Ultrasonography of Abdominal Organs in Cynomolgus Monkey (Macaca fascicularis) (Cynomolgus Monkey(Macaca fascicularis)에서의 복부 초음파에 관한 연구)

  • The purpose of this study is to construct fundamental information about the ultrasonographic diagnosis for extrinsic and intrinsic abdominal disease. Normal ultrasonography of liver, gall bladder, spleen, kidney, urinary bladder, stomach, pylorus, duodenum, and heart of 4 cynomolgus monkey(Macaca fascicularis) were determined by use of ultrasonography. One cynomolgus monkey was autopsied at the time of euthanasia which is performed 24 hours after ultrasonography, and above mentioned organs were measured actually. In ultrasonography of cynomolgus monkey, the gall bladder was 17.5 cm long, and 6.6 cm wide. The width of spleen was 8.8 mm. The right kidney was 35.5 mm long, 23.7 mm wide, and 15.2 mm deep. The ultrasonographic measurements of the left kidney in calves was similar. The urinary bladder was 27.7 mm long, and 20.5 mm wide.

Ultrasonography of the Kidney in Mongrel Dogs in Korea (한국산 잡견에서 신장의 초음파검사법)

  • Ultrasonography observations were made of the kidneys of 21 dogs. Ultrasonography images obtained in transverse and longitudinal planes. A 3.5-MHz linear transducer was used. All examinations were peformed on dogs in dorsal recumbency under anesthesia. The right kidney was 3.2~3.8cm long, 1.7~3.5 cm wide and 1.0~1.9cm deep in 0~5 of body weight group, 3.6~4.6cm long, 2.4~3.6cm wide and 1.4~2.1cm deep in 5~10 of body weight group, and 4.6~6.4cm long, 3.3~4.2cm wide and 2.0~2.0cm deep in 10~15kg of body weight group(p< 0.01). Diameter of the parenchyma of the right kidney ranged between 0.4~0.9cm in 0~5kg group, 0.6~1.0cm in 5~l0kg group and 0.9-1.1cm in 10~15kg group(p<0.01). Diameter of the renal sinus of the right kidney ranged between 0.3~0.5cm in 0~5kg group, 0.4~0.7cm in 5~10kg group and 0.5~0.9cm in 10~ 15kg group(p< 0.01). Similar ultrasonic measurements were obtained for the left kidney. It was concluded that the ultrasonographic findings described in this study can be used as references for diagnosis of morphologic changes in e canine kidney.

  • Ultrasonographic diagnosis of kidney and urinary bladder disease was performed in native Korea cattle. Ultrasonogaphic appearence of kidney and urinaryy baldder according to the growth were determined from 6 to 13 months by monthly examinations in 9 native Korean cattle. The kidney and urinary bladder were examined in standing position. The position, dimension, and structure of both kidneys were determined at 12 intercostal space and paralumbar fossa by use of ultrasonography. Ultrasonograms were obtained with 3.5 or 5.0-MHz sector transducer. The width and depth of the kidney, and the diameter of renal parenchyma and renal sinus were observed. Both kidneys were observedfrom the body surface until 8 months old age, but over the 9 months old age, only right kidney was easily accessible. The width of right kidney at the 6, 7, 8, 9, 10, 11, 12 and 13 months old age were 5.7, 6.3, 6.9, 7.6, 8.4, 9.3, 9.8 and 10.7 cm, respectively. The depth 5.8, 6.1 and 6.6 cm, respectively. The size of left kidney was similar to right kidney until 8 months old age. After the 9 months old age, only right kidney was easily accessible. The circumference of urinary bladder at the 6, 7, 8, 9, 10, 11, 12 and 13 months old age were 19.1, 21.7, 22.5, 23.0, 24.2, 25.3, 27.8 and 29.1 cm, respectively. Calculi in urinary bladder were observed by ultrasonography.

Radiogyaphic and Ultrasonographic Diagnosis of Pyelonephritis in 2 dogs (개에서 신우신염의 방사선학적 및 초음파학적 진단 2례)

  • A female 8-year-old Pug weighing 7.3 kg and a female 10-year-old Maltese dog weighing 3.5 kg showing anorexia and vomiting for a few weeks were referred to Veterinary Medical leaching Hospital, Seoul National University. Radiographic findings were an enlarged right kidney in a pug dog and a radiopaque material on the right ureteral region lateral to the third lumbar vertebrae with indefinite right kidney contour in a Maltese dog, repectively. Excretory urography performed in a Pug dog revealed a poor opacified enlarged right kidney with absent of pelvic recesses and pelvic dilation with proximal ureteral dilation on contralateral kidney. Ultrasonographic findings were enlarged kidney with dilated pelvis and echogenic sediment within the medulla in both dogs and especially an engorged proximal ureter and a thin rim of functional renal tissue remains in a Maltese dog. Those diagnostic findings indicated high possibility of pyelonephritis and these were confirmed by pathologic examination. Radiography and ultrasonography, although not giving final diagnosis for pyelonephritis, are useful for assessment and diagnosis of pyelonephritis.

