers whose children are attending 9 different nursery schools or kindergartens. The instrument used in this study was developed by researchers and validated by nursing professor, first aid doctor and the president of nursery school, and consisted of 'most important thing in child rearing' ,'experiences of accident and visiting emergency center', 'type of trauma' and 'need for parent education program' the result of this study were as followed ; 1.There were 70.09% of the experience of visiting emergency center among the subjects. The cause of visiting emergency center were high fever and convulsion(42.86%), and trauma by accident(28.04%) 2. The most common type of the indoor accident were fall down and slipping(76.92%), the common type of trauma were contusion (76.92%), abrasion(47.01%), laceration(29.06%), dislocation or fracture(12.82%), burn(10.26%), piercing(8.55%). 3.47.01% of subjects had the experience of education about emergency care for parents. They were educated by child rearing journals (32.72%), small books which were published by hospitals or community health center (25.45%), mass media(21.8%), parent seminar (12.72%), nursery school or kindergarten (5.45%). In conclusion, the need of emergency care for children was strongly found. Therefore, on the basis of above results, the parent education program which fits their cognitive level and their health care needs So, it must be directed on the further study of parents' knowledge about emergency care for children. To enhance the effectiveness of program and accomplish the children's health promotion, advanced instructional media and demonstration must be included.

The Study on the Development and Effectiveness of Parent Role Education Program I -Focusing on the six-months results- (영아의 건강증진을 위한 부모역할훈련 프로그램의 효과 -생후 6개월의 결과를 중심으로-)

  • Recent research indicates that the new mothers want to learn about childrearing, and have burden in care of infants. Also it indicates that the new mothers say the lack of confidence and knowledge about infant care. This study was a prospective longitudinal reseach developing parent role education program and evaluating the effectiveness of this program for mother-infant interaction, childrearing environment and infant development. Longitudinal quasi-experimental design was used. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37weeks, and their mothers. The sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to December 14th in 1999. For the intervention group received programmed education that was consisted of discharge education, telephone counselling, and home visiting for maternal education. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program. This study was focused on the results of six months time point. The results were as follows: 1. There was significant difference in the mean score of mother-infant interaction(NCAST) between two groups. Intervention group showed higher scores in the subscales of sensitivity to cues, cognitive growth fostering, and caregiver total. 2. There was no significant difference in the mean score of child rearing environment (HOME) between two groups of six-month-infants. But when each subscale of HOME was examined, intervention group showed higher scores in the dimensions of opportunities for variety in daily stimulation, maternal involvement with child, and emotional, verbal response. 3. Six-month-infants of the intervention group showed significantly higher GQ in the Griffiths mental development scale. In conclusion, the maternal education program was effective in promoting the mother-infant interaction, organizing the childrearing environment, and fostering the infant development. These results were very meaningful that we found parent role education necessary for normal infants' mothers, and nurses can make a great contribution in promoting health of infants and mothers.

Information Sources and Knowledge on Infant Vaccination according to Online Communities (온라인 커뮤니티에 따른 영.유아 예방접종에 대한 정보습득 경로 및 지식수준 비교)

  • Objectives : To explore the information sources and knowledge on infant vaccinations of pro-vaccination community members and anti-vaccination community members on the internet. Methods : An online survey of 245 parents from three pro-vaccination communities and 92 parents from one anti-vaccination community was conducted from June 7 to June 23, 2006. Results : Parents from pro-vaccination communities usually gained the information regarding vaccination efficacy and risk mainly from healthcare providers (49.8%) and mass media (47.7%). Pro-vaccination community members considered healthcare providers as the most credible sources of information on vaccination, whereas the anti-vaccination community members usually gained their information regarding vaccine efficiency and risk from Internet child-care cafes and online vaccination communities. Parents of the anti-vaccination community considered the internet as the most credible information source (77.6% for efficacy, 94.8% for risk). In addition, the major reason why anti-vaccination community members didn't vaccinate and, will not vaccinate, was concern about possible side effects of the vaccine. The knowledge level on infant vaccination, education and economic status was higher in the anti-vaccination community. Conclusions : On-line communities concerned with vaccination are getting popular. The influence of anti-vaccination parents on the Internet is expected to be high. The government and healthcare providers need to increase their efforts to improve the credibility of information about vaccination. Our findings suggest that online communication regarding vaccinations needs to be considered as a means to increase vaccination rates.
