Purpose: A stability-measuring device that utilizes damping capacity analysis (DCA) has recently been introduced in the field of dental implantology. This study aimed to evaluate the sensitivity and reliability of this device by measuring the implant stability of ex vivo samples in comparison with a resonance frequency analysis (RFA) device. Methods: Six implant beds were prepared in porcine ribs using 3 different drilling protocols to simulate various implant stability conditions. Thirty-six pork ribs and 216 bone-level implants measuring 10 mm in height were used. The implant beds were prepared using 1 of the following 3 drilling protocols: 10-mm drilling depth with a 3.5-mm-diameter twist drill, 5-mm drilling depth with a 4.0-mm-diameter twist drill, and 10-mm drilling depth with a 4.0-mm-diameter twist drill. The first 108 implants were external-connection implants 4.0 mm in diameter, while the other 108 implants were internal-connection implants 4.3 mm in diameter. The peak insertion torque (PIT) during implant placement, the stability values obtained with DCA and RFA devices after implant placement, and the peak removal torque (PRT) during implant removal were measured. Results: The intraclass correlation coefficients (ICCs) of the implant stability quotient (ISQ) results obtained using the RFA device at the medial, distal, ventral, and dorsal points were 0.997, 0.994, 0.994, and 0.998, respectively. The ICCs of the implant stability test (IST) results obtained using the DCA device at the corresponding locations were 0.972, 0.975, 0.974, and 0.976, respectively. Logarithmic relationships between PIT and IST, PIT and ISQ, PRT and IST, and PRT and ISQ were observed. The mean absolute difference between the ISQ and IST values on a Bland-Altman plot was -6.76 (-25.05 to 11.53, P<0.05). Conclusions: Within the limits of ex vivo studies, measurements made using the RFA and DCA devices were found to be correlated under a variety of stability conditions.
연구 목적: 전통적인 초기의 접착성 고정성 보철물이 가능한 보존적이고 가역적인 치료를 추구했지만 유지력의 상실이 큰 문제가 되면서 임상적 성공률이 낮은 단점이 있었다. 이러한 한계를 극복하고자 하는 여러 연구들 중, 자연 치아의 언더컷과 구조물의 각기 다른 삽입로를 이용하여 유지력을 얻는 3-piece형태의 변형된 접착성 고정성 보철물이 제안되었다. 이 연구의 목적은 변형된 형태의 접착성 고정성 보철물을 이용하여 수복 치료를 받은 환자에 대한 후향적 연구를 통하여 임상가들의 상실치 수복을 위한 치료 계획 수립에 도움을 주고자 함이다. 연구 재료 및 방법: 한 개 혹은 그 이상의 치아가 상실되어 수복을 요하여 2007년부터 연세대학교 치과대학병원 보철과에 내원하여 자연치아의 언더컷과 구조물의 각기 다른 삽입로를 이용하는 변형된 형태의 접착성 고정성 보철물 수복 치료를 받은 23명의 환자중 주기적 검사가 가능한 21명을 대상으로, 후향적으로 임상 및 방사선 검사 결과를 평가하였다. 결과 및 결론: 자연치아의 언더컷과 구조물의 각기 다른 삽입로를 이용한 접착성 고정성 보철물을 이용한 21명증례, 71 unit의 최대 25개월 (평균 7개월)간의 후향적 연구에서 다음과 같은 결과를 얻었다. 1. 자연치아의 언더컷과 구조물의 각기 다른 삽입로를 이용한 접착성 고정성 보철물은 단기간의 follow-up 에서 보철물의 탈락, 파절등의 기계적인 실패가일어나지 않았다. 2. 보철물 주변의 치주조직은 비교적 안정되었다. 3. 방사선 사진상 유의성있는 골흡수를 보이지 않았다.
