Intraosseous ameloblastoma masquerading as exophytic growth: a case report

  • Intraosseous ameloblastoma is the most common and simple type of ameloblastoma prevalent among odontogenic tumors. Clinico-radiographically intraosseous ameloblastoma presents as slow, painless swelling or expansion of the jaws and described as multilocular expansile radiolucency that occurs most frequently in mandibular molar/ramus area. This article describes a case of follicular ameloblastoma involving 45 year old male which is different from the usual presentation, which includes-exophytic growth, different location and without expansion of the cortex.

  • Intracranial dural arteriovenous fistulas (DAVFs) are abnormal arteriovenous connections that lie within the dura. Intraosseous DAVFs involving diploic venous system are extremely rare. A 46-year-old woman presented with headache and right pulsatile tinnitus for three weeks. The tinnitus started after yelling. Digital subtraction angiography revealed DAVF within the basal portion of right parietal bone along the middle meningeal artery (MMA) groove. The fistula was fed by frontal branch of right MMA and drained into right transverse sigmoid sinus junction through dilated middle meningeal vein. The intraosseous DAVF involving diploic vein was successfully obliterated with Onyx embolization via transarterial route.

  • Primary intraosseous carcinoma(PIOC) is defined as a squamous cell carcinoma arising within the jaw, having no initial connection with the oral mucosa. The squamous cell carcinoma within the bone can be presumably developed from residues of the odontogenic epithelium, therefore, it is seen in the jaw only. Metastatic carcinoma from another primary site should be excluded in the diagnosis of Primary Intraosseous Carcinoma. This is a case of 62-year-old man, who initially diagnosed as odontogenic cyst on maxilla, but its pathologic examination was diagnosed as squamous cell carcinoma with odontogenic cyst. We treated this patient with partial maxillectomy, modified radical neck dissection(mRND), and postoperative radiation therapy.

  • Superficial lipoma was easy to diagnose, but deep lipoma, especially intraosseus lipoma, was difficult to detect pre-operatively. Intraosseous lipoma is very rare and had been difficult to differentiate from other tumors before image diagnosis was developed. Recently, the detecting ratio has increased due to advanced diagnostic tools. Preoperative diagnosis is crucial for the treatment plan and influences the approach method for excision. We experienced one case of misdiagnosis which was diagnosed as intraosseous lipoma according to the preoperative MRI but turned out to be a parosteal lipoma after the surgery.

  • A very uncommon tumor, primary intraosseous carcinoma (PIOC), is a carcinoma arising within the jaw. The definite diagnosis of PIOC is often difficult as the lesion must be distinguished from alveolar carcinoma that may invade the bone from the overlying soft tissues or from the tumors that have metastasized to the jaw from a distant site. A case of PIOC arising in the mandible is presented. The clinical, radiologic, and histologic features are described. This rare lesion should be considered in any differential diagnosis of a jaw radiolucency.

Radiographic study of the distribution of maxillary intraosseous vascular canal in Koreans

  • Background: This study aimed to investigate the distribution and prevalence of intraosseous loop (anastomosis between posterior superior alveolar artery and infraorbital artery) in Koreans detected on computed tomography (CT) images taken prior to sinus augmentation surgery. Methods: From the 177 patients who underwent sinus augmentation with lateral approach at Ewha Womans University Department of Implant Dentistry, 284 CT scans were evaluated. The canal height (CH), ridge height (RH), and canal height from the sinus floor (CHS) were measured on para-axial views at the first premolar, first molar, and second molar. The horizontal positions of the bony canals in the lateral wall were also classified. One-way analysis of variance (ANOVA) and t test were used to estimate the statistical differences (p < 0.05). Results: The intraosseous loops were detected in 92 CT scans (32 %). The mean vertical height of the bony canals from the alveolar crest (CH) was $23.45{\pm}2.81$, $15.92{\pm}2.65$, and $16.61{\pm}2.92mm$ at the second premolar, first molar, and second molar, respectively. In the horizontal positions of the bony canals, intraosseous type was the most predominant. The canal heights more than 15 mm and less than 17 mm were most prevalent (33.7 %) and those under 13 mm were 12.0 %. Conclusions: The radiographic findings in this study could be used to decide the lateral osteotomy line avoiding potential vascular complication. However, only one third of the canals could be detected in CT scans; a precaution should be taken for the possibility of severe bleeding during lateral osteotomy.

