하악부 에나멜모세포종의 수술후 방사선치료 (Postoperative Radiation Therapy in Ameloblsstoma of the Jaw -Report of Two Cases-)

  • 에나멜모세포종(ameloblastoma)은 하악부에 드물게 생기는 상피성 종양으로서 치원성(齒原性)의 모든 종양과 낭(囊)의 약 $1\%$를 차지한다. 이들은 서서히 자라는 종양이지만 국소적으로 침습성을 보이고 높은 재발율을 나타낸다. 반면에 원격전이는 드물다. 에나멜모세포종은 방사선에 저항적이라고 알려져 있었기에 드문 경우들외에는 주로 수술적 요법으로 치료되어져 왔다. 그러나 최근에 에나멜모세포종이 방사선에 반응을 보이므로 수술가 병용하거나 방사선치료 단독으로 치료에 쓰일 수도 있다는 보고들이 나왔다. 저자들은 수술후 방사선치료 여부를 결정하기 위해 본 치료방사선과에 의뢰된 2예의 에나멜모세포종 중 1예에서 수술후 방사선치료를 실시하였기에 이들 2예를 문헌고찰과 함께 보고하는 바이다.

Giant osteochondroma of the parapharyngeal space: a case report

  • Osteochondroma is a common benign tumor of the axial skeleton, especially in the distal metaphysis of the femur and the proximal metaphysis of the tibia, that can occur on the facial skeleton (albeit rarely). Osteochondroma is differentiated from chondroma, osteochondromatosis and osteoma. Osteochondroma shows an irregular radiopaque lesion and chondromatic area surrounded by the osteoma. When it develops in the long bone, it has a marked tendency to occur at 10 to 20 years of age and ceases with the end of pubertal growth. However, when it develops in the mandibular condyle, it is prevalent in the third decade and continuous to develop. Tumors that develop in the long bone have a predilection for men, but tumors in the mandible have a predilection for women. In osteochondroma of the mandibular condyle, clinical features presented include occlusal changes, facial asymmetry, headaches, pain and joint noise on the temporomandibular joint, mouth opening limitations, and jaw deviation at the involved site. The first choice of treatment for the massive osteochondroma is surgical removal. A 70-year-old female patient with an osteochondroma on her right mandibular condyle visited our clinic. We surgically removed the mass with favorable results. It is presented here along with a review of literature on osteochondroma.

악골 낭종에 대한 임상적 연구 (CLINICAL STUDY OF CYST IN THE JAW)

  • Cystic lesion of the jaw are frequently encountered clinically. Although they rarely lead to development of tumors, they can result in resorption of the jaw bone or asymmetry of the face may occur. The purpose of this study is to find the clinical and histopathological pattern of cysts and to help better understanding for the diagnosis and treatment of jaw cysts. The hospital chart, out-patient chart, panorama X-ray, histopathological report and operation report of 246 patients were reviewed who had been diagnosed as cyst. Sex distribution, age distribution, classification, anatomic distribution, clinical sign & symptoms, treatment, post-operation complications, recurrence rate were studied. Then significant difference between the diameter of cyst with bone graft and none-bone graft was calculated with SAS program. The results were as follows. 1. Among the total patient of 246 cases, male were 163 case(67.0%), and female were 83 case(37.0%), male predominated by the ratio of 1.98. 2. By age group, the 20's accounted for the largest proportion of the cases(27.2%) and the 30' accounted for the 2nd largest proportion of the case(19.5%). 3. Radicular cyst and dentigerous cyst were most common cysts, irrespective of 166 case(67.5%) and 62 case(25.2%). 4. Clinical sign & symptoms were swelling(167case), pain(85case), pus discharge(53case), teeth discoloration(28case), indicating that most complaints were related to inflammation and facial asymmetry. 4.9% of the total cases were discovered accidentally. 5. The primary site of cysts were maxillary anterior area(43.9%), the others were, in descending order, mandibular posterior area(25.6%), maxillary posterior area(14.6%). 6. Enucleation with endodontic treatment was a main treatment method(133 case, 54.1%) and 38 cases(15.4%) were enucleation with extraction, and 37 cases(15.0%) were only enucleation, and 21 cases(8.5%) were enucleation with bone graft. 7. The average diameter of cysts with bone graft was significally greater than with non-bone graft(p<0.05). 8. Post-operation complications occurred in 10 case(4.1%), all of this were due to secondary infection.

