Exploratory Structural Analysis on Eight Positions for New Formulations in Jingyuequanshu by Network Analysis (네트워크 분석을 이용한 『영악전서(景岳全書)』 신방팔진(新方八陣)의 탐색적 구조 분석)

  • Background: Eight positions for new formulations in Jingyuequanshu is a unique classifying system and each position can be presented as a network. Network analysis is an effective method of visualizing the relationship of textual information. Purpose: This study aimed to analyze the structure of the eight positions for new formulations in Jingyuequanshu, and to acquire information about composing materia medica. Methods: We constructed an initial database and binary matrix of eight positions for new formulation in Jingyuequanshu, including its herbs and formulations. With this data, we carried out frequency and network analysis. Results: We analyzed each of the eight positions for new formulation, entire positions, and five positions after removing 'causal/cold/hot' positions. We found that the formulas of the causal position are distributed throughout the new formulation, and those of the cold and hot positions are also distributed similarly but the two groups are exclusive to each other. The other 5 positions are distributed exclusively to one another. Conclusions: Eight positions for new formulation of Jingyuequanshu were classified into three axes by exploratory network analysis. One is an axis of causal position, another is an axis of cold/hot positions, and the last is an axis of the other five positions.

  • Jing-Yue Jang(張景岳, 1563{\sim}1640$), a medical scalar in the late Ming(明) Dynasty wrote many books such as Leijing(類經), Zhilyilu(質疑錄) and Jingyuequanshu(景岳全書). He was also famouse for the concept of 'Warming and Recuperating(溫補)' and 'Syndrome Differantation Treatment(辨證論治)', Jingyuequanshu was written between 1636 and 1640, and first published in 1700, It can be said that the book was imported from Ming Dynasty of China to Chosun(朝鮮) Dynasty, in what is now South Korea. between 1713 and 1724 In eighteen century. the book was quoted in Euimunbogam(醫門寶鑑), Jejungshinpyun(濟衆新篇) and Magwaheotong(麻科會通). In nineteen century. the book was quoted in Euijongsonik(醫宗損益), Bangyakhappyun(方藥合編), Dongeuisusebowon (東醫壽世保元) and Euigamjungma(醫鑑重磨). The number of quoted prescription and thesis during in nineteen century. is much more than that of eighteen century, Specifically, there are thirty prescriptions of Xinfangbazhen(新方八陣) in Bangyakhappyun, and the concept of 'Nourishing Yang(扶陽)' in Euigamjungma is also based upon Jing-Yue's thesis, This fact shows that we cannot consider the medicine of the late Chosun Dynasty as an abridgement of Dongeuibogam(東醫寶鑑). It is also said that the study focusing on medical books imported from China to Korea in this period is related to understanding the medical progress in the late Chosun Dynasty and the medical intercommunication between Korea and China.

  • Various materials related the names of objects were published in the late Joseon Dynasty. The medicine-related vocabulary contained in these data is important data for examining the practice of medicine at the time. Jaemulbo (Genealogy of talent and things) is a document on the names of things written by Lee Man-young in 1798. Through this study, the medical vocabulary in Jaemulbo was broadly investigated and analyzed and the following claims are made: 1. Jaemulbo provides a wealth of information on the medical terminology used in the 19th century. 2. The selection of medical vocabulary included was made according to the purpose of tools such as cultured books and dictionaries. 3. Jaemulbo is an example of using Jingyuequanshu (Whole book of Jingyue) before 1790. 4. Jaemulbo reflects the experience and knowledge of pharmacology after Donguibogam.

A Literature Study on interrogation (문진(問診)에 대한 서지학적 고찰)

  • Objectives : The treatment starts with physical examination. The four examinations(四診) is a method to examine the disease. The books on history of diagnostics about it contained substantial contents about inspection(望診) and palpation including pulse taking(切診), while they contained little contents about auscultation(聞診) and interrogation or questioning(問診) relatively. For this reason, this study was conducted as on interrogation first of all. Methods : I looked into the contents related to interrogation that were scattered in plenty of books. There were chapters of specialty in interrogation since the Ming dynasty, so I looked into these chapters in chronological order. Results & Conclusions : The level of the contents of interrogation prior to the Ming dynasty were rudimentary and recapitulative. However, there was a turning point by three kinds of books appeared in the Ming dynasty. Yixuerumen Wenzheng dealt with detailed contents. Yixuezhunshengliuyao Wenbingbixiang extended the range of contents. Jingyuequanshu Shiwenpian systematized Eight Principle Pattern Identification(八綱辨證) with increase of contents. Since then, books of the Qing Dynasty were mostly based on three kinds of books ahead. Among these, Yimenfalu Yimingwenbingzhifa(醫門法律 一明問病之法) and Xingsewaizhenjianma wenfa(形色外診簡摩 問法) mentioned the importance of a succinct interrogation that hit the mark and Yibian Yiwenzheng dealt with another detailed contents unlike the previous books based on substantial contents.

