Effects of social support on toothbrushing in children and adolescents (아동·청소년의 사회적 지지와 칫솔질 실천의 연관성)

  • Objectives: This study aimed to confirm the relationship between toothbrushing practice after lunch and typical oral health behaviors in children and adolescents according to the subject and type of social support. Methods: The study used data from the 2010 Korean Survey on the Health of Youth and Children and selected 8,704 elementary, middle, and high school students as final targets. Multiple logistic regression was conducted to confirm the link between social support and toothbrushing after lunch. Results: The results showed that in elementary and vocational high school students, statistical significance of all social support variables disappeared after controlling for both demographic and social factors and oral symptom experience. However, in the middle school students, the emotional support of the neighbors and the emotional support of the teachers showed statistically significant effect on brushing daily after lunch (p<0.05). In general high school students, there was a significant association between family emotional support and toothbrushing after lunch (p<0.01). Conclusions: The oral health promotion program for children and adolescents needs to be integrated and coordinated with the overall health promotion program. In particular, it is believed that the school-based program can strengthen the practice of health behaviors by inducing behavioral changes based on the formation of healthy relationships and trust of colleagues and teachers. Therefore, it is necessary to develop social network-based programs including social support such as emotional health and healthy relationships that can be applied among children for oral health promotion.

Need of Health Center-based Integrated Healthcare Services for the Elderly in Rural Area (농촌지역 보건소 중심의 노인 통합보건의료서비스 필요도)

  • Objectives: The challenge of an increasing elderly population has coupled with everpresent social concerns in Korea. A major problem in health center for the frail older people is that medical, healthcare, and welfare services are often fragmented in terms of providers and settings without appropriate coordination. The purpose of this study was to investigate the need of health center-based integrated healthcare services and its related factors for the elderly. Methods: A total of 110 elderly people who had visited at a county Health Center were interviewed using a self-administered questionnaire from November to December, 2005. The questionnaire consists of five domains according to the Program of All-inclusive Care for the Elderly. Results: Respondents had high need (total mean score with the 5-point Likert-type sacle: 3.67) of health center-based integrated healthcare services including home visiting service (mean: 4.08), chronic disease care service (mean: 4.06), and transportation service (mean 4.05). According to the results of hierarchical multiple regression analysis, among three regression models the magnititude of the variance of full model that is explained by the need of welfare-domain service was significantly larger than two reduced model. Income was a significant variable in increasing the need of health care and welfare services. Conclusions: This study suggests that the health center-based integrated healthcare services for the elderly must be continuously developed and provided for the health promotion and improved the quality of life of the elderly who live in rural area in Korea.

  • In aging Korean society, many elderly people have difficulty in the areas of health, economic insecurity, role-loss(both work & spouse), loneliness, and family conflicts. Above all, retirement from work is one of the most important stressful life events having influences an elderly's persons economic & social activities, mental state, health status, and Lifelong educational programs could help the aged cope with these situations in later life. Health and social welfare are commonly recognized as an important value of life. They also have an effect on each other. Most people agree that the promotion of health and social welfare is to take a short-cut toward achieving well-being. Thus we need to develop a substantially integrated program of lifelong education, before and after retirement, for the promotion of health and welfare. Universities have the advantage of practicing lifelong educational programs because they have more material, intellectual, and human resources than any other educational facilities. As a result, the purposed of this study is to suggest life-long educational programs for promoting both health conditions and the level of social welfare by utilizing an aging center affiliated with a university. Specifically, the developmental courses of pre-retirement, learning in retirement, elder-hostels, and the connection between pre and post-retirement educational programs are proposed in this study.

Evaluation of Oral Health Promotion Program Connected with Hypertension and Diabetes Management Programs: Use of a Logical Model (일부 보건소 고혈압·당뇨관리교실 연계 구강건강증진 프로그램 운영 및 평가: 논리적 모형을 이용하여)

