조영증강 MR 혈관 조영술을 이용한 경동맥 기하학의 평가 (Assessment of Carotid Geometry by Using the Contrast-enhanced MR Angiography)

  • 목적 : 조영증강 MR 혈관조영술을 이용하여 경동맥의 기하학(geometry)을 측정하고, 경동맥 기하학과 임상요인 사이의 상관관계를 분석하고자 하였다. 대상 및 방법 : 2007년 1월부터 6월까지 대동맥궁 상부 조영증강 MR 혈관조영술을 시행한 216명을 대상으로 하였다. 동맥경화 위험인자(나이, 고혈압, 당뇨, 고지혈증, 흡연), 성별, 체중, 신장, 체질량지수를 조사하였다. MR 혈관조영술에서는 경동맥 기시부 유형, 경동맥의 굴곡도, 내경동맥 분지각, 내경동맥 비틀림, 대동맥궁 소-유형(bovine type) 여부를 평가하였다. 경동맥 기시부 유형은 위치에 따라 3단계로 분류하였다. 기하학 요소와 임상정보간의 상관관계를 회귀분석을 이용하여 분석하였다. 결과 : 경동맥 기시부를 종속변수로 한 다항 로지스틱 회귀분석에서 나이가 많을수록 경동맥 기시부 1형의 빈도가 높았고, 체질량지수가 클수록 2형, 3형일 빈도가 높았다. 좌, 우측 경동맥의 굴곡도의 평균은 각각 $240.9{\pm}69.0^{\circ}$$154.4{\pm}55.0^{\circ}$였다. 다변량 회귀분석에서 고혈압 환자와 여자, 나이, 체질량지수가 클수록 경동맥의 굴곡도가 증가하였다. 내경동맥 분지각, 내경동맥 비틀림, 대동맥궁 소-유형은 통계분석에서 임상정보와 유의한 상관관계를 보이지 않았다. 결론 : 경동맥 기하학에 나이, 체질량지수, 성, 고혈압 여부가 영향을 미칠 수 있다. 이러한 정보는 대동맥궁 및 경동맥의 영상정보가 없는 경우, 경동맥 기하학 예측 및 침습적 혈관내 시술 전략 수립에 도움이 될 것이다.

Hemorrhagic Moyamoya Disease : A Recent Update

  • Moyamoya disease (MMD) is a progressive cerebrovascular disease with unknown etiology, characterized by bilateral steno-occlusive changes at the terminal portion of the internal carotid artery and an abnormal vascular network formation at the base of the brain. MMD has an intrinsic nature to convert the vascular supply for the brain from internal carotid (IC) system to the external carotid (EC) system, as indicated by Suzuki's angiographic staging. Insufficiency of this 'IC-EC conversion system' could result not only in cerebral ischemia, but also in intracranial hemorrhage from inadequate collateral anastomosis, both of which represent the clinical manifestation of MMD. Surgical revascularization prevents cerebral ischemic attack by improving cerebral blood flow, and recent evidence further suggests that extracranial-intracranial bypass could powerfully reduce the risk of re-bleeding in MMD patients with posterior hemorrhage, who were known to have extremely high re-bleeding risk. Although the exact mechanism underlying the hemorrhagic presentation in MMD is undetermined, most recent angiographic analysis revealed the characteristic angio-architecture related to high re-bleeding risk, such as the extension and dilatation of choroidal collaterals and posterior cerebral artery involvement. We sought to update the current management strategy for hemorrhagic MMD, including the outcome of surgical revascularization for hemorrhagic MMD in our institute. Further investigations will clarify the optimal surgical strategy to prevent hemorrhagic manifestation in patients with MMD.

동맥관개존증의 외과적 요법 (Surgical Management of Patent Ductus Arteriosus)

  • Seventeen cases of vascular injuries treated in Chungnam National University Hospital during the period from Apr. 1980 to Sep. 1988 were reviewed. Common causes of injuries were stab wound, automobile accidents and iatrogenic injuries. Of the 11 arterial injuries, 3 were femoral artery which was the commonest in frequency, the next was 2 cases of subclavian, common carotid, iliac artery. Of the 10 venous injuries, the subclavian vein and internal jugular vein were common in frequency. The most frequent type of injuries was laceration[13 cases in 21]. Vascular reconstruction was done by lateral suture repair in 8 cases, autogenous vein graft in 2, prosthetic vascular graft in 5, direct anastomosis in 1 case. Simple ligation was done in 5 cases. There was 1 case of visual field defect as a sequelae in right common carotid artery transaction and 1 case of mortality.

