A Clinical Observation of the Stroke Patients : Using Functional Independence Measure (Functional Independence Measure를 이용한 뇌졸중(腦卒中) 환자의 임상적 관찰)

  • BACKGROUND The purpose of this study was to investigate disability of the patients after stroke, clinical applicability and usefulness of the FIM in evaluating the functional abilities and to assess the characteristics of the patients after stroke. And this study examined difference by predictive factors on the treatment outcome. METHOD The study was composed of 31 stroke patients who were within 1 week after stroke and had been treated in hospital more than 4 weeks. Improving rate was measured by using the Functional Independence Measure(FIM), and the correlation was analyzed between Motor FIM and Cognitive FIM. We analyzed significant meaning statistically in the mean value of FIM. RESULT 1. There was no significant meaning statistically in the mean value of FIM that was between both at admission and after 2 weeks. But there was proportional correlation between both at admission and after 4 weeks, and significant meaning statistically. 2. There was proportional correlation between both FIM and Motor FIM, and significant meaning statistically. But there was no significant meaning statistically between both FIM and Cognitive FIM. 3. There was no significant meaning statistically between both FIM and sex, age, side of weakness, stroke subtype, hypertension, hypercholesterolemia, diabetes mellitus, heart disease, starting point of rehabilitation therapy and past history of cerebral vascular accident. While dysarthria affected functional recovery in stroke patients, and significant meaning statistically. CONCLUSION 1. FIM appeared to be a valid and reliable method to measure the functional abilities of the patients after stroke. 2. Only one thing between Motor FIM and score didn't influence FIM score. 3. Dysarthria affected functional recovery in stroke patients, and it is the statistically significant factor.

  • Generally, internal parameters of the motors and generators can be divided to the resistance and inductance components. They can become a cause of the changing internal parameters because they have sensitive characteristics due to external conditions. The changed parameters can generate the outputs which include error values from the speed and current controllers. Also, it can bring the temperature increase and mechanical damage to the system. Therefore, internal parameters of the motors and generators need to obtain their values according to the external conditions because it can prevent the mechanical damage caused by the changed parameters. In this paper, the off-line parameter identification method is verified using the Goertzel algorithm. The motor used in the simulation and experiments is an interior permanent magnet synchronous motor (IPMSM), and the proposed algorithm is verified by the simulation and experimental results.

  • Amyotrophic lateral sclerosis (ALS) is a progressive disorder that causes degeneration of motor neurons of the brain and spinal cord. ALS is a progressive, fatal neuromuscular disease characterized by loss of motor neurons leading to muscle weakness. Sensation and mental function stay intact during the course of the disease. ALS is characterized by both upper and lower motor neuron damage. Diagnosis includes magnetic response imaging (MRI) electromyogram (EMG), muscle biopsy, and blood test. There is no cure for ALS. We recently observed three cases of ALS. The patients were diagnosis with ALS by EMG and symptoms. This report was conducted to evaluate how oriental medical treatment can affect ALS. We report the change of their symptoms through oriental medical treatment compared with taking riluzole.

Banhasasim-tang Administration for Treatment of Functional Gastrointestinal Disorders after Motor Vehicle Accidents: Case Series (교통사고 이후 발생한 기능성 위장관장애에 대한 반하사심탕 투여 : 연속증례)

  • Objectives: These cases report the benefits of administration of Banhasasim-tang extracts or decoctions to patients with functional gastrointestinal disorders after motor vehicle accidents. Methods: The patients were treated with Banhasasim-tang extract or decoction three times daily. History-taking and imaging tests were used to differentiate other diseases. We evaluated the patients using the Gastrointestinal Symptom Rating Scale (GSRS), which is used to assess overall gastrointestinal symptoms. Results: Treatment with herbal medicine resulted in a decrease in the patients' complaints of symptoms. The GSRS scale showed improvement in all four cases. Conclusions: Banhasasim-tang, regardless of its form, has beneficial effects in patients with functional gastrointestinal disorders after motor vehicle accidents.

  • In this study, flow visualization method to simulate internal flows of solid rocket motor in a water tunnel is introduced. The tunnel provides excellent visualization of vortex flows and has been used to propellant grain design of the solid rocket motor. A water tunnel is suggested for the research and the visualization test using dye, hydrogen bubble generator and PIV has been studied and discussed.

The Effect of Attentional Focus on Performance of Task (집중방식이 과제수행에 미치는 영향)

  • The purpose of this study is to introduce the effect of attentional focus on performance of task. Previous studies has shown that motor learning can be enhanced by directing performers's attention to the effects of their movements(external focus), rather than to the body movement producing the effects(internal focus). Wulf and colleagues have invoked the 'constrained action hypothesis' to explain the comparative benefits of adopting an external rather than an internal focus of attention. This hypothesis proposed that when performers utilize an internal focus of attention, they may actually constrain or interfere with automatic control processes that would normally regulate the movement, whereas an external focus of attention allows the motor system to more naturally self-organize. Electromyography(EMG) was used to determine neuromuscular correlates of external versus internal focus differences. EMG activity was lower with an external relative to an internal focus. This suggest that an external focus of attention enhances movement economy, and presumably reduces 'noise' in the motor system that hampers fine movement control. Focusing on a more remote effect seems to facilitate the discriminability of the effect from the body movements that produced it and to be more beneficial than focusing on a very close effects. There might be an optimal distance of the effect, at which ti wis easily distinguishable from the body movement but at which it is also still possible for the performer to relate this effect to the movement techniques. Future Studies of motor learning of patient need to accommodate these new finding and account for the role of the learner's attentional focus and its influencing on learning.

