Doubled Thrust by Boundary Layer Control in Scramjet Engines in Mach 4 and 6

  • Boundary layer ingestion in airframe-integrated scramjet engines causes engine stall (“engine un start” hereafter) and restricts engine performance. To improve the unstart characteristics in engines, boundary layer bleed and a two-staged injection of fuel were examined in Mach 4 and Mach 6 engine tests. A boundary layer bleed system consisting of a porous plate, an air coolers, a metering orifice and an ON/OFF valve, was designed for each of the engines. First, a method to determine bleed rate requirements was developed. Porous plates were designed to suck air out of the Mach 4 engine at a rate of 200 g/s and out of the Mach 6 engine at a rate of 30 g/s. Air coolers were then optimized based on the bleed airflow rates. The exhaust air temperature could be cooled below 600 K in the porous plates and the compact air coolers. The Mach 4 engine tests showed that a small bleed rate of 3% doubled the engine operating range and thrust. With the assistance of two-staged fuel injection of H2, the engine operating range was extended to Ф0.95 and the maximum thrust was tripled to 2560 N. The Mach 6 tests showed that a bleed of 30 g/s (0.6% of captured air in the engine) extended the start limit from Ф0.48 to Ф1 to deliver a maximum thrust of 2460 N.

Rapid Detection of Enterobacter sakazakii Using TaqMan Real-Time PCR Assay

  • Enterobacter sakazakii is an emerging food pathogen, which induces severe meningitis and sepsis in neonates and infants, with a high fatality rate. The disease is generally associated with the ingestion of contaminated infant formula. In this study, we describe the development of a real-time PCR protocol to identify E. sakazakii using a TaqMan probe, predicated on the nucleotide sequence data of the 168 rRNA gene obtained from a variety of pathogens. To detect E. sakazakii, four primer sets and one probe were designed. Five strains of E. sakazakii and 28 non-E. sakazakii bacterial strains were used in order to ensure the accuracy of detection. The PCR protocol successfully identified all of the E. sakazakii strains, whereas the 28 non-E. sakazakii strains were not detected by this method. The detection limits of this method for E. sakazakii cells and purified genomic DNA were 2.3 CFU/ assay and 100 fg/assay, respectively. These findings suggest that our newly developed TaqMan real-time PCR method should prove to be a rapid, sensitive, and quantitative method for the detection of E. sakazakii.

  • Foreign body ingestions pose a significant health risk in children. Neodymium magnets are high-powered, rare-earth magnets that is a serious issue in the pediatric population due to their strong magnetic force and high rate of complications. When multiple magnets are ingested, there is potential for morbidity and mortality, including gastrointestinal fistula formation, obstruction, bleeding, perforation, and death. Many cases require surgical intervention for removal of the magnets and management of subsequent complications. However, we report a case of multiple magnet ingestion in a 19-month-old child complicated by gastroduodenal fistula that was successfully treated by endoscopic removal and supportive care avoiding the need for surgical intervention. At two-week follow-up, the child was asymptomatic and upper gastrointestinal series obtained six months later demonstrated resolution of the fistula.

Epidemiological Investigation of an Outbreak of Salmonellosis in Gyeongju, Korea

  • Objectives: A salmonellosis outbreak occurred within a community of Gyeongju residents who ingested catered food from a wedding in June 2009. We aimed to epidemiologically investigate the probable vehicle of the infection. Methods: We conducted a retrospective cohort study on 34 local residents who ingested the wedding food. Results: Among the 34 residents, 31 (91.2%) reported symptoms of infection after eating the food. Among all of the wedding foods, pan-fried foods were highly associated with the diarrheal attack rate. On bacteriological examination, Salmonella species were detected in the pan-fried foods among the leftover foods and in 17 of the 31 stool specimens from the cases. There were five different types of pan-fried foods, but the onset of symptoms was independent of the ingredients used. We found that the pan-fried food was prepared at a food store in Seoul and that eggs were a common ingredient. Conclusions: The major cause of the salmonellosis in this population was presumed to be the pan-fried food prepared with contaminated eggs. These food items might have been partially undercooked because of their irregular shape, which allowed the Salmonella species to survive and multiply before ingestion.

