Isolation of kpn I restriction endonuclease from klebsiella pneumonia (Klebsiella pneumonia로 부터 제한효소 Kpn I의 분리)

  • A restriction endonuclease, Kpn I has been isolated from Klebsiella pneumonia. Cells were broken by sonication. After ultracentrifugation the supernatant containing Kpn I activity was further purified by Sepharose-6B gel filtration, DEAD-Cellulose, Heparin-Agarose, and Aminohexyl-Agarose column chromatography. Final enzyme preparation was essentially free of contamination exonuclease and phosphatase, as judged by ligation-recut test. Total activity of the enzyme recovered from 10 grams of cells was $4.7\times 10^5$ units.

  • Massive lung gangrene is a rare but very rapidly progressing fatal complication of lobar pneumonia. Etiologic agents are Klebsiella pneumoniae, Pneumococcus and Aspergillus, etc. Chest X-ray shows firm consolidation of the involved pulmonary lobe and bulging fissure due to the volume expansion of involved lung. CT-scan shows extensive lung parenchymal destructions with multiple small cavitary lesions. Recommended treatment is the early surgical intervention combined with antibiotics. Without surgical intervention, lung gangrene is known to progress toward sepsis, multiorgan failure, and high mortality. We report two cases of rapidly progressing massive lung gangrene by Klebsiella pneumonia treated by the resectional surgery.

Appearance of globule leukocytes in tracheal and broncheal mucosa in Korean native cattle with pneumonia and rats experimentally infected with Klebsiella pneumoniae (폐렴우와 Klebsiella pneumoniae 실험적 감염 랫드의 기관 및 기관지 점막층에서 Globule leukocyte의 출현양상)

  • The study was aimed at identifying the globule leukocytes (GL) of tracheal mucosa layer of Korean native cattle showing symptom of pneumonia which have died as enterotoxemia and normal Korean native cattle in Kyungpook local area. In another set of experiment, Isolated Klebsiella pneumoniae from suddenly died cattle specimens was subjected to rat for a determining globule leukocyte appearance by using histochemical and immunohistochemical method. In histochemical study, globule leukocytes generally was existed in all the case of postmortem of Korean native cattle and 3 heads of slaughtered cattle which showing symptoms of pneumonia and it showed significant increase in tracheal mucosa of rats experimentally infected with Klebsiella pneumoniae. These increased number of globule leukocytes was moderately remained on early infection stage and gradually decreased in timedependent manner after infection. The granule patterns were also determined as an acidmucopolysaccharide. In immunohistochemical study, serotonin intensity in the treacheal mucosaepithelial cells of rat experimentally infected observed a strong immunoreactivity during early infection and gradually decreased in dependent of infection stage while no IgE immunoreactivity observed. These data show that globule leukocytes were increased in a pneumonia, therefore it was considered as a valuable cell that was associated with early stage of inflammatory response.

  • Klebsiella pneumoniae causes that liver abscess mostly, also spread to pneumonia, meningitis, urinary tract infections. Septic arthritis caused by K. pneumoniae is a quite rare and has not been reported in Korea. Therefore, the authors report a case of the septic arthritis in the knee joint caused by K. pneumoniae in a patient with type 2 diabetes mellitus and osteoarthritis of the knee that successfully treated by early detection and arthroscopic synovectomy.

  • Ammonia is a useful substance which is widely used in various industries. It is generally released by the decomposition of agricultural wastes and known to have toxic effects on human beings. Due to the common usage, it is possible to cause water pollution through either direct or indirect leakage. Such cases, it is preferable to use the adsorption capacity of zeolite to rapidly remove ammonium ions, but it is not sufficiently removed by the adsorption only. In this paper, the removal efficiency of ammonium ion through both the adsorption capacities of commercial synthetic zeolites and the biological mechanism of microorganisms were compared. In addition, microorganisms were immobilized on the zeolite in order to enhance the removal efficiency by applying a chemo-biological process. As a result, the standard commercial zeolite showed 67~81% of the removal efficiency in 2~4 hours at a 100 ppm concentration of ammonium, whereas the selected microorganism Klebsiella pneumoniae subsp. Pneumoniae showed up to 97% within 8 hours. When the microorganism was immobilized on the zeolite, the highest removal efficiency of approximately 98.5% were observed within 8 hours.

  • This article presents the case of a bilateral chronic subdural hematoma which was contaminated with Klebsiella pneumoniae and resulted in a life-threatening central nervous system infection. After repeated of bilateral burr-hole drainage, the patient became hyperpyrexic and drowsy. Suppuration within the subdural space was suspected and then the patient underwent bilateral fronto-temporo-parietal craniotomies, and pus was evacuated. Its cultures revealed Klebsiella pneumoniae. Intravenous meropenem was given for 6 weeks. He recovered completely. Microorganisms like Klebsiella pneumoniae may directly infect the subdural space with iatrogenic contamination.