Study on function evaluation tools for stroke patients (뇌졸중(腦卒中) 환자(患者)의 기능평가방법(機能評價方法)에 대(對)한 연구(硏究))

  • Our conclusions for function evaluation tools of Stroke patients are as follows. 1. Evaluating tools of Activities of Daily Living, Katz Index, Barthel Index, Modified Barthel Index have high validity and reliability because of ease of measuring, high accuracy, consistency, sensitivity and sufficient stastistics, but they mainly measure motor function except sense, mentation, language, and social conception. Therefore cerebrovascular disease and brain injury in trauma patients with lacked acknowledgement and sensation, we are not able to apply these tools. 2. PULSES Profile is a useful scale for measuring the patient's over-all status, upper and lower limb functions, sensory components, excretary functions, and intellectual and emotional adaptabilities. It is recognized as a good, useful tool to evaluate patient's whole function. 3. Motor Assessment Scale was designed to measure the progress of stroke patients. The scale was supplemented with upper arm function items. We believe that the Motor Assessment Scale could be a useful evaluation tool with inter-rater reliability ,test-retest reliability. 4. The existing evaluation tools, Katz Index, Barthel Index, Modified Barthel Index, PULSES Profile, Motor Assessment Scale, mainly measured the rehabilitational motor function of sequela of cerebrovascular patients. On the other hand CNS & INH stroke scale can measure cerebrovascular disease patient's neurologic deficits and over-all stautus, which are recognition ability, speech status, motor function, sensory function, activities of daily living. Those scales have been recognized as useful tools to measure function of cerebrovascular disease patients and have increased in use. 5. Every function evaluation tool was recognized to have some validity and inter-rater, test-retest reliability in items of each evaluation tool and total scores of each evaluation tools, but it is thought that none of these scales have been fully validated and proved reliable. Therefore afterward, the development of a highly reliable rating system may best be accomplished by a careful comparison of several tools, using the same patients and the same observers in order to choose the most reliable items from each. 6. Ideal evaluation tools must have the following conditions; (1) It should show the objective functional statues at the same time. (2) It should be repeated consecutively to know changed function status. (3) It should be easy to observe the treatment program. (4) It should have the same result with another rater to help rater exchange information with treatment team members. (5) It should be practical and simple. (6) The patient should not suffer from the observer.

  • Monoplegia is the paralysis of either the upper or lower limb. Monoplegia is commonly caused by an injury to the cerebral cortex; it is rarely caused by an injury to the internal capsule, brain stem, or spinal cord. Most cerebral cortex is derived from the occlusion of a brain cortex blood vessel due to thrombus or embolus. According to motor homunculus, lower limb monoplegia occurs from limited damage to the most upper part of the primary motor area(Brodmann's area 4, located in precentral gyrus). Clinically, lower limb monoplegia due to brain cortical infarction is commonly misunderstood as monoplegia due to spinal injury because the lesion is situated at the most upper part of precentral gyrus. We had many difficulties in finding lesion on brain CT, but we diagnosed two patients correctly by using an MRI, who have lower limb monoplegia due to brain cortical infarction oriental treatment.

Interobserver Reliabilities of Modified Barthel Index, and Motor Assessment Scale in Stroke Patients (Modified Barthel Index 및 Motor Assessment Scale을 이용한 검사자간의 신뢰도 검사)

  • We have conducted a study of the interobserver reliabilities of Modified Barthel Index and Motor Assessment Scale with 30 patients hospitalized with strokes in the department of circulatory internal medicine, Sang Ji University Oriental Hospital. The observations were performed by two staff and residents in the circulatory internal medicine department. Raters were assigned in random pairs to individual patients. Evaluations were performed independently by the two observers. In order to minimize the impact of fluctuations in the patients' clinical status, the second set of observations immediately followed the first. Each patient was used for only one pair of evaluations. The results were as follows. 1. Mean kappa value of 13 items in Modified Barthel Index(MBD was 0.742, which indicated excellent interobserver reliability. The kappa values indicated almost $perfect({\kappa}:\;0.81-1.00)$ for 4, substantial for $9({\kappa}:\;0.61-0.80)$, and moderate for $2({\kappa}:\;0.41-0.60)$ of 13 items. All items. except Grooming item, showed statistically significant interobserver agreement(p<0.01) 2. Mean kappa value of 8 items. except General tonus, in Motor Assessment Scale(MAS) was 0.823, which indicated excellent interobserver reliability. and this value of Motor Assessment Scale was more high than MBI' s value. 0.81. The kappa values indicated almost perfect for 5, substantial for 3 of 8 items. All items showed statistically significant interobserver agreement(P<0.01).

Internal Flow Characteristics of Simulated Dual Pulse Rocket Motor by Using the Hot Gas and Cold Gas (Hot Gas와 Cold Gas를 이용한 모사 이중펄스 로켓 추진기관의 내부 유동 특성)

  • Dual pulse rocket motor is a variant of solid rocket motor with two propellant grain separated by a pulse separation device. The major performance of such a rocket motor is influenced by the change in the hole area of pulse separation device to nozzle throat area ratio. In this study, we performed flow analysis to investigate the internal flow characteristics according to the pulse separation device hole area to nozzle throat area ratio change. Gases used flow analysis were used combustion gas of HTPB/AP composite propellant and nitrogen gas. Flow analysis results of the dual pulse rocket motor were validated by comparison with experimental results of pneumatics. Commercial CFD code ANSYS FLUENT 14.5 is used in this study to simulate flow analysis.