본 연구는 외부 전원 없이 광다이오드만을 이용하여 생성한 광전 자극을 통해 신경계를 효과적으로 자극하는 방법에 대한 것이다. 광을 통한 전류원 생성 및 전달은 생체 내에 집적된 광소자를 삽입하고 외부에서 광을 통해 신호와 전력을 전달을 한다. 이 기술은 특히 '눈' 이라는 광학적인 연결통로를 이용할 수 있는 인공망막과 같은 시스템에 매우 효과적이다. 그러나 광전 소자를 내부 전원 없이 구동시키는 경우, 광전류가 생체 저항에 직접적인 영향을 받게 되므로 자극에 충분한 전류를 생성할 수 없다. 무 전원 광다이오드를 통해 생성되는 광전류를 신경 자극에 적용하기 위해서는 생체 저항의 크기에 관계없이 활동 전위 생성에 충분한 전류 공급을 할 수 있는 안정된 전류원이 필요하다. 이를 위해서 본 연구에서는 병렬 저항을 도입하였다. 병렬 저항 추가 시 생체 저항을 포함한 전체 저항 값이 낮아지므로, 광원의 세기에 따라 최대의 광전류에 근접한 값을 얻을 수 있게 된다. 그러나 병렬 저항 값의 크기를 낮출수록 자극에 쓰이지 않는 전류량이 늘어나므로, 자극 전류량의 극대 값을 찾기 위해서는 병렬 저항 값의 최적화가 필요하다. 실험을 통해 측정된 실제 자극 전류량이 최대가 되는 병렬 저항 값의 범위는 500Ω∼700Ω 이고, 이때 전류량은 580uA∼860uA 이며 전류 효율은 47.5∼59.7%이었다. 자극의 크기와 빈1도를 변화시키면서 쥐의 좌골 신경을 자극하여 눈으로 확인 가능한 떨림 현상을 확인하였으며, 다채널 기록기를 이용해 활동 전위를 측정하였다. 이를 통해, 인공 망막에서의 광 자극 가능성을 확인할 수 있었다.
선형가속기에 의한 뇌정위적 방사선수술에 적용되는 원형 소조사변의 선량분포를 측정하기 위해 측정기 선정 이유와 선축 결정, 자체 제작한 소형 물 팬톰에 의한 선량분포 측정시 고려해야 할 점에 대해 논의하고, 치료계획에 필요한 자료인 Clinac-18의 10MV X-선의 TMR, OAR, 조사면 계수와 같은 선량분포 측정결과를 보고하고자 한다. 뇌정위적 방사선수술에 권고되고 있는 조사면 크기가 3cm 이하의 작은 조사면에 대한 선량 분포를 측정하기 위해서는 크기나 감도에 있어서 적합한 p-형 실리콘(Si) 검출기가 선량에 대한 선형성과 선량율 독립성이 적합한지 측정에 의해 판단하였다. 크기와 형태가 같은 아크릴 통을 두 개 제작하여 호스로 연결하여 하나는 물 팬톰으로 이용하고 다른 하나는 높이를 조절하여 측정기의 깊이를 조절하였다. 측정할 위치에서 직각 방향의 측방선량분포를 측정하여 선축의 위치를 찾았다. SAD 100cm 위치에서 조사면 크기 10, 20, 30, 40mm 네 개 콘에 대하여 TMR을 측정하였으며, 일정한 선원-측정기간 거리(SCD)에서 최대선량점깊이(d$_{max}$) 및 6, 10, 15cm 깊이에서 OAR을 측정하여 비교하였다. 조사면 계수는 MU당 SAD, d$_{max}$에서 콘에 대한 선량으로 실리콘 검출기로 측정하였다. 실리콘 검출기는 선량에 대한 선형성이 거의 완벽하였으며 감도는 선량율이 증가함에 따라 감소하였다. 낮은 선량율 때문에 조사면 밖의 선량을 약간 과대평가할 수 있을지라도 100MU/min 이상의 선량율에 대해서는 일정하였다. 직각 방향의 측방선량분포 측정에 의하여 선축을 찾는 방식은 간편하였다. 1cm 두께의 아크릴 판을 보조 물통 아래에 삽입ㆍ제거하는 방식으로 측정기의 깊이 조절도 간편하면서 정확하였다. 측정에 의한 TMR, OAR, 조사면 계수는 충분히 정확하여 뇌정위적 방사선수술의 치료계획에 이용할 수 있었으며, OAR은 조사면 범위 내에서는 깊이에 거의 무관하였다. 실리콘 검출기는 소조사면 선량분포 측정에 적합하였으며 직각 방향의 측방선량분포의 측정으로 0.05mm까지 정확히 선축을 찾을 수 있었고, 보조 물통과 아크릴 판을 이용하여 측정기의 깊이를 조절하는 것이 용이하였다. TMR, OAR, 조사면계수의 측정치는 뇌정위적 방사선수술의 치료 계획에 이용할 수 있을 정도로 정확하였으며, OAR은 하나의 깊이에서 측정해도 충분할 것이라고 사료된다.
