Skin Cancer: Clinico-Pathological Study of 204 Patients in Southern Governorates of Yemen

  • Background: Skin cancer is a group of heterogeneous malignancies, in general classified into non-melanoma skin cancer (NMSC) and melanoma skin cancer (MSC). Incidences are high in many parts in the world with considerable geographical and racial variation. In the Yemen, there has been scarce information about skin cancer. The aim of this study was to evaluate the demographic characteristics and histological trend of skin cancer in Southern Governorates of Yemen. Materials and Methods: This retrospective study covered 204 cases of skin cancer at the Modern Histopathology Laboratory and Aden Cancer Registry and Research Center, Faculty of Medicine and Health Sciences, University of Aden, for the period 2006-2013. Data were classified regarding different demographic and tumor related variables and analyzed using CanReg-4 for cancer registry and SPSS (version 21). Results: The commonest encountered skin cancer was NMSC (93.1%). Generally, skin cancer appears slightly more frequently in females than males with a 1:1.06 male: female ratio, with a mean age of 62.9 years. Slightly higher than one-third (36.3%) were from Aden governorate. The head and neck proved to be the most common site in both males and females (58%). Basal cell carcinoma (BCC) is the most common histological type of skin cancer (50.5%). Conclusions: Skin cancer is a common cancer in patients living in southern governorates of Yemen. The pattern appears nearly similar to the international figures with a low incidence of MSC.

  • ISO/IEC 11179 메타데이터 레지스트리(Metadata Registry, MDR)는 메타데이터의 등록 및 공유를 위해 개발된 국제표준이다. ISO/IEC 11179는 MDR을 객체 모델인 메타모델로 표현한다. 하지만 ISO/IEC 11179는 메타모델을 데이터베이스로 구축하기 위한 명확한 기준이 없으므로 표준 기반의 MDR 개발이 어려운 문제점이 있다. 이 논문은 MDR 구현을 위해 객체-관계 변환 방법론 기반의 MDR 데이터 모델을 설계한다. 이를 위해 연구된 객체-관계 변환 방법론의 변환 기법들을 관계성에 기준하여 분류하고, 이 변환 기법들의 사용 기준을 정의함으로 모델링 규칙을 제안한다. 이 논문은 MDR 데이터 모델에 대한 구현 결과로 관계형 데이터베이스 테이블을 구축한다. 실험 및 평가를 통하여 제안하는 모델링 규칙을 검증하고 구축된 테이블 구조의 적합성을 평가한다. 평가 결과로 제안 기법에 의해 구축된 테이블 구조는 표준 메타모델의 클래스와 관계성을 잘 보존함을 보인다.

Quantitative Evaluations on the Query Modeling and System Integrating Cost of SQL/MDR

  • SQL/MDR is a metadata registry query language used to consistently exchange and share the data between distributed metadata registries. It is an extension of the international SQL and is familiar to most database builders and administrators. It provides many advantages such as simplicity of query language, ease of use, an independent description for distributed querying, low cost for adding new systems, simplicity of exchanging mechanism, and so on. The goal of this paper is to evaluate and show its merits quantitatively. To achieve this goal, we define simulation models to compare with an existing approach and then describe the evaluation results. In the quantitative evaluation results, the good points of SQL/MDR can be identified and known.

Pediatric heart transplantation: how to manage problems affecting long-term outcomes?

  • Since the initial International Society of Heart Lung Transplantation registry was published in 1982, the number of pediatric heart transplantations has increased markedly, reaching a steady state of 500-550 transplantation annually and occupying up to 10% of total heart transplantations. Heart transplantation is considered an established therapeutic option for patients with end-stage heart disease. The long-term outcomes of pediatric heart transplantations were comparable to those of adults. Issues affecting long-term outcomes include acute cellular rejection, antibody-mediated rejection, cardiac allograft vasculopathy, infection, prolonged renal dysfunction, and malignancies such as posttransplant lymphoproliferative disorder. This article focuses on medical issues before pediatric heart transplantation, according to the Korean Network of Organ Sharing registry and as well as major problems such as graft rejection and cardiac allograft vasculopathy. To reduce graft failure rate and improve long-term outcomes, meticulous monitoring for rejection and medication compliance are also important, especially in adolescents.

  • 특이하게 운영(運營)되는 STN International은 세계 최대의 과학기술(科學技術) 온라인 정보검색(情報檢索) 시스템으로서 세 비영리기관인 미국(美國)의 Chemical Abstracts Service, 독일(獨逸)의 FIZ Karlsruhe, 일본(日本)의 과학기술정보(科學技術情報)센터가 공동으로 운영하고 있다. 본고에서는 STN에서 빠르고 효율적인 정보검색(情報檢索)을 위해서 설계(設計)한 Messenger 검색(檢索) 소프트웨어 기능, 검색시간(檢索時間)과 노력을 줄이는 진보된 소프트웨어인 SYN Express, 그리고 STN에 수록된 데이터베이스에 관해서 기술(記述)하였다.

