단국대학교 치과대학 부속치과병원에서 시행된 정주진정에 대한 연구 (A Study of Intravenous Sedation in Dankook University Dental Hospital)

  • Background: Intravenous sedation is effective for dental patients who are anxious. Recently, target-controlled infusion (TCI) has begun to be used widely to administer and titrate propofol and remifentanil during sedation. To investigate the effect and safety of the pharmacologic agents used in anesthetic department, we performed a retrospective study. Methods: Retrospective study of a series of dental procedure under intravenous sedation performed in department of anesthesiology in Dental Hospital of Dankook University was carried out with propofol or propofol/remifentanil between January and August 2011 and January and April 2012. All patients received oxygen by nasal cannula. The average propofol and remifentanil target was 0.5 ${\mu}g/ml$ and 1.0 ng/ml, respectively using a TCI pump. The average peripheral oxygen saturation ($SpO_2$), heart rate, blood pressure, respiratory rate, nasal end-tidal $CO_2$ were recorded at 5-10 minute intervals. The age, gender, weight, procedure and sedation time, type of procedure were also recorded. Results: We included 22 cases of 19 adults (group A) and 6 cases of children (group B). In group A, 4 patients received propofol (group A-P), and 15 patients received propofol with remifentanil (group A-PR). In group B, 6 patients received propofol only. The mean age of group A was 41.1 years old and that of group B was 9.5 years old. No clinically significant complications were noted. There were no case of de-saturation <90%. The median respiratory rate was 13.1 (range 6 to 36) in group A and 19 (range 13 to 25) in group B. The median end tidal $CO_2$ was 36.7 mmHg(range 8 mmHg to 56 mmHg) in group A and 41.7 mmHg (range 30 mmHg to 53 mmHg) in group B. Conclusions: Based on our results, dental sedation using propofol/remifentanil in adult and propofol in children with TCI pump seems to appear as a safe and effective procedure while performing dental procedure.

구강건조증 환자의 타액선 스캔의 진단학적 가치에 관한 연구 (The Study on the Diagnostic Value of Salivary Gland Scintigraphy in Patients with Xerostomia)

  • The present study was performed to investigate the relationship between the salivary flow rate and the interpretation results of salivary scan in the patients with dry mouth. Twenty-five patients with dry mouth who visited the Dept. of Oral Medicine & Oral Diagnosis, Seoul National University Dental Hospital, were included. The unstimulated whole salivary flow rate was determined by the spitting method and the stimulated whole salivary flow rate was measured with gum-base chewing. Salivary scan was performed after the infusion of $^{99m}technetium$ pertechnetate(Tc) and interpreted. The obtained results were as follows: 1. The unstimulated and stimulated whole salivary flow rate were significantly decreased compared to normal value, which reflected the extensive destruction of salivary gland function in the patients with dry mouth. 2. The unstimulated and stimulated whole salivary flow rate were decreased in the group with decreased function in salivary scan compared with the group with normal function in salivary scan. However, there was no statistical significance between groups. 3. The difference between the stimulated and unstimulated whole salivary flow rates was greater in the group with normal function in salivary scan compared with the group with decreased function in salivary scan. 4. There was significant positive correlation between the stimulated and unstimulated whole salivary flow rates. The level of correlation was higher in the group with decreased function in salivary scan than the group with normal function in salivary scan. Collectively, these data suggested that salivary scan had the limited value. The comprehensive evaluation including history taking, clinical examination, clinical laboratory as well as the measurement of salivary flow rate are need for patients with dry mouth.

