A Clinical Case of Dermatomyositis Complicated by Henoch-Schonlein Purpura (피부근염에 이환되었던 환아의 Henoch-Schonlein 자반증 증례 1례)

  • Dermatomyositis is an inflammatory cutaneo-muscular disease of unknown etiology, characterized by symmetrical proximal muscle weakness and typical dermatologic manifestation. Henoch-Schonlein purpura is most common in children. In Korea, dermatomyositis or Henoch-Schonlein purpura associated with herbal medicine has not been reported. The subject patient is a seven year-old girl diagnosed with dermatomyositis in August 1999 and Henoch-Schonlein purpura in January, July 2001. There were symptoms of dermatomyositis such as headache, arthralgia, eruption, itching and abdominal pain. I think the herb-prescription composed or Radix astragali (Huangqi), Herba agrimoniae (Xianhecao), Fructus jujubae (Dazao) had an ameliorating effect on Henoch-Schonlein purpura by decreasing these symptoms.

A Clinical Report of Adult Acute Viral Hepatitis Type A Treated with Herbal Prescriptions

  • Owing to improvement of socioeconomic status during recent decades in Korea, incidence of hepatitis A has rapidly decreased, especially among children. However, this status has paradoxically caused a steady increase of adult patients with HAV infection, causing new medical issues associated with aggravated clinical symptoms. The present study reports an adult case of acute viral hepatitis type A treated with oriental medicine. The elevated biochemical findings(AST, ALT, gamma-GTP, bilirubin), physical symptoms (general weakness, nausea, right flank pain, itching sign), and serological makers (anti-HAV IgM and anti-HAV IgG) were normalized within four weeks. Also, sonographic examination showed a normal pattern on an enlarged liver image. This study informed us about the clinical capacity of oriental medicine for adult patients with acute viral hepatitis type A.

  • Objectives: This study reports using Hyungbangsabaksan to cure Soyangin Yangdokbalban (陽毒發斑) appealing Systemic Allergic Contact Dermatitis. Methods: After diagnosing the patient showing Papuloerythematous with no blisters in the body as a Soyangin Yangdokbalban (陽毒發斑), we prescribed the patient with Hyungbangsabak-san. We compared the condition of the patient before and after medication with photo shots. Results: After treatment, Papuloerythematous which was seen with the naked eye disappeared and various skin symptoms such as itching and warmth in the flare area also improved. Conclusions: After treatment, the symptoms of the patient who after eating lacquer chicken showed improvement.

  • To evaluate the antiallergic effect of the dried root of Astragalus membranaceus Bunge (AM) (Leguminosae), which inhibited the mouse passive cutaneous anaphylaxis (PCA) reaction in a preliminary experiment, its main constituents, astragalosides I and IV, were isolated and their antiallergic effects were investigated. Astragalosides I and IV inhibited the PCA reaction induced by the IgE-antigen complex, and the scratching behaviors induced by compound 48/80. These constituents reduced the protein expressions of $TNF-{\alpha}$ and IL-4 in IgE-induced RBL-2H3 cells. These findings suggest that astragalosides I and IV as well as AM can improve IgE-induced anaphylaxis and scratching behaviors.

  • Cutaneous injuries stung by jellyfish are not uncommon in human exposed to marine environments. Most of the cases occur to scuba divers, fishermen, and travelers swimming at the beach. The symptoms vary from mild dermatosis to fatal systemic reaction. Some group of jellyfish like Atlantic Portuguese man-of-war (Physalia physalis) provokes acute severe skin injuries with systemic symptoms of nausea, bradycardia, and rarely anaphylaxis. But it is unusual case that allergic dermatitis caused by Scyphistoma which is polyp stage of jellyfish (Aurelia aurita) happened to zoo keepers working at indoor dolphin pool. This case report is about dermatosis with symptoms such as painful, irritant, itching, and erupted skin lesions on the neck and face of zoo keepers working at dolphinarium in Seoul zoo, Korea.

  • An unusual over-gravid female of Enterobiw uemiculcris was recovered from a 15-month old child by cello-tape anal swab. The patient resided in Inchon and complained of severe anal itching. The worm measured 7.8 mm in length and 0.5 mm in width, and retained typical morphologic features of E. uemiculnris such as cephalic alas and a sharply pointed posterior end. In this gravid female, peculiarly. the uterus was tremendously distended, and about 99% of the whole body length was completely packed with a great number of eggs. Other internal organs were difficult to observe. This paper describes a peculiar over-gravid female of E. vemiculcyis.

  • Introduction: Atopic dermatitis is chronic eczematous recurrent disease of skin and till now its mechanism is not cleary known. It is related to a family history of atopic disease. It has severe itching and it causes eczema secondarily. Atopic dermatitis has mostly severe symptoms and progresses to chronic relapsing. Objectives: The purpose of this study was to estimate the efficacy of herb- medicine treatment on atopic dermatitis. Subjects: 1 treated 5 patients who visited Chungbalsan Kyunghee oriental medical clinic with atopic dermatitis from September, 2001 to October, 2002 partiticipated in the study and were observed for over 2 months.   • Dyshidrotic eczema is a chronic vesicular skin disease due to stress, sweating and nickel dermatitis. It has the characteristic symptoms of a cluster of vesicles and frequent recurrence. We diagnosed a 63-year old male patient case with hyperkeratosis, itching and vesicles at his right foot as dyshidrotic eczema and treated him with application of herbal medicine(bo-jung-ik-ki-tang) and acupunture at Dong-su Oriental-medicine hospital in 2003. We had a good result, so we report this case.

  • Hemorrhoid generally refers to a prolapsed nodule in the anus, and major symptoms are swelling, bleeding, pain. itching, and rectal discharge. For severe conditions, that is stage Ⅲ & Ⅳ, hemorrhoidectomy is commonly performed by surgeons. Yet, many patients are afraid of postoperative recurrence, and complications such as pain, wound bleeding, urinary retention, and dyschezia are not avoidable. We treated two patients with Stage Ⅱ and Ⅲ. and both of the cases were diagnosed as the hemorrhoid due to "moist and heat of the large intestine". After a couple of times of treatment only with SaAhm acupuncture, the symptoms were improved to the degrees where no discomfort could be found in the daily living.

  • Acontii Tuber is very toxious material, but In oriental medicine frequently used for emergency state like shock, coma, and severe pain and author study about Acontii Tuber in Bang-Yak-Hap-Pyun(方藥合編). Bang-Yak-Hap-Pyun(方藥合編) is widely used for clinical herbal prescpition book. Acontii Tuber has alkaloid. this chemical compound make toxious state like arrythmia, heart arrest, weakness, numbness, itching. And using this material must be careful. In Bang-Yak-Hap-Pyun(方藥合編) Acontii Tuber is used 1.2g ~ 12g. and it decrease pain, inflammation, increase activity of heart, and Acontii Tuber' commom use is severe pain disease. Raw Acontii Tuber(生附子) is rarely used, mostly roasted Acontii Tuber. In case of Acontii Tuber toxic state, in oriental medicine Gam-du-tang(甘豆湯) & Go-sam(苦蔘) is used, in western medicine atropine & lidocane is used.
