Impact of socio-demographic factors, lifestyle and health status on nutritional status among the elderly in Taiwan

  • BACKGROUND/OBJECTIVES: Aging is an imperative problem for many countries in this century, and presents several challenges for the maintenance of good nutritional status. This study aims to assess the impact of socio-demographic factors, lifestyle and health status on the nutritional status among the elderly in Taiwan. SUBJECTS/METHODS: A cross-sectional study was carried out in Taiwan. Data were obtained from the Mei Jau Health Management Institution, which is a private health evaluation provider with multiple health screening centers in Taiwan and Asia. This study included 7947 adults aged 65 years or above. The data were extracted between 2001 to 2010. Nutritional status was assessed using anthropometric data, biochemical data and dietary intake information. RESULTS: Among the 7947 participants with mean age of 70.1 (SD = 4.5) years, 20.2%, 6.6%, 10.5% and 52.5% experienced underweight, protein malnutrition, anemia and inadequate dietary intake in the past month, respectively. Age was negatively correlated with body weight (r = -0.19, P = 0.02), body mass index (r = -0.41, P < 0.001), albumin level (r = -0.93, P < 0.001) and hemoglobin level (r = -0.30, P = 0.008). Age above 70 years, gender, unmarried status, retirement, lack of education, low family income, smoking, alcohol drinking, sleep duration of 6-8 hours, vegetarian diet, multiple medications, comorbidity and dysphagia were positively associated with malnutrition in older adults. CONCLUSIONS: Underweight and inadequate dietary intake are prevalent among the elderly in Taiwan. Vegetarian diet, multiple medications, comorbidity, dysphagia and lifestyle factors such as smoking, alcohol drinking and sleep duration of 6-8 hours are risk factors for undernutrition in older adults.

Housing Policy for the Elderly and the Meaning of "Aging in Place" in Japan (일본의 고령자 거주문제와 주거정책: Aging in Place를 중심으로)

  • Recently, "aging in place" has become the principle philosophy of elderly housing policy in many countries. However, since there is ambiguity surrounding the universal definition of it, the meaning of AIP has been defined by political expediency. This study aims to discuss the meaning of AIP by examining the transition of elderly housing issues and policy in Japan. During the last two decades, the Japanese government has tried to launch a new elderly welfare system to shift the responsibility of elderly care from the family to the society and from the society to the region. And the focus of elderly housing policy has shifted from the promotion of three-generation dwellings to supporting the development of private residences. However, since the main goal has been limiting the welfare budgets rather than enhancing the quality of life, elderly housing issues have been reduced to a matter of choosing either elderly care facilities frequently located in suburbs or not. This paper suggest that we should pursue the true AIP idea in housing policy, and argues that to do so we need to consider AIP from diverse aspects such as temporal continuity, the range of space, the dynamics of place experience and the capability of communities.

Occupational Therapy in Long-Term Care Insurance For the Elderly Using Text Mining (텍스트 마이닝을 활용한 노인장기요양보험에서의 작업치료: 2007-2018년)

  • Objective : The purpose of this study is to quantitatively analyze the role of occupational therapy in long - term care insurance for the elderly using text mining, one of the big data analysis techniques. Method : For the analysis of newspaper articles, "Long - Term Care Insurance for the Elderly + Occupational Therapy for the Elderly" was collected after the period from 2007 to 208. Naver, which has a high share of the domestic search engine, utilized the database of Naver News by utilizing Textom, a web crawling tool. After collecting the article title and original text of 510 news data from the collection of the elderly long term care insurance + occupational therapy search, we analyzed the article frequency and key words by year. Result : In terms of the frequency of articles published by year, the number of articles published in 2015 and 2017 was the highest with 70 articles (13.7%), and the top 10 terms of the key word analysis showed the highest frequency of 'dementia' (344) In terms of key words, dementia, treatment, hospital, health, service, rehabilitation, facilities, institution, grade, elderly, professional, salary, industrial complex and people are related. Conclusion : In this study, it is meaningful that the textual mining technique was used to more objectively confirm the social needs and the role of the occupational therapist for the dementia and rehabilitation in the related key keywords based on the media reporting trend of the elderly long - term care insurance for 11 years. Based on the results of this study, future research should expand research field and period and supplement the research methodology through various analysis methods according to the year.

