Gastric Metastasis from Gastric-Type Mucinous Adenocarcinoma of Uterine Cervix: A Case Report (자궁경부 위형 점액샘암종의 위 전이: 증례 보고)

  • Gastric metastasis (GM) from cervical cancer is extremely rare, and only a few cases have been reported in the English literature. Gastric-type mucinous adenocarcinomas (GAS) of the uterine cervix are rare. GAS is an aggressive cancer commonly found in advanced stages; however, GM has not been reported. This study presents a rare case of GM from GAS of the uterine cervix in a 61-year-old female and describes the radiological findings of both the GM and cervical mucinous adenocarcinoma. GM appeared as a poor enhancing submucosal mass. The cervical mucinous adenocarcinoma appeared as an infiltrating mass with poor contrast enhancement. It exhibited mildly high and low signal intensities on the diffusion-weighted image and apparent diffusion coefficient map, respectively. This case is extremely rare and challenging to diagnose; however, if cervical cancer is an human papillomavirus-independent GAS type and a submucosal lesion is found in the stomach, the possibility of metastasis with a pattern similar to our case could be considered.

The 1998, 1999 Patterns of Care Study for Breast Irradiation After Breast-Conserving Surgery in Korea (1998, 1999년도 우리나라에서 시행된 유방보존수술 후 방사선치료 현황 조사)

  • Purpose: To determine the patterns on evaluation and treatment in the patient with early breast cancer treated with conservative surgery and radiotherapy and to improve the radiotherapy techiniques, nationwide survey was peformed. Materials and Methods: A web-based database system for korean Patterns of Care Study (PCS) for 6 common cancers was developed. Two hundreds sixty-one randomly selected records of eligible patients treated between 1998$\~$1999 from 15 hospitals were reviewed. Results: The patients ages ranged from 24 to 85 years(median 45 years). Infiltrating ductal carcinoma was most common histologic type (88.9$\%$) followed by medullary carcinoma (4.2$\%$) and infiltrating lobular carcinoma (1.5$\%$). Pathologic T stage by AJCC was T1 in 59.7$\%$ of the casses, T2 in 29.5$\%$ of the cases, Tis in 8.8$\%$ of the cases. Axillary lymph node dissection was peformed I\in 91.2$\%$ of the cases and 69.7$\%$ were node negative. AJCC stage was 0 in 8.8$\%$ of the cases, stage I in 44.9$\%$ of the cases, stage IIa in 33.3$\%$ of the cases, and stage IIb in 8.4$\%$ of the cases. Estrogen and progesteron receptors were evaluated in 71.6$\%$, and 70.9$\%$ of the patients, respectively. Surgical methods of breast-conserving surgery was excision/lumpectomy in 37.2$\%$, wide excision in 11.5$\%$, quadrantectomy in 23$\%$ and partial mastectomy in 27.5$\%$ of the cases. A pathologically confirmed negative margin was obtained in 90.8$\%$ of the cases. Pathological margin was involved with tumor in 10 patients and margin was close (less than 2 mm) in 10 patients. All the patients except one recieved more than 90$\%$ of the planned radiotherapy dose. Radiotherapy volume was breast only In 88$\%$ of the cases, breast+supraclavicular fossa (SCL) in 5$\%$ of the cases, and breast+ SCL+ posterior axillary boost in 4.2%$\%$of the cases. Only one patient received isolated internal mammary lymph node irradiation. Used radiation beam was Co-60 in 8 cases, 4 MV X-ray in 115 cases, 6 MV X-ray in 125 cases, and 10 MV X-ray in 11 cases. The radiation dose to the whole breast was 45$\~$59.4 Gy (median 50.4) and boost dose was 8$\~$20 Gy (median 10 Gy). The total radiation dose delivered was 50.4$\~$70.4 Gy (median 60.4 Gy). Conclusion: There was no major deviation from current standard in the patterns of evaluation and treatment for the patients with early breast cancer treated with breast conservation method. Some varieties were identified in boost irradiation dose. Separate analysis for the datails of radiotherapy planning will be followed and the outcome of treatment is needed to evaluate the process.

