Primary intraosseous squamous cell carcinoma mimicking periapical disease: a case report

  • Primary intraosseous squamous cell carcinoma (PIOSCC) is a rare carcinoma, which arises within the jaws without connection to the oral mucosa and presumably develops from a remnant of odontogenic epithelium. We present a case of solid type PIOSCC in a 52-year-old male patient complaining of dull pain on his left lower molar. In this case, early stage PIOSCC mimicking a periapical lesion might lead to a one-year delay in treatment due to the misdiagnosis of osteomyelitis after extraction of the third molar. The clinical, radiological, and histologic features are described. In this case, there was initial radiographic evidence for PIOSCC mimicking a periapical lesion. Incautious radiographic interpretation and treatment procedures had delayed the correct diagnosis and resulted in extensive bony destruction during the patient's disease progression.

Dentigerous cyst associated with an impacted mesiodens: report of 2 cases

  • Dentigerous cysts are the most common developmental cysts of the jaws, most frequently associated with impacted mandibular third molar teeth. Dentigerous cysts around supernumerary teeth, however, account for 5% of all dentigerous cysts, with most developing around a mesiodens in the anterior maxilla. This report describes two cases of a dentigerous cyst associated with an impacted mesiodens. Both of the patients complained of swelling in the maxillary anterior region. Radiographic examination revealed an impacted mesiodens surrounded by a large corticated radiolucency in both cases. A provisional diagnosis of infected odontogenic cyst was made. The cysts were enucleated with the removal of the mesiodens in the two cases. Histopathological examination confirmed the diagnosis of infected dentigerous cyst associated with an impacted mesiodens in both cases. The patients remained asymptomatic, and no complications were noted.

석회화치성낭의 두 증례보고 (Two cases report of Calcifying Odontogenic Cyst)

  • The calcifying odontogenic cyst (COC) is a rare disorder of the jaws and shows various radiographic features. The purpose of this study is to describe the different radiographic appearances of 2 cases of COC. Case 1 was located in the posterior maxilla extending into maxillary sinus, showing unilocular radiolucency with a well-defined margin. Cortical bone expansion and thinning were prominent. Root resorption of adjacent teeth was apparent. Case 2 showed unilocular radiolucency with a calcified material. Calcification was supposed to be dystrophic dental hard structures, detected at the periphery of the lesion. Ghost cell and proliferation of ameloblastoma-like tissues were common features for these two lesions on histopathological findings. This reports presented common and atypical radiographic features of the COC.

The effect of decompression as treatment of the cysts in the jaws: retrospective analysis

  • Objectives: The purpose of this study is to evaluate the treatment efficacy of enucleation after decompression. Materials and Methods: A total of 17 patients with cystic lesion of the jaw were treated with decompression followed by enucleation. Pre- and post-decompression panoramic radiographs were analyzed. Results: The mean percentage of reduction after decompression was 64%. The reaction was graded as good (>80%) in five patients (29.4%), moderate (50%-80%) in nine patients (52.9%), and poor (<50%) in three patients (17.6%). The reduction rate of larger cystic lesions was faster than that of smaller lesions. However, the reduction rate was not affected by age. The duration of follow-up ranged from one to eight years. There were no complications, and one case recurred. Conclusion: Decompression is an effective method for the initial treatment of jaw cysts.

치과 임플랜트 보철 장착자의 구강 감각 기능에 관한 연구 (A STUDY ON ORAL SENSORY FUNCTION IN THE IMPLANT-SUPPORTED PROSTHESES WEARERS)

  • In this study, oral sensory function in the osseointegrated root form implant-supported prostheses wearers was estimated by measuring occlusal tactile perception threshold of thickness and sensibility threshold against lateral static loading, and comparing with normal dentition subjects and complete denture wearers group. Osseointegrated root form implants seemed to be restored in the sensation to some extent, and so, dental implants restored edentulous patients in a wide meaning. Conclusions were summarized as following. 1. Occlusal tactile perception threshold of thickness was highest in complete denture wearers group, following by implant-supported prostheses wearers group, normal dentition subjects group. 2. In the implant-supported prostheses wearers group, occlusal tactile perception threshold of opposing artificial teeth case was higher than of opposing natural or opposing implantsupported teeth case. 3. Sensibility threshold against lateral loading of complete denture wearers and implantsupported prostheses wearers group was higher than that of normal dentition subject group. 4. In the implant-supported prostheses group, sensibility threshold against lateral loading was not significantly different between upper and lower jaws. 5. In occlusal tactile perception threshold of thickness and sensibility threshold against lateral loading test, there was no regularity among values of each tooth, and no significant difference between anterior and posterior teeth as well.

