Treatments of esophageal foreign body treated using Rigid esophagoscopy (강직형식도경술을 이용하여 치료한 이물에 대한 임상적 고찰)

  • OBJECTIVE : To study the management (diagnostic and therapeutic) of esophageal foreign bodies with rigid esophagoscopy. MATERIAL AND METHODS : All 100 patients admitted to the Dankook University Hospital for ingestion of foreign bodies between May 1994 and July 1999. The outcome for each patient was determined by examining hospital records of demographic information, identification of the foreign body and the removal procedure used. RESULTS : Rigid esophagoscopy was performed for suspected foreign bodies in 100 cases an impacted. The most frequent location was the upper third of the esophagus (68%). The most common type of foreign body was fish bones in adult(61%) and coins in children(70%). In 12 patients. flexible endoscopy had failed previously to remove the foreign body and 4 cases were migrated esophageal metal stent in esophageal stenosis. CONCLUSIONS : The rigid esophagoscopy is appropriate techniques for managing esophageal foreign bodies.

  • Objective: The purpose of this study was to present the clinical practice guideline of Korean medicine for malignant lymphoma.Background: Malignant lymphoma is the tenth most common cancer in Korea. The two main types of lymphoma are Hodgkin’s disease and non-Hodgkin’s lymphoma. Non-Hodgkin’s lymphomas are more common, comprising nearly 95 percent of all lymphomas. In China, the traditional Chinese medicine clinical guidelines for malignant lymphoma were published in 2014. Therefore, there is growing need for a clinical practice guideline in Korea, which has not thus far existed. This clinical practice guideline was created by reviewing the Chinese clinical practice guideline and Korean clinical reports. This study will be helpful in understanding malignant lymphoma and in understanding its treatment in Korean medicine.Conclusion: Further clinical research on malignant lymphoma is needed to develop a more advanced clinical guideline.

  • The N-unsaturated alkylcarbazole derivatives were synthesized by the nucleophilic unimolucular substitution reaction ($S_N1$) of carbazole with unsaturated alkyl chloride. These reactions between carbazole and unsaturated alkyl chloride were conducted in dimethyl sulfoxide (DMSO) containing alkali (NaOH or KOH) at room temperature for 4 hrs under nitrogen atmosphere. The mole ratios of carbazole, alkali and unsaturated alkyl chloride were 1:6:1, respectively. All of the compounds of starting materials and reaction products were characterized by CHN analysis, $^1H$-NMR and FR-IR spectroscopy.

  • In this paper, we propose mode selector for operating a switching system and regulator of linear system to detect the load current. The proposed mode selector can be a mode switching of linear system and switching system, and it has been proposed to compensate for the disadvantages of regulator of switching system with low efficiency in light load conditions. At light load conditions, the mode selector is possible to provide a high efficiency in light load condition by switching the mode to the regulator of linear system. The mode selector was designed to using a Dongbu Hitek $0.18{\mu}m$ CMOS process.

The Clinical Study on ShrinKage Rate of Graft following Strip Gingival Autografts (Strip 치은자가이식술후 이식편의 수축률에 관한 임상적 연구)

  • The purpose of this study was to evaluate clinical changes in graft size after treatment with strip gingival autograft in human. 57 premolar teeth in 27 patients having the following mucogingival problems were selected. The width of extension, attached gingiva including free marginal gingiva, width of transplant and clinical sulcus depth were measured at the initial examination, 2, 12 and 24 weeks following the strip gingival autograft and free gingival autograft. The change of width of extension, attached gingiva including free marginal gingiva, width of transplant and clinical sulcus depth according to healing process in both graft procedures was statistically analyzed by repeated measure ANOVA test and independent t-test using SPSS program. The results were as follows : 1. The change of keratinized gingiva in both graft procedures was increased significantly at 24 weeks post-op. 2. The clinical sulcus depth exhibited no marked changes throughout the entire investigation in both graft procedures. 3. No dimensional variation was seen in graft size in both graft procedures. 4. Shrinkage did not differ significantly in both graft procedures. From the day of grafting to 24 weeks after surgery the percentages of shrinkage were : strip gingival autograft 28% and free gingival autograft 29%.

A Pilot Study about The Effect of Chunwangbosim-dan(天王補心丹) on Heart Rate Variability of Healthy Subjects (천왕보심단이 정상인의 심박변이도에 미치는 영향에 대한 예비연구)

  • Objectives : This study was conducted to assess the effect of Chunwangbosim-dan(CB) on the autonomic nervous system by using heart rate variability measurement. Methods : The eligible subjects were composed of the healthy female group of ages 25 to 30 years. They were divided into two groups, the CB group(n=16) and the control group(n=16). We measured the heart rate variability respectively for 3 times at 14:00, 16:00, and 17:00. In the CB group, subjects were administered CB at 15:00. For the measurement values, Mean Heart Rate and SDNN(standard deviation of the NN intervals) were used as time domain analysis, and HF(high frequency), LF(low frequency), and LF / HF ratio were used as frequency domain analysis. Results : The degree of LF changes in the CB group was significantly lower than control group at 1 hour after administration. Conclusions : We suggest that Chunwangbosim-dan may be useful for stabilization of the autonomic nervous system by controlling sympathetic nerve in healthy people.