단삼(丹蔘), 홍화(紅花)가 흰쥐의 뇌허혈에 미치는 영향 (The Effects of Salviae miltiorrhizae Radix, Carthami Flos on Brain Ischemia Experimentally Induced from the Occlusion of Left Common Carotid Artery in Rats)

  • Objectives: This study investigates the effects of Salviae miltiorrhizae Radix, Carthami Flos on Brain ischemia of the rats induced from the Occlusion of Lt. Common Carotid Artery. Methods: I observed effects using light microscopes and examined tissue of parietal lobe and hippocampus and VEGF-immunoreactive cells. Results: A small number of VEGF-immunoreactive cells are observed in the control group. VEGF-immunoreactive cells in Salviae miltiorrhizae Radix-administered group were slightly increased compared with control group. VEGF-immunoreactive cells in Carthami Flos-administered group were significantly increased compared with control group. Neurons of parietal lobe and pyramidal cells of hippocampus in the control group were greatly damaged.(neuronal densitity, form of dendrite and axon) On the other hand, neurons of parietal lobe and pyramidal cells of hippocampus in Salviae miltiorrhizae Radix-administered group were less damaged. Neurons of parietal lobe and pyramidal cells of hippocampus in Carthami Flos-administered group were significantly less damaged compared with control group. Conclusion : Salviae miltiorrhizae Radix, Carthami Flos can effect on stimulating angiogenesis and reducinging the damage of neurons in the rats induced from the Occlusion of Lt. Common Carotid Artery.

Efficacy of Proximal Aspiration Thrombectomy for Using Balloon-Tipped Guide Catheter in Acute Intracranial Internal Carotid Artery Occlusion

  • Objective : Mechanical thrombectomy (MT) for acute intracranial internal carotid artery (ICA) occlusion is often complicated by difficult revascularization and non-involved territory embolization possibly related with larger clot-burden. This study aims to evaluate the efficacy of proximal aspiration thrombectomy (PAT) using a balloon-tipped guide catheter for clot-burden reduction in such cases with period-to-period analysis (period 1 : standard MT without PAT; period 2 : PAT first, then standard MT for the remaining occlusion). Methods : Eighty-six patients who underwent MT for acute intracranial ICA occlusion were included in this analysis from the prospectively maintained stroke registry (33 patients in period 1 and 53 in period 2). In period 2, 'responder' was defined as a case where some amount of clot was retrieved by PAT and the following angiography showed partial or full recanalization. Results : Fifteen of fifty-three patients in period 2 (28.3%) were 'responders' to PAT. There was a significantly higher incidence of atrial fibrillation in the 'responder' subgroup. Period 2 showed a significantly shorter puncture-to-reperfusion time (94.5 minutes vs. 56.0 minutes; p=0.002), a significantly higher Thrombolysis in Cerebral Infarction of 2b-3 reperfusion (45.5% vs. 73.6%; p=0.009), but only a trend for better 3-month favorable outcome (mRS 0-2; 36.4% vs. 54.7%; p=0.097). There was no increase in the incidence of procedure-related complications or intracranial hemorrhage in period 2. Conclusion : A strategy of PAT before standard MT may result in shorter puncture-to-reperfusion time and better angiographic outcome than a strategy of standard MT for acute intracranial ICA occlusion.

Is Stent-Assisted Coil Embolization for the Treatment of Ruptured Blood Blister-Like Aneurysms of the Supraclinoid Internal Carotid Artery Effective? : An Analysis of Single Institutional Experience with Pooled Data

  • Objective : Given the high risk of rebleeding and recurrence of blood blister-like aneurysms (BBAs), we treated ruptured BBAs of the internal carotid artery (ICA) with stent-assisted coil embolization (SAC). This study aimed to evaluate the efficacy and safety of SACs. Methods : We retrospectively reviewed clinical and radiological data from eight patients with ruptured BBAs of the supraclinoid ICA. The modified Rankin Scale (mRS) was used to assess clinical outcomes, while radiological outcomes were evaluated on angiographs. For a pooled analysis, data from literature reporting the outcomes of ruptured BBAs treated with SAC were collected and analyzed in conjunction with our data. Results : In our cohort, the mean Raymond classification score was 1.57±0.53 immediately after initial endovascular treatment. There were no perioperative complications or rebleeding events during the follow-up period. The mean mRS score at patient discharge was 1.00±0.81 and improved to 0.28±0.48 by the last follow-up day. The recurrence rate was 25% with an asymptomatic presentation and successful treatment with multiple stent insertion. Pooled analysis of 76 cases of SAC revealed a complete occlusion rate immediately after treatment of 54.8%, rebleeding rate 7.94%, and recurrence rate 24.2%. Good clinical outcomes with mRS score 0-2 were observed in 89.9% by the last clinical follow-up. Total mortality rate was 7.7%. Conclusion : This treatment appears to not only minimize the hemodynamic burden on the fragile dome specific to this type of aneurysm, but also provides an opportunity for safe and effective treatment in recurrent cases.

