망상적혈구 지수 및 미성숙망상적혈구 분획의 소아 빈혈에서의 임상적 의의 (Clinical Significance of Immature Reticulocyte Fraction and Reticulocyte Cellular Indices in Pediatric Anemia Patients)

  • 목 적 : 최근에 개발된 자동화혈구계산기를 이용하여 신속 간편하고 정확하게 IRF과 다양한 RCI 측정이 가능해졌다. 저자들은 정상 소아의 연령군별 RCI와 IRF의 정상치를 구하고, 소아빈혈 중 가장 흔한 원인질환인 철결핍성빈혈과 급성감염성빈혈을 포함한 소아빈혈군에서 RCI와 IRF치를 조사하여 소아빈혈의 조기진단과 철결핍성빈혈과 급성감염성빈혈의 감별진단에 유용한지를 알아보고자 하였다. 방 법 : 소아빈혈환자군 61명(급성감염성빈혈 34명, 철결핍성 빈혈 28명)과 정상대조군 57명을 대상으로 자동화혈구계산기 ADVIA 120을 이용하여 IRF과 CHr, MCVr, CHCMr, RDWr, HDWr 및 CHDWr를 포함한 RCI를 측정 후 대조군과 빈혈군 및 철결핍성빈혈군과 급성감염성빈혈군간의 평균치를 비교 분석하였다. 결 과 : 1) 정상대조군의 각 연령군별 IRF 평균치는 신생아군에서 가장 높았다. 정상대조군의 각 연령군별 RCI의 평균값을 비교해보면 신생아군에서 CHr, MCVr 및 HDWr이 다른 연령군에 비해 높았으나 CHCMr과 RDWr은 연령군별 차이를 보이지 않았다. 2) 빈혈군의 IRF는 정상대조군에 비하여 의미 있게 높았고, CHr과 CHCMr은 빈혈군에서 대조군에 비해 낮게 측정되었다. 그러나 MCVr은 빈혈군과 대조군 사이에 차이를 보이지 않았다. 3) 철결핍성빈혈군의 IRF는 정상대조군에 비하여 의미 있게 높았고, RCI 중 CHr과 CHCMr은 철결핍성빈혈군에서 대조군에 비하여 낮게 측정되었으며, RDWr은 철결핍성빈혈군에서 높았고, MCVr과 HDWr은 두 군간에 차이를 보이지 않았다. 4) 급성감염성빈혈군과 대조군의 IRF와 RCI를 비교해 보면 IRF는 급성감염성빈혈군에서 의미 있게 높았고, RCI 중 CHDWr만 차이를 보였으며, CHr, MCVr, CHCMr, RDWr 및 HDWr은 두 군간에 의의 있는 차이를 보이지 않았다. 5) 철결핍성빈혈군과 급성감염성빈혈군 간에 평균연령, 혈색소치 및 망상적혈구수는 차이가 없었다. IRF은 철결핍성빈혈군에 비하여 급성감염성빈혈군에서 의의 있게 높았고, RCI 중 CHr과 CHCMr은 급성감염성빈혈군에 비하여 철결핍성빈혈군에서 의의 있게 낮았으나, MCVr, RDWr 및 HDWr은 두 군간에 차이를 보이지 않았다. 결 론 : 빈혈의 감별진단에 IRF와 RCI가 매우 유용하리라 생각되며 특히 IRF는 급성감염에 의한 빈혈군에서 증가하는 것으로 보아 급성염증에 의한 골수반응이 있음을 생각할 수 있고, 또한 RCI 중 CHr은 빈혈의 감별진단에 아주 유용하고 특히 철결핍성빈혈군의 초기 진단에 유용하나 이에 대해 더욱 많은 연구가 필요하리라 사료된다.

