Expanding the aging self: Investigating successful aging among Korean older adults using grounded theory

  • Purpose: This qualitative study aimed to understand older adults' perspectives on successful aging and develop a model of a successful aging process, within the Korean socio-cultural context. Methods: This study used a Grounded Theory approach. Through theoretical sampling, 14 participants were selected from older adults at a public health center and a volunteer institution in Gyeonggi Province, South Korea. Results: The basic social process identified was "expanding the aging self", which was preceded by three phases, namely, adjusting to the changes, developing the valuable self, and embracing the environment. Participants used 2-3 strategies for successful aging in each phase. For adjusting to the changes, participants used strategies such as "adopting a positive attitude", "accepting the changes", and "being at the center of life". To develop the valuable self, "re-identifying capacities", "having something to do", and "advancing to a healthy lifestyle" were used. Participants embraced their environments with "sharing with others" and "embracing younger generations". The causal condition for expanding the aging self was the changes in participants' physical, mental, or psychosocial situations. Conclusion: These findings suggest a theoretical foundation for the development of potential nursing interventions to promote self-care management and the interpersonal relationship for successful aging among Korean elderly individuals.

Comparative Analyses of the Clinical Characteristic and Medical Cost against Surgical Procedures for Intertrochanteric Fracture in the Elderly Patients (노인의 대퇴전자간 골절의 수술적 방법에 따른 임상적 특성 및 의료비용 비교)

  • Purpose: Clinical characteristics and medical cost were analyzed according to the surgical procedures for intertrochanteric fracture in aged patients to assess the appropriateness of treatment expense and to find possibility of reducing the medical cost. Method: Variable for the statistical analysis were; the clinical characteristics, medical cost according to the surgical procedures, the treatment success rate, the total medical expense, and the average expense per case. SAS Package Version 8.02. was used to analyze the relevant data. Results: Operative procedures differ significantly according to the gender and by the location of institution. Only significant clinical variables according to the operative procedure were duration of general anesthesia and amount of blood transfusion. Average cost per treatment was the highest in the bipolar hemiarthroplasty followed by the gamma nail and hip compressing screw. Average cost for bipolar hemiarthroplasty was significantly higher than other surgical procedures. Conclusions: The difference in hospital costs for treatment of intertrochanteric fracture originates from the utilized surgical procedures, mostly by the materials used. The method of surgical treatment should be carefully determined by the purpose of the surgery, in order to improve the quality of medical care and also to reduce the hospital cost.

Skin Hydration Status and Skin Surface pH According to the Body Parts of the Aged with Immobilization (기동장애 노인의 신체 부위별 피부수분 상태와 피부표면 산성도)

  • Purpose: This study was performed to identify skin hydration status and skin surface pH according to the body parts of the aged with immobilization. Methods: The subjects were 101 aged patients in a hospital and a institution for the elderly in Seoul. Data for skin hydration and skin surface pH were collected using corneo-meter and skin pH-meter. The body parts of measurement were 10 parts; face, forearm, back of the hand, flank, upper abdomen, leg, back of the foot, heel, scapular, and sacrum. The data were analyzed by SPSS/WIN 12.0 program. Results: Skin hydration status were relatively low especially on lower extremity and back of body, and skin surface pH of scapular and sacrum were high. There was significant negative correlation between skin hydration status and skin surface pH on the 6 body parts. Conclusion: Skin hydration status and skin surface pH, especially on scapular, sacrum, and lower extremity need to be considered as important control factors for increasing skin health status of the aged with immobilization.

