Background : Hypertension is one of the most important risk factors of the cerebrovascular accident and coronary artery disease which are the major causes of mortality in Korea. In Korea, the quality of care provided by office-based physicians has not been evaluated formally. The purpose of this study is to assess the quality of hypertension management of office-based physicians. Method : Self-administered questionnaires were mailed to the office-based physicians with the speciality of internal medicine, general surgery, family medicine, and general practitioners. Among 2,045 physicians, 981 doctors(48.0%) replied the questionnaires. Contents of questionnaires were based on the recommendation from the JNC-V report(the Fifth Report of the Joint National Committee on Detection, Evaluation, and Treatment of High Blood Pressure), and included the criteria of diagnosis, treatment, follow-up interval, and other characteristics of physicians(age, sex, type of speciality, and location of practice). Results : Eighty four percent of the office-based physicians made diagnosis of hypertension with less than 3 times of blood pressure measurements. The performance rate of required examination for hypertensives was very low in most items. Rate of fundoscopic examination is the lowest one among them(5.9%). The performance rate of laboratory examination was also low in most items. Internists tended to order more frequent laboratory examinations than any other type of physicians. Only 11.4% of the physicians did appropriate treatments for the mild hypertension case. The antihypertensives selected by the physicians as a first line drug were in the order of beta blocker(26.4%), calcium channel blocker(23.4%), diuretics(23.1%), ACE inhibitors(14.3%). The visit interval for established hypertensives was very short. Proportion of physicians with follow-up interval longer than 4 weeks was only 4.3%. Conclusions : The overall quality of hypertension management of office-based physicians in Korea is very problematic in many aspects. So further investigations to find out the reasons of low quality arid quality of care should be initiated.
Purpose: Amelanotic melanoma represents a melanoma with an absence or a small number of melanin pigments and comprises 2% of all melanomas. These melanomas are frequently misdiagnosed, probably because of its nonspecific clinical features and difficulty in diagnosis, resulting in delayed diagnosis and treatment. We report a patient with amelanotic melanoma, who underwent surgical treatment with sentinel lymph node biopsy using gamma probe. Methods: A 32-year-old female was presented with a slowly growing ill-defined, hypopigmented nonerythematous lesion with nail defect on right index finger tip. Preoperative punch biopsy was performed, showing an amelanotic melanoma. Sentinel lymph node biopsy was done using gamma probe(Crystal probe system, CRYSTAL PHOTONICS GmbH, Germany) and confirmed no evidence of regional lymph node metastases. The patient underwent amputation at the proximal interphalangeal joint. Results: Histopathologic findings showed superficial spreading melanoma. There were no melanin pigments in Hematoxylin & Eosin stain but positive immunohistochemical stainings for S-100 protein and Hmb45, which were consistent with amelanotic melanoma. Patient's postoperative course was uneventful without any complication and had no evidence of recurrence of tumor in 6 months follow-up period. Conclusion: Amelanotic melanoma is extremely rare subtype of malignant melanoma with histopathologic findings of atypical melanocytes without melanin pigments. Early detection is crucial since survival is strongly related to tumor thickness and tissue invasion at the time of diagnosis. Wide excision is the treatment of choice and other conjunctive therapy has not been successful.

Interval-censored data are commonly found in studies of diseases that progress without symptoms, which require clinical evaluation for detection. Several techniques have been suggested with independent assumption. However, the assumption will not be valid if observations come from clusters. Furthermore, when the cluster size relates to response variables, commonly used methods can bring biased results. For example, in a study on lymphatic filariasis, a parasitic disease where worms make several nests in the infected person's lymphatic vessels and reside until adulthood, the response variable of interest is the nest-extinction times. Since the extinction times of nests are checked by repeated ultrasound examinations, exact extinction times are not observed. Instead, data are composed of two examination points: the last examination time with living worms and the first examination time with dead worms. Furthermore, as Williamson et al. (2008) pointed out, larger nests show a tendency for low clearance rates. This association has been denoted as an informative cluster size. To analyze the relationship between the numbers of nests and interval-censored nest-extinction times, this study proposes a joint model for the relationship between cluster size and clustered interval-censored failure data.
Calibration and Validation are major element of any space borne Earth Observation Mission. These activities are the major objective of the commissioning phases but routine activities shall be maintained during the whole mission in order to maintain the quality of the product delivered to the users or at least to fully characterise the evolution with time of the product quality. With the launch of ERS-l in 1991, the European Space Agency decided to put in place a group dedicated to these activities, along with the daily monitoring of the product quality for anomaly detection and algorithm evolution. These four elements are all strongly linked together. Today this group is fully responsible for the monitoring of two ESA missions, ERS-2 and Envisat, for a total of 12 instruments of various types, preparing itself for the Earth Explorer series of five. other satellites (Cryosat, Goce, SMOS, ADM-Aeolus, Swarm) and at various levels in past and future Third Party Missions such as Landsat, J-ERS, ALOS and KOMPSAT. The Joint proposal by the European Union and the European Space Agency for a 'Global Monitoring for Environment and Security' project (GMES), triggers a review of the scope of these activities in a much wider framework than the handling of single missions with specific tools, methods and activities. Because of the global objective of this proposal, it is necessary to put in place Multi-Mission Calibration and Validation systems and procedures. GMES Calibration and Validation activities will rely on multi source data access, interoperability, long-term data preservation, and definition standards to facilitate the above objectives. The scope of this presentation is to give an overview of the current Calibration and Validation activities at ESA, and the planned evolution in the context of GMES.