Use of separate single-tooth implant restorations to replace two or more consecutive posterior teeth: a prospective cohort study for up to 1 year

  • PURPOSE. The aim of this study was to evaluate the periodontal and prosthodontic complications of multiple freestanding implants in the posterior jaws for up to 1 year of function. MATERIALS AND METHODS. Eight patients received 20 implants posterior to canines. Two or more implants were consecutively inserted to each patient. Single crowns were delivered onto the implants. Marginal bone loss, implant mobility, probing depth, and screw loosening were examined to evaluate the clinical success of such restorations for maximum 1 year of functional loading. RESULTS. All the implants performed well during the observation period. Neither periodontal nor prosthodontic complications were found except a slight porcelain chipping. While the marginal bone level was on average 0.09 mm lower around the implant after 6 months of loading, it was 0.15 mm higher after 1 year. CONCLUSION. Within the limits of this investigation, separate single-tooth implant restorations to replace consecutive missing teeth may clinically function well in the posterior jaw.

가족성 섬유성 이형성증(Cherubism)에 대한 고찰 (A REVIEW ON CHERUBISM)

  • Cherubism is an uncommon, benign fibrosseous lesion of the jaws that causes a progressive, painless, symmetrical expansion of the maxilla and mandible. It's autosomal dominance pattern of inheritance has been confirmed. A characteristic deformity is specific to this disease: hypertrophy of the mandible, swelling of cheeks and sometimes hypertrophy of maxilla with eyes tending to look up ; that looks like the Renaissance cherubs. It usually makes figures during childhood between 2-4 years of age and progresses until puberty, after which it spontaneously regresses 10 most cases. As a result of this case review, the management strategies for cherubismic children in standpoint of pediatic dentistry can be summarized as follows: 1. It can be detected early in childen through its characteristic clinical and radiographic features, which is confirmed by histopathological examination and familial history. 2. The supervision of arch space is required against its frequent sequelae, the early missing of primary teeth and eruption disorders of permanent teeth. 3. It is highly recommended to continue the periodic check-up with clinical and radiographic examination, leading to surgical intervention in cases of aggravation.

악골에 발생한 점액종(섬유점액종) (Myxoma(Fibromyxoma) of the Jaw)

  • The auhors have studies clinically and histopathologically on the 6 rare cases of odontogenic myxoma or fibromyxoma of the jaw which were obtained from 72 odontogenic tumor of Biopsy cases during 10 years from 1961 to 1971 at the Dept. of Oral Pathology, college of Dentistry, Seoul National University. The results are are follows; 1. In 72 cases of odontogenic tumor, myxoma or fibromyxoma were 6 cases (8.34%) 2. 4 cases in 6 cases of myxoma (fibromyxoma) were occurred in male and 2 cases were in female, and the median age of 6 cases was 27 and the age range was 4 to 44. In the location, each 3 cases located in upper and lower jaw. 3. All 6 cases were occurred in jaws with central origin by expansive slowly growth, painless. the operation prognosis was almost good without one case. 4. The histopathological findings of 6 cases consists of loosely arranged stellate shaped cells with long, anastomosing cytoplasmic process. The cytoplasm of these cells are basophilic poorly stain and nuclei are avoid and hyperchromic.

악골에 발생한 낭종의 임상적 연구 (CLINICAL STUDY OF CYSTS IN THE JAWS)

  • A clinical study of selected patients with cyst which were managed in the Department of oral and maxillofacial sugery from March 1994 to February 1998 was done. An following results were obtained 1. Male were involved more than females by cyst in a ratio of 1.95:1 and most of the cases occurred in the secondary decades 2. The primary site of radicular cysts were the anterior incisor area, dentigerous cysts were anterior incisor area and canine area, incisive canal cysts were maxillary anterior area, odontogenic keratocyst were the mandibular ramus area. 3. The common symptoms were swelling(65 cases), pain(12 cases) and the mean duration of syptomatic period was 10 days. 4. The rates of histopathologic classification were radicular cyst(58%), dentigerous cyst(22%), incisive canal cyst(9%), odontogenic keratocyst(11%). 5. Average of Alk. phosphatase was 235(IU/L) at pre-twenty age and 102(IU/L) at post-twenty age. 6. In treatment modalities, enucleation was most common, odontogenic keratocyst was treated by enucleation and curettage for prevention of recurrence.

Bisphosphonate, 구강악안면외과 영역의 새로운 위험 요소인가? (BISPHOSPHONATE, IS IT AN EMERGING RISK FACTOR IN ORAL SURGERY?)

  • Since the first description of bisphosphonate related osteonecrosis of the jaw (BRONJ) in 2002, the number of report on the disease has rapidly been increasing. Now, BRONJ is considered as a new entity, which is emerging problem in oral and maxillofacial surgery. Bisphosphonates (BPs) can be categorized into 2 groups: nitrogen-containing and non-nitrogen containing, and nitrogen-containing BPs are considered to have more efficacy and toxicity possibly. It is unusual for osteonecrosis to occur in the maxilla but BRONJ is found in both the mandible and the maxilla, which is one of the special features of BRONJ compared with common infectious osteomyelitis of the jaws. Intravenous BPs are usually more likely to cause BRONJ than oral BPs which are frequently prescribed for osteoporosis and osteopenia. Nonetheless, the use of intravenous BPs cannot be prevented because of systemic condition of the patients. Although it is rare that oral BPs cause BRONJ in osteoporosis/osteopenia patients, we should be aware of BRONJ since the population of the patients is exceedingly increasing with the prolonging of life expectancy. So, we'd like to enlighten upon the problems and solutions of BRONJ.
