Investigation of fracture prevalence of instruments used in root canal treatments at a faculty of dentistry: a prospective study

  • Objectives: The aim of this study was to examine the use of hand or rotary files by pre-graduation (fourth- and fifth-year) and postgraduate students in endodontic treatments and to determine the incidence of file fracture and the management of cases with broken instruments. Materials and Methods: A total of 2,168 teeth undergoing primary endodontic treatment were included in this study. It was determined that 79 of these teeth resulted in broken tools. In the case of broken tools, the education level of the treating clinician, the tooth that was being treated, the canal and fracture level, the curvature of the tooth and the management of the broken instrument were recorded. Periapical radiographs of the patients were used to calculate curvature following the Schneider method. Results: There was no significant difference in the incidence of broken tools according to education level (p > 0.05). The incidence of file fracture in molar teeth (73.4%) was higher than in other teeth (p < 0.05). More files were broken in the mandibular molar MB canal (20.25%) and in the apical third of the canals (72.1%). The risk of instrument fracture was high in teeth with moderate (44.3%) and severe (38%) curvature canals. The management of apically broken (80%) files mostly involved lefting (p < 0.05). Conclusions: There was no statistically significant difference between fourth-year students, fifth-year students and postgraduate students in terms of instrument fracture.

Retrieval of a separated nickel-titanium instrument using a modified 18-guage needle and cyanoacrylate glue: a case report

  • During root canal preparation procedures, the potential for instrument breakage is always present. When instrument breakage occurs, it leads to anxiety of the clinician and as well as a metallic obstruction of the canal which hinders further cleaning and shaping. Separated instruments must always be attempted for retrieval and if retrieval is not possible bypass should be tried. With the increased use of nickel-titanium (NiTi) instruments the incidence of separated instruments has increased. A considerable amount of research has been done to understand the various factors related to the fracture of NiTi instruments to minimize its occurrence. This paper presents a review of the literature regarding the fracture of NiTi instruments and also describes a case report showing the use of a modified 18-guage needle and cyanoacrylate glue to retrieve a separated NiTi instrument from the mesiolingual canal of a mandibular first molar.

Canal preparation with nickel-titanium or stainless steel instruments without the risk of instrument fracture: preliminary observations

  • This report introduces a novel technique that allows a safe and predictable canal negotiation, creation of a glide path and canal preparation with reciprocating nickeltitanium or stainless steel engine-driven instruments in canals where the use of rotary and the newly developed reciprocating instruments is contraindicated. In this novel technique, the instruments are used in reciprocating motion with very small angles. Hand files are not used regardless of the complexity of the canal anatomy. It also allows achieving predictable results in canal negotiation and glide path creation in challenging canals without the risk of instrument fracture.

Fracture incidence of Reciproc instruments during root canal retreatment performed by postgraduate students: a cross-sectional retrospective clinical study

  • Objectives: To evaluate the fracture incidence of Reciproc R25 instruments (VDW) used during non-surgical root canal retreatments performed by students in a postgraduate endodontic program. Materials and Methods: From the analysis of clinical record cards and periapical radiographs of root canal retreatments performed by postgraduate students using the Reciproc R25, a total of 1,016 teeth (2,544 root canals) were selected. The instruments were discarded after a single use. The general incidence of instrument fractures and its frequency was analyzed considering the group of teeth and the root thirds where the fractures occurred. Statistical analysis was performed using the χ2 test (p < 0.01). Results: Seven instruments were separated during the procedures. The percentage of fracture in relation to the number of instrumented canals was 0.27% and 0.68% in relation to the number of instrumented teeth. Four fractures occurred in maxillary molars, 1 in a mandibular molar, 1 in a mandibular premolar and 1 in a maxillary incisor. A greater number of fractures was observed in molars when compared with the number of fractures observed in the other dental groups (p < 0.01). Considering all of the instrument fractures, 71.43% were located in the apical third and 28.57% in the middle third (p < 0.01). One instrument fragment was removed, one bypassed, while in 5 cases, the instrument fragment remained inside the root canal. Conclusions: The use of Reciproc R25 instruments in root canal retreatments carried out by postgraduate students was associated with a low incidence of fractures.

  • In this paper, the problems of the external fixator that have developed for a distal radius fracture so far are analyzed, and accordingly, the characterizations, which must have a prototype, are arranged. C-Arm is used. This instrument makes it possible for the real play of the internal body by x-ray permeability. From this data, it is possible to induce important design factors. Finally, a basic mechanism, which has to be applied, is decided, and the Solid Edge program, which used a 3-D design tool, completes then total instrument design.

