Comparison of Myogenous and Arthrogenous Pain Patients of Temporomandibular Disorders using Research Diagnostic Criteria for Temporomandibular Disorders (측두하악장애 연구진단기준(RDC/TMD)를 이용한 측두하악장애의 근육성 동통과 관절성 동통 환자군의 비교)

  • The purposes of this study were to compare psychological profiles, to investigate the differences in the clinical characteristics, and to compare treatment outcomes between myogenous pain and arthrogenous pain subgroups of temporomandibular disorder (TMD) based on Research Diagnostic Criteria for Temporomandibular disorders (RDC/TMD). Two hundred and fifty two patients diagnosed as TMD were divided into three groups based on the RDC/TMD axis I diagnostic guidelines; myogenous pain group, arthrogenous pain group, and mixed pain (both myogenous pain and arthrogenous pain) group. RDC/TMD history questionnaire was administered to each patient and depression, somatization, jaw disability, pain intensity, disability days, and graded chronic pain scale were analyzed. Bruxism, clenching, insomnia, headache, and unilateral chewing were assessed in a standardized TMD dysfunction questionnaire and the duration of onset, chronicity of pain, treatment period, the effectiveness of the treatment, and improvement of symptoms also analyzed. Myogenous pain group had higher depression (p=0.002), and somatization scales (p<0.001) than the arthrogenous pain group. Mixed pain group showed higher pain intensity (p=0.008), disability days (p<0.001), graded chronic pain scale (p=0.005), somatization (p<0.001), and depression scores (p=0.002) than the arthrogenous pain group. Jaw disability did not show any significant differences among the three groups (p=0.058). Arthrogenous pain group reported more limitation of mouth opening than myogenous pain group (p=0.007). Duration of onset showed that the arthrogenous pain group had lowest prevalence of chronicity among three groups (p=0.002). Mixed pain group patients showed lowest symptom improvements among three groups (p=0.007). Multiple linear regression analysis results showed that the treatment effectiveness was significantly associated with somatization score (${\beta}$=-0.251, p=0.03).

Clinical Manifestations in Orofacial Movement Disorders (구강안면 운동장애의 임상적 증상 발현)

  • This study was a preliminary study to establish diagnostic criterias and treatment for Orofacial Movement Disorders. The 33 Orofacial Movement Disorder patients who were visited in the department of Oral Medicine from September, 2007 to December, 2007 were selected for this study. We analyzed the age, sex, systemic diseases, the diagnosis and the cause of the patients' chief complaints, the self-consciousness and the types of orofacial movements. The obtained results were as follows : 1. Female were predominant in orofacial movement disorders(81.82% vs 18.18%) and mean age was 78.78(56 to 87) years. 2. They almost had systemic diseases(81.82%). Hypertenstion was the most common disease(22.41%) and diabetes mellitus(17.24%), depression(8.62%), gastritis(8.62%) in turns. 3. In clinical manifestation, temporomandibular disorder was the most frequently complained symptom(33.33%), and soft tissue disease(21.57%), burning mouth syndrome(17.65%), orofacial movement itself(15.69%), diffuse orofacial pain(6명, 11.76%) in turns. 4. Most orofacial movement disorders are idiopathic(72.73%), and related to prosthetic treatment(24.24%), related to antidepressant medication(3.03%) in turns. 5. The jaw-closing type was the most common type of orofacial movement disorders, and lateral type(33.33%), jaw-opening types(16.67%) in turns. 6. There were more patients who did not conscious of their orofacial movements than those who did.(54.55% vs 45.45%). In conclusion, dentists must be consider the orofacial movement disorders in patients who have orofacial pain. Also, dentists should obtain a proper history and perform a clinical examination to avoid misdiagnosis and inappropriate, irreversible treatment.

