One case Treated Cerebral Infarction with Aphasia by Jihwangumja (지황음자(地黃飮子)를 투여(投與)한 중풍(中風) 실어증(失語症) 환자 치험1례)

  • Aphasia is speech disorder caused by injuries on the speech nerve center. It usually occur due to a disease in the right cerebral cortex and is divided into the various aphasia such as Global aphasia, Broca's aphasia, Wernicke's aphasia, conduction aphasia, Anomic aphasia, etc. Jihwangumja is used the cerebral infarction with Aphasia due to deficiency syndrome of kidneys. The purpose of this study is to examine the efficacy of oriental treatment for cerebral infarction with aphasia by Jihwangumsa. In the hospital, the patient showed the symptoms of mental disorder, Rt. hemiparesis G3/G4, aphasia, chest discomfort, obstipation, frequent urination, etc. The case showed that acute infarction on Lt. fronto-temporo-parietal lobe in Brain-CT. We identified the patient's clinical conditions and treated accordingly. As a result of treatment, symptoms were markedly improved and he was discharged. Further elaboration of oriental diagnostic classification could possibly lead to the fundamental treatment.

  • Subcortical Aphasia is not among the typical eight types of aphasia. It is characterized as cerebral infarction or hemorrhage occurring in the region of the left basal ganglia internal capsule, accompanied with ventrical rounding and white matter. Subcortical aphasia is similar to transcortical aphasia in its symptom of repetition, and classification is difficult. Language ability can change rapidly over several months so a patient's convalescence is unpredictable. Because of the peculiarities of subcortical aphasia there are many conflicting hypotheses. Various clinical approaches and medical treatments for speech disorder(aphasia) have been presented. In Korean medicine, there are many studies about speech disorder, but few reports are related to subcortical aphasia. An aphasic stroke patient who was treated with acupuncture, herbal medicine(Jihwangumja), and speech-language therapy is here reported. Significant improvement in language abilities was seen over three months. Further clinical study of subcortical aphasia is needed.

A Case Report of the Effect of Korean Medicine on Liver Function Test of Drug-Induced Liver Injury Patient Treated with Western Medicine (약인성 간손상 환자에게 협진을 위한 한약의 추가 처방이 간수치에 미치는 영향에 대한 증례보고)

  • Objective : The purpose of this study is to report that patient diagnosed as drug-induced liver injury treated with herbal and western medicine improve liver function test. Methods : We treated the patient diagnosed as drug-induced liver injury caused by antibiotics with Hepatotonics and Cheonggan-tang Gagam(淸肝湯加減). Then, We followed up the result of liver function test. After the result of liver function test improved, we diagnosed the patient with Korean medical methods, and evaluated the symptom of Cerebral infarction and liver function test treating the patient with Jihwangumja (地黃飮子). Result : After our treatment, Liver function test and symptoms of Cerebral infarction result improved. Conclusion : Use of Western medicine with herbal medicine can improved liver function test of drug-induced liver injury effectively and herbal medicine do not disturb the liver function.

Korean Medical Treatment and Language Therapy in Patient with Fluent Aphasia after Stroke : Case Report (한방치료 및 언어치료를 병행한 중풍 유창성 실어증 환자 치험 1례)

  • In this study, we evaluated the effects of herbal medicine, acupuncture and language therapy in a patient with fluent aphasia after stroke. We treated the patient with Jihwangumja, special acupuncture points and Language-Oriented Treatment(LOT). The language abilities of the patient in this study was markedly increased in Korean-Western Aphasia Battery(K-WAB) and Communicative Ability in Daily Living Test(CADLT). Aphasia quotient, fluency, comprehension, repetition and naming score of K-WAB were improved in second and third examination compared with those of the first examination. Each item of CADLT was improved in second examination compared with that of the first examination.