  • Korea currently provides long-term care benefits for the elderly with poor functionality, but most of the service providers are private businesses. This is the time when quality management of care services is required, which is just around the corner of the super-aged era. In this study, we would like to look at the case in which 'A company', which operates a long-term care institution, attempted to make voluntary changes ahead of social demands. The company tried to transform the social needs of quality management by judging them as opportunities, not threats, and establishing an integrated database of centers. First, the company processed data and built a cloud-based database system. Second, the company automatically linked data from existing systems for the efficiency of data utilization. Third, the company pursued visualization for the convenience of data utilization. This allowed the company to make data-driven strategic decisions internally. This is expected to increase sales as it will soon lead to securing new customers and pioneering new markets. It is also significant in that it can provide best practices for the long-term care industry.

Feasibility and Efficacy of Concurrent Chemoradiotherapy in Elderly Patients with Esophageal Squamous Cell Carcinoma: a Respective Study of 116 Cases from a Single Institution

  • Background: To evaluate the safety and efficacy of combined chemoradiotherapy or radiotherapy alone in elderly patients with esophageal carcinoma to identify the best method of treatment. Materials and Methods: One hundred and sixteen patients with esophageal carcinoma aged 70 and older who received definitive radiotherapy or chemoradiotherapy entered the study. Overall survival (OS), disease-free survival (DFS) and treatment-related toxicities were assessed. Results: The median OS of the overall population was 17.9 months. For patients treated with cCRT, sCRT and radiotherapy alone, the median OS was 22.3 months, 18.0 months and 12.4 months respectively(P=0.044). Median OS for patients treated with radiotherapy dose ${\geq}60Gy$ and <60Gy was 20.2 months and 10.9 months respectively (p=0.017). By univariate analysis, Chemoradiotherapy (include cCRT and sCRT) and radiotherapy dose ${\geq}60Gy$ were found to achieve higher survival rates compared with radiotherapy alone and radiotherapy dose <60Gy (P=0.015, P=0.017). By multivariate analysis, chemoradiotherapy (HR=1.645, P=0.022) and radiotherapy dose ${\geq}60Gy$ (HR=1.642, P=0.025) were identified as independent prognostic factors of OS. Conclusions: Definitive concurrent chemoradiotherapy could be considered as a feasible and effective treatment in esophageal carcinoma patients aged 70 and older. Radiotherapy dose 60Gy is an effective treatment option compared with standard dose radiotherapy, while higher doses are not beneficial to improve survival.

Association between the self-reported periodontal health status and oral health-related quality of life among elderly Koreans (한국노인의 자가보고 치주건강상태와 구강건강관련 삶의 질의 연관성)

  • Purpose: This study assessed the impact of self-reported periodontal health on the oral health-related quality of life among elderly Koreans. Methods: Four hundred twenty one elderly Koreans in Seoul and suburban areas were selected with a cluster (institution) sampling method, and were requested to take oral examinations and finish questionnaires on the Oral Health Impact Profile-14(OHIP-14). and self-reported periodontal health status, such as periodontal symptoms, self-rated periodontal health and periodontal treatment need. As the dependent variable, OHIP-14 showed a positive skewed distribution (skewness: 1.17), we transformed to square-root form to apply parametric analyses. Bivariate analysis by t-test and ANOVA, and multivariate analysis with the two-level regression model accounting clusters were implemented. Results: Mean age of the subjects was 74.6 years and 66.5% were women. Fourteen items of OHIP-14 were summarized to one factor explaining 78.6% of total variance and produced the Chronbach alpha coefficient of 0.92. Results from the multivariate model, adjusting for age, sex, type of institutions, ability to pay, and number of teeth present, showed significantly lower OHIP-14 with reporting less than 3 periodontal symptoms (p(O.OOO1), rating their own periodontal health as above average level (p=O.0144), and thinking they don't need any periodontal treatments in the near future (p=O.0148), than their counterparts. The intraclass-corrrelation estimated by the final model was 0.028. Conclusion: This study demonstrates a significant association between self-reported periodontal health status and the oral health-related quality of life.