EFFECT OF LOW - POWER LASER IRRADIATION ON PAIN RESPONSE (저출력 레이저조사가 동통반응에 미치는 영향)

  • The aim of this study was to investigate the effect of low - power laser used in the medical field for various purposes to suppress pain responses evoked by noxious electrical or mechanical stimuli. After both inferior alveolar nerves and the left anterior digastric muscle of cats under general anesthesia were exposed, a recording electrode for the jaw opening reflex was inserted into the anterior digastric muscle. The right inferior alveolar nerve was dissected under a surgical microscope until the response of the functional single nerve could be evoked by the electrical stimulation of the dental pulp or oral mucosa. The electrical stimulus was applied with a rectangular pulse of 10 ms duration for measuring the threshold intensity of a single nerve fiber in the inferior alveolar nerve which responds to stimulation of dental pulp and oral mucosa. Then a pulse of 1 ms duration was applied for determination of conduction velocity. A noxious mechanical stimulus to the oral mucosa was applied by clamping the receptive field with an arterial clamp. The Ga-As diodide laser(wave length, 904 nm ; frequency, 1,000 Hz) was irradiated to the prepared tooth cavity, inferior alveolar nerve and oral mucosa as a pulse wave of 2 mW for 6 minutes. This was followed by a continuous wave of 15 mW for 3 minutes. The action potential of the nerve and EMG of the digastric muscle evoked by the noxious electrical stimulus and nerve response to noxious mechanical stimulus were compared at intervals of before, immediately after, and at 5, 10, 20, 40, 60 minutes after laser irradiation. The results were as follows: The conduction velocity of the intrapulpal $A{\delta}$- nerve fiber recorded from the inferior alveolar nerve before irradiation had a mean value of $6.68{\pm}2.07m/sec$. The laser irradiation did not affect the conduction velocity of the AS - nerve fiber and did not change the threshold intensity or amplitude of the action potential either. The EMG of the digastric muscle evoked by noxious electrical stimulation to the tooth was not changed by the laser irradiation, whether in latency, threshold intensity or amplitude. The laser irradiated to the receptive field of the oral mucosa which was subjected to noxious stimuli did not affect the amplitude of the action potential or the frequency either.

A COMPARATIVE STUDY ON THE GENIOGLOSSUS MUSCLE AND ORBICULARIS ORIS MUSCLE ACTIVITY IN THE ANTERIOR OPEN BITE AND NORMAL OCCLUSION (전치부 개방교합자와 정상교합자의 이설근 및 구륜근 활성도에 관한 비교 연구)

  • This study was undertaken to investigate the activities of genioglossus and orbicularis oris muscle between normal occlusion and anterior open bite group. 39 subjects without the experience of orthodontic treatment and T.M.disorder were selected for this study. 20 subjects were normal occlusion. 19 subjects were anterior open bite. The twenty items were measured from the cephalometric headplates, and EMG recording of the genioglossus, orbicularis oris muscle were taken at rest position, water swallowing, jaw opening, isometric tongue protrusion, maximum tongue protrusion. All data were analyzed and processed with the computer statistical method. The following results were obtained: 1. Except at rest position. the muscle activities of genioglossus muscle in anterior open bite were higher than in normal occlusion with singificatn difference. 2. Except druing water swallowing, the muscle activities of orbicularis oris muscle in anterior open bite were higher than in normal occlusion with significant difference. 3. During maximum tongue protrusion, the geniolossus muscle of anterior open bite subjects showed the highest muscle activity. 4. Anterior open bite showed closer interrelationship between facial morphology ad the genioglossus, orbiculars oris muscle activities than that of nomral occlusion with significatn difference.

CHANGES OF JAW-OPENING REFLEX DEGREE ACCORDING TO ELECTROACUPUNCTURE DURATION ON ZUSANLI (족삼리의 전기침 자극에 대한 개구반사 크기의 변화)