  • This study aimed to design and operate a complementary integrated health management program based on the connection between the hypertension and diabetes management programs and the oral health programs at a public health center. It also proposed to suggest the phased evaluation indicators. In this study, 48 adults registered in the hypertension and diabetes management program were selected from the Gangneung public health center. The clinic-specific programs were led by dental hygienists and operated for visitors twice every two weeks. The programs were designed based on the logical model, and indicators for evaluating the structure, process, and outcome were presented and applied to the input, process, output, and outcome. The evaluation indices consisted of quantitative and qualitative indicators, and the planning and operation, goal achievement, and effect of each program were assessed. The process evaluations were assessed by the appropriateness of the managers and the operating fidelity of the programs. Indicators for evaluating the outcomes were gingival bleeding, oral health knowledge, oral health awareness, and the satisfaction of the participant and the manager. The clinic-specific programs resulted in positive changes in the evaluated outcomes. The integrated health management of visitors to the hypertension and diabetes management program is important as the general and oral health has common risk factors. Furthermore, long-term operation and continuous monitoring of oral health programs are necessary to evaluate the common factors in chronic disease management.

  • The purpose of this study was to develop, apply, and test the effectiveness of integrated palliative care empowerment program for the clinical nurses caring the cancer patients. Methods: The subjects were selected by a convenience sampling method from the nurses working at two university hospitals in D city. The study targeted a total of 37 nurses, 17 for the experimental group and 20 for the control group. The integrated palliative care empowerment program was implemented once a week for seven weeks to the experimental group from October 5th to November 11th in 2016, and it was analyzed by using the program IBM SPSS Statistics version 19.0 for Windows. Result and Conclusion: The study proved that the integrated palliative care empowerment program was effective on improving the integrated palliative care, empathy capacity, and ethical climate of the nurses. Based on the results of the study, it is necessary to develop an applicable program in collaboration of various health professions.

Effects of Interprofessional Nurse-led Rehabilitation Intervention Program for Subacute Stroke Patients and their Caregivers: a non-randomized study

  • Purpose: Rehabilitation nursing requires an interdisciplinary comprehensive team approach that allows for enough time to promote patients' functional recovery. We identified the effects of a nurse-led rehabilitation intervention program on subacute stroke patients' activities of daily living, anger, and muscle strength, as well as their chief caregivers' satisfaction. Methods: Intervention group participants (n = 20) completed a rehabilitation intervention program, which integrated physical activities with psychological and social approaches. Results: The program did not significantly improve patients' activities of daily living or anger management; however, it significantly affected both anger-in (t = 3.384, p = .002) and anger-out (t = 2.167, p = .037) attitudes. Caregivers' satisfaction among the intervention group (t = 6.935, p < .001) decreased more significantly than that of the comparison group (t = 0.224, p = .825). Conclusion: Rehabilitation intervention program should enlist nurses' participation and promote a team-based approach during the rehabilitation program. Further, patients and their caregivers should be encouraged to express their emotions during counseling.

Causality Analysis of Muscle Activation, Physical Strength and Daily Living Abilities Change among the Elderly due to a Health Promotion Exercise Program (건강운동에 의한 노인의 근활성, 체력 및 일상생활능력 변화의 인과관계)

  • Purpose: The purpose of this study was (1) to examine the effects of a Health Promotion Exercise Program (HPEP) on functional improvement, physical strength (PS) and muscle activation (MA), and the interactive influence with activities of daily living (ADL) and instrumental activities of daily living (IDAL) in the elderly, and (2) to develop and provide a HPEP for the elderly in order to prevent declines and impairments in integrated nerve function and physical capacity. Methods: Our study included relatively healthy elderly people aged 65 years or older. The experimental group, which was composed of 30 people, participated in a HPEP 50~60 minutes a day, 3 times a week, for a total of 12 weeks. Results: Confirmatory factor analysis (CFA) validated the measurement models for MA, physical strength, ADL and IADL with a p<0.01. This confirms (i) the beneficial effects of the ADFP on their MA and PS, and (ii) that their improvement in PS, in turn, can improve their IADL. The results of this study indicates that ADFP can help Korean seniors 65 years or older improve their MA, PS, ADL, and IADL, and do so synergistically. Conclusion: An HPEP, when organized in such a way that the elderly can easily do it, and when exercise items and intensity can be programmed and reorganized accordingly to individual physical and physiological characteristics by presenting 5 categorized health domains of physical strength, may be useful, especially because it can be practiced irrespective of time and place.

