인슐린은 지방세포 또는 근육세포에서 포도당 흡수 조절 통로단백질이 함유되어 있는 소포제를 세포막으로의 이동을 촉진시킨다. 우리는 여기서 지방세포로의 분화는 인슐린에 의한 포도당 흡수에 대한 반응이 증가됨을 보였다. 반면에 지방세포로의 분화는 PDGF에 의한 포도당 흡수 반응이 감소됨을 보였다. 인슐린 수용체나 caveolae는 지방세포로의 분화과정 동안 발현이 증가된다. 또한 지방세포로의 분화는 인슐린에 의한 Akt의 활성을 증가시켰다. 하지만 PDGF에 의한 Akt의 활성은 크게 감소하였다. 하지만 인슐린은 지방세포 또는 섬유아 전구세포에서 ERK의 활성을 유도하지 않았다. PDGF에 의한 ERK 활성 또한 지방세포로의 분화과정에 따라 감소하였다. P13K의 저해제인 LY294002는 지방세포 뿐만 아니라 섬유아 전구세포에서 인슐린에 의한 포도당 흡수를 저해하였다. 마지막으로 인슐린 수용체, Akt, SHIP2, p85등이 lipid raft/caveolae에 존재함을 확인하였고 인슐린에 의해 이런 단백질들이 lipid raft/caveolae로 이동함을 관찰하였다. 이런 결과를 토대로 lipid raft는 포도당 홉수를 위한 인슐린의 기능적 작용을 하는데 매우 중요한 환경을 제공함을 주장한다.
The large and rapid changes of glucose utilization in lactating mammary tissue in response to changes in nutritional state must be largely related by external signal of insulin. This also must be related with the quantity and composition of the diet in vivo. To characterize the mode of growth factors and gut regulatory peptides with insulin, in vitro experiment was conducted with HC11 cells. All the growth factor alone and the combinations of growth factors significantly (p<0.05) increased in glucose uptake. Insulin, EGF and IGF-1 exhibited a stimulation of glucose uptake for at least 24 h. Furthermore, the highest (p<0.05) synergistic effect was shown in EGF plus IGF-1 and the second synergistic effect in insulin plus EGF while no synergistic effect was found between insulin and IGF-1. However, the gut regulatory peptides neither potentiated nor inhibited the action of insulin on glucose uptake. Although growth factors did not modulates glucose uptake via increasing the rate of translation of the GLUT1 protein, RT-PCR analysis indicated that the growth factors significantly (p<0.05) increased the expression of GLUT1. The growth factors are therefore shown to be capable of modulating glucose uptake by transcription level with insulin in HC 11 cells.
Rabson Mendenhall syndrome은 Rabson과 Mendenhall에 의해 1955년 처음 발표된 증후군으로서, 심한 insulin resistance를 보이는 희귀한 상염색체 열성 유전성 질환이다. Insulin receptor의 돌연변이나 insulin이 작용하는 다른 target cell의 결함에 의해 나타나며, 일반적인 증상으로는 흑색가시세포종(acanthosis nigricans), 다모증(hypertrichosis), 손발톱 비대(onychauxis), 성장 지체 (growth retardation), 성 조숙(precocious puberty), 생식기의 비대, 팽만된 복부(protuberant abdomen), 건성 피부(xerotic skin) 등이 나타난다. 악안면 영역에서는 치아의 이형성, 거친 얼굴의 피부, 하악 전돌, 균열 혀 (fissured tongue)와 같은 특징적 증상이 나타난다. 이에 본 증례에서는 Rabson Mendenhall syndrome으로 본원에 내원한 4세 환아에게 관찰된 치과적 특징들에 관하여 보고하는 바이다.
Impaired insulin action in Type 2 diabetes is thought to lead to hyperglycemia, with both environmental and complex genetic factors playing key roles. Although the primary lesion in Type 2 diabetes is unknown, a number of studies suggest that metabolic defects in the liver, skeletal muscle and fat, and pancreatic ${\beta}$-cells contribute to the disease. These metabolic abnormalities are characterized by the overproduction of hepatic glucose, impaired insulin secretion, and peripheral insulin resistance. In current pharmacological treatment of Type 2 diabetes, sulfonylurea (SU) drugs have mainly been used as oral hypoglycemic drugs to stimulate endogenous insulin secretion from ${\beta}$ cells. SU drugs, however, sometimes aggravate the disease by causing fatigue of the pancreatic ${\beta}$ cells, which leads to reduced drug efficacy after long-term treatment. This class of drugs also leads to enhanced obesity arising from the stimulation of endogenous insulin secretion in obese Type 2 diabetic patients, plus an increased incidence of SU-induced hypoglycemia. Since 1980, a major challenge has been made by us to develop a potential pharmacological therapy for the treatment of insulin resistance in peripheral tissues and/or suppression of abnormal hepatic glucose production in Type 2 diabetic patients. Such a drug would be expected to have fewer side effects and retain long-term efficacy.
