Streptococcus pneumoniae is the major pathogen that frequently causes serious infections in children, the elderly and immunocompromised patients. S. pneumoniae is known to produce reactive oxygen species (ROS) and S. pneumoniae-produced ROS is considered to play a role in pneumococci pathogenesis. SIGN-R1 is the principal receptor of capsular polysaccharides (CPSs) of S. pneumoniae. However, there is a considerable lack of knowledge about the protective role of SIGN-R1 against S. pneumoniae-produced ROS in SIGN-$R1^+$ macrophages. While investigating the protective role of SIGN-R1 against ROS, we found that SIGN-R1 intimately bound to peroxiredoxin-1 (Prx-1), one of small antioxidant proteins in vitro and in vivo. This interaction was increased with ROS generation which was produced by stimulating SIGN-R1 with dextran, a polysaccharide ligand of SIGN-R1. Also, SIGN-R1 crosslinking with 22D1 anti-SIGN-R1 antibody increased Prx-1 in vitro or in vivo. These results suggested that SIGN-R1 stimulation with CPSs of S. pneumoniae increase the expression level of Prx-1 through ROS and its subsequent interaction to SIGN-R1, providing an important antioxidant role for the host protection against S. pneumoniae.
To develop a new natural antibiotics from Koran plant resources for dealing with the current situation regarding the rapid increase in antibiotic-resistant pathogen, the in vitro inhibitory activities of essential oils from the young leaves of Glehnia littoralis (Umbelliferae) as well as its main constituents were evaluated against susceptible and resistant species of Streptococcus pneumoniae. The essential oil fraction of G. littoralis and its main components, $\alpha-and\;\beta-pinene$, exhibited significant inhibitory activities against the antibiotic-susceptible and resistant strains of S. pneumoniae, with MICs (minimum inhibiting concentrations) ranging from 4.0mg/ml to 16mg/ml. No remarkable differences were shown between the susceptible and resistant strains. Moreover, the disk diffusion test disclosed that these inhibitory activities were dosedependent. Furthermore, data from the checkerboard titer test with FICIs (fractional inhibiting concentration indices) from 0.15 to 0.28 indicated synergisms between norfloxacin and $\alpha-or\;{\beta}-pinene$ in activity against S. pneumoniae KCCM49629 and S. pneumoniae CCARM4059.
목 적:호흡기 감염의 증상이 없는 소아들을 대상으로 다중 역전사중합효소연쇄반응법(multiplex reverse transcription-polymerase chain reaction; mRT-PCR)을 이용하여 비인두 상주균의 이환율을 알아보고자 하였다. 방 법:2008년 7월 25일부터 28일까지 33명의 소아들을 대상으로 비강 면봉채취법으로 검체를 채취하였으며, 이들은 검체 채취 당시 심각한 호흡기 감염의 증상이 없었다. 모아진 검체에서 DNA를 추출한 후 multiplex primer set ($Seeplex^{(R)}$ PneumoBacter ACE Detection, Seegene, Seoul, Korea)로 PCR을 진행하였다. 증폭된 반응산물은 2% agarose gel과 전기 영동 자동화 시스템인 screen tape system (Lab901, Scottland, UK)에 각각 전기 영동하여 확인하였다. 결 과:전체 33명의 소아 중 남아는 15명 여아는 18명이었으며, 나이는 3.2세에서 16.3세로 중앙값은 8.2세였다. mRT-PCR 결과 30명(90.9%)의 소아들에서 양성을 보였으며(S. pneumoniae, H. influenzae, C. pneumoniae, B. pertussis), 이들 중 13명(39.4%)에서 2가지 이상의 균이 검출되었다. 균의 종류로는 12명(36.4%)에서는 S. pneumoniae와 H. influenzae, 1명(3.0%)에서는 S. pneumoniae, H. influenzae와 C. pneumoniae이 검출되었다.. 결 론:mRT-PCR은 비인두 상주균의 동정에 있어서 민감도가 높은 방법으로 생각된다. 하지만 비인두 상주균에 대한 PCR 결과가 소아들의 임상 양상과 어느 정도 일치할지에 대해서는 더 많은 연구가 필요할 것으로 생각된다.
