Subjective Xerostomia and Jaw Functional Limitation Related Quality of Life of the Elderly (노인의 주관적 구강건조증과 악기능제한 관련 삶의 질)

  • This study is a descriptive research one and its purpose is to understand subjective xerostomia, JFL and quality of life of the elderly and to apprehend its variable correlation and variables which influence on the OHRQOL. Subjects in this study were over 65-year-old the elderly in city B and the data of 116 ones were collected using the questionnaires from May 6, 2013 to June 7 from them. Collected data were analyzed as descriptive statistics, t-test, one-way ANOVA, Sheffe, Pearson's correlation coefficient and Multiple Regression using program SPSS 18.0. The most influenced factors of OHRQOL were JFL(${\beta}$=-.366), xerostomia(${\beta}$=-.248) and diseases of oral cavity (gingivitis)(${\beta}$=-.172) in order. These factors occupied 73% of OHRQOL. Therefore, development of nursing intervention to develop a program for the improvement in behavior and education to reduce xerostomia, JFL, gingivitis and to improve the OHRQOL elderly development of nursing interventions are required.

Relationship between disease stage and renal function in bisphosphonate-related osteonecrosis of the jaw

  • Objectives: Bisphosphonate is the primary cause of bisphosphonate-related osteonecrosis of the jaw (BRONJ). Bisphosphonates are eliminated from the human body by the kidneys. It is anticipated that bisphosphonate levels in the body will increase if the kidney is in a weak state or if there is systemic disease that affects kidney function. The aim of this study was to analyze the relevance of renal function in the severity of BRONJ. Materials and Methods: Ninety-three patients diagnosed with BRONJ in Pusan National University Dental Hospital from January 2012 to December 2014 were included in this study. All patients underwent a clinical exam, radiographs, and serologic lab test, including urine analysis. The patient's medical history was also taken, including the type of bisphosphonate drug, the duration of administration and drug holiday, route of administration, and other systemic diseases. In accordance with the guidelines of the 2009 position paper of American Association of Oral and Maxillofacial Surgeons, the BRONJ stage was divided into 4 groups, from stage 0 to 3, according to the severity of disease. IBM SPSS Statistics version 21.0 (IBM Co., USA) was used to perform regression analysis with a 0.05% significance level. Results: BRONJ stage and renal factor (estimated glomerular filtration rate) showed a moderate statistically significant correlation. In the group with higher BRONJ stage, the creatinine level was higher, but the increase was not statistically significant. Other factors showed no significant correlation with BRONJ stage. There was a high statistically significant correlation between BRONJ stage and 'responder group' and 'non-responder group,' but there was no significant difference with the 'worsened group.' In addition, the age of the patients was a relative factor with BRONJ stage. Conclusion: With older age and lower renal function, BRONJ is more severe, and there may be a decrease in patient response to treatment.

  • Stellate ganglion block is extensively performed in pain closing to treat a diversity of diseases. Stellate ganglion phenol neurolysis, however, has not been not popular because of risk and complications such as: permanent horner's syndrome, hoarseness, pneumothorax and intravascular or intraspinal injection. But Racz recently performed stellate ganglion phenol neurolysis successfully, under fluoroscopic guide, minus significant complication. Three patients were recently treated at our pain clinic by repeated stellate ganglion block with local anesthetics. Patients showed immediate signs of improvement but prolonged pain relief was not achieved. Therefore we reported to performing stellate ganglion phenol neurolysis following Racz's technique. We successfully treated: two cases of reflex sympathetic dystrophy of the upper extremity, and a case of postherpetic neuralgia of jaw, neck and upper chest wall, by stellate ganglion phenol neurolysis, devoid of any significant complications.

  • Noma is a gangrenous disease of the orofacial region that leads to severe facial tissue destruction and is a significant cause of death among children. With the advent of modern antibiotics and improved nutrition, children with noma may survive into adulthood, but must face the challenge of undergoing repair of the sequelae of noma. This report describes a case of bony fusion of the maxilla and mandible in a 28-year-old female patient, which was a sequelae of a childhood case of noma.