면양에서 생리적 분비형태의 테스토스테론이 황체형성호르몬의 분비 억제에 미치는 효과 (The inhibitory effect of physiological pattern of testosterone on luteinizing hormone secretion in sheep)

  • 본 연구의 목적은 거세한 숫양에서 테스토스테론의 생리적인 주기적 분비형태가 황체형성호르몬의 분비억제에 미치는 효과를 연구하는 데 있다. 이를 위해, 3개의 서로 다른 실험들을 수행하였다. 실험 1에서는 정상적인 숫양에서 관찰되는 테스토스테론의 분비방식과 동일한 분비형태를 인위적으로 만들기 위해 필요한 테스토스테론의 주입비율과 분비형태를 얻기 위하여 3가지 다른 양(25, 50 그리고 $100{\mu}g$)의 테스토스테론을 정맥에 주입시켜 테스토스테론의 약리동력학을 분석한 결과, 테스토스테론의 평균 제거율상수, 분포용적, 그리고 총 체청소율은 각각 $0.18min^{-1}$, 0.531/kg, 그리고 0.091/min/kg 이었다. 실험 2에서는 테스토스테론의 처리 시간의 경과에 따른 테스토스테론의 황체형성호르몬 평균 농도의 억제 효과를 조사하기 위하여 3가지 다른 분비율(192, 384 그리고 $768{\mu}g/kg/24h$)의 테스토스테론을 주기적으로(4시간 간격) 3일 동안 정맥에 주입시킨 결과, 테스토스테론의 처리 시간이 증가함에 따라 혈액 내 황체형성호르몬의 평균 농도는 서서히 감소하였다. 테스토스테론을 2일 또는 3일간 처리하였을 때는 테스토스테론을 처리하기 전에 비해 황체형성호르몬의 평균 농도는 현저히 감소하였다. 그러나 테스토스테론의 1일간 처리는 황체형성호르몬의 평균 농도를 감소시키지 못하였다. 실험 3에서는 두 가지 다른 분비 형태(지속적 분비 형태와 주기적 분비 형태)의 테스토스테론의 황체형성호르몬의 분비에 미치는 효과를 비교 조사하였다. 지속적 분비형태를 만들기 위하여 테스토스테론을 3일간 지속적으로($32{\mu}g/kg/h$) 정맥 주입시켰고, 주기적 분비 형태를 만들기 위하여 테스토스테론을 4시간 간격으로 3일간 주기적으로($128{\mu}g/kg/h$) 정맥 주입시켰다. 지속적 방법과 주기적 방법 간에 동일한 양($768{\mu}g/kg/h$)의 테스토스테론이 주입되었다. 혈액은 테스토스테론의 정맥 주입전 4시간 동안과 3일간 정맥 주입 기간 중 마지막 4시간 동안 각각 10분 간격으로 경정맥에서 채취하였고, 황체형성호르몬과 주입된 테스토스테론의 혈액 내 농도는 각각의 방사성면역방법을 이용하여 측정하였다. 황체형성호르몬의 펄스 간격(p<0.034)과 황체형성호르몬의 평균 분비량은(p<0.045) 주지적 방법 보다 지속적 방법의 테스토스테론의 주입에 의해 현저히 증가하였다. 황체형성호르몬의 펄스 분비량은 주기적 방법과 지속적 방법 간에 차이가 없었다. 이상의 결과들은 숫양에서 지속적 방법의 테스토스테론이 주기적 방법의 테스토스테론의 보다 황체형성호르몬의 분비를 저하시키는데 보다 더 효과적임을 나타내 보이고 있다.

가와사끼병에서 감마글로불린 재투여가 필요했던 경우의 임상적 특징 (Clinical Characteristics in Patients with Kawasaki Disease Who Received Intravenous Gamma-globulin Retreatment)

