  • More than 120 surgical methods for the correction of hallux valgus deformities have been reported. For the correction of moderate to severe hallux valgus deformities with aesthetic demands, minimally invasive surgery at the proximal area can be considered. This paper reports a case of moderate hallux valgus deformity treated by a minimally invasive proximal transverse metatarsal osteotomy followed by intramedullary plate fixation.

  • Chronic invasive fungal sinusitis (CIFS) is a relatively rare disease which occurs in immunocompetent or immunocompromised patients. We report a case of CIFS in ethmoid sinus with concurrent fungal ball in maxillary sinus. After surgical debridement and antifungal therapy, CIFS recurred with submucosal invasion in nasopharynx and osteomyelitis of clivus. Although CIFS encompasses a slowly progressing invasive fungal disease but potentially aggressive. It should be noted that CIFS could progress and extend through submucosal route without evidence of mucosal lesion.

  • Accurate diagnosis of Helicobacter pylori (H. pylori) infection is mandatory for the effective management of many gastroduodenal diseases. Currently, various diagnostic methods are available for detecting these infections, and the choice of method should take into account the clinical condition, accessibility, advantage, disadvantage, as well as cost-effectiveness. The diagnostic methods are divided into invasive (endoscopic-based) and non-invasive methods. Non-invasive methods included urea breath test, stool antigen test, serology, and molecular methods. Invasive methods included endoscopic imaging, rapid urease test, histology, culture, and molecular methods. In this article, we provide a review of the currently available options and recent advances of various diagnostic methods.

A Study on Jaundice Computer-aided Diagnosis Algorithm using Scleral Color based Machine Learning

  • This paper proposes a computer-aided diagnostic algorithm in a non-invasive way. Currently, clinical diagnosis of jaundice is performed through blood sampling. Unlike the old methods, the non-invasive method will enable parents to measure newborns' jaundice by only using their mobile phones. The proposed algorithm enables high accuracy and quick diagnosis through machine learning. In here, we used the SVM model of machine learning that learned the feature extracted through image preprocessing and we used the international jaundice research data as the test data set. As a result of applying our developed algorithm, it took about 5 seconds to diagnose jaundice and it showed a 93.4% prediction accuracy. The software is real-time diagnosed and it minimizes the infant's pain by non-invasive method and parents can easily and temporarily diagnose newborns' jaundice. In the future, we aim to use the jaundice photograph of the newborn babies' data as our test data set for more accurate results.

  • Chronic invasive aspergillosis is a life-threatening disease, especially in immunocompromised patients. The diagnosis and treatment of tracheal aspergillosis (TA) are challenging because of its rarity and nonspecific clinical presentations. The treatment standard of TA has been medical treatment like other forms of invasive aspergillosis, but patients with medically resistant TA require surgical intervention. We demonstrated a successful surgical outcome of chronic invasive TA in a 16-year-old patient with immunocompromised status related to acute myelocytic leukemia.

  • Klebsiella pneumoniae is known to cause an invasive syndrome characterized by primary liver abscess associated with metastatic infection. The characteristics of the metastatic infection involving the musculoskeletal system in this invasive syndrome are not well understood. The authors present a case report of a patient who developed abscesses of the lower extremities along with abscesses of multiple organs, such as the liver and eye, caused by K. pneumoniae. The patient was diagnosed early, and the infection was successfully controlled after several surgical treatments.

  • We have discovered Vicia grandiflora Scop., a newly invasive alien species in Baekun-ri, Okcheon-gun, Chungcheongbuk-do, Korea. This species is native to regions from Central and Southeast Europe to Central Asia and Iran and is reported as an invasive species in North America and Japan. This species is similar to the Vicia sativa complex (V. sativa subsp. sativa, V. sativa subsp. nigra) but can be readily distinguished by the undivided ovate to semi-hastate stipules of the upper leaves, yellowish petals, large size of its flower, and elongated hilum. In the field, V. grandiflora grows in disturbed sites near cultivated land, suggesting that their seeds are typically transported by vehicles along with fertilizer or livestock feed. Here, we present the morphological description, photographs, and sites of V. grandiflora growth, which will be useful in guiding the management of this invasive alien plant.

  • A minimally invasive procedure refers to a procedure that minimizes tissue damage due to incision. By limiting the size of the incision, it can be expected to reduce the time taken for wound healing, reduce pain, and reduce the risk of infection. Recently, studies have been attempted to dissect the structures of the neuromusculoskeletal system using ultrasound-guided minimally invasive technique. Among those, dissecting thread may be utilized in several musculoskeletal diseases, including carpal tunnel syndrome, cubital tunnel syndrome, trigger finger, and selective crural fasciectomy. In this brief review, the use of dissecting thread in treating neuromusculoskeletal diseases are described.

Considerations for Invasive Dental Treatment in Disabled Patients

  • Dental treatment for the disabled should be a customized that considers the characteristics and degree of cooperation of each disabled patient. There are additional considerations during implant treatment and tooth extraction in disabled patients. Since some brain lesion disorder or cardiac disease patients may be taking antiplatelet or anticoagulant medications, it is necessary to evaluate whether these medications should be discontinued before an invasive procedure. Precautions should be taken for patients with heart valve disease considering the risk of infective endocarditis, especially during invasive dental procedures. Moreover, disabled patients may have difficulty in following instructions and cautions. There are specific considerations for each stage of implant treatment in disabled patients. In the case of patients who are in the pre- or post-transplant state, it is necessary to assess their general condition and oral disease due to the risk of infection. Since disabled patients with various systemic diseases may visit the dental clinic, it is important to understand their characteristics and treatment process in order to flexibly adjust the dental treatment plan accordingly.

Invasive sphenoid sinus aspergillosis with normal findings on initial diagnostic tests that mimics Tolosa-Hunt syndrome-a diagnostic dilemma: a case report

  • Invasive sphenoid sinus aspergillosis can mimic Tolosa-Hunt syndrome (THS), leading to frequent misdiagnoses and potentially fatal consequences. We report a case of invasive sphenoid sinus aspergillosis initially misdiagnosed as THS. A 79-year-old man presented with right periorbital pain, ophthalmoplegia, and loss of vision. Initial evaluations including magnetic resonance imaging (MRI), were normal. He was first diagnosed with THS based on clinical features. The disease progressed despite high-dose intravenous steroid treatment, and an enhancing mass-like lesion was found in the right orbital apex, cavernous sinus, and sphenoid sinus on follow-up MRI. Aspergillosis was eventually confirmed by sphenoid sinus biopsy. The patient developed cerebral infarction and finally died despite being treated with amphotericin B. Given that invasive sphenoid sinus aspergillosis may initially resemble THS, high suspicion and rapid histological examination are important for diagnosis.