  • Histiocytosis-X is a disorder characterized by proliferation of cells, phenotype of Langerhans' cells. Traditionally, these diseases include three subtypes of commen histologic features and clinical findings respectively. Eosinophilic granuloma shows a solitary or multiple skeletal lesions. Hand-Schuller-Christian disease often fatal due to widespread extraskeletal lesions. Patient' chief compaints are frequent bone pain and swelling. During rescent 10 years in our hospital, 13 patients, 11 male and 2 female, were diagnosed as Histiocytosis-X. And we evaluated clinical features, radiographic finding, histological characteristics and treatment modalities according to the each subtypes. Major treatment methods of the eosinophic granulma was a chemotherapy combined with local currettage, wide resection of the lesion should be performed carefully in selective cases.

Full mouth implant rehabilitation with double scanning of provisional restoration (임시치아 double scanning을 이용한 전악 임플란트 수복 증례)

  • It is important to produce a provisional restoration reflecting the patient's jaw relation, occlusal plane, lip support, shape of teeth, and occlusion type for fully edentulous patients before making a definite prosthesis. The patient introduced in this study showed bad prognosis of remained tooth after severe periodontal diseases. Therefore, remaining teeth were extracted and replaced with dental implants. Provisional restorations were fabricated and the the patient's vertical and horizontal jaw relationship, occlusal plane, amount of overjet and overbite, size of teeth, and length of anterior tooth were recorded. Provisional restorations were scanned and CAD/CAM techniques were used to fabricate a monolithic zirconia bridge, which contour is identical with the provisional restorations. The patient was satisfied with the treatment results on functional, esthetic aspects and the prosthesis retained stable during the four-month clinical observation period.

  • The average life expectancy has been increased, so the proportion of elderly patients that visit to the dental clinic for prosthetic restoration has increased. Elderly patients have various chronic diseases. Recent trends show an increase of osteoporosis in elderly patients, and thus, the number of osteoporosis patients is expected to escalate. Currently, the most widely used drug for osteoporosis is bisphosphonate. However, osteonecrosis of the jaw has been reported as a side effect derived from longterm oral administration or injection treatment of the drug. Surgical dental treatment was the main cause of medication related osteonecrosis of the jaw (MRONJ). As MRONJ is very difficult to cure, it is important to take preventive measures. Surgical operation may be needed for the mouth preparation before prosthetic restoration. For successful treatment, the dentist should have a full understanding of osteoporosis and show a continued interest toward this disease for careful management.

Clinical study of diagnosis and treatment of bisphosphonate-related osteonecrosis of the jaws (비스포스포네이트 관련 악골괴사의 진단 및 치료에 대한 임상적 연구)

  • Introduction: Bisphosphonates is used widely for the treatment of the Paget's disease, multiple myeloma, bone metastases of malignant tumors with the prevention of pain and their pathological fracture. However, it was recently suggested that bisphosphonates related osteonecrosis of the jaw (BRONJ) is a side effect of bisphosphonate use. Materials and Methods: Twenty-four individuals, who were referred to the Department of Oral and Maxillofacial surgery, Dankook University Dental Hospital, were selected from those who had exposed bone associated with bisphosphonates from January, 2005 to December, 2009 according to the criteria of American Association of Oral and Maxillofacial Surgeons (AAOMS) for BRONJ. The patients group consisted of 7 males and 17 females between the age of 46 to 78 years (average 61.8 years). Each patient had panoramic imaging, computed tomography (CT), whole body bone scanning performed for a diagnosis and biopsy sampling from the necrotizing tissue. C-terminal cross-linking telopeptide of type I collagen (CTX) level of patients who had undergone surgical intervention was measured 7 days before surgery. Results: The main cause of bone exposure was post-extraction (15), chronic periodontitis (4), persistent irritation of the denture (3). Twenty people had undergone BRONJ treatment for two to eight months except for 4 people who had to maintain the bisphosphonates treatment to prevent a metastasis and bone trabecular pain with medical treatment. When the bisphosphonate treatment was suspended at least for 3 months and followed up according to the AAOMS protocols, the exposed necrotizing bones were found to be covered by soft tissue. Conclusion: Prevention therapy, interruption of bisphophonates for at least 3 months and cooperation with the physician for conservative treatment are the essential for treating BRONJ patient with high risk factors. The CTX level of BRONJ patients should be checked before undergoing surgical intervention. Surgical treatments should be delayed in the case of a CTX level <150 pg/mL.

