An isolated dislocation of the intermediate cuneiform bone is a rare midfoot injury. This paper reports a case of a 60 year old man who fell from a height with his foot in the plantar flexed position. An isolated dorsal dislocation of the intermediated cuneiform was confirmed. Good results were obtained after an open reduction and internal fixation with a Lisfranc screw and Kirschner wire.

The foot plays a vital role in standing and gait. This function results from harmonious interaction of bones, joints, and soft tissue. An imbalance or a defect in such structures can lead to impaired function of the foot. The mid foot, composed of cunieforms, navicular and cuboid bone, plays a vital role in maintaining longitudinal and transverse arches and injury or defects to this region can cause instability of the foot. This paper reports a case of complex foot injury; soft tissue defect of dorsum of foot, and medial and intermediate cuneiform bone defect, reconstructed in a single stage using vascularized osteocutaneous fibular free flap. Segmented to fit the defects of medial and intermediate cuneiform bones and a skin paddle providing adequate coverage, restored the stability to the arches and function of the midfoot. The fibula osteocutaneous free flap has appealing characteristics for reconstruction of the foot and the complex mid foot injuries can be considered to the long list of indications.

Malignant peripheral nerve sheath tumors (MPNSTs) usually arise in soft tissues; they are rarely found in the bone. This paper reports a case of MPNST in the foot and ankle joint involving the distal tibia, talus, calcaneus, navicular, medial intermediate, and lateral cuneiform, cuboid, and 2nd to 4th metatarsal bone. Palliative treatment was performed. The authors encountered a patient with intraosseous MPNST of the midfoot who presented with nonspecific clinical and radiologic findings. This case shows that a high index of suspicion and a histopathology examination, including immunohistochemistry, will be necessary for an accurate diagnosis.
