An Exploratory Study of the Effect of Ongyeong-tang Administration on Insomnia Disorder in Patients with Neurological Diseases: A Retrospective Chart Review Study

  • Objectives: The aim of this study was to retrospectively evaluate the efficacy and safety of Ongyeong-tang (OGT) in the treatment of insomnia in patients afflicted with neurological diseases, according to the medical records of one medical institution. OGT is a herbal prescription that originates from the "Synopsis of Prescriptions of the Golden Chamber" and has been used for menopausal symptoms, uterine diseases, skin diseases, insomnia, and other neuropsychiatric symptoms. Methods: Medical records of patients who visited Kyung Hee University Korean Medicine Hospital from January 1, 2021 to June 30, 2022, received outpatient or inpatient treatment, and met the inclusion criteria were used in this study. Results: Twenty-four patients satisfied the inclusion criteria and were included. The Pittsburgh Sleep Quality Index-Korean version (PSQI-K) score was used to evaluate the outcomes before and after treatment. After an average of 14.42±9.2 days of OGT extract intake, 20 patients (83.3%) experienced improvement. The remaining four (16.7%) had no change in their symptoms, but none demonstrated worsening of symptoms. The total PSQI-K score showed a statistically significant decrease from 13.08±4.54 to 10.42±4.58 (p<0.001). Sleep quality, sleep duration, sleep disturbance, use of sleeping medications, and daytime dysfunction also showed statistically significant improvements. Conclusions: Administration of OGT extract may be a relatively safe and effective therapeutic option for insomnia in patients with neurological diseases.

불면증 평가를 위한 수면다원검사 (Polysomnographic Evauation for Disorders of Initiating and Maintaining Sleep)

  • Disorders of intitating and maintaining sleep(DIMS : insomnia) may be a symptom of some other disease or the basic, root problem in some patients. DIMS is usually treated by private physicians or psychiatrists, initially. Many physicians can do to improve the quality of life of the sleep disordered patients by appropriate evaluation and management For the effective management of DIMS patients, the accurate differential diagnosis is essential. In polysomnographic evaluation for insomnia, the physician should consider the aspects of cost-effectiveness to provide the maximal benefits for the patients. For the purpose of this, evaluation of DIMS complaints outside the sleep disorder clinic, indication for referral to a sleep disorder clinic, the role of polysomnopraphy in evaluating DIMS, and polysomnographic findings of different types of DIMS are discussed, together with possible way to diagnose them.

수면 설문지를 통한 수면장애의 진단 (Diagnosis of Sleep Disorders Through Sleep Questionnaires)

  • It is very difficult to evaluate sleep disorders by simple history taking, because which covers very comprehensive areas such as psychobiosocial fields. Although polysomnography is used for the method of final diagnosis, systemic history taking and sleep question-aires are still critically important especially in evaluation of insomnia. Proper informations through sleep questionnaires can provide very precise data for effective treatment as well as exact diagnosis. Sleep questionnaires consist of largely four kinds of questionnaires, which are screening questionnaire of sleep disorders, sleep diary and questionnaire of sleep hygine, diagnostic questionnaire for specific sleep disorder and questionnaire of special symptoms of sleep disorders including insomnia, daytime sleepiness, cognitive function, mental symptom and personality, parasomnia, physical illness and sexual function. However, for more conclusive diagnosis especially in excessive daytime sleepiness nocturnal polysomnography and multiple sleep latency test should be performed.

  • Insomnia is one of the highest frequency neuropsychosis of neuropsychiatry patients and is primarily caused by major depressive disorder. This case study is about a remedy for a 33 year-old woman who has insomnia and intense depressed mood caused by the continuous family jar. Also, she had the feeling of squeezing her body, pouring cold water into her body, anorexia and feeling cold and hot turn and turn. I operated mindfullness with accupuncture, herb-medication and aroma theraphy. Especially, mindfullness and Gagamguibi-tang were effective for relaxation and the sound sleep in quality and quantity. According to this study, the traditional oriental medical remedy such as accupuncture, herb-medication is effective for the cure of insomnia and mindfullness fortifies the oriental medical remedy.

사역산(四逆散)의 불면 치료 효과 : 체계적 문헌 고찰 및 메타 분석 (The Effect of Sayeok-san for Insomnia: A Systematic Review and Meta-Analysis)

  • Objectives: This study evaluated the effectiveness of Sayeok-san for treating insomnia. Methods: We conducted a search using keywords, such as "Sayeoksan", "Si-ni-san", and "insomnia" in eight databases (CNKI, Wanfang, Pubmed, Embase, Cochrane, NDSL, OASIS, RISS) on July 17, 2012. There was no limit placed on the publication period of the papers, and we used Cochrane's risk of bias tool to assess their quality. Meta-analysis was classified according to the outcome measurements, such as total effective rate and PSQI. Results: Nine RCTs were selected. Meta-analysis showed that the treatment group, which used Sayeok-san, had a higher total effective rate than did the control group that used only Western medicine. The treatment group also had a lower average of PSQI and TSS (TCM symptom score) than did the control group. Conclusion: This study suggests that Sayeok-san is effective in treating insomnia. However, these findings should be interpreted cautiously due to unknown or high risk of bias of the included trials.

