여포성 종양을 동반한 침윤성 리들씨 갑상선염 (Riedel's Thyroiditis) 1예 (A Case of Invasive Riedel's Thyroiditis with Follicular Neoplasm)

  • Riedel's thyroiditis is an uncommon disorder of unknown etiology that is characterized by an invasive process that partially destroys the gland and extends into adjacent neck structures. Its clinical manifestation as a stonyhard, poorly defined enlargement over the thyroid gland and local compression of the trachea, esophagus and recurrent laryngeal nerve can mimic invasive thyroid carcinoma and mask the accompanied thyroid neoplasm. A case of Riedel's thyroiditis in a 59-year-old female patient, admitted with a previous diagnosis of adenomatous goiter, is reported. So, we present this case with the review of literatures.

Unilateral Biportal Endoscopic Spinal Surgery Using a 30° Arthroscope for L5-S1 Foraminal Decompression

  • Foraminal decompression using a minimally invasive technique to preserve facet joint stability and function without fusion reportedly improves the radicular symptoms in approximately 80% of patients and is considered one of the good surgical treatment choices for lumbar foraminal or extraforaminal stenosis. However, proper decompression was not possible because of the inability to access the foramen at the L5-S1 level due to prominence of the iliac crest. To overcome this challenge, endoscopy-based minimally invasive spine surgery has recently gained attention. Here, we report the technical skills required in unilateral extraforaminal biportal endoscopic spinal surgery using a $30^{\circ}$ arthroscope to enable foraminal decompression at the L5-S1 level. Two 0.8-cm portals were created 2 cm lateral from the lateral border of the pedicles at the L5-S1 level. After sufficient working space was made, half of the superior articular process (SAP) in the hypertrophied facet joint was removed using a high-speed burr and a 5-mm wide osteotome, whereas the remaining inside part of the SAP was removed using a Kerrison punch and pituitary punch. The foraminal ligamentum flavum should be removed to inspect the conditions of the L5 exiting root and disc. Removing of the extruded disc could decompress the L5 root. The extraforaminal approach using a $30^{\circ}$ arthroscope is considered a minimally invasive alternative technique for decompressing foraminal stenosis at the L5-S1 level that preserves facet stability and provides symptomatic relief.

The Role of Minimally Invasive Plate Osteosynthesis in Rib Fixation: A Review

  • More than a century ago, the first scientific report was published about fracture fixation with plates. During the 1950's, open reduction and plate fixation for fractures were standardized by the founders of Arbeitsgemeinschaft $f{\ddot{u}}r$ osteosynthesefragen/Association for the Study of Internal Fixation. Since the introduction of plate fixation for fractures, several plates and screws have been developed, all with their own characteristics. To accomplice more fracture stability, it was thought the bigger the plate, the better. The counter side was a compromised blood supply of the bone, often resulting in bone necrosis and ultimately delayed or non-union. With the search and development of new materials and techniques for fracture fixation, less invasive procedures have become increasingly popular. This resulted in the minimally invasive plate osteosynthesis (MIPO) technique for fracture fixation. With the MIPO technique, procedures could be performed with smaller incisions and thus with less soft tissue damage and a better preserved blood supply. The last 5 years rib fixation has become increasingly popular, rising evidence has becomeavailable suggesting that surgical rib fixation improves outcome of patients with a flail chest or isolated rib fractures. Many surgical approaches for rib fixation have been described in the old literature, however, most of these techniques are obscure nowadays. Currently mostly large incisions with considerable surgical insult are used to stabilize rib fractures. We think that MIPO deserves a place in the surgical treatment of rib fractures. We present the aspects of diagnosis, preoperative planning and operative techniques in regard to MIPO rib fixation.

개실술에서 우측흉골우연소절개법을 이용한 최소침투수술 (Minimally Invasive Cardiac Surgery through A Small Right Parasternal Incision)

  • Background: Minimally invasive techniques for open heart surgery are widely accepted in these days. There are minimally invasive approaches by the right or left parasternal incision and another approaches by mini-sternotomy of upper or lower half or sternum. We report the safety and efficacy of minimally invasive technique with right parasternal incision compared with the routine full sternotomy. Material and Method: From April 1997 through February 1999, 20 patients(Group A) underwent minimally invasive cardiac operations. We chose 41 patients(Group B) whose preoperative diagnosis were the same and general conditions were similar and who underwent routine full sternotomy before April 1997. We compared A group and B group in many aspects. We performed routine full median sternotomy in B group but we did a minimally invasive technique through a small right parasternal incision in A group. Result: mean age was 36.1 years in both groups. In disease entities, there were 11 cases of ASD, 9 cases of mitral valve disease in group A, and 16 cases of ASD, 25 cases of mitral valve diseases in group B. In ASD, operation time, cardiopulmonary bypass time of aortic occulusion time were 263 min, 82 min, and 41 min in group A and 180 min, 53 min, and 32 min in group B. In mitral valve disease, operation time, cardiopulmonary bypass time and aortic occlusion time were 267min, 106 min, and 70min in A group and were 207 min, 82 min, and 69 min in group B. There were significant differences in operation time, CPB time, and ACC time between group A and group B. There was a significant difference in the amount of bleeding in postoperative day 1 between group A and group B of mitral diasease. However, there was no significant difference in the amount of bleeding in other comparisons. Mean length of incision was 8.7 cm in group A. There was no significant difference in postoperative complications between A group and B group. There was no mortality in either group. Conclusion: We conclude that this minimally invasive technique with right parasternal incision is cosmetically excellent but it is not effective in reducing operative time and there was no significant difference in recovery time and postoperative complications compared with routine full sternotomy.

