The morphologic changes of small intestinal epithelium in pigs diagnosed as porcine epidemic diarrhea(PED} by virus isolation and immunohistochemistry were studied through light microscope and transmissible electron microscope. On semi-thin section, the histologic findings showed severe villous atrophy and fusion with hyperplasia of cuboidal epithelium in the villi, inflammatory cell infiltration in lamina propria, and increased mitotic figures in the crypt. The structural changes were mostly restricted to the cytoplasm of affected absorptive epithelium of villi. 3 types of epithelial changes were found; degenerated virus-affected cells, undifferentiated cuboidal cells, and normal columnar cells. On electron microscopy, round to spherical viral particles of 50∼l00nm in diameter were found within the dilated vesicles and endoplasmic reticulums of degenerated cells, which had decreased their cytoplasmic electron density due to dilated and missing organelles(e.g. mitochondria, ERs, etc.). Microvilli were shortened and sparse, leaving denuded terminal web of the villous epithelial cells. Fat globules were often found within slightly degenerated enterocytes. On the tip of villi, severely damaged cells were exfoliated and replaced by undifferentiated cuboidal cells We found distinct ultrastructural changes in the jejunal epithelium confirming PED virus infection is involved in malabsorptive diarrhea.
Background/Aims: Capsule endoscopy is a diagnostic method for evaluating the small bowel lumen and can detect undiagnosed lesions. The aim of this study was to evaluate the diagnostic yield and clinical impact of capsule endoscopy in patients with refractory diarrhea-predominant irritable bowel syndrome and functional abdominal pain. Methods: This study involved a retrospective analysis of prospectively collected data, maintained in a database. Patients with refractory diarrhea-predominant irritable bowel syndrome and functional abdominal pain within the period of March 2012 to March 2014 were included. Capsule endoscopy was used to detect small bowel pathologies in both groups. Results: Sixty-five patients (53.8% female) fulfilled the inclusion criteria and had a mean (${\pm}$standard deviation) age of $50.9{\pm}15.9$ years. Clinically significant lesions were detected via capsule endoscopy in 32.5% of the patients in the abdominal pain group and 54.5% of the patients in the diarrhea group. Overall, 48% of patients had small bowel pathologies detected during the capsule endoscopy study. Inflammatory lesions and villous atrophy were the most frequent lesions identified in 16.9% and 15.3% of patients in the abdominal pain and the diarrhea groups, respectively. Conclusions: Routine use of capsule endoscopy in patients with irritable bowel syndrome should not be recommended. However, in patients with refractory conditions, capsule endoscopy may identify abnormalities.

Objective: The study was conducted to investigate the impact of boron supplementation on nutrient digestibility, inflammatory responses, blood metabolites and diarrhea index, and their relevance to growth performance in weaned pigs housed in good and poor sanitary environments for 14 days after weaning. Methods: A total of 108 male pigs (Duroc×[Yorkshire×Landrace]) weaned at 21 days of age were used in a randomized complete block design with 2×3 factorial arrangement. Pigs were assigned to three boron treatments (0, 5, and 10 mg/kg) under two environments (good and poor sanitary) to give six replicates per treatment (3 pigs per replicate). On 0, 7, and 14 days, one pig per replicate was euthanized to collect, ileum tissue samples, and rectal fecal samples. Results: Boron supplementation quadratically influenced (p<0.001) feed intake and weight gain in pigs housed in good sanitary conditions from 1 to 14 days post-weaning where pigs offered 5 mg/kg boron optimized weight gain and feed intake. There is a quadratic interaction (p = 0.019) on feed intake for 1 to 14 days post-weaning where 5 mg/kg boron increased feed intake in good sanitary conditions. Pigs housed in the poor sanitary environment decreased (p<0.001) villus height and crypt depth in ileum at days 7 and 14. On day 7 and 14, crude protein digestibility was quadratically influenced (p<0.05) by boron supplementation. Boron supplementation linearly increased (p<0.05) plasma calcium and cholesterol levels whilst linearly (p = 0.005) reducing plasma triglyceride concentrations. Diarrhea index was quadratically influenced (p<0.05) by boron supplementations regardless of sanitary conditions where 5 mg/kg boron inclusion achieved the lowest diarrhea index. Conclusion: Pigs offered 5 mg/kg of boron increased weight gain which may be deduced by improved dry matter, crude protein, and energy digestibility regardless of the sanitary conditions.
Geranium thunbergii is a perennial plant commonly used as an oriental medicine for prevention of diarrhea, constipation, and gastrointestinal disorders. However, its anti-inflammatory effect has not been evaluated thus far. Therefore, the present study aimed to investigate the anti-inflammatory effect of G. thunbergii. In this study, G. thunbergii extracted with methanol; this methanol extract was further partitioned using various solvents, and G. thunbergii ethyl acetate fraction (GTEF) was obtained. To determine the anti-inflammatory activity of G. thunbergii, the effects of GTEF on nitric oxide (NO) production in lipopolysaccharide-stimulated RAW 264.7 cells were evaluated. GTEF suppressed NO production in a dose-dependent manner without any toxic effects. In addition, western blotting was performed to examine the effect of GTEF on expression of inducible nitric oxide synthase and cyclooxygenase-2. These results suggest that GTEF as a phytoextract may be useful for the prevention or treatment of inflammation.