  • An ameloblastoma is one of the most common odontogenic tumors. Ameloblastoma is cytologically a benign tumor, but is clinically characterized by infiltrative growth and high recurrency. The treatment of ameloblastoma has been controversial. The aim of this paper is to consider effectiveness of curettage and enucleation after SSRO in the small-sized multilocular intraosseous ameloblastomas that have been treated more frequently by radical treatment. They were radiographically characterized by the cortical bone that was expanded or eroded locally and histopathologically by solid multilocular ameloblastomas. It is considered that curettage and enucleation after SSRO and long-term follow-up enable the small-sized multilocular intraosseous ameloblastomas that were characterized by almost destroyed cancellous bone and expanded cortical bone to treat minimizing facial disfigurement and masticatory dysfunction and sociopsychological impact produced by radical treatment. We recommend that the small-sized multilocular intraosseous ameloblastomas without involvement to the surrounding soft tissues be first treated by curettage and enucleation after SSRO.

Two Cases of Primary Osteolytic Intraosseous Meningioma of the Skull Metastasizing to Whole Skull and the Spine

  • We report here two cases of primary intraosseous meningioma with aggressive behavior. A 68-year-old man presented with a one year history of a soft, enlarging mass in the right parietal region. Magnetic resonance image (MRI) revealed a 6 cm sized, heterogeneously-enhancing, bony expansi1e mass in the right parietal bone, and computed tomograph (CT) showed a bony, destructive lesion. The tumor, including the surrounding normal bone, was totally resected. Dural invasion was not apparent Diagnosis was atypical meningioma, which extensively metastasized within the skull one year later. A 74-year-old woman presented with a 5-month history of a soft mass on the left frontal area. MRI revealed a 4 cm sized, multilobulated, strongly-enhancing lesion on the left frontal bone, and CT showed a destructive lesion. The mass was adhered tightly to the scalp and dura mater. The lesion was totally removed. Biopsy showed a papillary meningioma. The patient refused adjuvant radiation therapy and later underwent two reoperations for recurred lesions, at 19 and at 45 months postoperative. The patient experienced back pain 5 years later, and MRI showed an osteolytic lesion on the 11th thoracic vertebra. After her operation, a metastatic papillary meningioma was diagnosed. These osteolytic intraosseous meningiomas had atypical/malignant pathologies, which metastasized to whole skull and the spine.

  • Intraosseous hemangioma is a rare, benign vascular tumor of endothelial origin. It accounts for fewer than 1% of all hemangiomas, and very rarely occurs in the face. Intraosseous hemangioma usually presents as an asymptomatic lesion, but symptoms can occur due to the mass effect. The authors describe a case of intraosseous hemangioma of the zygoma with a review of the relevant literature. A 44-year-old man presented with a chief complaint of painless swelling on the left zygomatic region that had been slowly growing for the past year. On physical examination, a hard, non-movable mass in a deep layer was palpated. On computed tomography performed to evaluate its layers and extent, trabeculation was found inside the mass, but the lack of destruction of the surrounding bone suggested that the mass was benign. Complete surgical excision was performed under local anesthesia. After complete excision of the mass, slight erosions remained on the cortical bone of the zygoma, but because it was small enough not to cause a facial deformity such as depression or asymmetry, no additional reconstructive procedure was performed. There were no symptoms or recurrence during a 8-month follow-up period.

  • Intraosseous phlebography was performed to determine the healing effect of the dosing treated with vitamin-D$_3$ and/or calcium phosphate on the fractured bone in male rabbits. Femoral shafts of 16 adult male rabbits were fractured and these animals were divided into 3 treated groups and 1 control group l. The 1st treated group of rabbits was dosed with vitamin-D$_3$(VD), the 2nd treated group of rabbits was given calcium phosphate(CP), and the 3.d treated group of rabbits received vitamin-D$_3$ and calcium phosphate. The control group of rabbits was dosed with saline alone. Radiographs were taken weekly in each rabbit over 6 weeks period and the results obtained were as follows. 1. Sinusoidal vein was visualized at 3rd week in eve animal received CP and VC, at 5 week in animals received VD and in control animals. 2. Main nutrient vein was observed more clearly in treated groups than in control group of animals. 3. These results indicate that venous reconstruction of the fractured bone in groups of rabbits received CP and VC was earlier and more complete than those of other groups of rabbits. 4. Therefore, intraosseous phlebography is suitable for determination of the bone healing effect of vitamin-D$_3$ and/or calcium phosphate in femoral fractured rabbits.