Cerebral Arteriovenous Malformation Associated with Moyamoya Disease

  • The coexistence of moyamoya disease (MMD) with an arteriovenous malformation (AVM) is exceedingly rare. We report two cases of AVM associated with MMD. The first case was an incidental AVM diagnosed simultaneously with MMD. This AVM was managed expectantly after encephalo-duro-arterio-synangiosis (EDAS) as the main feeders stemmed from the internal carotid artery, which we believed would be obliterated with the progression of MMD. However, the AVM persisted with replacement of the internal carotid artery feeders by new external carotid artery feeders from the EDAS site. The AVM was eventually treated with gamma knife radiosurgery considering an increasing steal effect. The second case was a de novo AVM case. The patient was initially diagnosed with MMD, and acquired an AVM eight years later that was slowly fed by the reconstituted anterior cerebral artery. Because the patient remained asymptomatic, the AVM is currently being closely followed for more than 2 years without further surgical intervention. Possible differences in the pathogenesis and the radiologic presentation of these AVMs are discussed with a literature review. No solid consensus exists on the optimal treatment of MMD-associated AVMs. Gamma knife radiosurgery appears to be an effective treatment option for an incidental AVM. However, a de novo AVM may be managed expectantly considering the possible risks of damaging established collaterals, low flow characteristics, and probably low risks of rupture.

가성동맥류의 수술적 가료 (Surgical Management of Pseudoaneurysm)

  • Objective : Surgical experiences of pseudoaneurysms such as traumatic, mycotic and ill-defined unknown causes of aneurysms are rare. The authors have studied the results of surgical management from such cases in our series. Patients and Method : In the last 17 years, 1320 patients with cerebrovascular aneurysms were managed surgically. Among these, 16 patients showed the pseudoaneurysms. The authors analyzed retrospectively the clinical characteristics, treatment methods, management outcomes and problems in the managements. Results : There were 6 patients with traumatic aneurysm, 4 mycotic aneurysms and 6 ill-defined unknown causes of aneurysm. The sites of traumatic aneurysms were cavernous portion of the internal carotid artery(n=3), distal portion of the anterior cerebral artery (n=2) and vertebral artery(VA : n=1). Good outcomes in 5 cases could be obtained by extracranial - intracranial bypass followed by parent vessel occlusion or resection of aneurysm followed by re-anastomosis of parent vessel. The sites of mycotic aneurysm were peripheral portions of middle cerebral artery(MCA : n=3) and posterior cerebral artery(PCA : n=1). The outcomes of the patients with a mycotic aneurysm were relatively poor. It was partially due to the development of new aneurysm after treatment in one. The sites of ill-defined unknown causes of aneurysm were extracranial carotid artery(n=3), V2 portion of the VA(n=1), peripheral portion of the PCA (n=1) and MCA(n=1). Good outcome in all cases could be obtained by resection of aneurysm with or without saphenous vein graft. Conclusion : For the treatments of cerebrovascular pseudoaneurysm, combinations of aggressive medical, endovascular and surgical managements seem mandatory. Insertion of stent for a extracranial carotid artery aneurysm and coiling for a peripheral mycotic aneurysm can be option in future.

Treatment for Giant Fusiform Aneurysm Located in the Cavernous Segment of the Internal Carotid Artery Using the Pipeline Embolization Device

  • The pipeline embolization device (PED) is a new endovascular device for treatment of complex, fusiform and wide-neck intracranial aneurysms. The main mechanism of this stent is to divert the flow in the parent artery with reduction of inflow in the aneurysm leading to thrombosis. We treated a 40-year-old woman who had left facial pain and orbit discomfort. Angiography showed a giant fusiform aneurysm located in the cavernous segment of the left internal carotid artery. A PED was successfully deployed across the aneurysm. The procedure and post-procedural course were uneventful. After 3 months, angiography showed complete obliteration of the aneurysm with good patency of the branching vessels originating from the deployed segment. The patient's symptoms improved completely without complications.