일본에서는 녹나무 줄기에서 분리한 정유의 하나인 camphor는 강심제등 약용으로 사용하고 있지만, 녹나무 잎 추출물에 대한 연구도 적고 특히 항당뇨 효과에 대한 연구는 거의 없었다. 본 연구에서는 녹나무 잎 추출물이 in vitro에서 인슐린 작용, 인슐린 분비 또는 탄수화물의 소화에 효과적인지를 조사함으로 항당뇨에 효과적인지 여부를 조사하였다. 녹나무 잎을 $70\%$ 에탄올로 추출한 후 메탄올과 물을 섞은 용액으로 단계별로 XAD-4 column으로 분획 하였다. 녹나무 잎 추출물은 고농도(1 mg/mL)에서도 MTT로 측정하였을 때 세포 독성을 나타내지 않았다 녹나무 잎 추출분획 물 중 40과 $60\%$ 메탄올층은 3T3-L1 지방세포에 처리하였을 때 인슐린의 작용을 향상시켜 포도당의 흡수를 증가시키는 효과가 매우 컸다. 이 추출분획 물들은 3T3-L1 지방세포에 존재하는 인슐린의 작용을 향상시켜 인슐린을 10 nM을 처리한 것보다도 효과적으로 포도당 흡수를 증가시켰다. 또한 $40\%$ 메탄올층은 3T3-L1 섬유아세포에 분화 유도물질를 처리하였을 때 PPAR$\gamma$인 pioglitazone과 마찬가지로 지방 세포로의 분화를 촉진시키고 지방의 축적도 증가시켰다. 그러므로 $40\%$ 메탄올층에는 PPAR-$\gamma$ agonist로 작용하는 물질이 함유되어 있을 가능성이 높다. 베타세포라인인 Min6세포에 녹나무 잎 추출분획물을 처리한 후 저농도와 고농도 포도당 자극시 인슐린 분비를 측정하였을 때 두 농도에서 모두 인슐린 분비에 영향을 미치지 않았다. 또한 녹나무 잎 추출분획물은 탄수화물의 소화에 작용하는 효소인 $\alpha$-glucoamylase의 활성에 영향을 미치지 않았다. 결론적으로 녹나무 잎에는 인슐린 분비나 탄수화물의 소화에 관여하는 성분의 없지만, 인슐린 작용을 향상시키는 인슐린 민감성물질이 함유되어 있을 가능성이 높다. 특히 $40\%$ 메탄올층에는 PPAR$\gamma$ agnist로 작용하는 인슐린 민감성 물질이 존재할 가능성이 높다.
국내의 한약재 시장 등에서 당뇨병의 민간요법 약재로 많이 판매되고 있는 엄나무(Kalopanax pictus NAKAI)수피의 열수 추출물에서 인슐린성 물질과 인슐린 민감성 약제로서의 가능성을 탐색하기 위해 3T3-L1 섬유아세포(fibroblast)가 3T3-L1 지방세포(adipocytes)로 분화할 때 미치는 영향과 지방 세포내로의 포도당 섭취(glucose uptake)에 미치는 영향을 조사하였다. 엄나무의 수피를 열수 추출하고 동결건조하여 만든 분말 엑스시료를 증류수에 녹여 각각 $10{\mu}g/ml$ 및 $l{\mu}g/ml로$ 3T3-L1 섬유아세포를 처리한 후, 분화유도 물질만을 첨가한 대조군의 분화 정도를 100%로 정하였을 때 이에 대한 백분율로 환산하여 결정하였다. 분화유도물질이 첨가되지 않은 경우 $1{\mu}g/ml$와 $10{\mu}g/ml$의 농도에서 각각 $115.4{\pm}5.1%(p<0.05)$와 $122.3{\pm}2.1%(p<0.05)$로 유의성있게 분화를 촉진시켰다. 분화유도물질을 첨가하였을 때, $1{\mu}g/ml$에서는 $127.0{\pm}5.3%(p<0.001)$ 분화를 촉진시켰고, $10{\mu}g/ml$에서는 분화에 거의 변화가 없었다. 3T3-L1 지방 세포내로의 포도당 섭취량은 소량의 인슐린(3ng/ml)과 함께 $0{\sim}10%$ 메탄올 분획층(Fr. 1)과 30% 메탄올 분획층(Fr. 3)을 0.3와 $3{\mu}g/ml$ 첨가하였을 때 분획층을 첨가하지 않은 대조군에 비해 모두 $3.3{\pm}0.5$배 증가시켜 인슐린 50ng/ml에서 반응하였을 때와 유사한 섭취 정도를 나타내었다. 엄나무의 수피는 인슐린성 물질을 함유하고 있으며, 추출물을 분획한 Fr. 1과 Fr. 3은 인슐린 저항성을 감소시키는 물질이 함유되어 있어 인슐린 민감성 제재로 개발할 수 있는 가능성을 암시하고 있다.