Clinical Investigation of Patients with Acute Paraquat Poisoning and Case Reports Who Survived with Oriental Medical Treatment (Paraquat중독(中毒) 치험(治驗) 3례(例))

  • Paraquat has been widely used as a herbicide in Korea since 1970. Human ingestion results in transient impairment of liver and kidney function and a characteristically respiratory failure with a high mortality rate. Clinical investigation and Oriental Medical Treatment were done on 3 cases of paraquat poisoning admitted to the department of 1st Internal Medicine. Kyung San University Oriental Medical Hospital from August 1991 to December 1992. and following results were obtained. The symptoms of paraquat poisoning can be regarded as Sim hua chi song(心火熾盛), Kan hua sang yom(肝火上炎). um hu hua wang(陰虛火旺), Kan tam sup yol(肝膽濕熱), Yang myong yol tok(陽明熱毒) in Oriental Medicine. and the principle of treatment for paraquat poisoning can be applicable to Chong yol Ii sop(淸熱利濕), Chong yol hae tok(淸熱解毒). Sa ha hae tok(瀉下解毒). Ja um kang hua(滋陰降火) and so on. Three cases were treated by HuangYonHaeTokTang(黃蓮解毒湯) and KamDuTang(甘豆湯): all of them have survived without complications. According to the result above, it suggests that determination of treatment based on the differentiation of symptoms and signs(Pyon jung si chi ;辨證施治) of Oriental Medicine may prove to be helpful in saving lives of patients with paraquat poisoning.

  • Steven-Johnson syndrome (SJS) and toxic epidermal necrolysis(TEN) are severe mucocutaneous reaction which are most frequently caused by drugs. Although the incidence of SJS and TEN is known to be relatively low, outcomes may be fatal. A systematic approach is required because morbidity rate is currently increasing and oral lesion is frequent. We investigated the clinical features and outcomes of 6 patients diagnosed as SJS and TEN and referred from the department of dermatology, Chonnam National University Hospital for oral care. Ketoconazol, Ofloxacin, Chlorphenesin, Amoxicillin, Pontal, Harnal, and Ciprofloxacin were suspected as the causative drugs. Average treatment period was 3.2 weeks, and two patients were referred to 'burn-patients' hospital. Most of oral lesion were cured be normal tissue, but scares with discoloration were observed. For intraoral management, antibiotic disinfection and steroid application were performed according to systemic treatment principles. Additionally, ingestion of zinc, antioxidants, and vitamin was recommended. The establishment of oral treatment principles is demanded because it has not been yet. Also, through investigation of drug side effect and careful prescription are required.

Korean Red Ginseng as a Postoperative Immune Modulator in Patients with Advanced Gastric Cancer

  • In this paper, we present evidence that the red ginseng from Panax ginseng C.A. Meyer inhibits the recurrence of advanced gastric cancer and shows immunomodulatory activities during postoperative chemotherapy. Flow cytometric analyses for peripheral T-lymphocyte subsets showed that the red ginseng powder restored CD4 levels to the initial preoperative values during postoperative chemotherapy. Depression of CD3 during postoperative chemotherapy was also inhibited by the red ginseng powder ingestion. This study demonstrated a 5-year disease free survival and overall survival rate that was significantly higher in patients taking the red ginseng powder during postoperative chemotherapy vs. control $(68.2\%\;vs.\;33.3\%,\;76.4\%\;vs.\;38.5\%,$ respectively, p<0.05). The mean value of serum IL-10 of the ginseng group was reduced progressively during the postoperative chemotherapy. The values of the ginseng group were close to that of the control group on postoperative months 3. These studies suggest that the red ginseng may have some immunomodulatory properties associated with CD3 and CD4 activity and interleukin 10 during postoperative chemotherapy and some potential of improving prognosis in patients with advanced gastric cancer.

