Effects of Isoflurane/Remifentanil and Isoflurane/Fentanyl Anesthesia in Beagle Dogs (비글견에서 아이소플루란/레미펜타닐 및 아이소플루란/펜타닐 조합의 마취 효과)

  • This study was performed to compare two opioid drugs with isoflurane and to determine the difference between isoflurane/remifentanil anesthesia and isoflurane/fentanyl anesthesia in terms of the anesthetic effects in beagle dogs. Isoflurane was maintained at 0.5 MAC, and the opioid drug was administered as a constant rate infusion. The anesthesia was maintained for 2 hours, and isoflurane and opioid drugs were discontinued 2 hours later. After discontinuing the anesthetics, the extremity movement time, eye global positioning time, gag reflex time, head up time, sternal recumbency time, standing time, walking time and complete recovery times were recorded for each dog. Both of the studied anesthetic protocols were suitable in beagle dogs because the anesthetic status was well maintained until the end of the procedure, and rapid recovery times were demonstrated in this experiment. And this study shows that the isoflurane/remifentanil group was more reliable than the isoflurane/fentanyl group because the recovery time CV was lower. Therefore, isoflurane/remifentanil combination anesthesia could be a better choice than isoflurane/fentanyl anesthesia if the patient is severely ill and stable recovery time is needed.

Circadian variation of IV PCA use in patients after orthognathic surgery - a retrospective comparative study

  • Background: An understanding of the features of postoperative pain is essential for optimal analgesic dosing strategies. Using a visual analogue scale (VAS) score and patient controlled analgesia (PCA) infusion pattern analysis, an anesthesiologist can estimate when and how severely patients suffer from pain. Several reports have been published about circadian changes in the pain threshold. Postoperative pain was analyzed retrospectively in 250 patients who underwent orthognathic surgery. Methods: A total of 250 patients were allocated into two groups according to the time of recovery from anesthesia. Patients in the early group (group E) recovered from anesthesia before 06:00 p.m. Patients in the late group (group L) recovered from anesthesia after 06:00 p.m. All patients received intravenous patient controlled analgesia (IV PCA) at the end of the operation. The VAS score of pain intensity was measured. Self-administration of bolus analgesic from the IV PCA device was also analyzed according to actual time and elapsed time. Results: VAS scores showed no difference between the two groups except 36 hours after recovery from anesthesia. On POD1, there were two peaks for self-administration of bolus analgesics in group L and one peak in the morning for group E. Two peaks each in the morning and in the afternoon were shown in both groups on POD2. Conclusions: Diurnal variance in pain should be considered for effective dosing strategies.

  • Infiltration is one of detrimental problems occurring in nursing or medical settings. Early detection of infiltration is essential to minimize the risk of injury from infiltration. To perform a preliminary study on the point of care and automated infiltration detection system, bioelectrical impedance was investigated using bioelectrical impedance analyzer. We would like to report experimental results that allow impedance parameters to effectively distinguish infiltration. Electrodes were attached to both sides of the transparent dressing on the fusion site where IV solution was being infused. Then, impedance parameters before and after infiltration were measured as a function of time and frequency. The experimental results are as follows. After infiltration was intentionally induced by puncturing the vein wall with a needle, the resistance gradually decreased with time. That is, when an alternating current having a frequency of 20 kHz was applied to the electrodes, the resistance gradually decreased with time, reflecting the accumulation of IV solution in the extracellular fluid since the current could not pass through the cell membrane. Impedance parameters and equivalent circuit model for human cell were used to examine the mechanism of current flow before and after infiltration, which could be used for early detection of infiltration.

Decreased Diethylnitrosamine-induced Liver Preneoplastic Lesions by Estradiol-3-benzoate Treatment

  • To clarify whether inhibitory effect of estrogen on liver tumor is associated with cell proliferation, we investigated its role in diethylnitrosamine (DEN)-induced rat preneoplastic lesions, with time sequenced manners. F344 male rats (n = 90) were divided into three groups at 5 weeks of age. The mini-osmotic pumps providing a continuous infusion of DEN was implanted into the abdominal cavity of each animal in group 1, 2 and 3 at 6 weeks of age. To see the effect of estrogen, pellet containing 1 or 10 ${\mu}g$ of estradiol-3-benzoate (EB) was implanted subcutaneously in the animals of groups 2 or 3, respectively, one week prior to DEN treatment. Ten animals of each group were euthanized at 10, 14 and 18 weeks after DEN treatment. Liver tissues at each time point were fixed in 10% phosphate-buffered formalin and were processed and embedded in paraffin and 5 ${\mu}g$ sections mounted on a silanized slide. Glutathione S-transferase placental form (GST-P) positive foci and 5-bromo-2-deoxyuridine (BrdU) labeling cells were detected at each time point. Area of GST-P positive foci in DEN+EB 1 or 10 ${\mu}g$ group was significantly decreased compared to DEN alone at 14 weeks (p < 0.01 or p < 0.05, respectively) an at 18 weeks (p < 0.05 or p < 0.01, respectively). BrdU index in DEN+EB 1 or 10 ${\mu}g$ groups was significantly decreased compared to DEN alone at 14 weeks and at 18 weeks (p < 0.01). Taken together, we conclude that EB treatment decrease the DEN-induced liver preneoplastic lesions and this may be associated with decrease of cellular proliferation.