상악골에 발생한 원발성 골내암종 (Primary intraosseous carcinoma occurring in the maxilla)

  • Primary intraosseous carcinoma (PIOC) is a rare odontogenic carcinoma defined as a squamous cell carcinoma arising within a jaw having no initial connection with the oral mucosa, and presumably developing from residues of the odontogenic epithelium. A 56-year-old patient who complained of delayed healing after extraction of upper left central incisor visited our department. The conventional radiographs showed a bony destructive lesion with ill-defined margin and moth-eaten appearance. On the computed tomographic images, the lesion perforated the labial cortex of alveloar bone, elevated the left nasal floor superiorly, and perforated partially both nasal floor. The magnetic resonance images showed low signal intensity at T2 and Tl weighted images at the area and adjacent soft tissue. Histologically, there were irregular epithelial islands with cell atypia, nuclear hyperchromatism, pleomorphism, atypical mitosis. The final diagnosis was PIOC.

치성섬유점액종;증례보고 (ODONTOGENIC FIBROMYXOMA;A CASE REPORT)

  • Odontogenic fibromyxomas are extremely rare benign odontogenic tumors that probably arise from interstitial portion of jaw bone. The plain X-ray showed osteolytic destructive lesions in the molar root area. CT scans demonstrated osteolytic expansile lesions with mild enhancement of the solid portion of the mass. Histologically, the tumor is composed of fibrous tissue and myxoid tissues. In this paper a case of odontogenic fibromyxoma of the maxilla and a review of the literature is presented.

  • Ameloblastoma is a common odontogenic tumor originating from the dental lamina, reduced dental epithelium and rests of Malassez, and represents 10% of all odontogenic tumors of the jaw. Unicystic ameloblastoma is normally encountered in young patients, and often occurs in the mandible, and is particularly associated with an impacted tooth. We encountered an unicystic ameloblastoma arising from a dentigerous cyst after the treatment of a radiolucent lesion on the mandible.

  • 하악 과두에 발생하는 연골육종은 전세계적으로 매우 드물게 보고되는 질환으로 저자등은 하악골 과두돌기부에 발생한 연골육종을 하악골 상행지 수평골 절단술에 의해 종물을 제거후 A-O reconstruction plate로 재건하고 술후 4년1개월까지 재발의 소견이 없는 양호한 결과를 얻었기에 문헌고찰과 함께 보고하는 바이다.

Metastasis of breast carcinoma to the whole mandible

  • Metastatic tumors to the jawbones are an infrequent but not rare phenomenon. The most common site of distant primary tumor metastasis to the jaw bones is the breast. The clinical signs and symptoms, and radiographic appearance of these lesions can be quite variable. In this report, an invasive ductal carcinoma of the breast that metastasized to the whole mandible is presented. The patient's medical history revealed that she had undergone a modified radical mastectomy on the right breast eight years ago.

Odontogenic myxoma: a case report with recent image modalities

  • The odontogenic myxoma is an benign, slow growing neoplasm which is of ectomesenchymal origin. This neoplasm occurs almost exclusively in the jaw bones and comprises 0.2% to 17.7% of odontogenic tumors. The odontogenic myxoma may show a wide spectrum of radiographic appearances, unilocular, multilocular radiolucency and a distinct or diffuse border, making the differential diagnosis difficult. We present a case of the odontogenic myxoma in the maxilla with conventional and recent image modalities. Occlusal film revealed a medially extended multilocular lesion with intralesional fine and straight trabeculations from the scalloped margin and buccal expansion and thinning of cortical bone. Computed tomogram revealed lesion showed equivalent density to the muscles in the left maxillary sinus with partial cortical discontinuity of medial wall and the tennis-racket pattern with internal straight trabeculations. MRI revealed intermediate signal intensity on Tl weighted image and high signal intensity on T2 weighted image. In Gd enhanced MR image, the peripheral portions of the lesion were enhanced.

하악에 발생된 법랑모세포섬유치아종 (Ameloblastic fibro-odontoma of the mandible)

  • Ameloblastic fibro-odontoma is an extremely rare odontogenic tumor composed of proliferating ectodermal and mesenchymal components of odontogenic tissue, containing hard tooth structure. It occurs predominantly in children and young adults. The mandibular molar-ramus area is the most frequently observed location, presenting radiographically as a well-circumscribed and radiolucent-radiopaque tumor. A case involving a 24-year-old woman presenting with a large ameloblastic fibro-odontoma of the posterior mandible is reported. This case is of radiologic interest because two distinct calcification patterns were observed.
