Osteoradionecrosis of the jaws (악골에 발생된 방사선골괴사)

  • Osteoradionecrosis is one of the most serious complication after heavy irradiation of bone and most particularly following treatment of cancers within the head and neck. The irradiated bone has the decreased vascularity and is easily infected. As a result, the spread of infection may cause a nonhealing wound that is very difficult to be treated. A comprehensive prophylactic dental care as well as proper blocking of the radiation field before radiotherapy must be considered to reduce the risk of osteoradionecrosis. We present three cases of osteoradionecrosis which developed after extraction of teeth in irradiated patients.

ORTHOPANTOMOGRAPH STUDY OF THE ALVEOLAR BONE LEVEL ON PERIODONTAL DISEASE. (ORTHOPANTOMOGRAPH에 의한 치주병환자의 치조골흡수에 관한 연구)

  • The author had measured the alveolar bone level of periodontal disease on 50 cases of orthopantomogram to detect the degree of alveolar bone resorption of both sexes of Korean. The results were obtained as follows; 1. Alveolar bone resorption of mesial and distal portion was similiar in same patient. 2. The order of alveolar bone resorption was mandibular anterior region, posterior region, canine and premolar region of both jaws. 3. The degree of alveolar bone destruction was severe in shorter root length than longer. 4. The degree of alveolar bone resorption was severe in forth decades.

  • Orthopantomograph는 치과진료에서 가장 일반적인 국외촬영방법으로 이용되고 있다. 저자는 1983년 서울대학교병원 치과진료부 치과방사선과에 래원한 환자중 Orthopantomogram을 촬영한 2,160명을 대상으로 골경화증의 성별, 연령별 발생빈도와 발생부위, 경화상의 크기, 모양등을 분류하고 특발성 골경화증과 condensing osteitis에 관하여 관찰하였든 바 다음과 같은 결과를 얻었다. 1. Idiopathic osteosclerosis의 발현율은 16.0%, condensing osteitis의 발현율은 9.8%였고 남녀간의 차이는 없었다. 2. 골경화증의 호발부위는 하악소구치, 대구치부위였고, 상악에서의 발생율은 적었다. 3. 골경화상의 크기는 idiopathic osteosclerosis는 10㎜이내였고, condensing osteitis는 10-20㎜의 폭경을 가지고 있었다. 4. 골경화상의 형태, 건강골과의 경계 및 발현되는 상태는 일정하지 않았다.

  • Gorlin-Goltz syndrome is an infrequent multi-systemic disease which is characterized by multiple keratocysts in the jaws, calcification of falx cerebri, and basal cell carcinomas. We report a case of Gorlin-Goltz syndrome in a 23-year-old man with emphasis on image findings of keratocyctic odontogenic tumors (KCOTs) on panoramic radiograph, computed tomography, magnetic resonance (MR) imaging, and Ultrasonography (US). In this case, pericoronal lesions were mostly orthokeratinized odontogenic cyst (OOC) concerning the MR and US study, which tended to recur less. The aim of this report was to clarify the characteristic imaging features of the syndrome-related keratocysts that can be used to differentiate KCOT from OOC. Also, our findings suggested that the recurrence rate of KCOTs might be predicted based on their association to teeth.

  • The authors observed the two cases of Rieger's syndrome in 21-year-old male and 13-year-old female with a chief complaint of Partial anodontia of permanent teeth on both jaws. We had done the laboratory and ophthalmic examinations and had taken radiographs. Final diagnosis was established as Rieger's syndrome. We obtained the results as follows, 1. There was no peculiar hereditary tendency in them. 2. The patients had no disturbance of general physical activity and mentality. 3. The male patient had pseudoprogenathism, concave profile, congenital missing on upper anterior and second premolar teeth, and conical shaped crown of upper len celral incisor. The female patient had also pseudoprognathism, concave profile, thickened upper labial frenum, decreased upper dental arch width, congenital missing on upper anterior teeth, and delayed eruption of second premolars. 4. The ophthalmic symptoms that were ins abnormalities, decreased visual acuity, and increased intraocular pressure were present. 5. The abnormalities except dental and ophthalmic abnormalities were not round.