Comparison of Effect of SSRIs and SNRIs on Depression, Pain and Somatic Symptoms in Elderly Patients with Major Depressive Disorder (노인 우울증 환자에서 선택적 세로토닌 재흡수 억제제, 세로토닌 노르에피네프린 재흡수 억제제가 우울증상, 통증 그리고 신체증상에 미치는 효과 비교)

  • Objectives : The aim of this study was to compare the effect of Selective Serotonin Reuptake Inhibitor (SSRI) and Serotonin Norepinephrine Reuptake Inhibitor (SNRI) for mood symptoms, pain, and somatic symptoms in elderly depression patients with pain and somatic symptoms. Methods : This study is a prospective open-label study conducted by a single institution. A total of 43 subjects diagnosed with major depressive disorder under the DSM-5 diagnostic criteria participated in this study (average age: 72.53, 58.1% women). The subjects were classified as SSRI and SNRI groups. Depressive symptoms, pain, and somatic symptoms were evaluated by Korean version of the Hamilton Depression Rating Scale (K-HDRS), visual analogue scale (VAS) and Patient Health Questionnare-15 (PHQ-15) respectively at baseline and six weeks later. Two-way repeated-measure ANOVA was performed to analyze changes in the KHDRS, VAS, and PHQ-15 scores. Results : In the SSRI and SNRI groups, K-HDRS, VAS, and PHQ-15 all showed significant improvement after 6 weeks compared to each baseline values. There were no differences in therapeutic effect between the two groups. Conclusions : We found that SSRI and SNRI both improved somatic symptoms and pain in elderly depression patients. The results of this study are thought to help select antidepressants when administering medication to elderly depression patients who complain pain and somatic symptoms. Further research is needed on the longterm effects of the SSRI and SNRI.

Favorable Outcome in Elderly Asian Patients with Metastatic Renal Cell Carcinoma Treated with Everolimus: The Osaka Urologic Oncology Group

  • Background: In clinical trials with no upper age limit, the proportion of older patients is usually small, probably reflecting the more conservative approach adopted by clinicians when treating the elderly. An exploratory analysis of elderly patients in the RECORD-1 Trial showed that patients ${\geq}$ 65 y.o. had superior median PFS than overall RECORD-1 population (5.4 months and 4.9 months, respectively). We investigated the efficacy, relative benefit and safety of Everolimus (EVE) as sequential therapy after failure of VEGFr-TKI therapy for older patients with metastatic renal cell cancer (mRCC), in daily practice. Materials and Methods: 172 consecutive IRB approved patients with mRCC (median age 65, M:F 135/37, 78% clear cell) who received salvage EVE at 39 tertiary institutions between October 2009 and August 2011 were included in this analysis. Some 31% had progressed on sunitinib, 22% on sorafenib, 1% on axitinib, 41% on sequential therapy, and 5% had received other therapy. Patients with brain metastases were not included and 95% of the patients had a ECOG (Eastern Cooperative Oncology Group) performance status (PS) of 0 or 1. Previous radiotherapy was an exclusion criterion, but prior chemotherapy was permitted. Adequate organ function and hematologic parameters were mandatory. EVE administration was approved by the institutional review board at each participating institution and signed informed consent was obtained from all patients. Results: Median time of the whole cohort to last follow-up was 3.5 months (range 0.4-15.2 months). Forty four percent were continuing to take EVE at last followup. There were 86 (50%) patients ${\geq}$ 65 y.o. and 86 (50%) <65 y.o. The percentage of patients who showed PR/SD was higher in the older group than in the younger one (5.9%/61.2% vs 1.2%/46.5%, respectively). Median survival of older patients was also significantly longer (3.5 +/- 0.31 vs 3.1 +/- 0.34, hazard ratio=0.45, CI; 0.255-0.802). Analysis using Cox regression model adjusted for gender, PS, number of metastases, site of metastases, histology, smoking history and age detected an association between age and PFS (p=0.011). The frequency of adverse events in elderly patients treated with EVE was no greater than that in younger patients, although such toxicity may have had a greater impact on their quality of life. Conclusions: Older patients should not generally be excluded from accepted therapies (mTOR inhibitors after failure of VEGFr-TKI therapy) for mRCC.

