CIDP와 CMT 1형의 전기생리학적 특성에 대한 정량 분석: 감각신경연구 (Quantitative Analysis of Electrophysiological Characteristics of CIDP and CMT Type 1: Sensory Nerve Research)

  • 선천성 유전질환인 CMT와 후천성 면역 매개 질환인 CIDP는 임상적 증상이 유사하므로 두 질환의 감별진단을 위해서는 말초신경의 전기생리학적 특징을 비교하는 것이 도움이 될 수 있다. 본 연구는 CIDP와 CMT 1형으로 확진된 환자의 신경전도검사 결과 중 감각신경전도검사의 주요 지표별 결과를 후향적으로 정량분석하여 두 질환군의 전기생리학적 특징을 규명하고자 하였다. CIDP (N=35)와 CMT1 (N=30)로 확진된 환자의 dCNAP와 SNCV를 이용하여 두 질환군의 중증도 분석, 유의성 검정, 비정상 범위별 비율분석 및 상관분석을 실시하여 통계적 차이를 기반으로 특징을 비교하였다. 두 질환 모두 전신성 말초신경다발신경병증의 특징이 관찰되었고, 장딴지신경의 손상이 가장 심한 것으로 확인되었다. CMT1군은 탈수초성 및 축삭성 신경병증을 동반하는 전신성 신경병증이고, CIDP보다 더 중증의 신경병증임이 확인되었다. 또한, 상관계수 및 산점도 분석에서 CMT1은 신경 손상 범위가 전체 신경에서 균등한 전기생리학적 특징이 관찰되었다. 감각신경전도검사의 결과를 기반으로한 본 연구결과가 CIDP와 CMT 1형의 감별진단 및 연구에 도움이 될 것으로 사료된다.

  • Objective: The aim of this study was to investigate 1) the difference in static lower extremity alignment (SLEA) according to a history of lateral ankle sprain (LAS), 2) to identify SLEA factors affecting LAS, and 3) to present the cut-off value and 4) the usefulness and limitations of the SLEA measurement. Method: This case-control study recruited 88 men (age: 27.78±4.69 yrs) and 39 women (age: 24.62±4.20 yrs) subjects with and without LAS. SLEA measurement protocol included Q angle, tibiofemoral angle, genu recurvatum, rear foot (RF) angle, tibal varum and torsion, navicular drop, ankle dorsiflexion range of motion (DF ROM). Independent t-test, logistic regression and receiver operating characteristic (ROC) curve were used for statistical analysis. Results: Men with a history of LAS had significantly smaller Q angles both in standing and in supine position, while women with a history of LAS had significantly greater DF ROM in non-weight bearing (NWB; p < 0.05). Logistic regression model suggests tibial varum (OR = 0.779, p = 0.021) and WB DF ROM (OR = 1.067, p = 0.045) were associated with LAS in men. In case of women, there were no significant SLEA factors for LAS, however, ROC curve analysis revealed standing RF angle (AUC = 0.647, p = 0.028) and NWB DF ROM (AUC = 0.648, p = 0.026) could be affecting factors for LAS. Conclusion: There are differences in SLEA according to the history of LAS, furthermore, the identified items were different by sex. In case of men, tibial varum and WB DF ROM affect LAS occurrence. Standing RF angle and NWB DF ROM of women could be a predictor for LAS. However, since the sensitivity and specificity in most of the SLEA measurements are low, kinematic in dynamic tasks should be considered together for a more accurate evaluation of LAS risk.

  • 혈소판의 과도한 활성화 및 응집은 심혈관계 질환의 주요원인이 된다. 따라서, 혈소판 활성화 및 응집을 억제하는 것은 심혈관계 질환을 예방하고 치료하는데 있어서 매력적인 치료 표적으로 여겨진다. 특히, 혈관 내피세포에서 분비되는 collagen에 의한 강력한 혈소판 활성화와 응집이 혈관질환에서 특징적이다. Artesunate는 Artemisia 또는 Scopolia 속 식물의 뿌리에서 추출한 화합물이며, 항암 및 알츠하이머 병 분야에서 효과가 있다고 보고된 바 있다. 그러나, artesunate가 collagen 유도의 혈소판 활성화와 응집에 미치는 영향과 그 기전에 대해서는 규명된 바가 없다. 본 연구에서는 collagen이 유도하는 사람 혈소판 응집에 있어 artesunate이 미치는 영향을 확인하였고, artesunate의 작용 기전을 명확히 하였다. Artesunate은 혈소판가 활성화 될 때 신호 전달 과정에 작용한다고 알려진 인단백질인 PI3K/Akt 및 MAPK의 인산화를 억제하였다. 또한, artesunate는 TXA2 생성을 감소시켰고, ATP 및 serotonin 방출 등의 혈소판 내 과립 분비를 감소시켰다. 그 결과, artesunate는 혈관 내피세포에서 분비되는 강력한 응집 유도 물질인 collagen으로 유도된 혈소판 응집을 106.41 μM의 IC50로 강력하게 억제하였다. 이 결과들을 통해, artesunate가 혈관 손상을 통해 일어나는 사람 혈소판의 활성화 및 응집을 억제하는 데 유효한 항혈전 물질로 가치가 있음을 제안한다.

