Relationship between Some Obesity Indices and Coronary Artery Stenosis in Health-Screen Examinees (건강검진 수검자에서 비만 지표들과 관상동맥협착증과의 관련성)

  • Purpose : Obesity indices are major predictive markers for coronary artery stenosis, but there are few studies about the relationship between obesity indices and coronary artery stenosis in the Korean population. Therefore, we analyzed the association between obesity indices and coronary artery stenosis among health-screen examinees. Methods : This study included 99 males and females who visited a health-examination center. The obesity indices included body mass index (BMI), waist circumference (WC), visceral adipose tissue (VAT), visceral adipose tissue/subcutaneous adipose tissue ratio (V/S ratio). All subjects had their degree of coronary artery stenosis measured using coronary computed tomography angiography (CCTA). A multiple logistic regression test was conducted to analyze the association between obesity indices and coronary artery stenosis. Results : This study was taken by multiple logistic regression test adjusted by age, sex, smoking status, frequency of alcohol drinking/week and frequency of exercise/week. The adjusted odds ratio for the presence of coronary artery stenosis for subjects with abdominal obesity (abdominal obesity defined as a waist circumference ≥ 90 cm in males; ≥ 85 cm in females) was 6.263 (95 % confidence interval (CI) 1.974-19.869), for subjects with visceral obesity by visceral adipose tissue (visceral obesity defined as a visceral adipose tissue ≥ 100) was 11.430 (95 % CI 3.044-42.928). Conclusion : In this study of adults, WC and VAT were independent predictors of coronary artery stenosis. These results suggest that WC and VAT may be useful markers of coronary artery stenosis.

Qualitative study on the key elements of obesity counseling in Korean Medicine

  • Objectives: Weight counseling is an inevitable process for patients with obesity who intend to lose weight; however, the contents of real-world counseling practices remain uncertain in Korean medicine (KM) practice. This study aimed to explore the structure and constituents of obesity counseling in KM. Methods: Qualitative methods were used to explore the structure of weight counseling and clinicians' approaches to weight counseling, particularly the advice and main constituents of weight counseling in KM. Nine KM clinicians (4 women, 5 men) from nine clinics completed face-to-face, individual and semi-structured interviews from September 2019 to February 2020 in Seoul, Daejeon, Cheonan, Seongnam, Asan, and Hwaseoug. The interviews were digitally recorded, transcribed verbatim, and categorized based on their constituents. Results: During weight loss counseling, the six principal constituents of KM identified were: planning with patients, motivation, correcting misinformation on weight loss, dietary and exercise advice, medication guidance, and customized guides for each type. Conclusion: Clinicians with in-depth knowledge about obesity prescribed personalized guidance for exercise, diet, and medication through an integrative approach. The study results provide an in-depth understanding of weight counseling in KM. These six constituents could guide the counseling provided by clinicians in obesity care.

A case of pulmonic stenosis in a Shihtzu dog

  • A 3-year-old, intact female, Shih-tzu dog was presented with a 15-day history of vomiting,depression, and anorexia. On physical examination, systolic ejection murmurs with precordial thril atthe left heart base were detected. A diagnosis of congenital pulmonic stenosis (PS) was made mainlyfrom the thoracic radiography, electrocardiography, and echocardiography. On complete blood counts andconfirmed that main pulmonary artery was tremendously buldged and electrocardiography was suggestiveof severe right ventricular hypertrophy. Echocardiographic findings revealed the pulmonic valve stenosiscontaining valvular dysplasia and poststenotic dilation. On Doppler echocardiography, ejection velocityof the lesion accounted for 3.38 m/sec, meaning mild velocity through the stenotic area. The dog'sproblem and resulted in death. However, there has been no reliable relation between PS and CRF. Primarymalformation of pulmonic valve was confirmed at necropsy after death.

A Case of Fibromyalgia Treatment Using Yin-yang Balancing Therapy of Temporomadibular Joint (TMJ) (턱관절음양균형요법을 이용한 섬유근육통증후군 치험1례)

The balancing appliance was used in patient with fibromyalgia syndrome, and clinical outcome measurements were performed through clinical observations in specific movement and actions. The patient displayed positive changes after the treatment and the effect was well-maintained during the treatment period. For the more precise evaluation, clinical and biological studies of temporomadibular balancing therapy (TBT) are needed.