수면클리닉을 방문한 환자들의 불면증에 대한 인식조사 (Survey of Knowledge on Insomnia for Sleep Clinic Clients)

  • 목 적 : 본 연구의 목적은 수면장애로 수면클리닉에 내원하는 환자들을 대상으로 수면클리닉을 방문하게 된 이유, 찾게 된 경로, 수면클리닉을 찾게 되는 가장 흔한 증상이자 독립질환인 불면증에 대한 인식을 조사하고 그 의미를 해석하여 향후 수면장애의 치료와 환자교육에 도움이 되고자 하였다. 방 법 : 2018년 4월부터 10월 사이 6개월 간 국립정신건강센터의 수면클리닉을 방문한 외래환자들 중 만 19세 이상의 초진환자를 대상으로 자기보고식 설문을 진행하였다. 이때, 지적장애, 치매 등 심한 인지기능을 동반한 환자등을 배제하였고, 총 44명(남성 24명, 여성 20명)이 연구에 참석하였다. 설문지는 자체개발한 설문지로 수면클리닉을 방문한 이유, 수면클리닉을 알게 된 경로, 수면클리닉 방문 이전 수면질환의 치료력, 불면증의 인식에 대한 질문, 불면증의 치료에 대한 인식, 불면증의 동반질환(comorbodity)에 대한 지식을 묻는 질문으로 구성되었다. 결 과 : 연구에 참가한 대상자들은 총 44명으로 평균 연령은 $54.11{\pm}16.3$세로 남성은 24명(54.5%), 여성은 20명(45.5%)였으며, 40대 이상의 중장년층이 전체의 77.3%를 차지했고, 81.8%(36명)가 고졸이상의 학력을 갖추고 있었다. 환자들의 72.7%(32명)가 공존질환을 갖고 있었으며, 22.7%에서 고위험음주를 보였다. 수면클리닉에 내원한 이유는 잠들기 힘들고 중간에 자주 깸, 낮에 피곤하고 졸림, 불규칙한 수면시간, 악몽을 자주 꿈, 코골이와 수면 중 숨막힘 등의 순서였고, 93.2%에서 불면증을 인식하고 있었고, 가족 친지, 방송, 인터넷 등을 통해 본인이 불면증에 대한 지식을 얻게 되었다고 하였다. 불면증의 치료에 대해서는 70.5%가 약물치료를 떠올렸고, 이에 대한 정보는 인터넷과 의료진을 통해 주로 얻고 있었으며 불면증의 비약물학적인 치료방법에 대해 52.3%가 들어본 적이 없다고 하였다. 불면증의 치료선호도를 묻는 질문에 45.5%가 특별한 선호도 없이 의사의 권유대로 따르겠다고 하였고, 불면증의 공존질환 중 우울증, 신체질환, 알코올사용장애을 알고 있는지를 묻는 질문에 대해 각각 75%, 50%, 38.6%의 응답률을 보였다. 수면제 복용 시 의사의 지침을 준수해야 한다는 것에 대해서는 75%에서 알고 있었지만, 전체 응답자의 68.2%가 수면제 처방에 대해 걱정을 하고 있었고 이들 중 70%가 약물의존에 대해 걱정된다고 하였다. 결 론 : 내원자의 56.8%가 내원전 타의료기관에서 수면장애로 치료를 받고 왔음에도 불구하고, 불면증의 치료, 특히 비약물학적인 치료에 대한 잘 알고 있지 못했으며, 본인의 수면제 복용여부와 상관없이 수면제에 대해서는 약물의존에 대한 걱정을 많이 하고 있었으나 상대적으로 불면증과 공존질환으로서의 알코올 사용문제에 대해서는 간과하고 있었다. 향후 일선 일차의료담당자의 불면증치료에 대한 이해증진 및 표준화된 불면증치료안의 개발이 시급할 것으로 생각된다. 또한 알코올 사용장애에 대한 구체적인 평가가 초기 면담에서 반드시 필요하다고 하겠다.

