요추간판 수핵 탈출증 수술환자의 족관절 척측굴곡근과 배측굴곡근의 등속성운동 평가 (Isokinetic Evaluation of the Ankle Dorsi Flexors and Plantar Flexors of Operated Laminectomy & Disectomy Patients)

  • Isokinetic test of muscle strength was performed on 20 patients operated laminectomy & discectomy. Peak torque of ankle dorsi flexors & plantar flexors at the angular velocity of $30^{\circ}$/sec and average power and total work of ankle dorsi flexors & plantar flexors at the angular velocity $90^{\circ}$/sec were measured and analyzed. The result were as follows: 1. Peak torque of non - dominant dorsi flexors at $30^{\circ}$/sec showed no significant difference statistically, but dominant dorsi flexors at $30^{\circ}$/sec showed significant difference statistically(p<0.05). 2. Peak torque of dominant & non - dominant plantar flexors at $30^{\circ}$/sec showed significant difference statistically(p<0.05). 3. Average power of dominant & non - dominant dorsi flexors at $90^{\circ}$/sec showed no significant difference statistically. 4. Average power of dominant & non - dominant plantar flexors at $90^{\circ}$/sec showed significant difference statistically (p<0.05). 5. Total work of dominant & non - dominant dorsi flexors at $90^{\circ}$/sec showed no significant difference statistically. 6. Total work of dominant & non - dominant plantar flexors at $90^{\circ}$/sec showed significant difference statistically(p<0.05).

  • 최근 일상생활이나 산업현장에서 생산공정의 기계화 또는 자동화로 인하여 수 작업이 많이 감소하였으나 아직도 정형화 되지 못한 물건의 운반, 부품조립 등 위 험요소를 내포한 여러가지 형태로 상지 동작(arm exertion)이 요구되는 인력운반 (manual material handling) 작업이 실제 작업현장에서 행새지고 있는 실정이며, 이러한 작업으로 인해 작업자들의 근골격계 부상으로신체장애의 사례가 늘어나고 있다.

21-68세 남녀 100명의 슬관절 굴근과 신근의 근력에 대한 등속성 평가 (Isikinetic evaluation of the knee flexors and extensors on muscle strength in 100 men and women aged 21 - 68yr)

  • The purpose of the present study was to the investigate the effect of aging in men and women on muscle strength of knee extensor and flexors by using the cybex 6000 isokinetic dynamometer. A total of 100 volunteers participated in this study and were divided into five groups according to their chronological age as follows: 20s, 30s, 40s, 50s, 60s, 10 men and 10 women in each decade respectively. Isokinetic ($60^{\circ}{\cdot}s-l$) knee extensor and flexor peak torque, peak torque to body weight ratio, opposing muscles(flexor/extensor) peak torque ratio, deficit of peak torque between dominant and non-dominant were measured. The results obtained were as follows: 1) In men, While the aged increased. the peak torque of the knee flexor and extensor statistically sig nificant decreased in the dominant and non-dominant side. 2) In women, Statistically significant difference of knee extensor peak torque was found as the aged increased in the dominant and non-dominant side, but significant difference of knee flexor peak torque did not that. 3) In men, No significant difference in the peak torque of knee flexor to body weight ratio was found as the aged increased in the non-dominant side, but statistically significant difference in the peak torque of knee flexor to body weight weight ratio was found as the aged increased in the dominant side. 4) In women. No significant difference in the peak torque of knee flexor to body weight ratio was found as the aged increased in the dominant side, but statistically significant difference in the peak torque of knee flexor to body weight ratio was found as the aged increased in the non-dominant side. 5) In men and women, While the aged increased, statistically significant difference was found the dominant and non-dominant side in the peak torque of knee extensor to body weight ratio. 6) Peak torque of hamstring to quadriceps ratio of dominant and non-dominant side in men and women were not significantly different as the age increases. 7) Mean deficit of peak torque between dominant and non-dominant side in men and women were not significantly different as the age increases. From these results we conclude a proper exercise program is need before 50s decade to preserve in muscle strength of knee flexors and extensors.

근육 파라미터 최적화를 통한 발목관절 모멘트 추정 모델 개발 및 평가 (Development and evaluation of estimation model of ankle joint moment from optimization of muscle parameters)