Efficacy of arthrocentesis and lavage for treatment of post-traumatic arthritis in temporomandibular joints

  • Objectives: Joint injuries frequently lead to progressive joint degeneration that causes articular disc derangement, joint inflammation, and osteoarthritis. Such arthropathies that arise after trauma are defined as post-traumatic arthritis (PTA). Although PTA is well recognized in knee and elbow joints, PTA in the temporomandibular joint (TMJ) has not been clearly defined. Interestingly, patients experiencing head and neck trauma without direct jaw fracture have displayed TMJ disease symptoms; however, definitive diagnosis and treatment options are not available. This study will analyze clinical aspects of PTA in TMJ and their treatment outcomes after joint arthrocentesis and lavage. Materials and Methods: Twenty patients with history of trauma to the head and neck especially without jaw fracture were retrospectively studied. Those patients developed TMJ disease symptoms and were diagnosed by computed tomography or magnetic resonance imaging. To decrease TMJ discomfort, arthrocentesis and lavage with or without conservative therapy were applied, and efficacy was evaluated by amount of mouth opening and pain scale. Statistical differences between pre- and post-treatment values were evaluated by Wilcoxon signed-rank test. Results: Patient age varied widely between 20 and 80 years, and causes of trauma were diverse. Duration of disease onset was measured as 508 post-trauma days, and 85% of the patients sought clinic visit within 2 years after trauma. In addition, 85% of the patients showed TMJ disc derangement without reduction, and osteoarthritis was accompanied at the traumatized side or at both sides in 40% of the patients. After arthrocentesis or lavage, maximal mouth opening was significantly increased (28-44 mm on average, P<0.001) and pain scale was dramatically decreased (7.8-3.5 of 10, P<0.001); however, concomitant conservative therapy showed no difference in treatment outcome. Conclusion: The results of this study clarify the disease identity of PTA in TMJ and suggest early diagnosis and treatment options to manage PTA in TMJ.

A COMPARATIVE STUDY ONMAGNETIC RESONANCE IMAGE AND SECTOGRAPH OF HUMAN TNJ (악관절의 자기공명영상과 시상단층 방사선촬영상에 관한 비교연구)

  • For understanding of anatomy, physiology, and diseases of human TMJ, it is required to evaluate quantitatively the movement of the disc and condyle head of mandible. The histologic section of cadaver TMJ were examined, and the magnification of the MR image and its details of anatomy were evaluated. And then a quantitative analytic method, by comparing the Sectograph and the MR image of vital human TMJ, was proposed. For this study, 15 subjects(Male, 24~35years) were selected from a prosthodontic examination randomly, and each subject’s five interocclusal rubber registration records were made on the ICP, and 5, 10, 15, and 20mmjaw opening positions. All subjects were radiographed with a Denar Quint Sectograph Image System(Denar Corp., USA), and imaged with a MRP-20EX MR Image System(0.2T, Permanent Magnet Type, Hitachi Medical Corp., Japan) using an 100mm diameter bilateral type surface coil. These images were traced on the acetate tracing paper, and analyzed In this study, the findings led to the following conclusions. 1. In comparison of the histologic section of autopsy specimen with the MR image at the same section, the size(dimension) of MR image was 70% of the real one. It was possible to recognize the shape of articular disc, anterior and posterior attachments, and adjacent soft tissues, because of the excellent reproducibility of anatomical structure. 2. When we compared the amount of joint space on MR image with that of joint space on sectograph, the amount of joint space on sectograph was significantly greater than that of joint space on MR image, except at the top of condylar head. 3. The position of minimum joint space on sectograph at intercuspal position didn't coincide with the middle position of articular disc on MR image, and was approximately in the anterior third of posterior band of articular disc. 4. The amount of condylar movement on MR image at opening movement was greater than that of articular disc movement. From Intercuspal position to 5mm jaw-opening movement, the condylar movement showed hinge one, and over the range 5mm jaw-opening it suggested hinge & translatory one. 5. In terms of area variation of articular disc measured on MR image in sagittal plane, the area of posterior band increased with increasing the amount of Jaw opening, but the area of anterior band decreased conversely.