  • The clinical study was carried out the 33 patients with insomnia who were treated in Daejeon University Oriental Hospital from 17 March 1997 to 12 May 2001. The results were summarized as follows. 1. The ratio of male and female was 10:23 and sleep initiation insomnia and sleep maintenance insomnia were the highest frequence(97%), the beginning of insomnia was frequent at 30s(27.3%). 2. Living accident by mental attack was the most inducing factor and many patients came to our hospital by way of west-neuropsychiatry(55.6%). 3. In admission period most of the patients were within 15 days(78.8%) and Liver-Qi- depression was the main cause and in classification of Four Human coporeal constitution the number of patients Sho-Eum-In(少陰人) was remarked mostly and in distribution of the prescription drugs of growing heart and warming gall bladder such as GUIBIONDAMTANG(歸脾溫膽湯) and ONDAMTANGGAMI (溫膽湯加味) were many, 4. Relatively most patients were well treated(69.7%) and insomnia was mainly caused by Anxiety Disorder and Depression and Hwabyoung and in the age distribution the highest frequence was 30s but we know insomnia appeared at all ages. 5. In distribution of the period of the clinical history was various and within 15 days were all improved and the treatments group of drug and acupuncture and aid treatments etc. and hypnotics was more improved than not used hypnotics treatments group but using hypnotics was temporal. 경계성 인격 장애 환자의 수면 (Sleep in Borderline Personality Disorder Individuals)

  • Borderline personality disorder (BPD) is characterized by identity and interpersonal problem, affective dysregulation and pervasive severe impulsivity. Although sleep disturbances are not primary symptoms of BPD, they are important aspects of this disorder. However, clinicians and researchers did not give much attention to the sleep symptoms of BPD yet. Measured by nocturnal polysomnography, increased sleep latency as well as reduced total sleep time and sleep efficiency, and 'depression-like' REM abnormalities (i.e., reduced REM latency and increased REM density) are found in BPD patients. Co-morbid sleep disorders such as chronic insomnia, nightmare disorder or circadian rhythm sleep disorder associated with BPD have been reported. Clinicians should focus on the sleep complaints of BPD patients, and carefully manage such symptoms with sleep hygiene education, cognitive psychotherapy or light therapy.

시론영위주야절률성재수면활동중적작용(试论营卫昼夜节律性在睡眠活动中的作用) (Study on The Rhythmical Rules of The Circulation of Yeong(營) and Wi(衛) during Sleep)

  • This paper according to the sleep theory of Yeonggi(營氣) and Wigi(衛氣)'s circulation in "Naegyeong(內經)", explains that Yeonggi and Wigi's circulation is the internal basic for human's sleep-wake rhythm, and it's relationship with other sleep theories, such as mental activity sleep theory, soul sleep theory, Jangbu(臟腑) organs sleep theory etc. It can be seen that the circulation law of Yeonggi and Wigi for day and night is the key of sleep activities, and Yeonggi and Wigi's circulation disorder is the key of insomnia, so we should use coordination of Yeonggi and Wigi as basic method for treating insomnia, based on treatment after differentiation of syndromes.

중풍환자(中風患者)의 불면증(不眠症)에 피내침(皮內針)의 유효성(有效性) 및 적응증(適應證) 평가(評價) (The Effect of Intradermal Acupuncture on The Patients suffering from Insomnia after Stroke)

  • Objective : Insomnia is the disorder of initiation or maintenance in sleeping that results in daytime sleepiness and dysfunction, and it arises from multiple psychological, physiological and environmental factors. A number of stroke patients suffer from insomnia are classified as sleep disorder patients with physical illness. In addition, insomnia may have profound deleterious effects on the natural course of stroke. We are to assess the effectiveness of intradermal acupucture on stroke combined with insomnia. Methods : We recruited hospitalized patients with insomnia after stroke. Then, the subjects were allocated into a treatment group and a control group by randomization. The treatment group received intradermal acupuncture therapy at He-7(神門), EH-6(內關) correctly. However in control group, intradermal acupuncture were just attached to He-7(神門), EH-6(內關) without insertion. Treatment over time at first day and second day a various indexes were repeatedly checked such as sleep latency, total sleep time, sleep quality, condition on awakening, ability of concentration, morning sleepiness, Insomnia Severity Index, and Athens Insomnia Scale. In the second analysis, we divided the treatment group into a response and a non-response group by their response to intradermal acupuncture. With the acupuncture treatment, accompanied symptoms were checked Results : Among thirty two subjects, two of them were dropped out: One complained the pain from needle insertion, and the other underwent the change of his herbal medication. At baseline investigation with the residual 30 subjects, the control and the treatment group were assessed to have an equal comparability. In the treatment group, sleep latency, total sleeping time, a number of awakenings, sleep quality and ability of concentration showed improvement significantly compared with the control group. The non-response Group showed symptoms such as nausea and halitosis, belching and acid regurgitation, abundant expectoration, while Responder Group showed palpitation, oppressed feelings in chest and sleep disorder. Conclusions: The result of this clinical study suggests that the intradermal acupuncture at He-7(神門), EH-6(內關) is one of the effective treatments for the insomnia after stroke.