승모판 성형술에 있어 최초 침습적 수술방식과 고전적 정중 흉골 절개술을 통한 접근방식의 비교 (Comparison of Mitral Valve Repair between a Minimally Invasive Approach and a Conventional Sternotomy Approach)

  • 배경: 최소 침습적 심장 수술은 전통적인 정중 흉골 절개술에 비해 수술 후 조기 회복 및 우수한 미용적 효과 등의 장점이 있지만 승모판막 성형술과 같이 복잡한 개심술에서의 성적은 아직 국내에 보고된 바 없다. 본 연구에서는 승모판 폐쇄 부전증의 환자군에서 로봇을 이용한 최소 침습적 승모판 성형수술의 유용성에 관해 정중 흉골 절개술과 비교하여 알아보고자 하였다. 대상 및 방법: 1993년 12월부터 2005년 12월까지 승모판막 폐쇄 부전으로 승모판막 성형술을 시행받은 520명의 환자를 연구대상으로 하였다. 이 중 정중 흉골 절개술을 통하여 승모판 성형술을 시행한 군을 S군(n=432), 최소 침습적 우측 개흉술을 이용한 환자군을 M군(n=88)으로 하여 임상 결과를 비교하였다. 우측 개흉술시에는 AESOP (Automated Endoscopic System for Optimal Positioning) 3000이라는 로봇을 이용하여 5 cm 이하의 절개로 수술을 시행하였다. 결과: 양 군 간의 나이, 성별, 수술 전 좌심실 구출률 등은 양 군 간의 차이는 없었다. S군에서 2예의 조기 사망이 있었으나 M군에서는 수술 사망예가 없었다. M군이 대동맥 차단 시간과 심폐기 가동 시간은 S군에 비해 유의하게 길었지만, 중환자실 재원기간과 수술 후 재원 일수는 S군에 비해 유의하게 짧았다. 제한된 절개에도 M군에서 승모판막 성형술식에 사용된 수술 술기는 제한이 없었으며, 술 후 의미있는 잔존 승모판막 폐쇄부전증의 빈도는 양 군에서 유사하였다 결론: 최소 침습적 우측 개흉술을 통한 승모판막 성형술을 시행하더라도 조기 성적은 차이가 없었다. 향후 최소 침습적 승모판막 성형술의 중-장기 성적의 관찰을 요하며 본 연구를 바탕으로 승모판막 성형술의 일차적인 접근 방법으로 최소 침습적 개흉술이 보다 폭넓게 시행될 수 있을 것으로 기대한다.

Robot-Assisted Cardiac Surgery Using the Da Vinci Surgical System: A Single Center Experience

  • Background: We report our initial experiences of robot-assisted cardiac surgery using the da Vinci Surgical System. Methods: Between February 2010 and March 2014, 50 consecutive patients underwent minimally invasive robot-assisted cardiac surgery. Results: Robot-assisted cardiac surgery was employed in two cases of minimally invasive direct coronary artery bypass, 17 cases of mitral valve repair, 10 cases of cardiac myxoma removal, 20 cases of atrial septal defect repair, and one isolated CryoMaze procedure. Average cardiopulmonary bypass time and average aorta cross-clamping time were $194.8{\pm}48.6$ minutes and $126.1{\pm}22.6$ minutes in mitral valve repair operations and $132.0{\pm}32.0$ minutes and $76.1{\pm}23.1$ minutes in myxoma removal operations, respectively. During atrial septal defect closure operations, the average cardiopulmonary bypass time was $128.3{\pm}43.1$ minutes. The median length of stay was between five and seven days. The only complication was that one patient needed reoperation to address bleeding. There were no hospital mortalities. Conclusion: Robot-assisted cardiac surgery is safe and effective for mitral valve repair, atrial septal defect closure, and cardiac myxoma removal surgery. Reducing operative time depends heavily on the experience of the entire robotic surgical team.

Feasibility of the AtriClip Pro Left Atrium Appendage Elimination Device via the Transverse Sinus in Minimally Invasive Mitral Valve Surgery

  • Background: Achieving external access to and manual occlusion of the left atrial appendage (LAA) during minimally invasive mitral valve surgery (MIMVS) through a small right thoracotomy is difficult. Occlusion of the LAA using an epicardial closure device seems quite useful compared to other surgical techniques. Methods: Fourteen patients with atrial fibrillation underwent MIMVS with concomitant surgical occlusion of the LAA using double-layered endocardial closure stitches (n=6, endocardial suture group) or the AtriClip Pro closure device (n=8, AtriClip group) at our institution. The primary safety endpoint was any device-related adverse event, and the primary efficacy endpoint was successful complete occlusion of blood flow into the LAA as assessed by transthoracic echocardiography at hospital discharge. The primary efficacy endpoint for stroke reduction was the occurrence of ischemic or hemorrhagic neurologic events. Results: All patients underwent LAA occlusion as scheduled. The cardiopulmonary bypass and aortic cross-clamp times in the endocardial suture group and the AtriClip group were 202±39 and 128±41 minutes, and 213±53 and 136±44 minutes, respectively (p=0.68, p=0.73). No patients in either group experienced any device-related serious adverse events, incomplete LAA occlusion, early postoperative stroke, or neurologic complication. Conclusion: Epicardial LAA occlusion using the AtriClip Pro during MIMVS in patients with mitral valve disease and atrial fibrillation is a simple, safe, and effective adjunctive procedure.