Geology, Mineralization, and Age of the Pocheon Fe(-Cu) Skarn Deposit, Korea (한국 포천 철(-동) 스카른 광상의 지질, 광화작용 및 생성연대)

  • The Pocheon iron (-copper) deposit, located at the northwestern part of the Precambrian Gyeonggi massif in South Korea, genetically remains controversial. Previous researchers advocated a metamorphosed (-exhalative) sedimentary origin for iron enrichment. In this study, we present strong evidences for skarnification and Fe mineralization, spatially associated with the Myeongseongsan granite. The Pocheon deposit is composed of diverse carbonate rocks such as dolostone and limestone which are partially overprinted by various hydrothermal skarns such as sodic-calcic, calcic and magnesian skarn. Iron (-copper) mineralization occurs mainly in the sodic-calcic skarn zone, locally superimposed by copper mineralization during retrograde stage of skarn. Age data determined on phlogopites from retrograde skarn stage by Ar-Ar and K-Ar methods range from $110.3{\pm}1.0Ma$ to $108.3{\pm}2.8Ma$, showing that skarn iron mineralization in the Pocheon is closely related to the shallow-depth Myeongseongsan granite (ca. 112 Ma). Carbon-oxygen isotopic depletions of carbonates in marbles, diverse skarns, and veins can be explained by decarbonation and interaction with an infiltrating hydrothermal fluids in open system ($XCO_2=0.1$). The results of sulfur isotope analyses indicate that both of sulfide (chalcopyrite-pyrite composite) and anhydrites in skarn have very high sulfur isotope values, suggesting the $^{34}S$ enrichment of the Pocheon sulfide and sulfate sulfur was derived from sulfate in the carbonate protolith. Shear zones with fractures in the Pocheon area channeled the saline, high $fO_2$ hydrothermal fluids, resulting in locally developed intense skarn alteration at temperature range of about $500^{\circ}$ to $400^{\circ}C$.

The Effect of Surface Sealing on the Microleakage of Class V Composite Resin Restorations (제V급 복합레진 수복물의 표면전색이 미세변연누출에 미치는 효과)

  • This in vitro study was performed to assess the effect of surface sealing on the microleakage of class V composite resin restorations that underwent several aging treatments. Class V cavities were prepared on the buccal surface of 100 sound extracted premolars and restored with a hybrid light-cured composite resin according to the manufacturer's instructions. They were randomly divided into two groups consisting of 50 samples: group I, without surface sealing, and group II, in which margins were etched and surface sealant was applied. After thermocycling, each group was divided into five subgroups, respectively, to represent the five aging treatments: group A = no further treatment (only thermocycling), B = toothbrushing, C = load cycling, D = toothbrushing followed by load cycling, and E = aging treatment in deionized water for six months. Microleakage was assessed by examining the penetration of 2% methylene blue dye. The following results were obtained: 1. At occlusal and cervical margins in groups without surface sealing, there was no significant difference in microleakage after the several aging treatments (p>0.05). 2. The occlusal margins of groups with surface sealing showed no significant differences after the several aging treatments (p>0.05). 3. In the cervical margins of groups with surface sealing, microleakage significantly increased after load cycling or aging in deionized water for six months (p<0.05). 4. The no-further-treatment group and the toothbrushing group with surface sealing showed less microleakage than the corresponding groups without surface sealing (p<0.05). 5. The surface-sealed groups with load cycling or aging in deionized water showed no significant difference in microleakage to the corresponding groups without surface sealing (p>0.05). In conclusion, the results of this study suggest that the surface sealant infiltrating through the gap of the cervical margin exerted a positive effect on microleakage at the initial stage, but the effect was not sufficient to overcome the stress generated by the cuspal flexure during occlusal loading and water absorption.