The Development of Phlebitis in Relation to 18 Gauge Peripheral Intravenous Catheter Dwell Time among Surgical Patients (수술환자 18 Gauze 말초 정맥관 유지시간과 정맥염 발생률)

  • Purpose: The purpose of this study was to evaluate the appropriateness of the replacement time intervals of 18 gauge peripheral intravenous catheters (PICs) by investigating the development of phlebitis. Methods: The subjects were 200 hospitalized patients over 18 yrs old aged who have 18 gauge PICs placed for surgery. After the insertion of PICs, the researcher monitored the insertion site daily for 96 hours for any signs of phlebitis. Results: Phlebitis developed in 25.7% of patients. Patients who developed phlebitis were significantly older and were receiving fluids with faster infusion rate. However, patients with and without phlebitis were not different by gender, insertion site, fluid osmolality, or pH of drugs administered. The incidence rate of phlebitis was higher than 10%(12.9%) starting 24~48 hours after the insertion of 18 gauge PICs. Conclusion: It is recommended to replace 18 gauge PICs within 24~48 hours after insertion. Close monitoring of the PICs insertion site for the signs of phlebitis is recommended.

  • In this study was explored minimize side effects due to the additional injection of contrast medium and maintaining a high resolution imaging applied to the inspection and analysis of the contrast medium that affect the peak time biomechanics factors. Included 48 patients using the test bolus method, after measuring a patient's biomechanics factors of inspection before and during the test, correlation between contrast medium peak time and learn, matches the regression equation calculated and measured contrast medium peak time was assessed by the Bland Altman plot. Research result, inspections of SBP, HR contrast medium peak time and a significant negative correlation was, step 1, every increase, the contrast medium peak time significantly to -0.018 and -0.159 decreased, a fairly high concordance no difference between the two method. In conclusion, the regression equation using the existing methods, while maintaining excellent image quality that contrast medium is reduced to a patient, it can conclude that the alternative to the existing methods.

  • Respiration is one of the most important functions which are carried out in stomatognathic system. When nasal orifice is obstructed or the resistance of upper airway is increased mouth breathing is initiated. Mouth breathing is regarded as an important etiologic factor of dentofacial anomalies. This experiment was performed to observe the influences of metabolic acidosis, tracheal resistance and vagotomy on mouth breathing. After rabbits were anesthetized with sodium pentobarbital, a pair of wire electrode was inserted into mylohyoid muscle, anterior belly of digastric muscle and dilator naris muscle to record EMG activity. Femoral vein and artery were cannulated for infusion of 0.3N HCl and collection of blood sample to determine the blood pH, and tracheal intubation was done to control airway resistance. Mouth breathing was induced by metabolic acidosis. Increase of the airway resistance through tracheal cannula intensified the activity of dilator naris, mylohyoid and digastric muscle. The higher the resistance, the larger the EMG amplitude. After bilateral vagotomy, respiratory volume and inspiatory time were increased and the activities of dilator naris, mylohyoid and digastric muscle were strengthened. It was concluded that the muscle activity related to mouth breathing was induced by metabolic acidosis and increase of tracheal tube resistance.

  • A non-intrusive Planar Laser-Induced Fluorescence(PLIF) technique was applied to study the turbulent mixing process in a Rushton turbine reactor. Instantaneous and ensemble averaged concentration fields are obtained by measuring the fluorescence intensity of Rhodamine B tracer excited by a thin Nd:Yag laser sheet illuminating the whole center plane of the stirred tank. The gray level images captured by a 14-bit cooled CCD camera can be transformed to the local concentration values using a calibration matrix. The dye injection point was selected at the tank wall with three quarter height (3/4H) from the tank bottom to observe the mixing characteristics in upper bulk flow region. There exist distinct two time scales: the rapid decay of mean concentration in each region after the dye infusion reflects the large scale mixing while the followed slow decay reveals the small scale mixing. The temporal change of concentration probability functions conjectures the two sequential processes in the batch type mixing. An inactive column of water existed above the impeller disk, in which the fluid rotates with the shaft but is isolated from the mean bulk flow.

  • This study was to determine the procedure for ultrasonographic imaging of the canine external ear canal(ECC). By infusion of warm saline solution into the ear canal, a real-time B mode ultrasonographic examination of the ECC was performed in five healthy Beagle dogs and two dogs with severe otitis externa. The ultrasonography was very useful to detect the structures of the ECC and the tympanic membrane. An 11 NHz linear probe was permitted to assess an anechoic lumen of the ECC and a hyperechoic cartilage surrounding the ear canal. In two dogs with otitis externa, it could be assessed the ECC with ultrasonography without ear cleansing. A lumen of the ECC was irregular and a cerumen could be seen, but edematous inflammatory changes of the ECC could not be identified. The results suggest that ultrasonography of the ECC with an 11 NHz linear probe after infusing the saline solution into the ear canal may be an accurate, non-invasive, rapid, and widely available method for assessment of the ear canal diseases such as otitis externa, polyp, stenosis, hyperplasia, and foreign body in dogs.