Herpes Zoster mimicking odontogenic pain

  • ;The Varicella zoster virus can cause two infectious diseases. The primary infection of this virus leads to Chicken pox and it goes into a latent period until it reveals itself again. When secondarily shown. it is called Herpes Zoster. Herpes Zoster can usually be seen between T-3 and L-2 of immunodepressant patients but in 13%. it is seen in the trigeminal nerve branch. During the predrominal stage. burning sensation, itching. and sharp pain can be experienced unilaterally where the nerve branch is distributed. Patients can also express tooth pain in this stage. After 2-4 days the acute stage begins with its characteristic unilateral macule showing vesicle formation. In this stage fever and fatigue can be experienced. Continuous pain even after the acute stage is called postherpetic neuralgia; deep pain and burning sensation can be expressed by the patient. The sequelae of this disease consists of rare osteomyelitis. necrosis of the jaws. or loss of teeth.teeth.

  • Odontogenic keratocyst is a central destructive lesion of the jaws characterized by a thin, fragile layer of orthokeratinizing or parakeratinizing stratified squamous epithelium. Correlation between the histologic type and the recurrence ratio remains a subject of controversy, and multiple cysts are known to be associated with the nevoid basal cell syndrome. We experienced a case of multiple odontogenic keratocyst in a 25 year-old male patient involving bilateral mandible and maxilla. The cystic mass of the right maxilla was removed by Caldwell-Luc's approach and the right mandibular mass was removed by intraoral approach but the teeth that were impacted in the mandibular bone were remained in order to prevent an iatrogenic fracture.

THE STUDY OF IMPACTED AND SUPERNUMERARY TEETH IN YOUNG MAN BY THE FULL MOUTH ROENTGENOGRAMS (청년 전악X선사진에 있어서 매복치 및 과잉치에 관한 연구)

  • The author has observed 906 full mouth roentgenograms in Korean young man who had the impacted and supernumerary teeth. Impacted and supernumerary teeth were analysed upon several items, such as incidences and positions in the dental arch. The result were as follows; 1. The incidences of impacted teeth were 59.05% and the ratio of upper jaw to lower one was 1:3. 2. The frequency of the supernumerary teeth were 5.52% and the ratio of maxilla to mandible was 49:1. 3. No significant differences between right and left side in both jaws, but incidence rate of the supernumerary teeth was 74% in the maxillary central incisor area. 4. The positions of impacted and supernumerary teeth were vertical and inverted shapes in maxilla, whereas horizontal and mesio-angular forms in mandible.

ROENTGENOGRAPHIC STUDY OF NUTRIENT CANALS AND BONE TRABECULAR PATTERN (X선상으로 본 영양관과 골주상구조에 관한 연구)

  • The purposes of this investigation was to determine the appearance rate of nutrient canals and the aspect of alveolar bone trabecular patterns. 1949 subjects, averages 23 years old, who have normal occlusion were used. The film used in this investigation was full mouth roentgenographic survey. The obtained results were as follows: 1. In the trabecular pattern, coarse form was 35.17% and fine one was 64.14% in both jaws. 2. The appearance rate of nutrient canals was much higher in mandibular anterior segment(89.13%) than in the maxillary posterior segment(0.86%). 3. In the trabecular forms, small trabecular space and horizontal relation appeared in the mandible, and vertical relation in the maxilla.

A CASE REPORT OF OSTEOPETROSIS (Albers-Schonberg disease의 일례보고)

  • The authors had observed a rare case of osteopetrosis from 8 years old male who had complained nasal obstruction at, Department of Dental Radiology, Kyung Hee University. In the serial roentgenograms the following results were revealed. 1. This osteopetrosis was considered to be malignant rather than benign. 2. In the skull, the greatest degree of radiopacity was found in the base. The pituitary fossa appeared to be small and posterior clinoid process revealed clubbing and thickening. 3. In the frontal and nasal bones were slightly enlarged with marked radiopacity and paranasal sinuses were obscured. 4. The maxilla was markedly affected and bony trabeculae were seen to be coarse and thickened but mandibular posterior segment slightly. 5. There were long retardation of the tooth eruption and physiologic resorption of the deciduous teeth, and tooth root revealed stunted and dwarfed appearance in the both jaws. 6. In this case, the thickened alveolar lamina dura was indistinguishable.
