  • Purpose: This study was carried out to identify premenstrual discomforts and coping patterns and their effects. Method: The participants of this study were 297 female university students in C area. The Menstrual Discomfort Questionaire (MDQ) and coping method lists were used as measurement tools. Results: There were significant differences premenstrual discomforts according to age (F=5.76, p=.003) and according to health condition (F=3.43, p=.034). The mean scores of the sub-categorical factors of premenstrual discomfort were 2.35 points for pain, 2.29 for instability and 2.25 for water retention. The worst symptoms among the subcategories of premenstrual discomfort were as follows: backache (M=2.68) in the pain subcategory, irritability (M=2.53) in the instability subcategory and swelling(M=2.40) in the water retention subcategory. Their common coping patterns were 'coping according to menstrual period', 'active behavioral coping' and 'evasional coping'. Frequently used coping methods were 'taking a rest and sleep (99.3%)' and 'taking a warm shower (86.2%)'. Effective coping methods were 'taking a rest and sleep (89.5%)' and 'taking a warm shower (87.1%)'. Conclusion: Most participants have their own coping pattern. Some methods were very effective to PMS but some were not effective to PMS. To manage PMS, effective coping methods should be encouraged for female university students. To keep up with effective coping, education and counseling should be continued. It is considered necessary to make the same research with a larger number of samples and more specified assessment.

  • Septic arthritis of the elbow is a rare, but serious disorder in neonates. However, neonates often present with few systemic signs other than irritability or poor feeding. When the clinical findings are minimal or absent, joint aspiration gives us helpful information in making a diagnosis. Although many clinicians rely on its result, we experienced a case of early clotted joint effusion, which resulted negative joint aspiration in a neonate with septic elbow arthritis. Ultrasonography helped us to make an early decision to perform surgical intervention as showing different echogenicity of intracapsular portion from generally presented in septic effusion. Presenting case shows that ultrasonography can be an excellent second step examination of neonatal elbow presenting atypical clinical findings of septic arthritis.

  • Neonatal sepsis is a frequent and important cause of morbidity and mortality in the neonatal period. This study was undertaken to observe the 55 cases of proven neonatal sepsis among 6,717 newborn infants under 4 weeks of age, admitted to the nursery of Yeungnam University Hospital from May 1, 1983 to April 30, 1988. We observed following results : 1. The morbidity rate of male(1.12%) was higher than that of female(0.44%) significantly (p<0.05). 2. The morbidity rate of prematurity(2.65%) was higher than that of full term(0.76%) significantly (p<0.05). 3. The incidence of perinatal obstetric complications in early onset neonatal sepsis was higher than that of late onset neonatal sepsis (38.5% vs 10.3%, p<0.05). 4. The common clinical manifestations were poor feeding(52.7%), jaundice(45.5%), diarrhea(30.9%) and irritability (30.9%). 5. Among the causative organisms, gram positive organisms were predominated and Staphylococcus epidermidis was the most common organism. There was no difference in the causative organisms between early onset and late onset sepsis.

  • Citrullinemia is a rare inborn error of metabolism of the urea cycle, and was first reported by McMurray, et al. in 1962. It is inherited as an autosomal recessive trait. The normal synthesis of argininosuccinic acid is blocked in this disease due to a deficiency of argininosuccinic acid synthetase(AS), which has been demonstrated in liver cells and fibroblasts. The clinical symptoms are vomiting, lethargy or irritability, convulsion and mental retardation. The diagnosis is made by the finding of an increased plasma citrulline level. Every effort should be made to reduce the blood ammonia level as rapidly as possible before irreversible brain damage occurs. This report describes a case of citrullinemia that was diagnosed through organic acid analysis and amino acid analysis, and reviews the related literatures.

  • Purpose: Although Bednar's aphthae are common and regress spontaneously, these lesions may lead to feeding intolerance and are often misdiagnosed, rendering examinations useless. This study sheds new light on the clinical features of Bednar's aphthae. Methods: Sixteen neonates and infants were newly diagnosed with Bednar's aphthae via routine health check-ups in an outpatient clinic. Medical records were retrospectively reviewed, and the following parameters were analyzed; sex, gestational age, birth weight, mode of delivery, and perinatal problems. A physical examination was carried out during the next outpatient visit to examine the healing process and check for the existence of scars or complications. Results: Initial presentation included changes in feeding habits (n=10), longer feeding time, reduced intake, and increased irritability. In 6 patients, Bednar's aphthae were discovered incidentally, without prior symptoms. Feeding posture and method of feeding are important causes of Bednar's aphthae. Eleven patients were fed in a horizontal position, whereas 5 patients were fed in a semiseated position. Fifteen patients were bottle-fed, whereas 1 patient was exclusively breastfed. After correcting the feeding position, the ulcerative lesions disappeared within 1 month of diagnosis. During the follow-up period, lesions did not recur in any of the patients. Conclusion: This study suggests that Bednar's aphthae are caused by mechanical pressure. A diagnosis of Bednar's aphthae should be considered when lesions are found on the palate of infants and when symptoms seem to be feeding related. Proper education of parents can both treat Bednar's aphthae and easily prevent its recurrence.