  • The purpose of this study was to investigate the effect of various electroacupuncture duration induced by acupuncture point-Zusanli ($S_{36}$) electrical stimulation on inhibition of amplitude of digastric electromyogram (dEMG) evoked by noxious electrical stimuli around the mental foramen. intraperitoneal sodium pentobarbital in an initial dose of 50mg/kg and maintenance doses of 4.5mg/kg/h were given through a cannula in the femoral vein using a constant infusion pump. A pair of stimulating electrodes were inserted for noxious stimuli around the mental foramen. An irritant electronic stimuli pulse (0.2 Hz, 0.1 ms duration) was produced with an intensity of about $1.5{\times}2$ times threshold for evoking the dEMG. The anterior belly of the digastric muscle was exposed and a pair of 0.1mm wire electrodes were inserted for dEMG recording. Acupuncture point stimulation on Zusanli (2 Hz, 250 ${\mu}s$, biphasic pulse, 2 V) was delivered by Dental Electronic Anesthesia (3M, U.S.A). For periods of electronic stimulation of 10, 20, and 30min, the amplitudes of dEMG were measured on the oscilloscope and on the monitor connected to the amplifier. The following results were obtained: The dEMG was decreased to 73.4% of that in the control set after 10 min electroacupunture stimulation (Group I); The dEMG was decreased to 77.1% (10min), 54.0.% (20min) of that in the control set after 20minutes of electroacupunture stimulation (Group II). The dEMG was decreased to 73.3% (10min), 61.9% (20min), 76.2% (30min) of that in the control set after 30 min of electroacupunture stimulation (Group III). From these results, it may be that in the electroacupuncture stimulation on the Zusnali resulted in a reduction of amplitude of dEMG and that the most effective electroacupuncture stimulation period was 20min.

Morphologic and positional assessment of temporomandibular joint disk in facial asymmetric patients by magnetic resonance imaging (자기 공명 영상을 이용한 안면비대칭환자의 측두하악관절원반의 형태와 위치에 관한 연구)

  • The present study was conducted to examine the morphometrics and function of the disk on both sides among patients with facial asymmetry (FA) and to elucidate plausible correlations between internal derangement (ID) and FA. The sample was composed of 10 males and 27 females with FA. The disk status of all subjects was evaluated by bilateral high resolution magnetic resonance scans in the sagittal (closed and open) and coronal (closed) planes. Five types of disk displacement were identified accordingly. The disk function was diagnosed as normal disk function, disk displacement with reduction, and disk displacement without reduction. The disk shape on sagittal MRI in closed position was classified as bi-concave, biplanar, funnel/hemiconvex, and deformed. The disk position, translation and rotation were also measured. The difference between the shifted side and non-shifted side was analyzed by statistical analysis. Approximately $70\%$ of the patients in the present study showed unilateral or bilateral ID. It was found that anterior disk displacements (ADD), especially rotational ADD, occurred more frequently in the shifted side, while normal disk position was observed mainly in the non-shifted side (p<0.01). The disk of the shifted side showed significantly deformed configuration and inferior-anterior disk position. However, the disk of the non-deviated side showed hyper-mobility during jaw opening movement. These results demonstrate that in FA patients, the disks status of the shifted side is different from that of the non-shifted side, a phenomenon that could be correlated to facial asymmetry.

COMPARISON OF BITE FORCE WITH DENTAL PRESCALE AND UNILATERAL BITE FORCE RECORDER IN HEALTHY SUBJECTS (Dental Prescale과 편측 교합력 측정기를 이용한 정상성인의 교합력 비교)