A Case of Superior Vena Cava Syndrome Caused by Klebsiella Pneumonia (폐렴간균 폐렴에 의해 유발된 상대정맥 증후군 1예)

  • Superior vena cava(SVC) syndrome is mostly related to a malignant process, but many different benign causes have also been described. We report a case of SVC syndrome caused by Klebsiella pneumonia diagnosed by sputum culture and serial chest X-ray changes. A 27-year-old man had been in stable health until three days before admission, when he complained of pleuritic chest pain, facial flushing, and shortness of breath. Examination of the head and neck disclosed edema of face and both arms, and jugular venous distention to the angle of the jaw. The chest auscultation revealed decreased breath sound without crack1e on right upper lung field. The chest roentgenogram showed homogenous air space consolidation on right upper lobe, asociated with downward displacement of minor fissure and contralateral displacement of trachea, but air bronchogram was not seen. We began antibiotic therapy under impression of pneumonia after available culture was taken from blood and sputum. SVC scintigraphy showed stasis of drain of right brachiocephalic vein at the proximal portion with reflux into the right internal jugular vein and faintly visible SVC via the collaterals. Sputum culture revealed Klebsiella pneumoniae. Antibiotic therapy resulted in a cure of infection and disappearance of facial swelling. Follow-up SVC scintigraphy after 20 days showed normal finding. We first report a case of SVC syndrome caused by klebsiella pneunonia.

  • Inspiratory pneumoniais one of the common complications in stroke patients who have dysphagia. Pneumonia due to Klebsiella bacteria shows higher fatality than other bacteria. For this reason, K. pneumoniae can be fatal to immune deficient patients. The purpose of this paper is to report the adapting pattern identification according to principles of defense, Gi, nutrients and blood on K. pneumoniae treatment. In this case, improvement was seen in a stroke patient with dysphagia diagnosed with K. pneumoniae by X-ray, sputum culture, and by using Cheonghobyulgaptang as a way of nourishing Eum and clearing heat, because in this case K. pneumoniae was presumed to cause Eumdeficiency due to the chronic low fever, cough, sputum and weakness. This paper proposes that adapting pattern identification according to principles of defense. Gi, nutrients and blood can increase the efficiency of K. pneumoniae treatment.

A Case of Massive Pulmonary Gangrene Complicated by Klebsiella Pneumonia (클렙시엘라 폐렴의 합병증으로 발생한 광범위 폐괴저)

  • Pulmonary gangrene is a rare and severe complication of bacterial pneumonia, where a pulmonary segment or lobe is sloughed due to parenchymal devitalization of the parenchyma, with secondary anaerobic infection and necrosis caused by pulmonary vascular thrombosis. Prior to the antibiotic era, massive pulmonary gangrene was potentially fatal. Herein, a case of pulmonary gangrene in a 67-year-old man is reported. He complained of fever, chills, dyspnea and purulent sputum of 5 days duration. The plain chest radiograph showed well-marginated right upper lobe consolidation, with bulging minor fissure, suggestive of a Klebsiella infection. A contrast CT scan demonstrated consolidation of the right upper lobe, with a central necrotizing portion. Klebsiella species was confirmed from both sputum and blood cultures. After appropriate antibiotics, the chest X-ray and CT scan 3 weeks later showed a large cavity with an air-fluid level, sloughing-off and extrusion of necrotic lung tissue, suggestive of pulmonary gangrene. Seven months later, the right gangrenous lung showed severe volume loss on a chest radiograph. The management of pulmonary gangrene has been somewhat controversial. Herein, it was managed without surgical drainage or resection. If the antibiotic therapy had failed, then a surgical approach would have been considered.

Acute Osteomyelitis of the Mandible by Extended-Spectrum β-Lactamase Producing Klebsiella Pneumoniae: A Case Report

  • Acute osteomyelitis caused by Klebsiella pneumoniae is rare in the oral and maxillofacial region. Klebsiella pneumoniae is a Gram-negative bacillus and the normal flora of the human body, but it can cause pneumonia, urinary tract infection, meningitis, and osteomyelitis in patient with compromised immune systems. These infections are mainly caused by nosocomial infection. Microbacterial osteomyelitis was developed by clinical cause such as tooth extraction, fracture, and surgical history, which requires long-term antibiotic administration and surgical treatment. This report describes that a 56-year-old male patient with acute osteomyelitis caused by Klebsiella pneumoniae infection after implant placement was treated with intravenous administration of ertapenem without open surgery treatment. Through this case, we report that antibiotic susceptibility test is essential for the treatment of acute osteomyelitis caused by a bacterial infection resistant to empirical antibiotics, and early administration of appropriate antibiotics can reduce the possibility of extensive bone destruction or additional open surgery.