  • Objective: The detection rate of traumatic subdural hygroma(TSH) has increased after the development of computed tomography and magnetic resonance imaging. The treatment method and the mechanism of development of the TSH have been investigated, but they are still uncertain. This study is performed to evaluate the effectiveness of subduroperitoneal shunt in traumatic subdural hygroma. Methods: Five hundred thirty six patients were diagnosed as TSH from 1996 to 2002, among them, 55 patients were operated with subduroperitoneal shunt. We analyzed shunt effect on the basis of clinical indetails, including the patient's symptoms at the diagnosis, duration from diagnosis to operation, changes of GCS, hygroma types. We classified the TSH into five types (frontal, frontocoronal, coronal, parietal and cerebellar type) according to the location of the thickest portion of TSH. Results: The patients who have symptoms or signs related to frontal lobe compression (irritability, confusion) or increased intracranial pressure (headache, mental change), had symptomatic recovery rate above 80%. However, the patients who have focal neurological sign (hemiparesis, seizure and rigidity), showed recovery rate below 30%. The improvement rate was very low in the case of the slowly progressing TSH for over 6weeks. We experienced complications such as enlarged ventricle, chronic subdural hematoma, subdural empyema and acute SDH. Conclusion: Subduroperitoneal shunt appears to be effective in traumatic subdural hygroma when the patients who have symptoms or signs related to frontal lobe compression or increased ICP and progressing within 5weeks.

  • The prevalence of postural hypotension among over 65 year old men is 10-30%. The postural hypotension commonly causes the discomfort such as dizziness, irritability or fainting spell, and for the result it diminishes the quality of the life of aged. Therefore the assessment and intervention of postural hypotension during changing position is very important. The purpose of this study is to investigate the effect of changing position from supine to standing upright, from supine through left lateral to standing upright, from supine through sitting to standing upright of aged. The results obtained are as follows : 1. The average systolic blood pressure of normotension group is $132.68{\pm}16.04mmHg$ at supine position. The average diastolic blood pressure of normotension group is $80.72{\pm}9.82mmHg$ at supine position. The average systolic blood pressure of hypertension group is $153.92{\pm}20.12mmHg$ at supine position. The average diastolic blood pressure of hypertension group is $93.74{\pm}15.53mmHg$ at supine position. 2. There is no significant difference of blood pressure after three different procedures as mentioned above from supine to standing upright. 3. The prevalence of postural hypotension at standing upright is 13.5% which is 18.8% of the hypertension group and 9.4% of the normotension group. There is statistical difference of the prevalence of postural hypotension between hypertension group and normotension group.

  • Koreans are one of the fastest growing immigrant groups in America. Adjsting to life in foreign country produces a great deal of stress. Differences in culture, language, expectation and social behavior can lead to misunderstandings. The pregnancy and delivery event is one of maturational crisis in life cycle. The purpose of this research was to understand the structure of the lived experience of pregnant women and postpartum mothers who had immigrated to the United States. The research question was "What is the structure of the experience of pregnant women and postpartal mothers?" The sample consisted of 16 women registrated at the Obstetrics and Gynecology Department of one local clinic in Hawaii. The unstructured interviews were carried out from Jnuary 5, through January, 30, 1994. They were audio-recorded and analyzed using Van Kaam's method. There are different views on the causative factors of stress. Maladjustement to the immigrant life, spousal conflicts, anxiety related to bringing up the bay and conflicts between mother-in-law and daughter-in-law are considered to be related factors. The experiences of pregnant women over the period of the pregnancy can be varied and can include change of body image, emotional and physical change. The experiences of postpartum mothers over the postpartum period can be varied and can include postpartum depression emotional irritability, fear related to bringing up the baby and disappointment with husband. Positive experiences over the period of pregnancy and postpartum were the strengthen-ing beliefs. Sources of support were, first, spouse then mother and faith. Support was also received from the physician in charge and through self-control. The nurse, by providing empathetic support, should be a person with whom they can express their feelings and share their experiences.

  • Myofascial pain is a condition associated with regional pain and muscle tenderness characterized by the presence of myofascial trigger points. In this case report, a subject complaining of nighttime bruxism was clinically assessed, and a latent trigger point of the anterior temporalis muscle was identified with manual palpation. A surface electromyographic (SEMG) exam of the anterior temporalis muscle harboring the latent trigger point demonstrated several SEMG features, including post-contraction irritability, delayed relaxation following contraction and accelerated muscle fatigue. It was concluded that a SEMG exam may detect abnormal masticatory muscle function and, therefore, assist in the evaluation of myogenous temporomandibular disorders.

  • Childhood intussusception is usually idiopathic, and pathological lesions as the leading point are found in limited cases. Sixteen operative cases with leadpoints among 2,889 cases of childhood intussusecption treated at the surgical departments of the affiliated hospitals of Catholic University over 19 years are reviewed. The approximate incidence of pathological lesions as the leading point was 0.6%. The male to female ratio was 2:1. The mean age was 3.5 years. There was not an age preponderance. The symptoms were vomiting(63%), abdominal pain(38%), irritability(38%), bloody stools(25%), fever(25%) and abdominal mass(6%). The average duration of the symptoms was 2.4 days(1-10days). The most common lesion was Meckel's diverticulum, followed by malignant lymphomas, polyps, ectopic pancreas, and cecal duplication. An ileocolic type was most frequent, followed by ileoileocolic and ileoileal. Segmental resection or wedge resection of the ileum was done in 10 cases, ileocecectomy in 3, and right hemicolectomy in 3. Surgical reduction was done only in an ectopic pancreas, with no later recurrence. The average hospital stay was 10 days. Postoperative adhesive ileus occurred in two cases, and in one of them adhesiolysis was performed. One case of malignant lymphoma died at 28 days after surgery due to chemotherapy related complication.