  • Objectives : As for the effect of Polygonum multiflorum on malaria treatment, this study analyzed the characteristics of Herenyin use in Korea and the contents mentioned in major Herbs books of the past. Methods : This study searched, collected and analyzed in the literature and data DB including the contents in herbs books on the treatment of malaria with Polygonum multiflorum, and the contents of various medical books about Herenyin used in Korea, etc. Results :Since Ming and Qing dynasties, Polygonum multiflorum was used mainly for patients with malaria, whose vitality gets weakened with time. Such effects are described in numerous Herbs books. Herenyin, consisting of major medicinal materials of Polygonum multiflorum and ginseng, was also frequently used in malaria treatment in Korea in late Joseon, rather than in China since it was listed in "Jingyuequanshu". Conclusions : Based on the mechanism studies of Polygonum multiflorum with malaria treatment effects and the study of malaria treatment medication including Polygonum multiflorum, it is hoped that the development of new therapies for complete eradication of malaria will be made in the future.

A Study on Zhang Jiebin's Discussion of Treating Insomnia (장개빈(張介賓)의 불면(不眠) 논치(論治) 연구(硏究))

  • Objectives : This study aims to improve the diagnosis and treatment of contemporary insomnia by examining Zhang Jiebin's discussion on treating insomnia. Methods : The classical texts from the 'Insomnia' chapter of the Jingyue Quanshu were examined threefold in terms of symptom, treatment, and prescription analysis, after which the treatment discussion part was examined within the historical context of discussions on insomnia in major medical texts starting from the Huangdineijing. Results : According to Zhang, the cause of insomnia could be divided into two, after which criteria for diagnosis and treatment were set as excess pathogen and vital qi deficiency. He argued that insomnia could be naturally resolved through improvement of various pathogenic situations. Discussions on insomnia from various medical texts since the Huangdineijing suggest that pathology related to psychological function and emotions gradually increased and expanded over time. Conclusions : Zhang's discussion on symptom, treatment and prescriptions of insomnia suggests a new framework that could improve treatment effects through a Korean Medical Mind-Body approach, rather than the contemporary classification of organic insomnia and non-organic insomnia.

A Study on the Korean Medical Treatment of Recurrent Pregnancy Loss: A Contemporary Understanding and Application of Classical Texts (습관성 유산의 한의학적 치료에 대한 연구 - 고문헌의 현대적 이해와 활용 -)

  • Objectives : The aim of this study is to establish foundation for applied research and clinical application of the treatment principle and imperativeness for Korean Medical treatment of recurrent pregnancy loss through the study of classical texts. Methods : The Donguibogam, Yixuerumen, Jingyuequanshu, together with gynecological texts such as the Fuqingzhunuke and the Nukejinglun were studied in terms of treatment, post-miscarriage cultivation methods and prevention for various types of pregnancy loss. Results : For cases of missed miscarriage, Foshousan is applied, while for recovery post surgery, Shenghuatang-based formulas are applied. For cases of imminent miscarriage, prescriptions should be selected based on major symptoms such as stomach pain, lower back pain and bleeding, while for cases of chemical miscarriages, treatment methods that cultivates the Liver and calms qi should be applied. After pregnancy loss, elimination of stagnant blood should be prioritized according to the principle of 'first eliminate blood, then tonify deficiency'. For prevention of pregnancy loss, Jinguidangguisan-based formulas should be applied according to medicinal administration principles of Korean Medicine. Conclusions : For an integrative treatment and prevention of recurrent pregnancy loss, Korean Medical treatment according to clinical representation is required.

A Comparative Research of Eight Principle Pattern Identification - based on Zhang Jie-Bin, Cheng Guo-Peng, and Jiang Han-Tun - (팔강변증(八綱辨證)에 대한 비교 연구 - 장개빈(張介賓).정국팽(程國彭).강함돈(江涵暾)을 중심으로 -)