만성폐쇄성폐질환자에서 기류제한 및 COPD 복합지수와 말초산소포화도의 연관성 (Association of Airflow Limitation and COPD Composite Index with Peripheral Oxygen Saturation in Patients with Chronic Obstructive Pulmonary Disease)

  • Objective: Chronic obstructive pulmonary disease (COPD) is characterized by progressive airflow obstruction that is only partly reversible, inflammation in the airways, and systemic effects. This study aimed to investigate the association between low peripheral oxygen saturation levels (SpO2), and composite indices predicting death in male patients with (COPD). Method: A total of 140 participants with post-bronchodilator FEV1/FVC ratio less than 0.7 were included. Three composite indices (ADO, DOSE, BODEx) were calculated using six variables such as age (A), airflow obstruction (O), body mass index (B), dyspnea (D), exacerbation history (E or Ex), and smoking status (S). Severity of airflow limitation was classified according to Global Initiative for Obstructive Lung Disease (GOLD) guidelines. SpO2 was measured by pulse oximetry, and anemia and iron deficiency were assessed based on blood hemoglobin levels and serum markers such as ferritin, transferrin saturation, or soluble transferrin receptor. Results: Participants with low SpO2 (<95%) showed significantly lower levels of %FEV1 predicted (p=0.020) and %FEV1/FVC ratio (p=0.002) compared to those with normal SpO2 levels. The mMRC dyspnea scale (p<0.001) and GOLD grade (p=0.002) showed a significant increase in the low SpO2 group. Receiver Operating Characteristic analysis revealed higher area under the curve for %FEV1 (p=0.020), %FEV1/FVC(p=0.002), mMRC dyspnea scale (p=0.001), GOLD grade (p=0.010), ADO (p=0.004), DOSE (p=0.002), and BODEx (p=0.011) in the low SpO2 group. Conclusion: These results suggest that low SpO2 levels are related to increased airflow limitation and the composite indices of COPD.

종합병원 소아청소년과 내원 초등학생의 비만도와 관련 요인 분석 (Obesity Index and Related Factors among Elementary School Students Visiting Pediatric Department of General Hospital)

  • The present study was conducted to investigate dietary habits and biochemical indices according to the obesity index of elementary school students visiting a hospital for diagnosis. The subjects of this study were 150 elementary school students and their mothers. The overweight rates of child subjects were 37.5% for males and 40.9% for females, whereas the rates of underweight children were 35.0% for males and 14.5% for females. Children's obesity index significantly decreased with an increase in the average monthly income of children's families. Children's obesity index, mothers' weights, BMI, and percent ideal body weight were all positively correlated. Blood biochemical values in children (hemoglobin, hematocrit, albumin, globulin, glucose, total cholesterol, and triglyceride levels) and their obesity index also showed positive correlation. For the underweight group, the frequency of iron-deficiency anemia was significantly high, whereas for the overweight group, the frequency of dyslipidemia was high. There was a significant increase in children's obesity index and a decrease in frequency of regular exercise with time spent on the TV/computer. The normal weight group recorded the highest total scores for mothers' nutritional knowledge, whereas the overweight group recorded the lowest total scores. Therefore, to maintain healthy weight in children, nutritional education programs for mothers and children should be developed.

충북지역 노인들의 약물복용 및 영양상태 II. 도시와 농촌 노인들의 영양실태 (Drug Consumption and Nutritional Status of the Elderly in Chung-buk Area II. Nutritional Status of Urban and Rural Elderly)

  • The nutritional status of 362 elderly men and women in Chung-bud area was evaluated in terms of their nutrient intakes, biochemical and anthrophometric measurements by interviews with questionnaires from August to October 1996. Mean intake of all nutrients except ascorbic acid did not meet the RDA for this sample. Protein, vit A, reboflavin, calcium were the most likely to be deficient on the basis of propotions of elderly consuming less than 75% of the RDA. The subjects nutrient intake was significantly affected by gender, marital status, number of family, family composition, educational level, pocket money, and region. Men in rural areas and women over 75 in urban areas were the most vulnerable groups with nutritional deficiency. According to serum biochemical indices, mean level of cholesterol, triglycerides, LDL, total protein, albumin and iron belonged to normal range but mean level of HDL showed below the normal range. More elderly men and women in urban areas showed a higher percentage of abormal level of cholesterol, triglycerides, LDL and HDL than those in rural areas. More elderly men and women in rural areas had abnormal levels of RBC, Hematocrit and hemoglobin compared to those in urban areas. Mean height and weight of elderly men was 161.4cm and 56.2Kg, respectively and 149.1cm and 50.5kg for women. The elderly in rural areas were taller than those in urban areas but had less weight, MAC, TSF, MAMC. Mean BMI of this sample belonged to normal range. However, the elderly in rural areas had a higher rate of underweight and lower for overweight than those in urban areas. The elderly in urban areas had higher blood pressure than those in rural areas.