Study on the Changes in Distributions and Expressions of Aquaporin5 (AQP5) in Salivary Glands of Mice After Alcohol Ingestion

  • Alcohol intake is known to affect various organs in the human body, causing reduction of salivation in the oral cavity. Hypo-salivation effect of alcohol is a common feature, but the mechanism in salivary glands is still poorly studied. Therefore, in this study, the changes in salivary secretion and water channel protein (aquaporin5, AQP5) in salivary glands of mice were investigated after ethanol administration. Animals were divided in to 4 groups with the control, 4 g/kg ethanol, 8 g/kg ethanol and 16 g/kg ethanol administration groups. One hour after ethanol administration, saliva was collected from the oral cavity, and the animals were killed and parotid and submandibular glands were extracted to analyze the histopathology, AQP5 immunihistochemistry and AQP5 protein level. According to the results, the salivation rate decreased irrespective of the ethanol dose in mice, and viscosities increased with increase in ethanol dose. However, there were no pathological changes in parotid and submandibular glands due to ethanol administration. Expression of AQP5 in parotid and submandibular glands decreased with increase ethanol administration These results indicate that the reduction of salivary secretion due to acute alcohol intake is closely related to decrease of the water channel protein such as AQP5 in parotid glands and submandibular glands, rather than the damage of salivary glands.

Co-treatment of Sewage Sludge and Cow Manure by Vermistabilization (Vermistabilization에 의한 하수 슬러지와 가축분뇨의 병합처리)

  • The objective of this study was to investigate the effect of cow manure as additive material on the ripening of sewage s sludge for vermistabiJization. The changes of the waste properties by ripening for 50days were observed as a function of the v various mixture ratios of sewage sludge ‘and cow manure. The pH values of the mixture wastes decreased from 7.5-7.67 to 6_ 9~7.2 by the ripening for 50days, and the mixture ratio made dIfferent pH values. The initial value of oxidation-reduction p potential (Eh) of the mixture waste was a negative (-) value indicating an unfavorable condition for earthworm after, but the v values of Eh increased with the opening time. The Increase rate at Eh value was prop$\alpha$rtional to the mixture ratio of cow m manure. The value of alkalinity was also changed into the favorable range for earthworm after 50days except for non-use of 1 the cow manure When the mixture ratio of the cow manure increased from 10% to 30%, the growth of earthworms increased h from 63.7% to 88.3 % tor the survival rate, 265% to 321% for the liveweight increasing rate and 66.7_7% to 91% for hatching f rate of the cocoons. It can be concluded that the proper content of tbe cow manure in the sewage sludge to ensure effective v vermistabilization was over 20%, when the mixture was ripened during 50 days. The quantity of ingestion and 며ectian at 20%-30% was found to be O.15--i.L18g sludge and 0 1l--O.14g solid per capacity earthworm per day, respectively.

General Characteristics for Poisoning-Induced Transient or Sustained Hyperammonemia (급성 약물중독에 합병된 일과성 혹은 지속성 고암모니아혈증의 특성)

  • Purpose: In patients with altered mentality caused by drugs or unknown causes, ammonia is checked to facilitate differential diagnosis or diagnose hepatic coma. This helps early prevention and treatment of brain damage due to hyperammonemia. This study was conducted to evaluate clinical characteristics of intoxicated adult patients with hyperammonemia. Methods: We evaluated 95 patients with hyperammonemia among intoxicated patients above the age of 15 who visited our ED from January 2013 to December 2015. We analyzed the demographic characteristics and type of poisoning substance, reason for ingestion, toxicological characteristics such as elapsed time from ingestion to hospital visit, lab, clinical progression and complications. Data were evaluated using the student's t test or Mann-Whitney U test for continuous variables, and Chi-square test and Fisher's exact test for frequency analysis of categorical variables. Results: When compared to healthy individuals, patients with hyperammonemia showed statistical significance on their SOFA score (p=0.016) and poison severity score (p<0.001). Additionally, patients with hyperammonemia showed significantly different initial serum AST level (p=0.012) and maximum serum AST level during the hospital stay (p=0.026) when compared to healthy individuals. Moreover, individuals with sustained hyperammonemia compared to transient hyperammonemia showed clinically significant SOFA scores (p<0.001), poison severity scores (p=0.007), mortality rates in the ICU (p=0.021), as well as different duration of hospital stay (p=0.037), serum creatinine level (p=0.002), erythrocyte sedimentation rate (p=0.025), and serum myoglobin (p=0.015). Conclusion: Most poisoning-induced hyperammonemia cases were transient and recovered without special treatment. Therefore, hyperammonemia is almost non-specific among poisoning patients.