Significant Factors Related to the Intention of the Elderly to Live in a Community:The Case of Busan Metropolitan City (노인의 지역사회 거주의사에 영향을 미치는 요인: 부산광역시 노인을 중심으로)

  • This study aims to find the extent to which the elderly intends to live in their community and significant factors related to their intention and to provide basic but important empirical data in approaching to various community resources for community care service for the elderly. This study analyzed the raw data titled social welfare needs of residents of Busan Metropolitan City surveyed in 2005. Within the data, 1,673 households were selected in which at least one senior citizen aged 65 and over lived together. Questionnaires from in each household were analyzed. Research findings indicate that 80% of the respondents intend to live in their home rather than in residential institutions and that the elderly without adult children(55.2%) have less intention for living in their home than the elderly without their spouse(76.4%). Their intention-related factors were the presence of adult children, recognition on community resources for the elderly and perceived number of chronic diseases, when they were presumed to be healthy. When they were presumably weak or ill, socio-economic factors such as home ownership and welfare recipience were found to be more influential factors than family-related variables. The elderly who intended to live in home rather than to live in a residential institution were less likely to use social services in community than expected. Policy and practice implications were suggested on the basis of the findings.

  • An Infotainment Service intended to enhance the human emotion is introduced in this paper. The service is to be installed on the robot helping elderly persons to live comfortable and enjoyable life. The research started with defining the undesirable status of emotion in everyday life, and the psychological skills to cope with the status were sought about. Then, a methodology for providing emotion contents reflecting the coping skill has been suggested. Based on the Cognitive Behavior Therapy, the coping skill is used to edit animation clips. A movie recommendation system to utilize the edited animation clips has been being developed. A series of process for developing the system is described, where the emotion elements are taken into consideration in addition to the user preference as the criterion for recommendation.

The Effects of Time-use on the Elderly for Facilities in Activity of Daily Living (시설 노인들의 시간 사용이 일상생활 수행능력에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effects of the activities of daily living on time-use of the Elderly in facility. This study is also to provide basic data of the redesign time management and lifestyle as occupational therapy. Methods : The subjects were 20 elderly people (over 65 years) who live in S institution of Daejeun from August 11th, to August 22th in 2008. We used time table, interview and FIM which was to evaluate the ability to collect the general characteristics of these subjects(gender, age, moving period, education). Results : There was no significant different between the general characteristics of the subjects and activity of daily living. The more they spent time in Active BADL and IADL, the more they got higher scores in ADL performance ability(p<0.01). The more they spent time in the more they got higher score in ADL performance ability(p<0.05). Conclusion : We could know the amount of spending time of the elderly in Daejeun area facility and it related to activity of daily living. To improve the efficiency of time-use of the elderly, it is needed the role of occupational therapists. They should prepare a intervention to maintain active and positive life of the elderly.

The Effect of the Range of Motion Exercise on Powerlessness and Life Satisfaction in the Elderly (근관절운동이 노인의 무력감 정도에 미치는 영향)

  • This study was a clinical Quasi-experiment to examine the effects of the range of motion exercise on powerlessness and life satisfaction among the E-Institutionalized elderly in Pusan. The purpose was to test the effect of the range of motion exercises on powerlessness and life satis-faction, and to examine the correlation between powerlessness and life satisfaction. The subjects for this study were 54 elderly people, both were over 60 and residing at I-Institution in Pusan. The research samples of 54 elderly were divided into an experimental and control group by matching randomized assignment. The instruments for this study were the Assessment Tool of Powerlessness in the elderly developed by Miller, Life satisfaction developed by Yoon Jin, and the Active-Passive Range of Motion Exercise Program which was developed and utilized by Shin, Jae-Shin. The data were collected from December 15, 1990 to November 1991. The paired t-test, t-test and ANCOVA were used to test hypothesis I and II, related to the Effect of the Range of Motion Exercises to powerlessness and life satisfaction. To identify the relationship of the range of motion exercises to powerlessness and life satisfaction, path analysis was done through multiple regression. The interrelationship of the variables was analyzed using t-test paired t-test. The results of the three hypothesis can be summarized as follows : 1. Hypothesis I -the experimental group which participated in the Range of Motion Exercise gets lower powerlessness score than that of the control group-was accepted by t-test (t = -3.40, p = .001) but not accepted by ANCOVA (F = .1, p =.96). 2. Hypothesis II - the experimental group which participated in the Range of Motion Exercise gets a higher Life Satisfaction Score than that of the control group-was not supported by t-test(t = 1.01, p = .316) but was accepted by ANCOVA (F = 4.39, p = .006). 3. Hypothesis III - if the powerlessness Score becomes higher, the Life Satisfaction Score will be-come lower - was accepted by Pearson Correlation Coefficient (pre-exercise : r : -.34, p=.006), (post exercise : r = -.06, p=.32) . The relation-ship between the score of powerlessness and life satisfaction was negatively correlated. The results obtained clearly showed that the effects of the range of motion exercises on powerlessness and life satisfaction are beneficial to the elderly The relation between the scores of powerlessness and life satisfaction was negatively correlated and the range of motion exercise had a direct influence to powerlessness. Therefore, it is concluded that the relationship between powerlessness and life satis-faction was not a causal one. Also it is concluded that the range of motion exercises can be applied as a nursing intervention to reduce powerlessness and to improve life satisfaction in the elderly.

