한국 폐경 여성의 골다공증 위험 인자에 관한 연구 (Risk Factors for Postmenopausal Osteoporosis in Korea)

  • Objective : To identify the risk factors for postmenopausal osteoporosis in Korea Materials and methods : Bone mineral density (BMD) at the lumbar spine and femoral neck was measured by dual energy X-ray absorptiometry in 808 apparently normal postmenopausal Korean women. Questions about life style, demographic parameters, medical history and social habits etc. were asked on these women; 2ll women with normal bone mineral density, and 597 women with osteopenia or osteoporosis. Results 'Age of >50 years, low body mass index (BMI; <18.5 kg/m2), long duration of menopause(>10 years), and previous history of fracture were associated with increased prevalence of osteopenia or osteoporosis. Women without the outside activity also showed a higher frequency of low bone mass, Risk for osteopenia or osteoporosis was low in women with high BMI (>23 kg/m2) and women with job. The prevalence of low bone mass appeared to be independent of the following parameters: socioeconomic status, familial history, smoking, drinking, exercise, previous use of oral contraceptive, coffee or milk consumption, and degree of sunlight exposure. Conclusion 'Age, BMI, duration of menopause, previous history of fracture and degree of outside activity are the risk factors for postmenopausal osteoporosis in Korea.

  • We discovered various sides of Lee Jema's tenure as the governor of Jinhae by looking at recently found historical materials of the central government of Joseon. Lee Jema went to his appointed post in Jinhae in February of 1887 and was replaced before the termination of his office in June of 1888. Since his replacement did not immediately arrive at Jinhae, Lee Jema continued to perform his duties until the first half of 1889. The difference between the date of his resignation and the arrival of his replacement reveals why various historical materials disagree about when he left office. After Lee Jema's official resignation in June of 1888, he was appointed to the honorary posts of Naegeumjang and Cheomji, which indicates the termination of his career as an official. Two superior officials conducted job assessments on Lee Jema three times during his service (once every six months) and these are the rare evaluations which were performed during his lifetime. Lee Jema was highly regarded in terms of job competency by two superiors in succession, which was unique for a local government official in the late Joseon dynasty. One of the job assessments referred to the potential of his performance as an official which usually implied that he was a suitable candidate for higher rank. This might have derived from Lee Jema's study of statecraft. Lee Jema's love for the people as an official was also mentioned in one of the job assessment reports. This is borne out by his quelling the revolt of Hamheung Province in 1896.

한국인 알코올중독 환자의 예후인자에 관한 연구 : 심리적, 사회적, 생물학적 요인을 중심으로 (A Study for The Prognostic Indicators of Korean Alcoholics in Psychological, Social and Biological Aspects)

  • The author studied prognostic indicators of sixty Korean male alcoholics in psychological, social and biological aspects who were divided into abstinent and drinking groups. Thirty patients were assigned to each group. They were controlled in age and sex. Both groups were compared in terms of the demographic characteristics, past drinking history, treatment history, famaily history, ego strength and personality factors differences and distribution of dopamine $D_2$ receptor gene Al allele. Also the author studied relation of clinical course, alcoholic family history and dopamine $D_2$ receptor gene Al allele in both groups. The results were as follows; 1) The abstinent group had higher rate of married state, higher economic status, longer education years and maintained more stable job than the drinking group. But made no differences in occupation and religion. 2) The abstinent group showed higher rate of living with family members than the drinking group, and wives and fellows of the alcoholics anonymous were important factors for maintenance of abstinence. Family loading and parent's characters were not different. 3) The abstinent group had longer maximal length of abstinence but mean amount of alcohol consumption per day were larger than the drinking group. But there were no differences in duration of past drinking, drinking pattern, main drinking time, first drinking age and preference of the kind of alcoholic beverage in the past drinking history. 4) The abstinent group showed stronger treatment motivation, absolute abstinence in treatment goal, more voluntary adimission and maintained longer therapeutic relationship otter discharge than the drinking group. But both groups showed negative attitude toward antabuse therapy. 5) The abstinent group had higher mean score in ego strength scale than the drinking group. 6) In the personality factor questionnaire, the abstinent group showed strong laugh poise and the trait of praxernia, conservatism personality but the drinking group showed tough poise, the trait of weak ego strength(unstableness) and tough mindedness personality. 7) In comparision of dopamine $D_2$ receptor gene A1 allele, the prevalence of A1 allele was seventy percent and the frequency was 0.38 in the abstinent group. The prevalence of A1 allele was sixty percent and the frequency was 0.42 in the drinking group. Both groups were not significantly different in A1 allele prevalence and frequency. 8) In comparision of dopamine $D_2$ receptor gene A1 allele according to alcoholic family history, the prevalence of A1 allele was seventy percent and the frequency was 0.43 in the family history positive group. The prevalence of A1 allele was sixty-one percent and the frequency was 0.38 in the family history negative group. Both groups were not significantly different in A1 allele prevalence and frequency. In comparision of past drinking history according to alcoholic family history, the family history positive group showed earlier first drinking and problem drinking, but the family history negative group hod longer duration of past drinking. The mean amount of alcohol consumption per day, the longest duration of abstinence and Alcoholism Screening Test of Seoul Natoinal Mental Hospital(NAST) results were not significant. In conclusion, the results suggest that successful prognostic indicators of Korean alcoholics are married state, higher economic status, longer education years, stable job, living with family members, longer abstinence during past drinking history, strong treatment motivation, absolute abstinence in treatment goal, voluntary adimission, maintained therapeufic relationship, strong ego strength and the trait of praxernia, conservatism personality. But occupation, religion, alcoholic family history, parent's characters, duration of past drinking, drinking pattern, main drinking time, first drinking age, preference of the kind of alcoholic beverage, attitude to antabuse therapy and distribution of dopamine $D_2$ receptor A1 allele were not significantly related to the prognostic indicators of Korean alcoholics.