  • Objective : Syndrome differentiation(辨證) has clinical importance in East Asian Traditional Medicine. There are several ways for Syndrome differentiation. However, Eight Principle Pattern Identification(八綱辨證) is the base of other Syndrome differentiations. Of the physicians focused on Eight Principle Patterns(八綱), I have researched for Eight Principle Pattern Identification concerning with Zhang Jie-Bin(張介賓), Cheng Guo-Peng(程國彭), and Jiang Han-Tun(江涵暾) in Ming(明) and Qing(淸.) Method : Applying to Eight Principle Pattern Identification, I have researched comparatively for 3 kinds for. First, Zhang Jie-Bin's Jingyuequanshu(景岳全書) that involves Yinyangpian(陰陽篇), Liubianbian(六變辨), Biaozhengpian(表證篇), Lizhengpian(裏證篇), Xushipian(虛實篇), and Hanrepian(寒熱篇), secondly, Cheng Guo-Peng's Yixuexinwu(醫學心悟) Hanrexushibiaoliyinyangbian(寒熱虛實表裏陰陽辨), at lastly Jiang Han-Tun's Bihuayijing(筆花醫鏡) Biaolixushihanreyinyangbian(表裏虛實寒熱辨). Results : All of sick cases can be explained totally by Eight Principle Patterns. Of Eight Principle Patterns, Yin(陰) and Yang(陽) include last Six Principle Patterns(六綱 : 表裏, 寒熱, 虛實). Six Principle Patterns can be divided normally by 6 pulses(六脈 : 浮沈 遲數 虛實). In all of pain cases, feeling comfortable(可按) or discomfortable(拒按) to palpation can be important foundation for distinguishing Xu(虛) from Shi(實). Physical constitution(體質) for Hanre Xushi(寒熱 虛實) and tongue-diagnosis(舌診) for Biaoli Hanre(表裏 寒熱) are used effectively. Related with tongue-diagnosis, tongue-coating(舌苔) for Biaoli and tongue status(舌質) for Hanre are used effectively. Symptoms should be divided following this sequence, Biaoli ${\rightarrow}$ Hanre ${\rightarrow}$ Xushi and lastly should be summarized of Yinyang. Conclusion : Diagnosing with Eight Principle Patterns, digestive function, urine, and feces should be checked at first. In addition, the pulse, tongue, physical constitution, and good or bad from palpation(觸診), these should be checked and give a result. And then the result can be an important evidence of syndrome differentiation. As a result, it would be the best to diagnosis that discriminating the sequence as Biaoli Hanre Xushi and summarizing with Yinyang.

A Review on the Concept Establishment of Stagnation Syndrome (울증(鬱證)의 개념 정립에 관한 문헌고찰)

  • Objectives: The purpose of this study was to identify the meanings and characteristic of the stagnation syndrome, a distinctive clinical syndrome in traditional Korean medicine (KM). Methods: The major ancient Oriental medicine literature, including Huangdi neijing (黃帝內經), Danxixinfa (丹溪心法), and Jingyuequanshu (景岳全書) were examined to identify the semantic change of the stagnation syndrome (鬱證). Also, recently published articles about the stagnation syndrome were searched from databases including MEDLINE, CENTRAL, KMBASE, KISS, NDSL, and OASIS. Results: The term of stagnation was originally used to describe not flowing and clogged situations, and the stagnation syndrome appeared as an independent syndrome in Danxixinfa. As the etiology became more sophisticated over time, emotional factors were mentioned for one of the causes of the stagnation syndrome. However, the major causes and symptoms of the stagnation syndrome were somatic factors. Various articles about stagnation were searched, and most of them used "stagnation" as the KM syndrome subtype of disease, some of them referred to the "stagnation syndrome" as an independent syndrome. The recently defined stagnation syndrome commonly shows distinctive symptoms of chest stuffiness, and an obstructing sensation in the throat. Conclusions: The semantic changes and characteristics of the stagnation syndrome were examined through searching ancient and modern literature. The meaning of the stagnation syndrome has evolved over time, and at its center, there are somatic and mental symptoms characterized by stagnation, distinguished from the depressive disorder.

Analysis of Differential Diagnosis System in Sasang Typology on the Basis of Greater-Lesser-Yin-Yang and Eight Principle Pattern Identification (태소음양(太少陰陽)과 팔강(八綱)의 분석에 근거한 사상인 변증분류체계 연구)

  • The purpose of this study is to evaluate the clinical implication of Eight Principle Pattern Identification, EPPI in Sasang Typology. Although EPPI is considered as the most basic differential diagnosis in Oriental Medical Science, it has not been used actively enough in clinical practice owing to the presumption that EPPI is not sufficient for the analysis of complex clinical symptoms. In Zhang Jiebin's Jingyuequanshu, he separates Yin-Yang with Six-identification in EPPI when explaining them. This implies that it is difficult to talk about each of EPPI in same perspective. Thus, for the active clinical usage of EPPI it is necessary to study in hierarchical perspective. Lee Je-ma's Sasang typology could be a good example that uses the hierarchical perspective of EPPI. In the book Dongeuisoosebowon Lee Je-ma talks about differentiating Yin with Yang by constitution, Exterior with Interior and Cold with Heat accordingly, to apply in prescriptions. After differentiation, Yin and Yang are placed in higher level from the rest of EPPI, thus Yin and Yang can influence the rest of EPPI. This study showed the importance of EPPI in understanding the differential diagnosis system of Sasang typology in clinical perspectives.