  • Objectives The purpose of this study is to analyze the trend and indication of clinical research about Visceral Chuna Manipulation and to suggest the direction of further study. Methods We searched 9 electronic databases (MEDLINE Pubmed, EMBASE, CENTRAL, CiNii, CNKI, KMbase, OASIS, NDSL, KISS) using the search terms "visceral manipulation", "visceral manual therapy" and identified relevant literature that investigated visceral manipulation as a intervention for various subjects. Results Among 142 studies were searched and screened, 16 met designated criteria. Visceral Chuna Manipulation showed the effectiveness in musculoskeletal disease, digestive disease, psychiatry, obstetrics, circulatory and oncology. Conclusions This study shows possibility of Visceral Chuna Manipulation as a treatment for many diseases. Better designed further study is necessary to establish the evidence of clinical efficacy of visceral manipulation and to suggest standard of techniques and treatment protocols for diverse patients.

Identification of Lactoferrin as a Human Dedifferentiation Factor Through the Studies of Reptile Tissue Regeneration Mechanisms

  • In this study, we performed two-dimensional electrophoresis with protein extracts from lizard tails, and analyzed the protein expression profiles during the tissue regeneration to identify the dedifferentiation factor. As a result, we identified 18 protein spots among total of 292 spots, of which proteins were specifically expressed during blastema formation. We selected lactoferrin as a candidate because it is the mammalian homolog of leech-derived tryptase inhibitor, which showed the highest frequency among the 18 proteins. Lactoferrin was specifically expressed in various stem cell lines, and enhanced the efficiency of iPSC generation upto approximately 7-fold relative to the control. Furthermore, lactoferrin increased the efficiency by 2-fold without enforced expression of Klf4. These results suggest that lactoferrin may induce dedifferentiation, at least partly by increasing the expression of Klf4.

Reliability of microarray analysis for studying periodontitis: low consistency in 2 periodontitis cohort data sets from different platforms and an integrative meta-analysis

  • Purpose: The aim of this study was to compare the characteristic expression patterns of advanced periodontitis in 2 cohort data sets analyzed using different microarray platforms, and to identify differentially expressed genes (DEGs) through a meta-analysis of both data sets. Methods: Twenty-two patients for cohort 1 and 40 patients for cohort 2 were recruited with the same inclusion criteria. The 2 cohort groups were analyzed using different platforms: Illumina and Agilent. A meta-analysis was performed to increase reliability by removing statistical differences between platforms. An integrative meta-analysis based on an empirical Bayesian methodology (ComBat) was conducted. DEGs for the integrated data sets were identified using the limma package to adjust for age, sex, and platform and compared with the results for cohorts 1 and 2. Clustering and pathway analyses were also performed. Results: This study detected 557 and 246 DEGs in cohorts 1 and 2, respectively, with 146 and 42 significantly enriched gene ontology (GO) terms. Overlapping between cohorts 1 and 2 was present in 59 DEGs and 18 GO terms. However, only 6 genes from the top 30 enriched DEGs overlapped, and there were no overlapping GO terms in the top 30 enriched pathways. The integrative meta-analysis detected 34 DEGs, of which 10 overlapped in all the integrated data sets of cohorts 1 and 2. Conclusions: The characteristic expression pattern differed between periodontitis and the healthy periodontium, but the consistency between the data sets from different cohorts and metadata was too low to suggest specific biomarkers for identifying periodontitis.

A Case Study of an Advanced Breast Cancer Patient Treated with a Combination of Traditional Korean Medicine and Chemotherapy (진행성 유방암에 대한 한양방 병용 치료 치험 1례)

  • Although patients with breast cancer receive standard treatments, they often experience recurrence or metastasis of tumors. Therefore, patients seeking treatment with traditional Korean medicine (TKM) in addition to conventional treatment have increased. We present a case of 46-year-old female with recurring breast cancer. She underwent surgeries and various hormone therapies since being diagnosed in 2007. Upon follow-up examination, she had metastatic lesions on the lung and multiple bones in 2015 and 2016. She received TKM treatments from May 2017 with Aromasin and Afinitor. However, hepatic metastasis was found after two months, so she started Capecitabine with TKM. After about two months, the liver nodules disappeared and a seeding nodule in the right paracolic gutter was decreased. After two months, the tumor response was stable disease. Back pain due to bone metastasis was improved. We suggest that combination treatment of TKM and chemotherapy is a promising method for treating breast cancer.