  • Estimation of muscle forces is important in biomechanics, therefore many researchers have tried to build a muscle model. Recently, optimization techniques for adjusting muscle parameters, i.e. EMG-driven model, have been used to estimate muscle forces and predict joint moments. In this study, an EMG-driven model based on the previous studies has been developed and isometric and isokinetic contraction movements were evaluated to validate the developed model. One healthy male participated in this study. The dynamometer tasks were performed for maximum voluntary isometric contractions (MVIC) for ankle dorsi/plantarflexors, isokinetic contraction at both $30^{\circ}/s$ and $60^{\circ}/s$. EMGs were recorded from the tibialis anterior, gastrocnemius medialis, gastrocnemius lateralis and soleus muscles at the sampling rate of 1000 Hz. The MVIC trial was used to customize the EMG-driven model to the specific subject. Once the subject's own model was developed, the model was used to predict the ankle joint moment for the other two dynamic movements. When no optimization was applied to characterize the muscle parameters, weak correlations were observed between the model prediction and the measured joint moment with large RMS error over 100% (r = 0.468 (123%) and r = 0.060 (159%) in $30^{\circ}/s$ and $60^{\circ}/s$ dynamic movements, respectively). However, once optimization was applied to adjust the muscle parameters, the predicted joint moment was highly similar to the measured joint moment with relatively small RMS error below 40% (r = 0.955 (21%) and r = 0.819 (36%) and in $30^{\circ}/s$ and $60^{\circ}/s$ dynamic movements, respectively). We expect that our EMG-driven model will be employed in our future efforts to estimate muscle forces of the elderly.

견갑면에서의 견관절 외전정도에 따른 등속성회전운동의 근력 평가 (An Evaluation for Isokinetic Strength During Shoulder Rotation Movement in the Scapular Plane with Various Abduction)

  • Glenohumeral internal and external rotation with shoulder abducted in the frontal plane often causes impingement of the supraspinatus tendon. whereas similar activity in scapular plane does not cause impingement. The Purpose of this study was to determine if assessment among the three positions as 30 degrees, 60 degrees, 90 degrees abduction in the sitting position of the scapular plane could be affected the comparison between intemal and external peak torque, total work, average power. In this study, Isokinetic shoulder rotational strength was evaluated in twenty healthy male university students, using the Cybex NORMTM System (CYBEX Division of LUMEX, Inc., Ronkonkoma, New York). Test data was gathered in the plane of the scapular, 30 degrees of horizontal flexion anterior to coronal plane, and the subjects performed the test with the arm 30, 60, and 90degrees abducted in the sitting position. also, test speed was set at deg/sec. Statistical analysis was performed using SPSS 7.5 for Windows software and mean and standard deviations were calculated. ANOVA was used to analyze the difference of the values in the three test positions. A paired t-test was used of examining the difference in the means peak torque between external and internal rotation. Not any significant difference was found among three abduction positions in scapular plane, even though there was a consistent pattern of greater strength in the abducted position of 60 degrees. Internal relation strength peak torque and total work were greater than those of external rotation in every test positions.

K3 리그 축구선수의 시즌별 운동기술체력에 관한 비교 연구 (Seasonal Variation in Physiological Fitness of a Semi-Professional K3 League Soccer Team)

  • 이 연구는 K3 리그 축구선수들의 시즌 중 체력 상태를 점검하기 위한 목적으로 종목 특이적인 체력측정을 시즌 초반과 후반에 실시하였다. 평가항목은 신체구성, 순발력, 민첩성, 지구력 및 등속성 하지근력 등으로 구성하였다. 측정 결과 시즌 초반과 비교하여 시즌 후반에 향상된 기능을 보여준 항목은 골격근량, 순발력, 민첩성 등이며, 신체조성, 지구력 및 등속성 하지근력은 차이점이 발견되지 못하였다. 시즌 후반부에 향상도를 보여준 항목은 시즌 중 반복적인 훈련과 시합 참여에 의한 학습효과 및 다양한 생리학적 기전에 의한 것으로 생각된다. 마지막으로 지구력과 하지근력은 별도의 특이적인 훈련을 시즌 중에 실시하지 못한 이유로 인하여 시즌 후반에 향상도를 보여주지 못한 것으로 판단된다. 이상의 결과를 종합한 결과, 선수들의 체력상태를 주기적으로 점검하는 연구를 통하여 시합기, 전이기 및 준비기에 적합한 체력 훈련프로그램의 개발 및 적용이 필요한 것으로 생각된다.

The effects of complex decongestive therapy on pain and functionality in individuals with breast cancer who developed adhesive capsulitis due to lymphedema: an evaluation by an isokinetic computerized system

  • Background: This study aimed to determine the impact of complex decongestive therapy applications on upper extremity function in breast cancer patients who developed adhesive capsulitis after lymphedema. Methods: Thirty patients who developed adhesive capsulitis due to lymphedema were divided into two groups as study (n = 15) and control (n = 15) groups. Both groups received 20 minutes of exercise five days a week for three weeks using a Biodex isokinetic dynamometer, as well as a hot pack and TENS (Transcutaneous Electrical Nerve Stimulation) treatment to the shoulder joint. The study group received 45 minutes of intensive decongestive therapy along with the adhesive capsulitis treatment. The visual analogue scale was used to assess pain, circumference, and volumetric measurements were used to assess edema, and the Arm, Shoulder, and Hand Problems Questionnaire (DASH: Disabilities of the Arm, Shoulder, and Hand) was used to assess upper extremity functionality. The shoulder range of motion was evaluated. Results: Both groups had improvements in pain (P < 0.001), shoulder joint range of motion (P < 0.001), and upper extremity functionality (P < 0.001) after the treatment. There was a significant decrease in circumference and volumetric measurements in the study group (P < 0.001). However, no differences were seen in measurements in the control group. Conclusions: The results showed that complex decongestive therapy was beneficial in reducing lymphedema in breast cancer patients who acquired adhesive capsulitis due to lymphedema. Consequently, the authors believe that supplementing conventional physiotherapy with complex decongestive therapy will benefit patients.