목적: Holmium-166은 류마티스성 관절염 및 간암 치료에서 이미 임상적으로 사용이 보고된 방사성 동위원소이다. 본 연구는 holmium-166-chitosan 복합체를 외과적으로 생체에 투입하여 임상에 적용 가능한 방법을 연구하고, 투여량과 기간, 투여 부위에 따른 조직학적 반응 정도를 정성 및 정량 분석하여 임상적용의 기초로 활용하는 것이다. 재료 및 방법: Absorbable gelatin sponge에 액체상태의 holmium-166-chitosan 복합체를 50 ${\mu}l}$ 도포하여 고형제로 만들었다. 최종 삽입전에 caliberator로 측정한 방사선량은 약 1.5 mCi였다. Wistar종 흰쥐에 외과적 시술을 통하여 대퇴근육 내부와, 그리고 대퇴골에 접하게 holmium-166-chitosan 복합체가 부착된 gelatin sponge를 삽입 후 봉합하였다. 대퇴 근육내와 대퇴골에 투여한 후 2주, 4주, 6주에 육안 및 현미경 소견하에서 조직 괴사의 깊이, 조직 변화의 양상, 삽입된 gelatin sponge의 변화 등을 관찰하였다. 결과: 2주후의 현미경 소견상 holmium-166과의 접촉면에서는 정상 근육세포의 모양을 찾아 볼 수 없을 만큼 심한 괴사 소견을 보였고 근육세포의 퇴화 및 재생, 근육간 부종, 염증세포의 침윤 소견을 보였으며 유발된 괴사의 깊이는 평균 3.3 mm 였다. 대퇴골에서는 holmium-166과의 접촉면에 있는 골피질은 골소강내 골세포의 소실이 관찰되었고 골수강내의 골수 세포도 파괴되고 섬유화 또는 염증성 반응을 보였다. 괴사의 깊이는 평균 2.9 mm 였다. 4주 때 근육에서는 석회화와 보다 심해진 섬유화 소견이 추가 되었고 괴사의 깊이는 평균 3.3 mm 였다. 골조직에서도 골수강의 섬유화가 더 증가되었고 괴사의 깊이는 평균 3.3 mm였다. 경과 관찰 6주 군에는 연부 조직의 위축과 섬유화의 증가, 육아조직의 형성, 염증 소견의 소실 등이 관찰되었다. 결론: Holmium-166-chitosan 복합체를 생체 흡수 가능한 gelatin sponge에 부착시킨 후 실험동물에 삽입한 결과, 실험동물의 사망을 유발하지도 않았으며 상처의 괴사, 염증, 감염 등의 부작용이 전혀 없어, 생체에 외과적 적용이 가능하였다. 유발된 조직 괴사의 범위는 약 3 mm로 미리 예측한 정도에 근접하는 것으로 악성종양 치료에 임상적 적용이 가능할 것으로 생각한다.
Objectives : To understand the strength and weakness of traditional warm needle acupuncture based on existing research outcomes in Korea mostly and to suggest how to build the desirable warm needle acupuncture apparatus by overcoming demerits of traditional and currently existing ones. Methods : We searched warm needle relating papers in Korean with the key words of '온침, 화침, 열자극' by using DBpia, Journal of Korean Oriental Medical Society, Journal of Korean Acupuncture & Moxibustion Society and Journal of Society for Meridian and Acupoint. Degree dissertations in Korean were also searched with the same words through the search engine of Library of Kyung Hee University. We also searched with words of 'warm, thermal, heat, needle, acupuncture, temperature' via Pubmed and found a small number of English written papers and large number of Chinese written ones. To find english translated version of those papers, we googled with the same words with no success. Results : About 20 papers on warm needle acupuncture written in Korean were found and analyzed with respect to experimental factors that affected the thermal properties or the amount of heat stimulus of the acupuncture. More rigorous descriptions seemed to be required on the insertion depth, duration and manipulation of warm needle treatment. A basic heat transfer model was presented for the clarification of heat loss through the needle of warm needle acupuncture. Environmental factors such as air flow on the properties seems to be considered for the warm needle acupuncture. Conclusions : Papers on warm needle acupuncture were reviewed and analyzed based on their thermal properties and tools such as needles and moxa cones. Several suggestions were made on the descriptions relating the properties. Necessary specifications were introduced to aim modernized warm needle systems.
Cochlear implant poses a contraindication to the magnetic resonance imaging(MRI) process, because MRI generates artifacts, inducing an electrical current and causing device magnetization. CT is relatively expensive and the metal electrodes scatter the image. Post-implantation radiological studies using anterior-posterior transorbital, submental-vertex and lateral views, the intracochlear electrodes are not well displayed. Therefore, the authors developed a special view, which we call the cochlear view. The patient is sitting in front of a vertical device. Then the midsagittal plane is adjusted to form an angle of $15^{\circ},\;30^{\circ}$, and $45^{\circ}$ with the film. The flexion of the neck is adjusted to make the infraorbitomeatal line(IOML) is parallel with the transverse axis of the film. The central ray is directed to exit from the skull at point which is 3.0 cm anterior and 2.0 cm superior to the EAM(external auditory meatus). Results have shown that single radiography of the cochlear view provides sufficient information to demonstrate the position of the electrodes array and the depth of insertion in cochlear. Radiography of the cochlear view in angle of $45^{\circ}$ is an excellent image. The cochlear view gives the greatest amount of medical information with the least radiation and lowest medical cost. It can be widely used in all cochlear implant clinics.