Cost-Benefit Analysis of Maternal and Child Health Program - Focusing on Lao People's Democratic Republic - (모자보건 증진사업에 대한 비용편익 분석 - 라오스 국가를 중심으로 -)

  • Objectives: The purpose of this analysis was to assess the maternal and child health program in Lao PDR, which was implemented to reduce maternal and child mortality in Laos. Target areas for the project included 2 provinces (Xiengkhuang, Houaphan) with all 18 districts within them. The beneficiaries of this project included 121,000 childbearing women and 62,000 children under 5 years old (LSIS, 2012). Methods: In this study, it was done for efficiency evaluation of the assistance projects of the health sector of maternal and child health promotion program in Laos that took place from 2010 to 2013. We conducted a cost-benefit analysis for the evaluation of the effectiveness of that program was being carried out effectively. Results: This evaluation adopted the Cost-Benefit analysis approach. Key findings of the Cost-Benefit analysis are the following: The Benefit-Cost Ratio of 1.012, Net Present Value of 84,250,000 Korean Won, an estimated Internal Rate of Return of 10.12%. These findings suggest that project activities were efficiently implemented. Conclusion: As a result, maternal and child health project in Laos has been analyzed that there was economic efficiency. Therefore, It is considered necessary and continued support expansion of program in the future. The direction of maternal and child health project in Laos, approach and community policy support must be included. Integrated approach between the program approach and overall health should be performed for healthy life habit.

A study on the relationship between the experiences of depression, suicidal thoughts, and habitual drugs and oral symptoms in middle and high school students (중·고등학생의 우울감 경험, 자살 생각 및 습관적 약물 경험과 구강 증상 경험의 관련성 연구)

  • Purpose: The purpose of this study was to identify the experiences of depression, suicidal thoughts, and habitual drug use in middle and high school students and examine their relationship with the oral symptoms experiences. Methods: The participants of this study were 54,948 middle and high school students who took the screening and health survey at the 16th "Youth Health Behavior Survey" (2020). The SPSS statistical software (IBM SPSS 23.0 for Windows; IBM) was used for data analysis. The significance level was set to 0.05. Results: Complex-sample logistic regression analysis was performed to confirm the relationship between the experiences of depression, suicidal thoughts, and habitual drug use and oral symptom experienced. The results indicated that the absence of depression, suicidal thoughts, or habitual drugs had a significant effect on oral symptom experience. Conclusion: A systematic counseling program for early detection of oral symptoms and oral health promotion as well as strategies for practicing correct oral hygiene are required. Additionally, it is necessary to develop a customized education program to promote health education in middle and high school students. It can be used as the basis for an integrated support system that students can use to grow healthy. A differentiated program on the topic of mental health promotion for each grade can be planned and its effects can be monitored.

A Systematic Review on the Vocational Pharmacy Education and Pharmacists' Role in the Singapore's Healthcare System (싱가포르 보건의료체계의 약사 양성교육과 약사 직능에 대한 체계적 고찰)

  • Background: Singapore has the stable healthcare system with utilizing pharmacist manpower in proper positions by demand of populations' health among Asian countries. Objective: This study aims to systematically review (1) the pharmacists' role and (2) the pharmacy education system of Singapore in comparison with Korea. Method: We searched for information about academic, medical and governmental institutions related to professional pharmacists' practice in Singapore by primarily using database such as DBpia, KISS, Google Scholar and ProQuest and the official website of the Singapore Ministry of Health. We contacted and arranged the visit schedules with National University of Singapore, National Health Group's polyclinics, Agency for Integrated Care, National University Hospital, and community chain pharmacies. During onsite visits, we interviewed pharmacists working in each institution and obtained additional documents and materials relevant to this manuscript work. Results: To become a registered pharmacist in Singapore, the pharmacy curriculum requires four full-time academic years and six additional months allotted for pre-registration training. Pharm.D. course is offered for pharmacy graduate students with additional two full-time years of study. Team teaching and inter professional education program seem the most significant method in pharmacy education. Pharmacists working at hospitals, polyclinics, and community pharmacies in Singapore take broader roles and offer more cognitive services such as smoking cessation program and medication reconciliation. Especially, pharmacists in Agency for Integrated Care fill the role of primary care providers for the continuing care of the community through the governmental support toward the patients-centered integrated care. Conclusion: Singaporean pharmacists take significant and active roles in collaboration with other healthcare providers. Efforts such as interprofessional pharmacy education and governmental endorsement of the systematic and interactive care between pharmacists and other medical providers in Singapore are needed to be urgently applied to Korea healthcare system for the promotion of population health.