다양한 extended-spectrum ${\beta}$-lactamase (ESBL)를 생성하는 폐렴막대균의 출현 및 확산은 세균에 의한 감염증 치료에 어려움을 가중시키고 있다. 본 연구에서는 충청지역에서 분리된 폐렴막대균을 대상으로 ESBL 유전자를 검출하고 항균제 감수성 양상을 조사하였다. 또한 같은 클론에서 유래하였는지를 확인하기 위해 repetitive element sequence-based (REP)-PCR을 수행하였다. 충청지역에서 분리된 폐렴막대균 102균주 중 21균주가 CTX-M-14 및/또는 CTX-M-15를 생성하는 것으로 나타났으며 이 균주들은 3세대 cephalosporin 계열 항균제에 대해 70% 이상의 높은 내성율을 보였다. 본 연구에서 분리된 CTX-M형 ESBL생성 폐렴막대균은 다양한 클론으로부터 유래되었으며 그 중 일부는 지역사회에 확산되어 있음이 확인되었다. 이러한 폐렴막대균의 감염 및 확산을 방지하기 위해서는 감염관리의 강화가 필요하다. 아울러 좀 더 효과적인 내성세균의 관리를 위해서는 내성유전자의 생물학적 조사와 통계학적 분석을 통해 통합적으로 구축된 데이터베이스를 모니터링 할 필요가 있을 것으로 사료된다.
DnaK is a major heat shock protein and known to be highly conserved in all species. Previously, the dnaK in Streptococcus pneumoniae was cloned and the immunogenic nature characterized. In this study, dnaK mutants were generated by insertion of duplication mutagenesis and their characteristics examined. They had defective growths at all temperatures ($20^{\circ}C-42^{\circ}C$)and cell divisions, and formed filaments after a temperature shift from 30 to 42. A unique feature of the dnaK mutants of S. pneumoniae, unlike those of E. coli and B. subtilis, was the growth capability at high temperature ($42^{\circ}C$) without producing the putative GroEL. Our results suggest that DnaK may serve as a regulator and/or modifier in GroEL gene expression.

We report a case of a 5-year-old girl who developed left hemiparesis and left facial palsy, 6 days after the initiation of fever and respiratory symptoms due to pneumonia. Chest radiography, conducted upon admission, showed pneumonic infiltration and pleural effusion in the left lung field. Brain magnetic resonance imaging showed acute ischemic infarction in the right middle cerebral artery territory. Brain magnetic resonance angiography and transfemoral cerebral angiography revealed complete occlusion of the right middle cerebral artery. Mycoplasma pneumoniae infection was identified by a 4-fold increase in IgG antibodies to M. pneumoniae between acute and convalescent sera by enzyme-linked immunosorbent assay. Fibrinogen and D-dimer levels were elevated, while laboratory exams in order to identify other predisposing factors of pediatric stroke were all negative. This is the first reported pediatric case in English literature of a M. pneumoniae-associated cerebral infarction involving complete occlusion of the right middle cerebral artery.

Small phytochemicals have been successfully adopted as antibacterial chemotherapies and are being increasingly viewed as potential antibiofilm agents. Some of these molecules are known to repress biofilm and toxin production by certain bacterial and yeast pathogens, but information is lacking with regard to the genes allied with biofilm formation. The present study was performed to investigate the inhibitory effect of burdock root extract (BRE) and of chlorogenic acid (CGA; a component of BRE) on clinical isolates of Klebsiella pneumoniae. BRE and CGA exhibited significant antibiofilm activity against K. pneumoniae without inflicting any harm to its planktonic counterparts. In vitro assays supported the ${\beta}$-lactamase inhibitory effect of CGA and BRE while in silico docking showed that CGA bound strongly with the active sites of sulfhydryl-variable-1 ${\beta}$-lactamase. Furthermore, the mRNA transcript levels of two biofilm-associated genes (type 3 fimbriae mrkD and trehalose-6-phosphate hydrolase treC) were significantly downregulated in CGA- and BRE-treated samples. In addition, CGA inhibited biofilm formation by Escherichia coli and Candida albicans without affecting their planktonic cell growth. These findings show that BRE and its component CGA have potential use in antibiofilm strategies against persistent K. pneumoniae infections.