Antenatally detected urinary tract dilatation: a pediatric nephrologist's point of view

  • Antenatally diagnosed urinary tract dilatation (UTD), previously referred to as antenatal hydronephrosis, is the most commonly detected abnormality by prenatal ultrasonography. Several grading systems have been developed for the classification of antenatal UTD using prenatal and postnatal ultrasonography. UTD comprises a wide variety of congenital abnormalities of the kidney and urinary tract ranging from transient UTD to more significant abnormalities such as vesicoureteral reflux, ureteropelvic junction obstruction, ureterocele, ureterovesical junction obstruction, posterior urethral valves, and non-refluxing megaureter. Optimizing the evaluation of antenatally detected UTD is essential to recognize children with important disorders while avoiding excessive investigations. Conservative approach with close follow-up is increasingly accepted as an appropriate treatment option for patients with asymptomatic vesicoureteral reflux and ureteropelvic junction obstruction in recent years. However, predicting permanent kidney damage in an unselected group of children with antenatal UTD is still challenging. The management and follow-up of children with UTD should be individualized based on recommendations from a pediatric nephrologist, a pediatric urologist, or both. Future research directed at predicting long-term outcomes of children diagnosed with UTD from mild findings to severe disease is needed to refine management for those at higher risk of kidney disease progression.

Analysis of the Correlation Between Kidney Function Indicators and Kidney Size According Age Groups in Ultrasonography (신장 초음파 검사에서 연령대에 따른 신장 기능 지표와 신장 크기 간의 상관관계 분석)

  • The purpose of this study was to analysis the correlation between kidneys function indicators and these size in ultrasonography. A total of 170 (male:86, female:84) patients of sex and age groups were examined by abdominal ultrasonography. The patients classified as those in their 20's, 30's, 40's, and over 50's. We measured the length, width, and cross-sectional height of the kidneys twice. At this time, the length of these were measured from the maximum upper to the maximum lower pole and the widest width in the same ultrasonography was measured to obtain the cross-sectional area. Other relevant indicators included body surface area, serum creatinine, glomerular filtration rate (GFR), MDRD (Modification of diet in renal disease) and C-G (Cockcroft-Gault). Significant comparisons of differences between relevant factors by age groups and sex were conducted with a one-way distribution analysis. Correlation analysis was also performed between relevant factors by using Pearson and Spearman correlation coefficient. It was defined as meaningful when the p-value was less than 0.05. As a result, the length, the width, and the cross-sectional area of kidneys were correlated with GFR, C-G, MDRD. Therefore, it is expected that the accuracy of diagnosis of kidneys disease will be increased if the relevant indicators are evaluated together rather than measuring only length of these in ultrasonography.

Diagnostic imaging features of calyceal diverticulum in a cat

  • A seven-year-old, castrated male, Korean domestic shorthair cat was referred because of a kidney abnormality. Radiography revealed left renal agenesis and right kidney enlargement. Ultrasonography and computed tomography (CT) showed amorphous calcified materials in a cyst-like lesion of the right kidney. In the excretory phase of the CT images, the lesion was opacified with contrast materials, indicating communication with the collecting system. Based on these findings, the cat was diagnosed with a calyceal diverticulum. A calyceal diverticulum may cause various clinical symptoms related to the urinary system. The excretory phase of CT is useful for diagnosing a calyceal diverticulum.

Height-Based Formula Predicting Renal Length in Korean Children derived from Technesium-99m Dimercaptosuccinic Acid Scan

  • Purpose: The aim of this study was to establish a simple formula to predict renal length in children using a Technesium-99m dimercaptosuccinic acid (DMSA) scan data, and to compare it with the formula derived from ultrasonography, which is widely accepted. Methods: Children who underwent a DMSA scan and ultrasonography were reviewed retrospectively, and those who had anatomical urinary tract abnormalities or urinary tract infections were excluded. Results: A total of 230 children (84 males and 146 females; age, 1 month to 16 years; mean age, $16.8{\pm}27.4$ months). Mean renal length measured by DMSA scan was longer than that by ultrasonography ($6.38{\pm}1.16$ vs. $6.02{\pm}1.14cm$; P < 0.001). Renal length was correlated with age, weight, height, and body surface area on the DMSA scan and ultrasonography, and showed the strongest positive correlation with height. The following formulae were established to predict renal length: mean renal length (cm) = 5.433 ${\times}$ height (m) + 2.330 (R2, 0.833) using the DMSA scan data, and mean renal length (cm) = 5.367 ${\times}$ height (m) + 2.027 (R2, 0.853) using ultrasonography data. Conclusion: We propose a simple height-based formula to predict renal length in children using a DMSA scan data, and validate it by comparing with ultrasonography formula.

A Female Infant with Segmental Multicystic Dysplastic Kidney (분절형 다낭성 이형성신 영아 보고 1례)

  • There have only been 35 pediatric cases and one adult case reported on segmental multicystic dysplastic kidney (MCDK) from our search in PubMed, including 19 cases detected antenatally. There is little documentation of segmental MCDK, particularly concerning its natural history. Segmental MCDK can be presented atypically, making diagnosis more difficult. We report an another case with segmental MCDK. Multicystic abdominal mass detected on antenatal sonogram in this infant was diagnosed as segmental MCDK by renal ultrasonography and computed tomography. If a definitive diagnosis of segmental MCDK can be made on imaging, surgery is not required for a diagnostic biopsy.