IMMUNOHISTOCHEMICAL ASSAYS FOR THE EXPRESSION OF EPIDERMAL GROWTH FACTOR-SIGNALING PROTEINS IN ADENOID CYSTIC CARCINOMAS OF HUMAN SALIVARY GLANDS (타액선 선양낭성암종에서 상피성장인자 신호전달 단백의 발현에 관한 면역조직화학적 연구)

  • Malignant tumors of the human salivary glands may arise from major or minor salivary glands. Adenoid cystic carcinoma (ACC) is the second most common malignant neoplasm in the salivary glands. ACC is occasionally highly aggressive tumor that readily invades adjacent tissues and metastasize to distant organs at early stages of the disease. Although ACC tends to grow slowly, treatment outcome may be poor due to wide local infiltration, perineural or intraneural spread and a propensity for hematogenous metastasis. Therefore, knowledge of cellular and molecular characteristics that influence the growth, survival and metastasis of tumor cells, is important for new treatment strategies of salivary ACC. I determined expressions of epiderma growth factor (EGF)-signaling molecules using surgical specimens of human ACCs. Protein expressions of EGF, transforming growth $factor(TGF)-{\alpha}$, EGF receptor (EGFR), phosphorylated EGFR (pEGFR), and human EGF receptor (HER)-2 were assessed in 18 cases of salivary ACC by immunohistochemical staining. Adjacent normal salivary tissues and mucosal tissues, uninvolved by the malignant tumor, served as internal controls. Most of the tumors, especially ACC with a tubulocribriform pattern, were positive for EGF signaling molecules. The overall percentages of the 18 specimens expressing EGF, $TGF-{\alpha}$, EGFR, pEGFR, and HER2 were 50, 89, 61, 61 and 83% respectively. Moreover, tumor-associated endothelial cells and infiltrating immune-related cells in the stroma of ACC, also expressed these biomarkers. Taken together, EGF-signaling molecules are actively expressed in salivary ACC. Therefore, we suggest that these biomarkers can be molecular targets for new treatment strategies of salivary tumors.

Shear bond strength of veneer ceramic and colored zirconia by using aqueous metal chloride solutions (염화수화물용액 침지법으로 제작한 유색 지르코니아와 전장도재의 전단결합강도)

  • Purpose: The purposes of this study was to evaluates shear bond strength between zirconia core and veneer-ceramic in order to examine the clinical practice of colored zirconia block fabricated by infiltration method into the metal chloride solution. Material and methods: CNU block and $Everest{(R)}$ ZS blank were used. VITA In-$Ceram{(R)}$2000 YZ Coloring liquid (LL1) and 3 aqueous metal chloride solutions containing chromium and molybdenum ingredients were used. 40 zirconia specimens were prepared into cuboid shape ($5{\times}5{\times}10 mm$). All specimens were divided into 5 groups by infiltrating into the coloring liquids. After that, porcelain was build up into the shape of $5{\times}5{\times}4mm^3$, followed by sintering. The maximum loading and shear bond strength was measured. Failure patterns and failure sites were examined. Results: 1. There were no statistical differences in shear bond strength between zirconia blocks (P > .05). 2. There were no statistically significant differences in shear bond strength between non-colored and colored zirconia blocks, while shear bond strength of non-colored zirconia blocks is higher than that of colored specimen (P > .05). 3. In the comparison with shear bond strength among colored zirconia blocks, there were no statistical differences according to kinds of coloring liquid (P > .05). 4. Mixed failure patterns were mainly observed in the failure between zirconia and veneering ceramic. The veneering ceramic failure of all specimens was observed in either interface of zirconia or veneering ceramic. Conclusion: Shear bond strength between colored zirconia and veneering ceramic shows lower tendency than non-colored zirconia, but there was clinically allowable value.

