Recent changes in the health care environment have directed increasing attention to the number and specialty mix of practicing physicians. A major concern identified in Korean health care system is the serious oversupply of specialists and a relative lack of primary care physicians. Currently only 21% of Korean physicians are primary care physicians(general practitioners and family physicians), and less than 10% of recent medical school graduates are choosing to enter primary care. More primary care physicians are needed to deal with major problems in the current health care system, such as cost and access. The infrastructure that relies on primary care physicians is needed to deliver cost-effective and efficient care. To achieve a better balance of primary care to non-primary care physicians. more medical students need to choose careers in one of the primary care specialties(family medicine. internal medicine and pediatrics). This paper suggests the necessity of reforming the Korean graduate medical education system, that is, establishing the path of training primary care physicians in internal medicine and pediatrics residency training programs.

Solenoid operated electromagnetic control valve (ECV) using in an external variable displacement swash plate type compressor is widely used for air conditioning control system because of its low energy consumption and high efficient characteristics. ECV controls the entire vehicle air conditioning system by means of a pulse width modulation (PWM) system that supplied from an external controller. Different pressure ports located within ECV has important functions to control the air/refrigerant flow through its internal passages. The flow paths are preciously maintained with acceptable ranges of leakage (gap) between the parts inside it which is followed by effective design and critical dimensioning of its internal features. Therefore, it saves energy losses from the solenoid operation as well as ensures the balance of forces within it. The research paper highlights analysis of the leakages (at different pressure ports) and dimensioning tolerance factors that affects the ECV performance.

Dreams are experiences of life that is recognized to have taken place in .the mind while asleep. The phenomenon of dreams is occured by external stimuli such as somatic factor and emotion-arousing pre-sleep stimuli. The contents of dreams reflect the dreamer's unique psychic situation, we can view them from category of Yin and Yang and the Five Elements(陰陽五行). Also dreams are characterized by an objectivity that provides whatever in necessary for psychic balance, regardless of the ego's wishes. Hence dreams are objective sources of psychotherapy. The doctor must not treat patient by the doctor's subject mind. The Orient has taken the idea that human beings have the divine center in the heart, so dreams in the psychotherapy of the oriental medicine are more valuable as representation of inner world. The images of dream are classified as the deficiency and excessiveness of Yin-Ki and Yang-Ki of the internal organs, an property and source of stimulus. Dream applies to diagnose the disease of internal organs and to decide to the method of medical treatment.

Atrial fibrillation (AF) can occur in acute coronary syndrome (ACS), which is a serious medical condition and may require the use of antiplatelet agents in addition to anticoagulants for stroke prevention. Recently, novel or non-vitamin K antagonist oral anticoagulants (NOACs) have been increasingly used for stroke prevention in patients with AF instead of traditional OACs. The duration of treatment or treatment with a stepwise approach (e.g. triple, double, or monotherapy) is determined depending on the clinical setting and the balance between the risks of ischemic stroke and bleeding. However, some concerns and controversies in the use of NOACs in patients with AF and ACS need to be addressed. Here, the current management for NOAC therapy in patients with ACS and AF will be reviewed based on recently published guidelines.

The cerebellum processes input from other areas of the brain, spinal cord and sensory receptors to provide precise timing for coordinated, smooth movements of the skeletal muscular system. A stroke affecting the cerebellum may cause vertigo, nausea, balance and coordination problems. The papers on central vertigo caused by cerebellar infarction were reported by Kim, Ku, Ko, Choi, and Han. Their cases were classed as Heo hoon, Dam hoon, and Sangchopungyeul yeul dam, and they treated with Jaeumkunbi-tang, Taeksa-tang, Cheonghunwhadam-tang gami, Cheonginwhadam-tang, Hachulbosim-tang. In this paper, three patients who suffered from vertigo were diagnosed as cerebellar infarction. They felt too giddy to walk without any help. After Oriental medical treatment based on Dongeuibogam, their sequelae of cerebellar infarction - vertigo, nausea, ataxia - had improved dramatically. So we report these cases, and suggest Oriental medical treatment based on Dongeuibogam have effect on cerebellar infarction. But more clinical case reports should be further examined.