Chromium is an essential nutrient and participates in glucose and lipid metabolism in human beings and animals. The present study was conducted to assess the effects of chromium picolinate (Cr-pic) on glucose tolerance and insulin sensitivity in type I and ll diabetic rats. The experimental groups were type I diabetic (streptozotocin-induced: 40 mg/kg, i.p.) and type II diabetic (Goto-Kakizaki rats) models. Each group was subdivided into control. low-dose and high-dose of Cr-pic treated groups. The Cr-pic was orally administered with Cr-pic (100 mg/kg for low dose group and 200 mg/kg for high dose group) for 4 weeks. And then we performed intraperitoneal glucose tolerance test (IPGTT) and insulin sensitivity test (ITT). The glucose tolerance test was carried out by inection of glucose (2 g/kg, i.p.). The peripheral insulin sensitivity test was con- ducted by injection of insulin (5 units/kg, s.c.) and glucose. We performed determining of blood glucose concentration at 0, 10, 30, 60, 90, and 120 min using automated glucose analyzer. The plasma insulin concentration was determined by rat insulin EIA kit. Administration of Cr-pic improved weight gain in all group s with higher significant in the low-dose group. There was no significance between the control and the Cr-pic treated groups in the area under the blood glucose curve and serum insulin concentration plots of IPGTT and peripheral ITT in type I diabetic rats. But Cr-pic treated groups showed significantly lower levels of the area under the blood glucose currie during IPGTT and ITT and the high-dose group showed less effects compared with the low-dose group in the type II diabetic rats. The plasma insulin concentration of both diabetic groups was not influenced by Cr-pic supplementation. We can conclude that chromium picolinate may improve the endogenous and exogenous insulin action and peripheral insulin sensitivity in type II diabetic rats.
The relation of endocrine in the western medicine and zangfu-organ functions in Korean medicine, related to growth disturbance, was studied and derived the following results. 1. Somatomedin is the most important factor of the growth factors, IGF in another word. The unification of IGF and secretion is controlled firstly according to growth hormone, however is very closely related to the nutrition status in the non-hormonic causes. Also, it is affected very much by the insulin. 5. Insulin is one of the important hormone related to the growth and is secreted from the pancreas. Pancreas belongs to the functional system of spleen in oriental medicine, thus the growth disturbance, occurred due to error in insulin secretion and nutrition(in another words, the lack of postnatal essential substance from food-stuff(水穀精氣)), is closely related to the spleen. 6. From the results driven above, the hormone action of endocrine and problem in secretion, related to the growth disturbance, must be focused on the differentiation of symptoms and signs of the kidney and the spleen in oriental medicine.
Non-insulin-dependent diabetes mellitus (NIDDM) is characterized by insulin resistance and impaired insulim secretion. The transgenic technology, in which a specific gene can be introduced or deleted to study its function, has been established. A number of transgenic mice, altered the expression of genes potentially involved in insulin action or pancreatic ${\beta}$-cell function, have recently been developed to address questions concerning NIDDM. Thransgenic mice model may help understanding the molecular basis of complex patho-physiologies of NIDDM. This review outlines the new insights obtained from the studies of transgenic mice that overxpress or show decreased expression of putative key genes involved in the regulation of insulin resistance and pancreatic ${\beta}$-cell function, therefore in the control of glucose homeostasis.