  • 양평(楊坪) 산간계곡(山間溪谷)의 수도(水稻) 및 산야초(山野草)의 피해(被害)는 현지(現地)의 피해상황(被害狀況), 피해증상(被害症狀)과 식물체중(植物體中)의 불소함량(弗素含量) 및 무기성분량(無機成分量)의 변화(變化)를 조사(調査)한 결과(結果)는 다음과 같다. 1) 수도(水稻)의 엽피해정도(葉被害程度)는 100m 지점(地點)까지는 95%, 500m 지점(地點)에서는 65%, 2㎞ 지점(地點)에서는 5%정도(程度)였다. 2) 피해증상특미(被害症狀特微) ${\cdot}$ 수도(水滔) : 심(甚)한 것은 백색(白色)으로 고사(枯死)하고 경미(輕微)한 것은 엽선단부위(葉先端部位)에 백색(白色)이나 적갈색(赤褐色)이 발견(發見) ${\cdot}$ 잡초(雜草) : 심(甚)한 것은 적갈색(赤褐色)으로 고사(枯死)하고 경미(輕微)한 것은 엽선단(葉先端)이 적갈색(赤褐色)으로 고사(枯死) 3) 식물체중(植物體中) 불소함량(弗素含量) ${\cdot}$ 수도(水稻) : 엽중(葉中) 불소함량(弗素含量)은 $170{\sim}3,225ppm$이고 근(根)에서는 $68.8{\sim}3,000ppm$이었으나 엽선단(葉先端)의 피해부위(被害部位)가 엽중심부(葉中心部) 보다 불소함량(弗素含量)이 $3{\sim}20$배(倍)많았다. ${\cdot}$ 잡초(雜草) : 무피해엽중(無被害葉中) 불소함량(弗素含量)은 $10{\sim}15ppm$이고 피해엽(被害葉)은 $130{\sim}242.5ppm$이였고, 엽선단(葉先端)의 피해부위(被害部位)는 중심부보다 $1.2{\sim}7$배정도(倍程度) 많았다. 4) 피해엽(被害葉)의 선단부위(先端部位)에 CaO, $K_2O,\;SiO_2$ Fe, 및 Mn 등(等)이 축적(蓄積)되었다. 5) 피해(被害)가 가장 적은 식물(植物)은 댕댕이덩굴이며 피해(被害)가 심(甚)한 식물(植物)은 산딸기, 두릅나무 등(等)이었다. 이상(以上)의 조사결과(調査結果) 본(本) 피해(被害)는 피해지(被害地) 린근(隣近)에서 불화수소산(弗化水素酸)을 리용(利用)한 세멘트 경화제(硬化劑) 제조시(製造時) 발생(發生)한 불화수소(弗化水素)가스에 의(依)한 피해(被害)로 판단(判斷)되었다.

말초 혈액 단핵구의 TNF-$\alpha$와 IL-8 발현에서 내독소에 대한 내성 기전에 관한 연구 (Mechanisms of Lipopolysaccharide-induced Lipopolysaccharide Tolerance in the Expression of TNF-$\alpha$ and IL-8 in Peripheral Blood Monocytes)