  • Statement of problem: The previous unilateral bite force recorder has several limitations for taking long time for measuring and causing discomfort to subjects. Because of these reasons, it could not use widely for epidemiological researches. However, 'Dental Prescale System' which is new equipment for measuring bite force, is more convenient for measuring bite force than previous unilateral recorder. Purpose: The purpose of this study was to compare a new technique(Dental Prescale System) using pressure sensitive foils for recording of maximal jaw closing force with conventional measurement using unilateral bite force recorder Material and method: This studies included 22 healthy dental students in college of dentistry Yonsei university in Rep of Korea. Mean age was 23.9 years. All subjects had continuous dental arches and no significant malocclusion or signs from the teeth and craniomandibular system. The Dental Prescale System (Fuji Film, Tokyo, Japan) consists of a horseshoe-shaped pressure sensitive sheet (50H, R type) and a computerized scanning system.(FPD705) We also used unilateral bite force recorder(Denbotics Co. Seoul, Rep.Korea) for comparing with Dental Prescale. Results and conclusion: The total bite force recorded with Dental Prescale System (1423 N) was systematically higher than that recorded by unilateral bite force recorder. (256 N) However, the maximum bite force values measured in the two ways were significantly correlated (r=0.46, p<0.05). The Dental Prescale bite force calculated for first molar (208 N) was lower than that recorded by unilateral bite force recorder. (256 N) The two values were also very significantly correlated. (r=0.66. p<0.001) There were significantly different in bite force between two measurement methods. The reasons were first, unilateral bite force recorder measured only the bite force of a part of teeth, and Dental Prescale measured the total teeth force. Second, in measurement, a difference in muscle contraction appeared by an extent of mouth-opening. Third, unilateral bite force recorder has the reducing effect of the bite force by protective tape. Fourth, Dental Prescale has limitations during the computer scanning procedure. Therefore, Dental Prescale System is considered to be a very promising alternative to be conventional bite force recording methods.

A study on the lip shape recognition algorithm using 3-D Model (3차원 모델을 이용한 입모양 인식 알고리즘에 관한 연구)

  • Recently, research and developmental direction of communication system is concurrent adopting voice data and face image in speaking to provide more higher recognition rate then in the case of only voice data. Therefore, we present a method of lipreading in speech image sequence by using the 3-D facial shape model. The method use a feature information of the face image such as the opening-level of lip, the movement of jaw, and the projection height of lip. At first, we adjust the 3-D face model to speeching face image sequence. Then, to get a feature information we compute variance quantity from adjusted 3-D shape model of image sequence and use the variance quality of the adjusted 3-D model as recognition parameters. We use the intensity inclination values which obtaining from the variance in 3-D feature points as the separation of recognition units from the sequential image. After then, we use discrete HMM algorithm at recognition process, depending on multiple observation sequence which considers the variance of 3-D feature point fully. As a result of recognition experiment with the 8 Korean vowels and 2 Korean consonants, we have about 80% of recognition rate for the plosives and vowels. We propose that usability with visual distinguishing factor that using feature vector because as a result of recognition experiment for recognition parameter with the 10 korean vowels, obtaining high recognition rate.

Sociopsychological Factors related to Prediction of Treatment Outcome of the Temporomandibular Disorders (측두하악장애 치료결과의 예측에서 사회심리학적 요인의 영향)

  • The purpose of this study was to investigate the sociopsychological factors which might influence the course of the temporomandibular disorders and to develop models for prediction of treatment outcome related to pain, dysfunction and sound. For this study, 268 patients with temporomandibular disorders were selected from the patients presented to department of Oral Medicine, Wonkwang university dental hospital. Chief complaints of these subjects were largely grouped into three categories such as pain, dysfunction and sound, and 10 cm visual analogue scale(VAS) was used to record the state of the three complaints every visit and Treatment Index(VAS TI) was calculated from VAS. All the forty-two items obtained from clinical examination or questionnaire were statistically processed with $SPSS^{(R)}$ windows. The results of this study were as follows: 1. The items showed a difference between male and female subjects were maximum mouth opening, palpation score, jaw jerk during mandibular movement, and sleep disturbance. Among questionnaires such as SRRS, HAD scale and PSQI, the HAD scales showed the most highly significant correlation with the each scale item of the SCL-90R. 2. Among the groups classified by VAS TI, the group with the lowest VAS TI showed the highest VAS score in the start of treatment but showed the lowest score in the end of treatment, without respect to the type of chief complaint. From these results, it is assumed that the active treatment duration for the subjects with lower VAS score in the first visit would be longer than the subjects with higher score with the somewhat poor treatment outcome. 3. With regard to all the three complaints, the items showing significant effect in the model for prediction of treatment outcome were from questionnaire, except one item, maximum mouth opening, which suggest that the sociopsychological factors would be strongly related to development and progress of the symptoms.