The purpose of this study was to test the hypothesis that the more thorough cure of lightactivated composite resin could be obtained if a new way of delivering the light source deep through the resin body is developed when compared to the conventional bulk cure or incremental cure. Using cylindrical resin blocks and natural teeth, various curing conditions were tested for their effects on the degree of microleakage and microhardness. Data were analyzed statistically using One-Way ANOVA and Scheff's Multiple Range test. The results of the present study were as follows: 1. The increment of reduction in microhardness with depth was shown to be higher in group I and II than in group II and V. 2. At the level of lower inner surface and the bottom surface, significantly lower microhardness values were obtained in group I and II when compared to group III, IV and V. Group IV showed the lowest value among three groups(III, IV, V) representing the only statistically significant difference from group V(p<05). 3. Differences between groups in the degree of microleakage detected were shown to be statistically significant (p<05) with the exception of group IV and V at both occlusal and gingival margins. 4. Based upon the above-mentioned results, commonly cited recommendation of incremental curing is strongly supported and the light-transmitting wedge insertion method can be considered as a effective and feasible clinical procedure for the better curing of composite resin. However, more studies under a variety of conditions should be completed before this method is applied actually to the clinical setting.
The purpose of the present study was to evaluate and compare the periodontal conditions of distally extending cantilever bridges(CB) and removable patial dentures(RPD). 78 patients treated with removable partial dentures were divided into 2 groups; bilateral free-ending (type 1) and unilateral free-ending(type 2). 43 patients treated with distally extending cantilever bridges were divided into 4 groups according to position of missing molars and number of including abutment tooth (or teeth) (type 1-4). Prosthodontic status of the abutment teeth were assessed; overcontoured crown, overhanged margin, embrasure closure, and others. Their Periodonted status were assessed with Plaque Index Gingial Index(GI) Sulcus bleedy Index (SBI), Tooth mobility(TM) & Posher depth(PPD). 35.9% of RPD group and 38.1 of CB grouphad disconfort on mastication with prosthetics. 16.7% of RPD group and 6.1% of CB group showed on opposing sites. Overhanged margins were 58.3% for RPD group and 86.6% for GB group. 45.5% of RPD group and 72% of CB group had embrasure closure on the abutment teeth. 30.5% of GB group had oversized pontic. Gb group showed much higher clinical parameters than RPD groups, but statistically not significant. Type 2 of RPD group showed lower GI and SBI, but statistically not significant. Type 3 of CB group showed the highest GI and SBI and type 2 showed the highest tooth mobility, but there is no differences between the same type of groups. In conclusion, only minor differences in periodontal conditions were recorded in RPD and CB group.
Background: Oncogenic Bmi-1 (B-lymphoma Moloney murine leukemia virus insertion region-1) belongs to the Polycomb-group (PcG) family of proteins and plays an important role in the regulation of proliferation, senescence, cell cycle and apoptosis, chromosome stability, activation of gene transcription. Methods: To clarify the roles of Bmi-1 in tumourigenesis and progression of gastric carcinomas, it was examined by immunohistochemistry (IHC) and real-time RT-PCR in gastric carcinomas, dysplasia, intestinal metaplasia (IM), and gastritis with a comparison of its expression with clinicopathological parameters of carcinomas. Results: There was gradually increased Bmi-1 protein expression from gastritis, IM, dyplasia to carcinoma (p<0.001). Bmi-1 expression was positively linked to tumor size, depth of invasion, lymph node metastasis and worse prognosis of carcinomas (p<0.001), but not to age or sex of carcinoma patients (p>0.05). There was higher Bmi-1 protein expression in intestinal-type carcinomas than diffuse-type ones (p<0.001). At mRNA level, Bmi-1 protein expression was increased from gastritis, IM, dysplasia and carcinoma (p<0.001). Bmi-1 overexpression was observed in gastric carcinoma with larger diameter, deeper invasion, lymph node metastasis, and intestinal-type carcinoma (p<0.05). Conclusion: These findings indicate that up-regulated Bmi-1 expression is positively linked to pathogenesis, growth, invasion, metastasis and differentiation of gastric carcinomas. It was considered as a promising marker to indicate the aggressive behaviors and prognosis of gastric carcinomas.

④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)

제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