국회기록보존소 직제 및 직무에 관한 연구 (A Study on Job and Organization of the National Assembly Archives)

  • 본 연구는 국회기록보존소를 중심으로 하는 국회 기록관리기관의 직제와 직무의 문제점을 파악하고 이에 대한 대안을 제시하는 데 목적을 두었다. 이론적 논의로 국회 기록관리 책임 부서의 직제 연혁을 살폈고, '영구기록물관리기준 표준운영절차'를 준거로 삼아 영구기록물관리기관의 직무를 파악하였다. 이를 바탕으로 국회기록보존소의 직제 현황을 분석하고, 국회기록보존소 직원을 대상으로 직무 조사를 실시하였다. 직제 영역의 개선방안으로 국회 영구기록물관리기관의 재지정, 국회 소속기관 설치, 헌정기념관의 통합적 운영을 주요 과제로 논하였고, 이를 반영한 조직 개편안을 제시하였다. 직무영역으로는 관리대상 기록물 범주 확대와 기록관리 전문 인력 강화의 필요성을 살폈으며, 앞서 제시한 직제 개편에 맞는 담당별 업무분장을 제안하였다.

방사선사의 조직문화 및 리더십, 조직몰입이 조직만족에 미치는 영향 (The Effect of Radiological Technologist Organizational Culture, Leadership and Organizational Commitment on Organizational Satisfaction)

  • 본 연구는 부산시내에 있는 병원에 근무하는 방사선사들을 대상으로 조직문화 및 리더십, 조직몰입이 조직만족에 미치는 영향에 대해 알고자 설문조사를 실시하고 분석을 시행하였다. 그 결과 3차 대학병원의 조직문화, 조직몰입, 직무만족의 점수가 통계적으로 유의하게 높게 나타났다(p<0.05, p<0.001). 이직경험이 없는 그룹이 조직몰입, 직무만족 및 조직 유효성의 점수가 통계적으로 유의하게 높게 나타났다(p<0.05). 핵의학과의 직무만족 점수가 통계적으로 유의하게 높게 나타났다(p<0.05).위계적 회귀분석 결과 방사선사의 직무만족은 조직몰입이 가장 높은 영향력을 미치는 것으로 나타났다(β=0.564, p<0.001). 방사선사는 질병의 진단에 있어서 병원 조직에서 큰 부분을 차지하고 있다. 따라서 본 연구의 결과를 바탕으로 방사선사의 조직문화, 리더십, 조직몰입과의 관계를 분석하여 방사선사들의 직무만족을 높일 수 있는 방안을 마련하는데 도움이 될 것으로 사료된다.

생산기관 직제분석 자동화 및 공통 활용 방안 (Automation and Common Utilization Plans of Job and Organization Analysis of Producing Institutions)

  • 직제분석 업무는 다양한 생산기관에 대한 변천 이력과 주요 업무 기능을 파악하는 과업으로, 영구기록물관리기관 내에서 공통적으로 수행되어야 하는 과업이며 다수의 작업자가 관련 지식을 공동으로 참조해야 한다. 그러나 현업에서는 한정된 수의 담당자가 개별적으로 수작업을 통해 수행하고 있으며 그 결과도 공유되지 않고 있다. 이에 본 연구는 직제 분석 프로세스의 자동화를 통해 기록물 담당자의 업무 부담을 경감시키고 영구기록물관리기관에서 공통으로 활용 가능한 기초 자원을 구축하고자 한다. 영구기록물관리기관의 실무 담당자와 FGI를 수행함으로써 직제분석 업무를 세분화하고 자동화 가능한 부분을 선별하였으며, 이를 실현할 수 있는 방안을 함께 제안하였다. 또한 전자기록관리 과정에서 공통으로 참조 가능한 기초분석자료를 도출하고 그 결과를 실무자를 통해 검증함으로써 효율적인 지식자원 활용 방안을 제시하였다. 나아가 규격화된 업무 프로세스 정립을 통해 일관된 체계적인 업무 수행을 지원하는 기반을 마련하였다.