IL-12 Regulates B7-H1 Expression in Ovarian Cancer-associated Macrophages by Effects on NF-κB Signalling

  • Background and Aim: B7-H1, a co-inhibitory molecule of the B7 family, is found aberrantly expressed in ovarian cancer cells and infiltrating macrophage/dendritic-like cells, and plays a critical role in immune evasion by ovarian cancer. IL-12, an inducer of Th1 cell development, exerts immunomodulatory effects on ovarian cancer. However, whether IL-12 regulates B7-H1 expression in human ovarian cancer associated-macrophages has not been clarified. Therefore, we investigated the effects of IL-12 on the expression of B7-H1 in ovarian cancer-associated macrophages and possible mechanisms. Methods: PMA induced THP-1-derived macrophages or human monocyte-derived macrophages were treated with recombinant IL-12 (rIL-12) or infected with adenovirus carrying human IL-12 gene (Ad-IL-12-GFP) for 24 h, then cocultured with the SKOV3 ovarian cancer cell line for another 24 h. Macrophages were collected for real-time PCR and Western blot to detect the expression of B7-H1, and activation of the NF-${\kappa}B$ signaling pathway. Moreover, supernatants were collected to assay for IL-12, IFN-${\gamma}$ and IL-10 by ELISA. In addition, monocyte-derived macrophages treated with IFN-${\gamma}$ were cocultured with SKOV3 and determined for the expression of B7-H1. Furthermore, the expression of B7-H1 in monocyte-derived macrophages was also evaluated after blocking NF-${\kappa}B$ signaling. Results: The expression of B7-H1 was significantly upregulated in monocyte-derived macrophages treated with rIL-12 or Ad-IL-12-GFP compared with the control groups (p<0.05), accompanied by a remarkable upregulation of IFN-${\gamma}$ (p<0.05), a marked downregulation of IL-10 (p<0.05) and activation of NF-${\kappa}B$ signaling. However, the upregulation of B7-H1 was inhibited by blocking the NF-${\kappa}B$ signaling pathway (p<0.05). Expression of B7-H1 was also increased (p<0.05) in monocyte-derived macrophages treated with IFN-${\gamma}$ and cocultured with SKOV3. By contrast, the expression of B7-H1 in THP-1-derived macrophages was significantly decreased when treated in the same way as monocyte-derived macrophages (p<0.05), and IL-10 was also significantly decreased but IFN-${\gamma}$ was almost absent. Conclusions: IL-12 upregulates the expression of B7-H1 in monocyte-derived macrophages, which is possible though inducing the secretion of IFN-${\gamma}$ and further activating the NF-${\kappa}B$ signal pathway. However, IL-12 downregulates the expression of B7-H1 in THP-1-derived macrophages, associated with a lack of IFN-${\gamma}$ and inhibition of expression of IL-10.

Ischemic Time Associated with Activation of Rejection-Related Immune Responses (허혈 시간과 거부반응 관련 면역반응)

  • Ischemia/reperfusion injury(I/RI) is the major cause of acute renal failure and delayed graft function(DGF) unavoidable in renal transplantation. Enormous studies on ischemia damage playing a role in activating graft rejection factors, such as T cells or macrophages, are being reported. Present study was performed to determine whether ischemia time would play an important role in activating rejection-related factors or not in rat models of I/RI. Male Sprague-Dawley rats were submitted to 30, 45, and 60 minutes of warm renal ischemia with nephrectomy or control animals underwent sham operation(unilateral nephrectomy). Renal function and survival rates were evaluated on day 0, 1, 2, 3, 5 and 7. Immunofluorescence staining of dendritic cells(DCs), natural killer(NK) cells, macrophages, B cells, CD4+ and CD8+ T cells were measured on day 1 and 7 after renal I/RI. Survival rates dropped below 50% after day 3 in 45 minutes ischemia. Histologic analysis of ischemic kidneys revealed a significant loss of tubular architecture and infiltration of inflammatory cells. DCs, NK cells, macrophages, CD4+ and CD8+ T cells were infiltrated from a day after I/RI depending on ischemia time. Antigen presenting cells(DCs, NK cells or macrophages) and even T cells were infiltrated 24 hours post-I/RI, which is at the time of acute tubular necrosis. During the regeneration phase, not only these cells increased but B cells also appeared in more than 45 minutes ischemia. The numbers of the innate and the adaptive immune cells increased depending on ischemia as well as reperfusion time. These changes of infiltrating cells resulting from each I/RI model show that ischemic time plays a role in activating rejection related immune factors and have consequences on progression of renal disease in transplanted and native kidneys.