Serum Concentrations of $\alpha$-Tocopherol, Carotenoids and Retinol of Normal Koreans

  • Five hundred and seventy-eight healthy subjects (351 men and 227 women) with a mean age of 44.8 years (45.2 for men, 44.3 for women) participated in this study. The serum concentrations of $\alpha$-tocopherol, \beta$-carotene, $\alpha$-carotene, zeaxanthin + lutein, cryptoxanthin, lycopene and retinol of normal Koreans were measured, and their relation to gender, age, BMI, alcohol consumption, cigarette smoking, and menopausal status were evaluated. The concentrations of $\alpha$-tocopherol, carotenoids and retinol in serum were measured simultaneously by reverse phase HPLC (multi-wavelength, gradient and computerized automatic system). Average serum levels of $\alpha$-tocopherol, \beta$-carotene and retinol were 10.1$\pm$0.41\mu$g/dl, 33.1$\pm$1.24\mu$g/dl, and 82.0$\pm$1.63\mu$g/dl for men and 11.1$\pm$0.74\mu$g/ml, 48.1$\pm$1.60\mu$g/dl and 64.5$\pm$1.96\mu$g/dl for women, respectively, Serum concentrations of $\alpha$-tocopheol \beta$-carotene, $\alpha$-carotene, cryptoxanthin and lycopene appeared to be higher in women than in men. The serum concentrations of zeaxanthin + lutein increased with the increase in age of men and those of $\alpha$-carotene and \beta$-carotene increased with the increase in age of women. For men, current smokers showed significantly lower serum concentrations of \beta$-carotene and $\alpha$-carotene than ex-smokers (p <0.05) . Current drinkers also showed significantly lower serum \beta$-carotene and zeaxanthin + lutein concentrations than ex-drinkers. For women, current smokers showed significantly love. serum concentrations of zeaxanthin+lutein and cryptoxanthin than ex-smokes (p<0.05). Men with BMI$\geq$24 showed significantly lower serum concentrations of \beta$-carotene, $\alpha$-carotene, lycopene and cryptoxanthin than men with BMI < 20. The mean concentrations of $\alpha$-tocopherol, \beta$-carotene, $\alpha$-carotene and lycopene far postmenopausal women were higher than those for premenopausal women (p<0.05). In conclusion, there were obvious differences in serum $\alpha$-tocopherol and carotenoids levels depending on gender, age, cigarette-smoking habits, alcohol consumption, BMI and menopausal status. further studies are required to establish the normal levels of these vitamins for children and the elderly and to elucidate their roles in different disease states.

The Relationship between Job Stress and Job Satisfaction of Geriatric Care Helpers (요양보호사의 직무스트레스와 직무만족 간의 관계)

  • The research on the analysis of relationship between job stress and job satisfaction has been conducted targeting 355 Geriatric Care Helpers who are caring for the elderly people in the geriatric care centers in the area of Busan by the Act of Long-term Care Insurance for the Geriatric. As the method for the analysis t-test was conducted to investigate their job stress, counter-stress experience and effort, and job satisfaction. Also regression analysis was conducted to analyse the factors having effect on the job satisfaction of geriatric care helpers. The results of analysis can be summarized as following. First, job stress of geriatric care helpers at hospitals has shown significant difference from that at geriatric care centers, while job satisfaction of geriatric care helpers at care centers has shown some difference from that at hospitals. Second, measurement experience and effort of job stress was shown to some difference according to each institution. Third, the effect factors on geriatric care helpers were employment type, job stress, stress with patients in the case of hospitals, while those on geriatric care helpers were average wage, employment type, the number of charging patients, job stress, job requirement stress, and stress with patients. However, regardless of the type of employed institution, job satisfaction was shown high in case of full time job, low job stress, and high stress with patients.