근로자들의 자율신경 활성 및 균형도에 대한 분석 (Analysis about Autonomic Nervous Activity and Balance of Workers)

  • Objective : This study was designed to analyze the results of autonomic nervous activity(ANA) and autonomic nervous balance(ANB) by heart rate variability test. The subjects were workers in the automobile manufacturing company. Method : The subjects consisted of 32,049 workers who had answered about questionnaires of a job position, age, smoking, drinking, exercise history and sex. The ANA and ANB were examined by SA3000P. I analyzed the differences of ANA and ANB according to job positions, ages, the state of smoking, drinking, exercise and sex by T-test or ANOVA with SPSS ver. 17.0. Results : Regarding the differences of ANA among job positions, the ANA was lowest in sales positions, highest in production workers. Among ages, the ANA was highest in 50s, lowest in 30s. In smoking, the ANA was higher in non-smoking group. In drinking, the ANA was higher in non-drinking group. In exercise, the ANA was higher in exercise group. In sex, there was no significant difference. The case of ANB, there were no significant differences except for ages. Among ages, the ANB was highest in 20s, the lowest in 40s. Conclusion : According to these results, we should establish the methods of controlling ANA and ANB from the perspective of Korean traditional medicine.

포항지역 사례를 중심으로 한 직업성 요통재해 현황 조사에 대한 고찰 (Investigation of job-related back injuries reported in the Pohang region)

  • This study is to analyze trends of back injuries reported for medical expense compensation in a territorial jurisdiction of Pohang regional office of Ministry of Labor. Back unjuries claimed in the Plhang region accounted for approximately 15% of all industrial accident and 0.96 billion won were spent to compensate for medical expenses in 1994. A majority of back injuries occurred in manufacturing, construction, and transportation, and, 98% of all back injuries during 1992-1994 were strain/sprain, herniated inter-vertebral dise(HIVD), and fracture. To achieve and effective back injury prevention, supplenentary reporting form should be provided to include the information about job description (e.g., weight of an object, task duration and working posture), employee's medical and work history, and working conditions.

데이터 마이닝을 이용한 당뇨환자의 관리요인에 관한 연구 (A Study on Factors of Management of Diabetes Mellitus using Data Mining)

  • 본 연구의 목적은 당뇨환자 관리와 관련된 요인을 규명하는데 있다. 2005년 국민건강 영양조사에 참여한 20세 이상의 성인 당뇨환자를 대상으로 하였다. 데이터마이닝 기법을 이용하여 로지스틱 회귀모형, 의사결정나무, 신경망 모형으로 당뇨환자관리모형을 개발한 결과 의사결정나무가 가장 설명력이 뛰어났다. 당뇨인지율과 관련된 요인으로는 연령, 거주지 및 직업이었고 중 연령이 가장 중요한 요인으로 나타났다. 당뇨치료율과 관련된 요인으로는 당뇨인지여부, 거주지 및 직업이었고 그 중 당뇨인지여부가 가장 중요한 변수로 나타났다. 당뇨환자의 관리프로그램은 당뇨환자의 특성별 군집으로 분류하고 그에 따라 관리해야 한다.

"동의보감(東醫寶鑑)" 등재(登載)의 의의(意義)와 현대적(現代的) 활용(活用) (The Significance of Dongeuibogam Registration and Its Modern Application)

  • The registration of Dongeuibogam as UNESCO's Memory of world was not only a national triumph but also an internationally important event. This event got rid of the opinions that suggested Dongeuibogam was a medical system practiced only un the limited area of Korea and this event signified the restoration of Dongeuibogam's international and universal position. Therefore, ih is now our job to find Dongeuibogam's modern applications and restore its value. This study was carried out with the purpose of examining the significance of Dongeuibogam's registration as Memory of world from a medical history's point of view and finding its modern applications. The author discussed significance in three parts of Past Significance, Present Significance and Future Significance. As for modern applications, the author reviewed currently used applications and considered the possible ways to restore its original value as an alternative for future medical science.