The Study on Quantifying and Evaluating for the Functions of Flood Control and Fostering Water Resources in Agriculture (농업의 홍수조절기능과 수자원함양기능 계량화 및 가치평가에 관한 연구)

  • In order to share the understanding agricultural multifunctionality with people, we carried out quantification and monetary evaluation for controlling flood and fostering water resources function in paddy and upland farming in Korea. The amount of water controlling flood and fostering water resources function in paddy farming was much greater than that in upland. The quantification of flood control function could be estimated by sum of the height of dike and water infiltrated during the flooding periods in paddy farming, and water excepting runoff water from precipitation at flooding time in upland farming. As results of estimation of flood control function, the amounts of water controlling flood have been evaluated as $294mm\;year^{-1}$ in paddy farming and $72.6mm\;year^{-1}$ upland farming, and was calculated 3.71 billion MT on a nation basis in 2006. When it was carried out monetary estimation as the cost of dam construction and the depreciation expense by using replacement cost method, flood control functions in paddy and upland were evaluated as 44,338.9 and 7,221.5 billion won, respectively. Comparing with previous reports, monetary value was analyzed much to increase because of rising price cost recently. Fostering water resource functions were also quantified in paddy and upland farming as the amount of water keeping and infiltrating water during the cultivation. In the basis of estimation model, it was showed that paddy and upland farming had been estimated to have $414.28mm\;year^{-1}$, $18.7mm\;year^{-1}$, respectively. They were also calculated to 4.49 and 0.137 billion MT on a nation basis in 2006, respectively. The economic values of fostering water resources function in paddy and upland farming were also estimated to 1,769.4 and 52.8 billion won, respectively, as replacing the amount of water to the cost of drinking water in 2006. There were differences by much to the amounts of controlling flood function and fostering water resource between paddy and upland farming. It means that paddy farming more play an important role in environment than upland farming in Korea.

Ultrastructural Effects of Irradiation on Squamous Cell Carcinoma of the Uterine Cervix (자궁경부의 편평상피암의 방사선치료에 수반되는 초미형태학적 변화)

  • Nineteen patients with previously untreated invasive squamous cell carcinoma of the uterine cervix were treated by irradiation alone at the Keimyung University Hospital from January, 1990 to July, 1991. The serial samplings of the tissue taken before and during radiation of the uterine cervix were studied by light and electron microscopic examination. Radiation-induced cellular changes, particularly nuclear degeneration was pronounced. The tumor invasion pattern remained unchanged but the number of mitosis and tumor cells decreased, The number of infiltrating inflammatory cells, multinucleated giant cells and karyolytic cells were increased with radiation. Fibrosis was also increased. Electron microscopically, the amount of tonofilament in the tissue samplings was increased in the postirradiated state, but the desmosomes were decreased in numbers. Fibroblasts began to appear after an irradiation dose of 2700 cGy. After an irradiation dose of 3000 cGy or more, tumor cells were nearly completely degenerated and displaced with mature fibrotic tissue. There was an increase of activated fibroblasts and collagen fibers but a decrease of inflammatory cells in the interstitial tissue. Swelling of the mitochondria and endoplasmic reticulum, loss of intercellular bridges and an increased number of secondary lysosomes were also found with radiation.