Significances and Outcomes of Mechanical Thrombectomy for Acute Infarction in Very Elderly Patients : A Single Center Experience

  • Objective : Mechanical thrombectomy is increasingly being used for the treatment of acute ischemic stroke. The population over 80 years of age is growing, and many of these patients have acute infarction; however, these patients are often excluded from clinical trials, so the aim of this study was to compare the functional outcomes and complication rates in very elderly patients (age ${\geq}80$ years) and aged patients (60-79 years) treated with mechanical thrombectomy. Methods : Between January 2010 and June 2015, we retrospectively reviewed 113 senior patients (over 60 years old) treated at our institution for acute ischemic stroke with mechanical thrombectomy. They were divided into a very elderly (${\geq}80$ years) and aged (60-79 years) group, with comparisons in recanalization rates, complications, death and disability on discharge be reported. Results : The mean age was 70.3 years in the aged group and 83.4 years in the very elderly group. Elderly patients had higher rates of mechanical thrombectomy failure than the younger group (40% vs. 14%; odds ratio [OR] 4.1; 95% confidence interval [CI] 1.4-11.9; p=0.012). Results from thrombolysis in cerebral ischemia and modified Rankin scale at discharge were worse in the older group (p=0.005 and 0.023 respectively). There were no differences in mortality rate or other complications, but infarction progression rates were significantly higher in the very elderly group. (15% vs. 2.2%; OR 8.0; 95% CI 1.2-51.7; p=0.038). The majority (92.3%) of the patients who failed in aged group were not successful after several trials. However, in half (4 of 8) of the very elderly group, the occlusion site could not be accessed. Conclusion : Patients older than 80 years of age undergoing mechanical thrombectomy for acute infarction were more difficult to recanalize due to inaccessible occlusion sites and had a higher rate of infarction progression, However, mortality and other complications were similar to those in younger patients.

Changes of Family Role on the Elderly Income Security in North Korea (북한 노후소득보장에서 가족의 역할에 대한 연구: 김일성 시대와 김정일 시대의 비교)

  • This study aims to examine the change of state-market-family as three welfare provisions. This exploratory study investigates the change of welfare provisions and life of the elderly for 25 North Korean refugees focusing the actual condition of income security for the elderly using the snowball sampling. The main results are followings. First, in the age of Kim Il Sung showed the state-led, market absence, family supplementation and in the age of Kim Jong Il and Kim Jong Un showed the state diminishment, market appearance, family supplementation in terms of welfare provisions. Second, there is disparity between institution and reality because the law don't prescribe the responsibility of market provisions for the people but the elderly should have made a living in the market since the early 1990s. The situation of 'the weaken state and strengthened market' in the age of Kim Jong Il have still continued in the age of Kim Jong Un. The formal income security for the elderly need to return to the former condition.

Possibility of Risk Accidents and Response Strategies for Participants in Agencies in Charge of Employment for the Elderly: Focusing on occupational risk accidents and COVID-19 risk accidents (노인일자리 전담기관 참여자의 위험사고 발생가능성과 대응 전략: 업무상 위험사고와 코로나-19 위험사고를 중심으로)

  • This study identified the level of awareness of dangerous accidents by empirically analyzing the possibility of hazardous accidents at work and the possibility of dangerous accidents due to COVID-19 targeting the participating seniors who are active in the Senior Club, an organization dedicated to job placement for the elderly. The purpose of this study is to provide basic data for preparing measures to improve risk accidents that may occur in organizations in charge of job placement for the elderly and to suggest countermeasures to prevent dangerous accidents. For this purpose, data were collected from the elderly participating in 16 senior clubs in Busan, and 425 of the collected 430 were ultimately used. As a result of this study, first, it was found that the elderly who participated in a job-dedicated institution for the elderly generally perceived the possibility of occupational risk accidents and risk factors due to COVID-19 to be low. Second, it was found that there was a statistically significant difference in the probability of occurrence of hazardous accidents at work according to demographic characteristics during the participation period, and the probability of occurrence of dangerous accidents with COVID-19 was found to have statistically significant differences in the classification and age of the participating elderly. Based on the results of this study, the project manager suggested creating a safety accident management record sheet, preparing a risk management manual, and establishing a risk management system reflecting the individual characteristics of the participants.