$^{18}F$-FDG PET/CT has led to advancement in diagnostic imaging, providing correlation of both physiology and anatomic information, and to new and innovative ways to utilize PET/CT imaging for the evaluation of musculoskeletal tumors. Recently, the most widely utilized musculoskeletal application of PET/CT imaging is for the detection and characterization of bone metastases, staging and restaging of primary malignant bone tumors and soft tissue sarcomas. And it is also useful in evaluating response to therapy for malignant musculoskeletal tumors and in detecting local recurrences or distant metastases during follow up. The future likely holds even more unique and potentially quite useful applications of PET/CT imaging for primary bone and soft tissue tumors. This article will review the useful applications of $^{18}F$-FDG PET/CT imaging for evaluating musculoskeletal tumors.
Purpose : To evaluate the diagnostic efficiency of the subacromial bursoscopy with glenohumeral air infusion technique for detection of the full thickness tear which we used to think as a partial thickness tear, because it was hidden by bursal tissue. Materials and methods : We chose 65 cases and divided them into 2 groups. The group I was 18 cases with partial thickness rotator cuff tears on glenohumeral arthroscopic evaluation, and the group 2 was 37 cases with full thickness rotator cuff tears which were repaired. We inflated the glenohumeral Joint with 50-100ml of air and observed air bubble leakage simultaneously on bursoscopy. Results : In group I, we could detect air leakage in 3 cases and found that it was full thickness tear. In group II, 2 cases revealed too much air leakage proved that it was incomplete repair and was in need of additional suture. Conclusion : Air infusion technique is though to be a valuable method in differentiation between the partial and full thickness tear and for evaluation of the security of the repair.
Approximating a divergence between two probability distributions from their samples is a fundamental challenge in statistics, information theory, and machine learning. A divergence approximator can be used for various purposes, such as two-sample homogeneity testing, change-point detection, and class-balance estimation. Furthermore, an approximator of a divergence between the joint distribution and the product of marginals can be used for independence testing, which has a wide range of applications, including feature selection and extraction, clustering, object matching, independent component analysis, and causal direction estimation. In this paper, we review recent advances in divergence approximation. Our emphasis is that directly approximating the divergence without estimating probability distributions is more sensible than a naive two-step approach of first estimating probability distributions and then approximating the divergence. Furthermore, despite the overwhelming popularity of the Kullback-Leibler divergence as a divergence measure, we argue that alternatives such as the Pearson divergence, the relative Pearson divergence, and the $L^2$-distance are more useful in practice because of their computationally efficient approximability, high numerical stability, and superior robustness against outliers.
There are three main kinds of service standards for 3G(Third-Generation) wireless communication as WCDMA, CDMA2000 and TD-SCDMA(Time Division-Synchronous Code Division Multiple Access). Compare with WCDMA and CDMA2000, TD-SCDMA system has distinguished technical characters. It is a TDD(Time Division Duplexing) based technology and deploys several advanced but in some respects complex technologies such as smart antenna, joint-detection and baton-handoff, etc. Therefore to analyze and design TD-SCDMA wireless network, it needs more efficient and systematic simulation tool. General simulation tool has so many analysis functions including path loss prediction, capacity and coverage analysis. For more suitable for TD-SCDMA, new additional technologies have to be implemented in simulation tool. Especially as the wireless network highly advancing focused on data service, it more needs to research and develop on the reliability of the simulation tool. In this paper, to give the concrete process and skill about how to implement TD-SCDMA simulation tool, we define the kinds of simulation tool and list basic analysis functions available for TD-SCDMA network design at first. And then we explain how to consider the effects of new technologies of TD-SCDMA and give the solutions about theses considerations.

Flap tear is a type of displaced meniscal fragments that is often clinically significant lesions requiring surgical intervention. If the displaced tear is located inferomedial to the tibial plateau and incarcerated into the articular capsule, it can be overlooked from preoperative magnetic resonance imaging evaluation and escape detection during arthroscopic examination. In addition, the clinical feature and treatment result of incarcerated flap tear has not been reported. We present 2 cases of medial meniscus flap tear incarcerated into the articular capsule that showed specific clinical features, in order to emphasize the importance of clinical suspicion of such a lesion for accurate preoperative diagnosis.

In this paper, we propose a method in which the negative acknowledge (NACK) message is used as command for cooperation and spectrum sharing. This allows for an automatic request for cooperation and sharing when the direct link of the primary user is in outage, and also allows for saving the number of control messages in cooperation-spectrum sharing based paradigm. In the sharing phase, the selected relay shares a power fraction of $1-{\alpha}$ for secondary transmitted signal while the remaining of ${\alpha}$ is for primary retransmitted signal. In the case of no relay collected, primary transmitter uses NACK as a command to retransmit the signal with fully power fraction (${\alpha}=1$). Both systems are assumed to employ BPSK signals. In this scheme, we propose the joint optimal decoding in the secondary user. The frame error rate (FER) performance at both systems is then analyzed. The theoretical and simulation results validate the analysis and confirm the efficiency of the protocol.