  • I. Objectives The purpose of this study is to determine if propagation of pre-existing manufacturing defects results in brittle fracture of NiTi rotary files by examining the fracture surface of the instrument by SEM, and to compare the fracture time of NiTi rotary files with different flute designs under cyclic loading. II. Materials and Methods This study examined three groups of rotary NiTi instruments (K3, ProFile, Hero). Of each group, 04 taper, size 30 and 06 taper, size 25 were selected.:120 in total.   • Endodontic rotary NiTi(Nickel-Titanium) files have several advantages, but they also have some problems. Fracture of instrument is the one of the problems. Cyclic fatigue fracture may not give a previous sign(ex, bending or distortion of the files) before it happen. It is affected by various factors. This study investigated time which takes for cyclic fatigue fracture to happen and fracture patterns with regard to different NiTi files and pecking motion.(omitted)

Micro-computed tomographic evaluation of canal retreatments performed by undergraduate students using different techniques

  • Objectives: This study evaluated the amount of remaining root canal filling materials after retreatment procedures performed by undergraduate students using manual, rotary, and reciprocating techniques through micro-computed tomographic analysis. The incidence of instrument fracture and the instrumentation time were also evaluated. Materials and Methods: Thirty maxillary single rooted teeth were prepared with Reciproc R25 files and filled with gutta-percha and AH Plus sealer by the continuous wave of condensation technique. Then, the specimens were assigned to 3 groups (n = 10), according to the retreatment technique used: manual, rotary, and reciprocating groups, which used K-file, Mtwo retreatment file, and Reciproc file, respectively. Retreatments were performed by undergraduate students. The sample was scanned after root canal filling and retreatment procedures, and the images of the canals were examined to quantify the amount of remaining filling material. The incidence of instrument fracture and the instrumentation time were recorded. Results: Remaining filling material was observed in all specimens regardless of the technique used. The mean volume of remaining material was significantly lower in the Reciproc group than in the manual K-file and Mtwo retreatment groups (p < 0.05). The time required to achieve a satisfactory removal of canal filling material and refinement was significantly lower in the Mtwo retreatment and Reciproc groups (p < 0.05) when compared to the manual K-file group. No instrument fracture was observed in any of the groups. Conclusions: Reciproc was the most effective instrument in the removal of canal fillings after retreatments performed by undergraduate students.

족관절 내과 골절시 보조적 관절경적 정복 및 내고정술이 필요한가? (Is an Arthroscopically Assisted Reduction and Fixation Necessary in the Medial Malleolar Fracture of the Ankle?)

  • We treated 10 cases of the medial malleolar fracture of the ankle by open reduction and internal fixation from June 1997 to December 1997. After the rigid internal fixation, we measured the gap of the fracture site and the step off of the articular surface by special instrument under the ankle arthroscopy whether it was reduced anatomically or not. And we tried to know the necessity of the arthroscopically assisted reduction and fixation in the medial malleolar fracture of the ankle. Under the arthroscopic view, all 10 cases were anatomically reduced as less than 1 mm of gap of the fracture site and less than 1mm of step off of the articular surface after open reduction and internal fixation in the medial malleolar fractures. In conclusion, through the arthroscopic management, it has advantage in finding and treating the accompanying intraarticular lesion, but also has disadvantage in setting the arthroscope and prolonging the operation time.

골절치료용 체외고정기기 개발을 위한 연구 (Development of the External Fixator for a Bone Fracture)

  • 본 논문에서는 연구를 통하여 지금까지 개발된 체외고정기기의 문제점을 파악하고, 기존의 단점을 보완하기 위하여 제품이 가져야 할 특성을 나열하였다. 이를 위해서 X-ray 투과성을 이용하여 신체내부를 연속적인 동화상으로 구현해주는 장치인 C-arm을 이용하였다. 이 데이터를 토대로 하여 실제 기구부 설계 시 중요한 설계요건을 도출할 수 있었다. 원위요골 (Distal Radius) 골절의 치료를 위해 적용되어야 할 기본 메커니즘을 확정한 후. 3차원 설계작업을 위한 tool인 Solid Edge program을 이용하여 실측에 가까운 기구부 설계를 정밀하게 구현하는 작업을 마침으로써 실제 시제품 제작을 위한 모든 제반과정을 수행하였다.