  • 목 적 : 저자들은 IVGG의 재투여가 필요했던 환아들의 검사소견 및 임상적 특징을 알아보고, 재투여가 필요 없었던 환아들과 비교하여 재투여가 필요한 환아를 예측할 수 있는지 알아보고자 하였다. 방 법: 1997년 10월부터 2001년 2월까지 적어도 6개월 이상의 추적관찰이 가능하였던 117명의 환자를 대상으로 통상적인 혈액검사, 심전도, 신호평준화 심전도, 심초음파도를 후향적으로 검토하였다. IVGG 한번 투여 후 recurrent fever 혹은 recrudescence fever를 보이는 6명(5.1%)에게는 IVGG를 한번 더 투여하였으며, 그래도 발열이 지속되는 경우는 methylprednisolone를 한번 내지 두번을 pulse로 주었다. 결 과: 전체 117명의 환아 중 23명(19.7%)에서 6개월 이상의 추적 기간중 심장의 합병증이 생겼으며, 초기 심장의 합병증이 23명에서, 후기 합병증은 4명에서 관찰되었다. IVGG의 재투여가 필요했던 환자는 6명(5.1%)이었으며, 초기의 심장합병증은 IVGG을 재투여한 군에서 1회 투여한 군에 비하여 높으며(P<0.0001), 장기적인 심장 합병증도 재투여군이 높았다(P<0.0001). IVGG 재투여군 중 m-PD까지 사용했던 군도 초기 합병증 및 후기 합병증이 1회 투여군에 비하여 현저히 높았다. 전체적인 합병증 유무에 따른 여러 가지 검사 소견 중 CRP만이 합병증이 있는 군에서 유의하게 증가되어 있었으며($11.94{\pm}7.43$ vs $6.97{\pm}6.41$ P<0.01), IVGG 재투여군이 1회 투여군보다 CRP가 현저히 증가되어 있었다($15.68{\pm}8.67$ vs $7.44{\pm}6.50$ P<0.05). 결 론: 초기 검사에서 CRP가 현저히 높은 군이 합병증의 발생빈도도 높고 IVGG을 재 투여할 가능성도 높다. 그러나, IVGG의 재투여가 관상동맥 합병증의 발생을 감소시키지는 못하였다. 이는 IVGG의 재투여가 필요했던 군에서는 CRP의 결과에서 보듯이 많은 양의 IVGG가 필요할 정도로 심한 염증반응이 있었거나 IVGG 자체로는 효과적으로 심장 후유증을 예방할 수 없기 때문이라고 생각된다.

개에서 발생한 전신성 낙엽상 천포창에 사람 면역글로불린의 임상적 적용 (Clinical Trial of Human Intravenous Immunoglobulin in a Dog with Generalized Pemphigus Foliaceus)

  • An American Cocker Spaniel (3-year-old, intact female, 6.0 kg) was referred to the Veterinary Medical Teaching Hospital of Chungnam National University for evaluation of pustules and crusts in the periocular region, dorsal and ventral region of the trunk, and digits. Complete blood count (CBC) revealed leukocytosis with mature neutrophilia, and a serum biochemistry profile revealed hypoalbuminemia. Tape strip tests identified numerous neutrophils and acatholytic cells. Histopathology identified intraepithelial pustules with neutrophils and acantholytic keratinocytes. Definitive diagnosis of pemphigus foliaceus (PF) was made by direct immunofluorescence (DIF) test with goat anti-canine IgG antibody. The human intravenous immunoglobulin (IVIG) was administered at a rate of 15 ml/h over 6 hours for 4 days. After that, the dog was maintained on prednisolone (2.2 mg/kg, PO, SID) and azathioprine (2.0 m/kg, PO, SID). An infusion of IVIG (0.5 g/kg) was repeated 3 days after 4 weeks. After 10 weeks, the dog showed the remarkable regression of lesions.

An Acute Postoperative Intractable Hyperventilation after an Endoscopic Third Ventriculostomy

  • This report describes a rare case of postoperative hyperventilation attack after an endoscopic third ventriculostomy in a 46-year-old woman. About 60 min after the termination of the operation, an intractable hyperventilation started with respiratory rate of 65 breaths/min and $EtCO_2$, 16.3 mm Hg. Sedation with benzodiazepine, thiopental sodium, fentanyl, and propofol/remifentanil infusion was tried under a rebreathing mask at a 4 L/min of oxygen. With aggressive sedative challenges, ventilation pattern was gradually returned to normal during the 22 hrs of time after the surgery. A central neurogenic hyperventilation was suspected due to the stimulating central respiratory center by cold acidic irrigation solution during the neuroendoscopic procedure.