  • The cell activities of bone metabolism is affected by growth factor rather than by hormone. The affects of growth factors on the bone activity were observed using various culture methods. Platelet-derived growth factor(PDGF) is produced from the well differentiated bone cell. It stimulates cell mitosis, synthesizes collagen in bone tissue and plays a role in healing response. The purpose of this study is to evaluate the effects that PDGF has on the activity and the proliferation of osteoblast by measuring the activity of alkaline phosphatase, the growth formation of calcified nodules, and osteocalcin production. In this study, HOS and ROS 17/2.8 osteoblastic cell line was used, along with variable concentrations of PDGF the were measured with osteoblastic proliferation. The cell proliferation of HOS and ROS 17/2.8 cells was stimulated dose- depentdently. Alakline phosphatase activity was significantly decreased by PDGF in osteoblastic cells. A number of small calcified nodules were observed in HOS cell treated with low concentrations(0.1, 0.4 ng/ml) of PDGF-BB and no significant difference from control group was found. High concentrations(10, 50 ng/ml) of PDGF suppressed calcified nodule formation. And osteocalcin production was inhibited with PDGF. These results suggest that PDGF stimulates the osteoblastic proliferation, whereas suppresses the individual cellular functions.

  • Objectives : Saam acupuncture has evident principles of prescription based on acupuncture tonification and purgation of Nan-gyeong but many prescriptions of Saam acupuncture are not typical patterns. This paper studies principles of prescriptions of the king fire and the premier fire. Methods : Through investigation of many medical documents concerning king fire and the premier fire, the author explained of physiological and pathological principle of king and the premier fire, also explained of interacting mechanism through five element. And The theoretical authorities of analysis are Nan-gyeong, works of Joo dan-gye and Li dong-won. Results & Conclusion : 1. The sufficiency and the control of the water is the key to treat the disease by fire. Considering the attack and the treatment of disease, heart and liver give rise to fire in its early stage and kidney and lung are the key to treatment. 2. Consequently the pathology of the disease from fire is corresponding to the theory- Yang Sufficient, Yin Deficient- of Joo dan-gye. It seems that the theory of Joo dan-gye influenced on the medical theory of Saam Acupuncture. 3. On that account I believe that the study of medical theories of the Geum and Won Dynasty that various medical theories had appeared is very available for Saam Acupuncture. At present global warming is accelerating because environmental pollution has been increasing. Therefore the damage of fire to human body will be more and more severe and extensive. Because in the future medical plants will not be free from environmental pollution, acupuncture will be very confidential treatment.

Combination Therapy of Gefitinib and Korean Herbal Medicines Could be a Beneficial Option for Patients with Non-Small-Cell Lung Cancer

  • Lung cancer has a high mortality rate and is often diagnosed at the metastatic stage. Gefitinib is a targeted molecular therapeutic drug used to treat patients with non-small-cell lung cancer (NSCLC). Korean herbal medicines may also have therapeutic efficacy against lung cancer, reduce the side effects associated with chemotherapy, and improve patient quality of life (QOL). This case report describes the effects of a Korean herbal medicine regimen combined with gefitinib in a patient with NSCLC and bone metastasis. The Korean herbal medicine regimen included woohwanggeosa-dan, hwanggibujeong-dan and geonchilgyebok-jeong. The computed tomography (CT) findings showed that following combination treatment, the size of the tumor was markedly decreased without serious adverse events. Moreover, the Eastern Cooperative Oncology Group (ECOG) performance status was improved and cancer-related pain was decreased. These results suggest that a combination of Korean herbal medicines and gefitinib may be an effective therapeutic option for patients with advanced NSCLC and bone metastasis. Further studies are needed to examine the mechanism and the clinical efficacy of Korean herbal medicines against NSCLC.

A Case Report of Postherpetic Neuralgia Treated with Kyung Hee Gongjin-dan and Hyangbujapalmul-tang-gagam (경희공진단과 향부자팔물군자탕가감으로 호전된 대상포진 후 신경통 치험 1례)

  • The most common definition of postherpetic neuralgia is the presence of pain more than a month after the onset of the eruption of zoster. This case report suggests the effect of Korean medicine on postherpetic neuralgia. An 85-year-old male patient with postherpetic neuralgia was treated with a combination of Korean medical remedies. Kyung Hee Gongjin-dan and Hyangbujapalmul-tang-gagam were applied for 23 days. The Numerical Rating Scale was used to measure pain intensity. The Barthel Index and Beck Depression Inventory were used for the functional assessment and depression evaluation, respectively. After treatment, the patient's pain intensity decreased. Additionally, the patient's independence increased during activity, and the severity of their depression decreased. Thus, this case suggests that a combination of Korean medicine treatments might be effective for postherpetic neuralgia. In particular, it shows that Kyung Hee Gongjin-dan and Hyangbujapalmul-tang-gagam can be therapeutic options in the treatment of postherpetic neuralgia.