Mycoplasma pneumoniae infection is usually confined to the respiratory tract but it can cause a variety of extrapulmonary manifestations such as rashes, myalgia, hemolytic anemia, cerebral infarction, transverse myelitis, cerebellar ataxia, Guillain-Barre syndrome and meningoencephalitis. Neurologic complications of Mycoplasma pneumonia have been rarely reported until now. Cerebral infarction as a complication of mycoplasma infection in children has been very rarely reported. In our case, in a young girl with M. pneumoniae infection, a cerebral infarct resulted in persistent and significant neurological dysfunction. We report a 11-year-old girl with cerebral infarction associated with clinical and serologic evidence of Mycoplasma infection.
목 적 : 소아 호흡기 감염의 주된 병원체인 M. pneumoniae는 일반적으로 4 내지 7년의 유행 주기를 가지는 것으로 알려져 있으나 M. pneumoniae 유행 기간 중 항체 양전율에 대한 정확한 국내 보고는 아직까지 없는 실정이다. 이에 저자들은 2003년 하반기 M. pneumoniae 유행 기간 전후로 항체 추적 검사를 시행하여 항체 양전율을 조사하고 나이, 초기 항체가, 아토피 유무가 항체 양전율에 미치는 영향을 알아보고자 본 연구를 시행하였다. 방 법 : 2003년 상반기 본원 외래를 방문하여 Mycoplasma 항체를 측정한 환아 중 혈청 항체가가 음성, 1 : 40, 1 : 80이었던 환아 191명을 대상으로 2004년 상반기에 추적 검사를 실시하였다. 추적 검사상 항체 역가가 4배 이상 증가한 경우를 항체 양전으로 정의하였고 항체 양전 여부에 따라 항체 양전군과 항체 비양전군으로 구분하였다. 혈청 항체 검사는 Serodia-Myco II 입자 응집법을 사용하여 시행하였다. 결 과 : 대상 환아들의 평균 나이는 $6.6{\pm}2.7$세였다. 남여 수는 각각 137명과 54명이었고 4세 이상 환아 중 아토피로 분류된 환아는 75명(44.6%)이었다. 대상 환아 중 항체 양전을 보인 환아는 83명(43.5%)이었다. 항체 양전군과 비양전군의 나이, 성별, 아토피 유무를 각각 비교하였을 때 두 군간의 유의한 차이는 보이지 않았다. 대상 환아들을 나이에 따라 네 군으로 나누어 비교하였을 때 각 군 사이 유의한 항체 양전율의 차이는 없었다. 초기 항체가가 음성, 1 : 40, 1 : 80이었던 환아들에서 항체 양전율은 각각 45.5%, 46.3%, 26.1%로 각 군 간 항체 양전율의 유의한 차이는 보이지 않았다. 아토피군과 비아토피군으로 나누어 비교하였을 때 항체 양전율은 각각 40.0%, 48.4%로 통계적으로 유의한 차이는 관찰되지 않았다. 결 론 : 2003년 하반기의 M. pneumoniae 유행 시기에 항체 양전율은 43.5%로 관찰되었으며 나이, 초기 항체가, 아토피 유무에 따른 항체 양전율의 차이는 없었다. 위 결과로 미루어 보아 M. pneumoniae 유행 시기에 소아 연령의 상당수에서 M. pneumoniae 감염에 이환되는 것으로 생각되며 M. pneumoniae의 혈청학적 역학 연구에 항체 양전율 조사가 유용하게 쓰일 수 있을 것으로 사료된다.

Klebsiella pneumoniae is a gram-negative, non-motile, rod-shaped, and encapsulated bacterium in the normal flora of the intestines, mouth, skin, and food, and has decarboxylation activity, which results in generation of diamines (cadaverine, agmatine, and putrescine). However, there is no specific information on the exact mechanism of decarboxylation in K. pnuemoniae. Specifically lysine decarboxylases that generate cadaverine with a wide range of applications has not been shown. Therefore, we performed a functional study of lysine decarboxylases. Enzymatic characteristics such as optimal pH, temperature, and substrates were examined by overexpressing and purifying CadA and LdcC. CadA and LdcC from K. pneumoniae had a preference for L-lysine, and an optimal reaction temperature of 37℃ and an optimal pH of 7. Although the activity of purified CadA from K. pneumoniae was lower than that of CadA from E. coli, the activity of K. pneumoniae CadA in whole cell bioconversion was comparable to that of E. coli CadA, resulting in 90% lysine conversion to cadaverine with pyridoxal 5'-phosphate L-lysine.
