  • Arsenic trioxide is one of the 'tooth pulp devitalizing agents' used through the dental history when anaesthesia was not available. But owing to its capacity to kill cells in surrounding tissues, the use of arsenic trioxide in vital pulpotomy has been reduced. Arsenic trioxide can cause necrosis of gingiva, bone which can cause osteomyelitis of the jaws. But some dentists still continue to use arsenic trioxide in their endodontic practices. The purpose of this article is to present arsenic trioxide induced osteomyelitis on maxilla and treatment process.

악안면 두개골격의 수직${\cdot}$수평적 부조화에 관한 두부방사선 계측학적 연구 (A CEPHALOMETRIC STUDY ON THE VERTICAL AND ANTEROPOSTERIOR DYSPLASIA OF THE CRANIOFACIAL SKELETON)

  • The purposes of the present study were to describe the vortical and anteroposterior facial dysplasia, and to identify morphologic differences associated with various facial patterns. Anteroposterior dysplasia was classified by traditional Angle's malocclusion classification and according to vertical relationships based on the SN-MP angle, facial pattern was subclassified to 3 vertical groups in each malocclusion group. Each vertical group composed of 20 samples and total 180 samples aged from 9 to 14 years. The results were as follows; 1. The skeletal differences that lead to disportionate lower facial height in vortical and anteroposterior facial dysplasia were closely related to mandibular morphology. 2. Hyperdivergent group, compared with hypodivergent group, demonstrated decrease of SNA and SNB and linguoversion of maxillary and mandibular central incisors in all malocclusion groups. 3. Irrespective of rotation of the jaws, Wits appraisal was a useful measurement in evaluation of relative anteroposterior relationship of maxilla and mandible. 4. As SN-MP angle increased, anterior facial height, especially lower anterior facial height, demonstrated significant increase and intermaxillary space also tended to increase. 5. The correlation coefficients of SN-MP angle and PTFH/ATFH demonstrated the highest value in all malocclusion groups.

한국인 아동의 악안면 구조의 사변형 분석에 관한 연구 (A STUDY ON QUADRILATERAL ANALYSIS OF FACIAL CONFIGURATION IN KOREAN CHILDREN)

  • The quadrilateral analysis is a proportional analysis which evaluates the skeletal configuration of lower face on the relations between both jaws in the horizontal as we]1 as vortical dimensions. This study was undertaken to analyse the harmony and disharmony of quadrilateral patterns in normal occlusion and malocclusion. The present study was carried out on lateral cephalograms of 530 Korean children; the subjects consisted of 135 normal occlusions (63 male and 72 female), 105 Class II division 1 malocclusions (52 male and 53 female), 109 Class III malocclusions (50 male and 59 female), 91 hypodivergent facial types (44 male and 47 female) and 90 hyperdivergent facial types (45 male and 45 female). The following conclusions were reached: 1. Means and standard deviation in each group and sex were obtained from normal occlusion and malocclusion. 2. Quadrilateral mean diagram in normal occlusion was constructed for male and female, respectively. 3. In normal occlusion, 1:1 ratio exists between the maxillary base length (A' to Ptm') and mandibular base length (B' to J'), but lower facial height is targer than above. 4. Difference is effective to estimate the degrees of Class II and Class III malocclusion, and lower facial height (LFH) and sagittal angle is effective to recognize the hypodivergent and hyperdivergent facial type. 5. Quadrilateral analysis is able to visualize the anteroposterior and vertical dysplasia of lower face, and it is helpful to recognize certain problems in malocclusion.