  • Global Electronic Trading means that trading partners trade each other via Global Electronic Trading Network, that is, global business to business electronic commerce. Recently, where the cargo arrives ahead of the B/L, the importer cannot take delivery of the cargo without the B/L. This situation is referred to as the B/L dilemma. But the BOLERO system will resolve this B/L dilemma. Bolero Project is developing a cross industry utility platform for the secure, electronic transfer of commercial trade information world wide. After the successful testing of an original pilot project, The Bolero Association was formed in 1995 by a group of interested cross industry companies. As a joint venture between SWIFT and TT Club, bolero.net is changing the way the world trades by providing a web-based, paperless mode of commerce that is designed to become a global standard. Bolero International Limited published the first edition of Bolero Rulebook in 1999. The Bolero Rulebook as amended from time to time, governing the relationship between Users and their rights and obligations arising from the Bolero system. The Bolero service will be governed by a multilateral contract called the Bolero Rule Book which specifies the rights and responsibilities of Bolero and its users. The Title Registry and Bolero Bill of Lading provide a fully functional equivalent to the paper bill of lading. The Bolero Bill of Lading can be created, transferred, amended, and surrendered by way of designating to order party, blank endorsement, refusal by the transferee etc. Thus, this study deals with the functions of Bolero Bill of Lading and the problems and solutions in the Bolero Bill of Lading in point of definitions and operation under the Bolero Rulebook.

  • There are three different approaches to chemical risk assessment which will be considered in this paper. The U.S. Environmental Protection Agency(EPA) Cancer Risk Assessment includes some of the approaches used by the International Agency for Research on Cancer (IARC). The Agency for Toxic Substances and Disease Registry (ATSDR) effort is an evaluated database approach similar to that used in the National Institute for Occupational Safety and Health (NIOSH) Criteria Documents and in the documentations prepared by the Occupational Safety and Health Administration (OSHA) for the Permissible Exposure Limits (PELs) and those of the American Conference of Governmental Industrial Hygienists (ACGIH) for the Threshold Limit Values (TLVs). A third approach is used by the Committee on Toxicology.

Changing Trends of Adult Lymphoma in the Kingdom of Saudi Arabia - Comparison of Data Sources

  • Background: Lymphoma is one of the most common malignancies affecting the young Saudi population. This disease has diversified pathologies and clinical stages that necessitate well optimized clinical management. Regular updates of epidemiological behavior of lymphoma from various parts of the world are available but studies from the Kingdom of Saudi Arabia (KSA) in this field are not consistent. Objectives: The aim of this study was to investigate the current trends in presentation and distribution of lymphoma with special reference to incidence and mortality, gender, age, histopathological subtypes, and clinical stages at King Faisal Specialist Hospital and Research Centre (KFSH&RC). Materials and Methods: Our study included lymphoma data from Saudi Cancer Registry, and relative comparison against KFSH&RC tumor registry data, Gulf country data and International Agency for Research on Cancer data. Results: Common tumors in the West (lung, colon, and prostate) were found to be much less frequent in KSA while leukemia, lymphoma and thyroid cancers were more common. Non-Hodgkin Lymphoma (NHL) ranked 3rd most common cancer with age-adjusted incidence of 6/100,000. Estimated age adjusted mortality was 4/100,000 in KSA. There was a peak rise in incidence of lymphoma in 1997-2007. Most common NHL was diffuse large B cell lymphoma at KFSH&RC. A total of 434 cases were diagnosed in 5 years with 55% of them at advanced stage and 35% demonstrating bulky disease and high risk. KFSH&RC registered 35% of Hodgkins and 21% of total NHL identified in entire Saudi Cancer Registry, 2009. Conclusions: Results of this study are very unique, and reveal diverse trends. The findings provide valuable insights in the understanding of current epidemiological features of lymphoma in this part of the world.

데이터준비를 위한 XML 기반의 분산 MDR 검색 시스템 설계 (A Design of XML-Based Distributed MDR Retrieval System for Data Preparation)

  • 데이터마이닝은 방대한 데이터로부터 다차원적인 정보를 추출하는 것이다. 방대하게 구축되어 있는 데이터베이스에서 임의의 테이블의 컬럼에 대해 참조 할 수 있는 정보는 단순하게 컬럼명과 자료형 혹은 간단한 주석 정도이다. 그러한 비구조적이고 빈약한 내용만으로는 데이터마이닝을 위한 자료수집 및 자료탐색 단계에서 컬럼의 용도와 특성 및 스키마를 파악하여 데이터를 정제하고 수집하는 것이 난해 할 뿐만 아니라 너무 많은 시간이 소요된다. 이러한 문제를 해결하기 위해 본 논문에서는 관계형 데이터베이스 환경에서 데이터준비 단계 에 대부분의 시간을 소요하는 문제를 해결하기 위한 방안을 제안한다. 즉, 데이터 준비 단계에서 유용한 요소들을 메타데이터의 표준인 ISO/IEC : 11179 MDR (MetaData Registry) 규격에 맞는 표준 메타데이터를 제안하고, 이기종 및 이질 DBMS간에 호환 가능한 XML 기반의 분산 MDR 검색 시스템 구조를 제안한다.

Positive and Negative Effects of IT on Cancer Registries

  • In the new millennium people are facing serious challenges in health care, especially with increasing non-communicable diseases (NCD). One of the most common NCDs is cancer which is the leading cause of death in developed countries and in developing countries is the second cause of death after heart diseases. Cancer registry can make possible the analysis, comparison and development of national and international cancer strategies and planning. Information technology has a vital role in quality improvement and facility of cancer registries. With the use of IT, in addition to gaining general benefits such as monitoring rates of cancer incidence and identifying planning priorities we can also gain specific advantages such as collecting information for a lifetime, creating tele medical records, possibility of access to information by patient, patient empowerment, and decreasing medical errors. In spite of the powerful role of IT, we confront various challenges such as general problems, like privacy of the patient, and specific problems, including possibility of violating patients rights through misrepresentation, omission of human relationships, and decrease in face to face communication between doctors and patients. By implementing appropriate strategies, such as identifying authentication levels, controlling approaches, coding data, and considering technical and content standards, we can optimize the use of IT. The aim of this paper is to emphasize the need for identifying positive and negative effects of modern IT on cancer registry in general and specific aspects as an approach to cancer care management.