  • 연구배경 : 그람음성 간균의 내독소에 의한 sepsis syndrome에서 단핵 식세포는 내독소에 의해 자극받아 여러 종류의 cytokine을 분비함으로써 개체를 방어하는데 있어 매우 중요한 역할을 한다. 그러나 불행히도 cytokine들은 개체를 방어하는 작용 뿐 아니라 TNF등의 경우처럼 심각한 조직손상을 가져오는 효과도 있다. 그러므로 생체 내에서 내독소에 반응하여 cytokine의 분비를 조절하는 기능은 매우 중요하다. 이전의 연구에서 미리 내독소에 노출된 단색 식세포는 내독소로 재자극 시 cytokine 생성능의 저하가 관찰되었는데 이러한 현상을 '내독소 내성'이라고 하며 cytokine 분비조절에 중요한 역할을 하리라 생각되나 그 기전 등에 대해서는 연구가 부족한 상태이다. 방 법 : 생체 외에서 내독소에 대한 내성획득의 조건을 확립하고자 정상인의 말초혈액 단핵구를 10ng/ml의 저농도 내독소로 24시간 전처치한 후 2회 세척하고 다시 1 ng/ml의 내독소로 각각 4시간, 6시간, 24시간 자극하여 TNF-$\alpha$와 IL-8의 단백량을 측정하고 총 RNA를 분비하였다. 내독소 내성 획득 기전을 밝히고자 내독소 전처치 시에 자가혈청, PMA, antiCD14 Ab, Indomethacin, $PGF_2$를 각각 첨가하여 내성획득에 영향을 주는지 알아보았다. TNF-$\alpha$와 IL-8의 단백량은 ELISA를 이용하여 측정하였고 분리한 RNA를 이용하여 TNF-$\alpha$와 IL-8에 대한 Northern blot analysis를 시행하였다. 결 과 : 말초혈액을 저농도 내독소로 전처치하면 TNF-$\alpha$ 단백 생성 및 mRNA 발현을 억제하였으나 IL-8에 대해서는 이러한 현상을 관찰할 수 없었다. 전처치 시에 antiCD14 Ab를 내독소와 같이 준 경우 억제된 TNF-$\alpha$ 생성이 부분적으로 회복되었다. PMA만으로 전처치 하여도 저농도 내독소 전처치와 유사하게 내독소 내성을 유도할 수 있었다. 결 론 : 내독소에 의한 내성획득에는 CD14가 관여하고 Protein kinase C 경로를 통하며 pretranslational 수준에서 조절되는 것으로 생각된다.

간척지 토양특성에 알맞은 사료작물 작부체계 연구 (Study on Forage Cropping System Adapted to Soil Characteristics in Reclaimed Tidal Land)

  • 본 연구는 새만금간척지 광활 및 계화지구 신간척지에서 조사료의 안정생산을 위한 적정 작부체계를 설정코자 2009년 10월부터 2011년 10월까지 수행하였다. 동계 사료작물로 청보리 (영양), 호밀 (곡우), 이탈리안 라이그라스 (passerel plus)와 하계 사료작물로 옥수수 (광평옥), 수수${\times}$수단그라스 (G7)를 재배하여 토양화학성, 양분흡수량, 사료가치, 생육 및 수량성을 검토하였다. 시험전 토양은 유기물, 유효인산 및 치환성칼슘 함량이 매우 적었고 치환성마그네슘 나트륨 함량이 많은 알칼리성 염류토양 이었다. 재배기간 동안 토양염농도 변화는 0.2%이하를 나타냈고 염피해는 없었다. 동계 사료작물 입모율은 호밀 > 청보리 > 이탈리안 라이그라스 순으로 양호하였고 생초 및 건물수량은 이탈리안 라이그라스 > 호밀 > 청보리 순으로 높았다. 조단백 함량은 이탈리안 라이그라스 > 청보리 > 호밀 순으로 총가소화영양 함량은 청보리 > 이탈리안 라이그라스 > 호 밀 순으로 많은 경향을 나타냈다. 하계 사료작물 조단백질 및 중성용매불용성섬유, 산성용매불용성섬유 함량은 수수${\times}$수단그라스에서 총가소화영양 함량은 옥수수에서 높았다. 동 하계 사료작물 경엽 및 곡실의 무기양분 함량은 이탈리안 라이 그라스와 옥수수에서 높았다. 시험 후 토양화학성은 pH가 낮아지고 총질소 및 유효인산, 치환성칼륨.칼슘 함량이 증가한 반면에 유기물, 치환성마그네슘 나트륨 함량이 감소되는 경향을 나타냈다. 동작물과 하작물을 연계한 수량성 ('10~'11)에서 생초수량은 청보리 - 옥수수 (74,740 kg $ha^{-1}$) 대비 IRG - 수수${\times}$수단그라스 10%, IRG - 옥수수 7%, 호밀 - 수수${\times}$수단그라스 6%, 호밀 - 옥수수 및 청보리 - 수수${\times}$수단그라스 3% 증수되었고 건물수량은 청보리 - 옥수수 (20,280 kg $ha^{-1}$) 대비 IRG - 수수${\times}$수단그라스 7%, 호밀 - 수수${\times}$수단그라스 6%, IRG - 옥수수 3%, 청보리 - 수수${\times}$수단그라스 3%, 호밀 - 옥수수 3% 증수되었으며 TDN수량은 청보리 - 옥수수 (13,830 kg $ha^{-1}$) 대비 IRG - 수수${\times}$수단그라스 2%, 청보리 - 수수${\times}$수단그라스 및 IRG - 옥수수 1% 증수되었다. 따라서 생초, 건물수량 및 TDN수량이 높은 "IRG - 수수${\times}$수단그라스" 및 "청보리 - 수수${\times}$수단그라스" 조합을 신간척지 토양특성 (사양토, 배수약간불량, 토양염농도 0.2% 이하)에 알맞은 사료작물 작부조합으로 판단되었다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.