함치성 낭종에 대한 방사선학적 연구 (A RADIOLOGIC STUDY OF DENTIGEROUS CYSTS)

  • A radiologic study of 139 dentigerous cysts (128 patients) indicated the followings: 1. There was higher incidence in males (62.99%) than in females (37.01%). The most common age group was 11-20 years and the average age was 23.6 years. 2. The most common clinical symptoms was swelling of the jaws (49 cases, 33.09%), and no symptoms was presented in 26 cases (18.71%). 3. Dentigerous cysts were found to be slightly more common in the maxilla (53.24%) than in the mandible (46.76%) and there was almost no difference in occurance between right and left side. The maxillary supernumerary tooth was the most frequent site of the dentigerous cyst and no dentigerous cyst was found to be related to a deciduous tooth. 4. The most common radiographic findings was root resorption of the adjacent teeth (33.09%), and the central dentigerous cysts were 78 cases (56.12%), the lateral dentigerous cysts were 61 cases (43.88%). 5. The increased radiolucency at the crown portion of the tooth in a cystic cavity was seen in 44 cases (31.65%), and a case of dentigerous cyst was found in a edentulous patient, and 16 cases of multiple dentigerous cysts were found in 5 patients. 6. The ameloblastic changes in 8 cases, the keratinization of the cyst wall in 2 cases, and a case of epidermoid carcinoma were confirmed microscopically.

백악질화성섬유종에 관한 연구 (A STUDY OF THE CEMENTIFYING FIBROMA)

  • Cementifying fibroma is an odontogenic tumor of periodontal ligament origin, consisting of a proliferation of fibrous tissue that forms cementum. Cemento-ossifying fibroma, cementifying fibroma and ossifying fibroma are difficult to distinguish on clinical and radiographic exmination, and on histopathology. However, when the calcified product is represented by ovoid or curvilinear deposits, the lesion is often referred to as cementifying fibroma. Cementifying fibromas of the jaws are well-circumscribed, generally slow-growing lesions. Although most lesions appear to be limited to the tooth-bearing areas, a few have extended into the angle-ramus area or encroached on the maxillary sinus. Radiographically, the cementifying fibroma could present as a radiolucent, radiopaque or mixed density lesion, depending on the degree of maturity. We have observed two cases of cementifying fibroma occurred in the left maxillary premolar-molar area of 38-year-old woman and 35-year old man. We obtained that two cases were shown the followed results; 1. Clinically, main clinical symptom was facial swelling and pain at palpation 2. Radiographically, well-circumscribed radiolucent lesion and radiolucent lesion with radiopaqu e foci were seen on the left maxillary premolar-molar areas. Loss of lamina dura and root resorption or divergency were also seen. 3. Histopathologically, many cementum-like calcifications were observed in the hypercellular connective tissue.

치아종의 방사선학적 연구 (A Radiographic Study of Odontoma)

  • The purpose of this study was to obtain information on the clinical and radiographic features of the odontomas in the jaws. For this study, the authors examined and analyzed the clinical records and radiographs of 119 patients who had lesion of odontoma diagnosed by clinical and radiographic examinations, The obtained results were as follows: 1. Odontoma occurred the most frequently in the 2nd decade(45.4%) and occurred more frequently III males (60.5%) than in females(39.5%). 2. The most common clinical symptom was the delayed eruption of the teeth(34.2%). 3. The type of lesions was mainly observed as compound odontoma(80.8%), and internal pattern of the complex odontoma was unevenly radiopaque(73.9%). 4. The compound odontoma frequently occurred in anterior portion of the maxilla(57.7%) and mandible(30,9%), and complex odontoma frequently occurred in anterior portion of maxilla(34.8%) and posterior portion of mandible(30.5%). 5. The effects on adjacent teeth were impaction of teeth(71.7%) and prolonged retention of deciduous teeth (31.7%), 6. The impaction of the teeth occurred in anterior portion of maxilla (44.2%) and mandible ( 19.2%), but root resorption of the adjacent teeth were not seen, 7. The boundary to adjacent structure was well-defined, the lesions appear as radiopaque mass with radiolucent rim.