Diltiazem의 신장작용에 대한 신신경제거의 영향 (Effect of Renal Denervation on Renal Action of Diltiazem in Dog)

  • This study was performed to elucidate the mechanism of antidiuretic action of diltiazem by infusion into the vein and carotid artery, of diuretic action into a renal artery in dog. Renal denervation caused a reversal of the effect of diltiazem from the antidiuretic to the diuretic when infused into vein or carotid artery, and potentiated the diuretic effect when infused into a renal artery. The changes of renal function in diuretic circumstances as described above included the increase in renal plasma flow, osmolar clearance, the amounts of sodium and potassium excreted in urine and the decrease in reabosrption rate of sodium and potassium in renal tubules. Above results suggest that antidiuretic action of diltiazem may be mediated by central nervous system, not by endogenous substance, diuretic action by direct renal action.

  • Recently, evaluation of brain transport of taurine which is possible to effect on Alzheimer's disease has investigated in rats. Also, internal carotid artery perfusion (ICAP) method is very useful for measuring of blood-brain barrier (BBB) permeability in rats. But ICAP has difficulties to evaluate of BBB permeability in mice especially. In the present study examines neuropharmaceutials permeability through the BBB in mice by common carotid artery perfusion (CCAP) method that modify ICAP method and require simple surgery. The external carotid artery (ECA) is cannulated with coagulating pterygopalatine artery (PPA) on ICAP method, while CCA is cannulated without coagulating PPA on CCAP method. The CCAP method require 4-5 fold higher infusion rate than ICAP method because an additional factor of 2 must be incorporated to adjust for fluid loss to the extracerebral circulation.

DelNido 심정지액의 심근보호효과 (Evaluation of Cardioprotective Effects of DelNido Cardioplegia)

  • Background: The aim of this study is to define the cardioprotective effects(functional and metabolic) of newly developed DelNido cardioplegic solution(containing plasma solution, mannitol, magnesium and lidocaine). Material and Method: This study assessed the function of rat hearts after itermittent infusion of DelNido cardioplegia with different preserving methods(Air or Icebox) for 2hours and perfusing the hearts on a Langendorff apparatus. Heart rate, left ventricular developed pressure(LVDP) and coronary flow, were measured at pre-ischemic, post-reperfusion 15min, 30min and 45min. Coronary flow was standardized to dry heart weight. Each weight was weighted to calculate water content. Creatine kinase-MB isoenzyme release was measured and ultrastructural assessment was done with electron microscopes. Result: DelNido group was better than St, Thomas group and Icebox group was better than Room-air group. Conclusion: DelNido cardioplegia have better myocardial protective effects than St. Thomas cardioplegia when they were preserved in the Room-air. But we can not tell the difference between Delnido cardiplegia with Air preserving method and St. Thomas cardioplegia with Icebox.

Sulphamethomidine의 젖소에 있어서의 유선과 신장을 통한 배출 (Mammary and renal excretion of sulphamethomidine in cows)

  • The mammary excretion of suphamethomidine after intravenous and/or oral administration was investigated in cow. The results show that sulphamethomidine is bound to plasma proteins to a great extent (80~90%). Ay a dosage of 60 mg./kg. maximal concenration in plasma of this sulphonamide was reached 7-10 hours after oral dosing. The sulphonamide concentration in plasma slowly declined after both oral and intravenous administration (fig. 1, 2, and 3) The concentration of sulphonamide in milk was very low and the excretion was completed in 7 days after a single oral dose and 5 days after intravenous injection while in the case of blood plasma it was 11 and 7 days, respectively. In addition, the renal excretion of sulphamethomidine was investigated while under continuous intravenous intravenous infusion. The excretion ratios varies according to self depression (table. 1). Blockade of the tubular secretion with diodone lowered the excretion of sulphamethomidine. It is concluded that the renal excretion of sulphamethomidine in cows occurs by filtration by slight tubular secretion and also by a high rate of back diffusion.