The objective of this work is to perform a geometric analysis of the net present value (NPV) and Internal Rate of Return (IRR), defining analytics and in verifying the relationship between geometric properties of such functions. For this simulation, was used the values of the cash flows for each period identical and equal to US$ 200.00 cash, the initial investment US$ 1,000.00 and investments of each identical and equal to US$ 50.00 period. In addition, the discount rate and time were considered a maximum of 2 years (24 months) at a rate between 0 and 100%. The geometric analysis of the characteristics obtained from the expressions of the Net Present Value and Internal Rate of Return possible to observe that besides the analytical dependence between these quantities , the geometric relationships are relevant when studied in relation to the zero NPV and expressed a great contribution the sense of a broad vision for the administrator in the analysis of analytical variables that in uences the balance sheet of the company.

A battery is the device that transforms the chemical energy into the direct-current electrical energy without a mechanical process. Unit cells are connected in series to obtain the required voltage while being connected in parallel to organize capacity for load current. Because the voltage drop down in one set of battery is faster than in two one it may result in the low efficiency of power converter with the voltage drop and cause the system shutdown. However when the system being shutdown. However when the system being driven in parallel a circular-current can be generated,. It is shown that as a result the new batteries are heated by over-charge and over-discharge and the over charge current increases rust of the positive grid and consequently shortens the lifetime of the new batteries. The difference between the new batteries and old ones is the amount of internal resistance. In this paper we can detect the unbalance current using the microprocessor and achieve the balance current by adjusting resistance of each set, The internal resistance of each set becomes constant and the current of charge and discharge comes to be balanced by inserting the external resistance into the system and calculating the change of internal resistance.

Combustion instability in solid rocket motors is a long-term open problem since the first rockets were used. Based on the numerous previous studies, it is known that the limit cycle amplitude is one of the key characteristics of the nonlinear combustion instability in solid rocket motors. Flandro's extended energy balance corollary, aims to predict the limit cycle amplitude of complex, nonlinear pressure oscillations for rockets or air-breathing engines, and leads to a precise assessment of nonlinear combustion instability in solid rocket motors. However, based on the comparison with experimental data, it is revealed that the Flandro's method cannot accurately describe such a complex oscillatory pressure. Thus in this work we make modifications of the nonlinear term in the nonlinear wave equations which represents the interaction of different modes. Through this modified method, a numerical simulation of the cylindrical solid rocket has been carried out, and the simulated result consists well with the experimental data. It means that the added coefficient makes the nonlinear wave growth equations describe the experimental data better.

In this work, experiments on hybrid sounding rocket were conducted to investigate the aerodynamic characteristics and analyze longitudinal static stability. Tests were performed on 1/10 scale models of sounding rocket through Mach number ranging from 1.75 to 2.5 and for angle of attack from $0^{\circ}$ to $6^{\circ}$. Aerodynamic forces and moments were measured by means of a 4 component internal balance. With measured forces and moments, static stability characteristics of rocket were calculated. Tests were made for three models with different length to determine the effect of body length. The visualization of shock waves was carried out by Schlieren optical system to observe variations of shock waves with Mach number and angle of attack.

Neiging Medicine(內經醫學) based on Yin-Yang and Five elements was the main stream of Korea Traditional Medicine until the end of 19th century. And the establishment of Sasang Constitutional medicine. In this paper, author proved that the Yin-Yang of Sasang Constitutional Medicine was different from that of Neiging Medicine and that explained the Yin-Yang of Sasang Constitutional Medicine as our own medicine. After considering the philosophical and medical background of Sasang Constitutional Medicine, and author researched the Yin-Yang of ontology, structure, function, symptoms in Sasang Constitutional Medicine. Also we compared this with that of Neiging medicine. The results were as follows 1. Neiging Medicine was based on the Yin-Yang and Five elements centering on the nature and Sasang joy, and pleasure centering on the human. 2. The ontology of Sasang Constitutional Medicine were the mind as a unitary substance, the mind and body as a dual substance, and affair, mind, body, and objects as a four substance which were the basic theory of philosophy. 3. Sasang Constitutional Medicine explained social phenomenon with a four substancial structure consisted of affairs, mind, body, and objects, a complex four substancial structure consisted of heaves, human, nature and order, and a three dimension structure including time. 4. On the other hand Neiging Medicine recognized seven emotions as five emotions, Sasang Constitutional Medicine recognized seven emotions as qi, nature, and emotions which consisted of sorrow, anger, joy, and pleasure. The qi had an effect on internal and external part of body and the nature and the emotion had an effect on the upward and downward of body. 5. It was taked great importance to control the up and down balance and the internal and external balance in Sasang Constitutional Medicine instead of reinforcing and reducting method of Neiging Medicine. 6. The Yin-Yang of Korean Traditional Medicine developed centering on the human from centering on the nature and Sasang Constitutional Medicine was a practical medicine compared with Neiging Medicine.