고등부 씨름선수의 체급별 기초·전문체력 비교분석 (A Comparison of Physical Fitness of High School Ssirum Elite-players According to Their Weight Class)

  • 본 연구는 고등부 씨름 선수들을 대상으로 체급별 기초·전문체력의 차이를 알아보는데 목적이 있다. 측정기간은 2019년 11월 25일-12월4일이며, 총 28명의 고등부 씨름 선수(80kg이하 7명, 90kg이하 8명, 90kg이하 7명, 100kg이상 6명)를 대상으로 체급별 기초·전문 체력요인(근력, 근파워, 민첩성, 무산소성파워, 유산소능력, 유연성, 등속성근기능)을 측정하여 비교·분석하였다. 자료처리는 SPSS 23.0을 활용하여 one-way ANOVA와 Duncan을 실시하였다. 그 결과 첫째, 악력과 팔굽혀펴기, 서전트점프, 제자리멀리뛰기, 체후굴, 체전굴, 사이드스텝, 반응시간에서 체급별 차이가 나타나지 않았다. 둘째, 배근력에서 100kg 이하인 그룹이 100kg 이상인 장사급보다 유의하게 낮게 나타났으며, 윗몸일으키기와 20m 셔틀런에서 장사급보다 유의하게 높게 나타났다. 셋째, 무산소성 최고파워와 평균파워의 상대적인 값은 100kg 이하인 그룹이 100kg 이상인 장사급보다 유의하게 높게 나타났다. 넷째, 등속성근기능의 경우 허리 신전/굴곡력과 무릎 관절의 우측 신전근력, 그리고 좌우측 굴곡근력에서 체급별 차이가 없었다. 본 연구 결과에서 얻은 결론은 고등부 씨름 선수의 경우 근파워, 유연성, 민첩성 그리고, 체중당 무산소성 평균파워와 등속성 허리근력, 무릎굴근력에서 체급에 따른 차이가 없다는 것을 확인할 수 있었다.

뇌졸중 환자의 경직측정을 위한 휴대형 장치의 유용성 (Clinical Effectiveness of New Portable System for Measuring Spasticity of the Stroke Patients)

  • Spasticity is a motor disorder characterized by a velocity dependent increase in muscle tone with exaggerated tendon jerks, resulting from hyper-excitability of the stretch reflex. The aim of this study is to develop a portable system for quantifying the grade of spasticity which could calculate the biomechanical as well as neurophysiologic parameters, and for determining the relationship between the Tonic Stretch Reflex Threshold (TSRT) and Modified Ashworth Scale (MAS). Eleven patients with stroke participated in the study (6 males and 5 females, the average age of $64.5\pm16.0$ years). As a results, the mean and standard deviation values of the TSRT were $129.8\pm4.2$, $123.4\pm5.2$ and $119.1\pm2.6$ in the MAS 1, MAS 1+ and MAS 2 groups, respectively. Also, there was a negative correlation between the TSRT and MAS (rho=-0.72, p<0.05). This demonstrated that the TSRT could be made clinically available for the more objective and reliable evaluation of the spasticity, instead of using the conventional clinical scales and an isokinetic dynamometer.

아킬레스건 파열에 대한 수술 후 조기 재활 치료 (Early Mobilization after Operative Treatment of a ruptured Achilles tendon)

  • Purpose: The purpose of the study is to evaluate the clinical outcomes of acute achilles tendon rupture patients treated by early mobilization after operative repair. Materials and Methods: In this retrospective study, 12 patients of acute Achilles tendon rupture were surgically treated from May 2001 to November 2002, with follow-up period of at least 6 months. There were 10 men and 2 women, and mean age was 35.9 year. The repair method was single Krackow suture technique. Two to three weeks after the operation, early ROM with removable ankle-foot orthosis(AFO) was started. We used Amer-Lindholm scale for the clinical evaluation and analyzed patient's satisfaction and subjective strength deficit. Results: We had 11 excellent results(92%), and 1 good results(8%). One-leg heel raising was possible in all patients. All the patients returned to the preinjury occupations and 7 patients(58%) reached the preoperative level of sports activities. There was no significant difference in ankle ROM and ankle plantar flexion isokinetic strength compared to contralateral side. There was, no reruptures. Conclusion: Early mobilization proved to be a well tolerated, safe, and effective means for the compliant patient.