Ulcerative colitis (UC) is a typical inflammatory colon disorder. Platycodon grandiflorum (PG) is known to exert various beneficial effects including anti-oxidative and anti-bacterial properties and improvements in liver function. However, the improving effect and mechanism of PG on intestinal inflammation are not fully understood. The present research was designed to investigate the effect of PG on the clinical signs of DSS-induced colitis in mice. The ameliorative effects of PG on inflammatory cytokine expression and the activation of hypoxia-inducible-factor (HIF)-1α in DSS-treated colon tissue were also determined. Our results showed that mice treated with DSS displayed the main clinical symptoms of colitis, including weight loss, bloody stools, decrease in colon length and diarrhea and PG treatment significantly improved the clinical features induced by DSS in mice. PG inhibited the increase in the levels of inflammatory cytokines caused by DSS in colon tissues. We also showed that the anti-inflammatory mechanism of PG involved suppressing the activation of HIF-1α in DSS-treated colon tissues. Collectively, the findings of this study indicate the prospect of developing new drugs from PG for UC treatment.
A 1-year-old, female Bengal tiger (Panthera tigris tigris) presented signs of weight loss and dark browncolored diarrhea. On fecal examination, numerous intact and broken red blood cells were found, but both parasites and inflammatory signs were absent. No significant findings were seen in serum biochemistry profiles, including electrolytes, with negative feline pancreatic lipase immunoreactivity (fPLI). Diagnostic kits using feces or peripheral blood were negative for feline parvovirus, feline coronavirus, feline immunodeficiency virus, and feline leukemia virus. Based on the result of feline trypsin-like immunoreactivity (fTLI) concentration ($4.6{\mu}g/L$), the tiger was provisionally diagnosed to have exocrine pancreatic insufficiency (EPI). After this diagnosis, pancreatic enzymes were prescribed. The feces of the tiger returned to normal form and her weight was increasing. EPI is uncommon and not described extensively in Felidae, including domestic cats. Feline EPI is associated with a variety of non-specific signs and it should be considered in the differential diagnosis of cases presenting with weight loss, diarrhea, and other gastrointestinal signs. In this case, the patient was strongly suspected to have EPI based on the very low fTLI concentration, though the concentration of fTLI in tigers has not yet been determined. This is the first report to present a suspected EPI case in Bengal tigers.
The present study investigated the prevalence rate of Cryptosporidium parvum as a cause of diarrhea. We examined 942 stools of unidentified reasons occurring in patients in whom no immunosuppression had been detected. We examined the stools for Cryptosporidium parvum via modified acid-fast staining. The clinical records of all of the positive patients were then analyzed. Nine ($1\%$) of the stools among the 942 diarrheal patients were positive for C. parvum. The positive rate in the males was $1.1\%$ (6/522) and the positive rate of the females was $0.7\%$ (3/420). Age distribution revealed that the highest positive rates were in patients in their sixties, with a positive rate of $2.5\%$ (4/158). In the clinical tests, levels of c-reactive protein, erythrocyte sedimentation rates, and neutrophil proportions were normally increased in the peripheral blood, whereas the lymphocyte proportion exhibited a tendency towards decrease. The pathological findings were compatible with an inflammatory reaction in the host.

Thus, the research of probiotics have been focused on the prevention and treatment of disorders associated with the gastrointestinal tract (GIT), including pathogen infection, traveler's diarrhea, antibiotic-associated diarrhea, and constipation. Probiotics have also been suggested as therapeutic agents against irritable bowel syndrome and inflammatory bowel diseases. An increasing amount of evidence from clinical studies suggests that they are effective in the prevention of atopic allergies and may have potential anti-carcinogenic effects. Until recent years many scientific research for this use has been based on empirical observations. Therefore, probiotics in the form of fermented milk products have been long part of attempts to maintain good health in world wide.
Most inflammatory disorders are usually treated using anti-inflammatory drugs including non-steroidal anti-inflammatory drugs (NSAID) and steroidal anti-inflammatory drugs (SAID). Prolonged uses of NSAIDs and SAIDs may frequently cause adverse side-effects such as nausea, vomiting, diarrhea, constipation, decreased appetite, kidney and liver failure, ulcers, and prolonged bleeding after an injury or surgery. Thus, it is necessarily required to develop a new anti-inflammatory drug with little side-effects. Dandelion (Taraxacum officinale) possesses the therapeutic abilities to eliminate body heat and toxins and to remove swelling and inflammation. In order to verify the anti-inflammatory activity of dandelion, TPA(12-O-tetra decanoylphorbol-acetate)-induced or croton oil-induced acute edema was developed in the mouse ears, and dandelion extract dissolved in acetone was applied to both sides of inflamed ears. It was found that dandelion could significantly reduce the ear swelling, compared to that of non-treated control. In the case of $20{\mu}{\ell}$ application of $100mg/m{\ell}$ dandelion solution (DA-100), its anti-inflammatory effect was comparable to that of indomethacin, a non - steroidal anti-anflammatory drug. Taken together, it could be concluded that topically applied dandelion extract exhibited its potentials as a new drug candidate with an effective anti-inflammatory activity.
Galla Rhois is a nest of parasitic bug, has been traditionally used for the treatment of the therapy of diarrhea, peptic ulcer, hemauria, etc., that showed various anti-inflammatory activity, and other biological properties. We studied the effect of Galla Rhois ethanol extract. we investigated whether compounds isolated from the ethanol extract of Galla Rhois, could modulate iNOS and COX-2 expression in RAW 264.7 macrophages stimulated with lipopolysaccharide (LPS). We found compounds that suppressed LPS-induced iNOS and COX-2 expression. Suppression of the expression of iNOS and COX-2 was in parallel with the comparable inhibition of the production of nitric oxide (NO) and prostaglandin E2 (PGE2). Our results suggest that compounds can inhibit NO and PGE2 productions through suppression of LPS-induced iNOS and COX-2 expression. Because COX-2- or iNOS-dependent mechanisms are involved in inflammation and tumor progression, our findings provide a new uncovering mechanism responsible for anti-inflammatory and antitumor effects of Galla Rhois.