  • Ameloblastoma is the most common aggressive benign odontogenic tumor of the jaws. Because of slow growth and tendency to local invasion of bone and soft tissue, high rates of recurrence are common. The treatment for ameloblastoma is still controversial and poses some special problems in children. Because of growth of the jaw and the different incidence, prognosis of the tumor make the surgical consideration different from adults. Radical resection cause facial deformity, jaw abnormal movement and masticatory disturbance especially to child and adolescents. So conservative treatment as enucleation, curettage is acceptable initial treatment of ameloblastoma in children who can be followed up in a precise, detailed manner. This report describes a case of unilocular plexiform ameloblastoma treated by enucleation and curettage followed by marsupialization.

The impact of the alveolar bone sites on early implant failure: a systematic review with meta-analysis

  • Dental implants are the first option for replacement of missing teeth. Failure usually involves additional cost and procedures. As a result, the physician should limit the risk factors associated with implant failures. Implant site is one of many factors that can influence the success or failure of dental implants. The association between early implant failure (EIF) and implant site has yet to be documented. This review aims to estimate the impact of insertion site on the percentage of EIFs. An electronic and manual search of studies that reported early failure of dental implants based on collection site. A total of 21 studies were included in the review and examined for the association between EIF and alveolar site. Subgroup analysis, including a comparison between implants inserted in four alveolar ridge regions of both jaws was performed. The early failure rate was higher for maxillary implants (3.14%) compared to mandibular implants (1.96%). Applying a random effect, risk ratio (RR), and confidence interval (CI) of 95% revealed higher failure in the maxilla compared to the mandible (RR 1.41; 95% CI [1.19, 1.67]; P<0.0001; I2=58%). The anterior maxilla is more critical for early implant loss than other alveolar bone sites. Implants in the anterior mandible exhibited the best success rate compared of the sites.

Implant-supported overdenture with prefabricated bar attachment system in mandibular edentulous patient

  • Implant-supported overdenture is a reliable treatment option for the patients with edentulous mandible when they have difficulty in using complete dentures. Several options have been used for implant-supported overdenture attachments. Among these, bar attachment system has greater retention and better maintainability than others. SFI-Bar$^{(R)}$ is prefabricated and can be adjustable at chairside. Therefore, laboratory procedures such as soldering and welding are unnecessary, which leads to fewer errors and lower costs. A 67-year-old female patient presented, complaining of mobility of lower anterior teeth with old denture. She had been wearing complete denture in the maxilla and removable partial denture in the mandible with severe bone loss. After extracting the teeth, two implants were placed in front of mental foramen, and SFI-Bar$^{(R)}$ was connected. A tube bar was seated to two adapters through large ball joints and fixation screws, connecting each implant. The length of the tube bar was adjusted according to inter-implant distance. Then, a female part was attached to the bar beneath the new denture. This clinical report describes two-implant-supported overdenture using the SFI-Bar$^{(R)}$ system in a mandibular edentulous patient.

외부광 차단을 위한 설진기 안면접촉부 설계 (Structural Design of Facial Contact Parts in Computerized Tongue Diagnosis System to Block Out External Light)

  • Objectives The aim of this study is to design a part in contact with the face of computerized tongue diagnosis system (CTDS), so that external light is effectively shielded even if the facial appearance and degree of protrusion differ when a patient opens or closes his/her jaws. Methods Each of the 4 researchers manually produced clay models of the part in contact with the face of CTDS. Shielding and contact feeling of the clay models were evaluated by 20 assessors. Based on the evaluation, we selected the appropriate model and produced the final silicon model. Then we evaluated the performance of the shielding of the completed silicon model. We took tongue pictures of 60 participants with a CTDS applying the silicon model in condition with external light and without it. The color values in RGB color model and gray scale of the tongue pictures in condition with external light were compared with those without external light. Results There was no significant difference between the color values of the picture taken in condition with external light and those without external light. Conclusions We concluded that the produced part in contact with the face of CTDS can effectively block out the external light.
