OROFACIAL ODONTOGENIC INFECTIONS ASSOCIATED WITH MEDICAL DISEASES (내과적 질환을 수반한 치성감염)

  • Pyogenic orofacial infections are most commonly odontogenic in origin. Although such infections are usually self-limiting and spatially confined, purulent material may occasionally borrow deeply into contiguous fascial space or planes far from the initial site of involvement. The incidence of orofacial infection remains low in this modern era of preventive dental care and antibiotic therapy, but severe orofacial infections are most frequently observed in the medically compromised patients. We experienced 5 cases of severe orofacial odontogenic infection associated with medical diseases, and then concluded as follows : 1. The average hospitalized period was about 5 weeks, and the signs that indicated that the infections were controlled usually appeared in third week after incision and drainage. 2. The involved medical diseases were diabetes mellitus iatrogenic Cushing's syndrome, rheumatoid arthritis, malnutrition, etc. 3. The medical diseases should be treated coincidently with control of infection.

Genomic Relationship of Salmonella enterica Serovar Typhimurium DT104 Isolates from Korea and the United States

  • Salmonella enterica serovar Typhimurium DT104 (Salmonella Typhimurium DT104 or DT104) has been emerging as a common pathogen for human in Korea since 1997. In order to compare the genomic relationship and to search for the dominant strains in Korea, we conducted pulsed-field gel electrophoresis (PFGE) and IS200 fingerprinting of 25 epidemiological unrelated isolates from human and animals from Korea and cattle from America. Two Salmonella Typhimurium DT104 isolates from human in Korea and all 8 isolates from American cattle had indistinguishable patterns from the PFGE and IS200 fingerprinting but multidrug-resistant Salmonella Typhimurium isolates, including DT104, from Korean animals had diverse genetic patterns. The data suggest that a dominant DT104 strain might have circulated between Korean and American cattle and that it had a high level of clonality.

  • Respiratory tract symptoms such as cough, wheeze, and strider may occur frequently or persist for long periods in a substantial number of children, others may have persistent or recurring lung infiltrates with or without symptoms. It is important to study on clinical significance of this symptoms for avoiding unnecessary test or treatment. Chronic or Recurrent Respiratory Symptoms are similar to cold(感冒) and cough(咳嗽) in Oriental Medicine. Diagnostic criteria is followed : in case of children less than three years old, upper respiratory infections are more than seven per year or lower respiratory infections are more than three ; in case of children three to five years old, upper respiratory infections are more than six per year or lower respiratory infections are more than two ; in case of children six to twelve years old, upper respiratory infections are more than five per year or lower respiratory infections are more than two. In oriental medical treatment is classified into the treatment of internal use, external use, internal and external use, acupuncture, cupping therapy, and combination treatment of chinese and western medicine.

Chronic Infections of the Urinary Tract and Bladder Cancer Risk: a Systematic Review

  • Literature on the relationship between recurrent urinary tract infections and urinary bladder carcinoma risk has been inconsistent. Therefore, we carried out this systematic review of observational studies to ascertain if there is any association between chronic urinary tract infection and urinary bladder carcinoma. A total of 10 databases were searched using Boolean: CINAHL, PUBMED, Google Scholar, Medline, Science Direct, SCIRUS, Cochrane, UK PubMed central, NHS evidence and WHO-website. The search yielded an initial hit of 3,518 articles and after screening and critical appraisal, seven studies were included for this review. Four articles reported an association between chronic urinary tract infections and bladder cancer while three concluded a weak or no association at least in one gender. Main findings in this review were that most of the studies reported an association between chronic urinary tract infections and bladder cancer risk. However, inferences about the causal association between chronic urinary tract infections and bladder cancer risk should be drawn cautiously considering the methodological limitations of case-control studies included in this review. Therefore, more empirical evidence is needed to determine the causal nature of relationships between chronic urinary tract infections and bladder cancer risk.

Factors Related to Surgical Site Infections in Patients Undergoing General Surgery (일반외과 환자의 수술부위 감염 관련 요인 분석)

  • Purpose: To identify risk factors for surgical site infections in patients undergoing general surgery, to analyze the prolonged hospital stay and extra cost for antibiotics, and to provide basic data for control of surgical site infections. Method: Surgical site infection was defined using the definition of the CDC and the data were analyzed by $x^2$-test and unpaired t-test. Results: The prevalence of surgical site infections was 9.7%, and it was related to wound class, duration of operation, number of operations, whether the operation was an emergency, trauma, drains, preoperative stays, presence of remote infection during operative period, and previous history of recent surgery. The mean duration for post-operative stay when a surgical site infection occurred was 9.5 days and in 56.9 % of the patients the surgical site infection appeared 7 days after the operation. Post-operative stays for infected patients were 20.3 days longer than that of uninfected patients. The mean cost of antibiotics for infected patients was higher than that for uninfected patients by 561,067 won per person. Conclusion: Surgical site infection results in an increased length of stay and extra-cost, thus, hospitals need to create strategies to reduce nosocomial infections through effective infection surveillance and by considering factors related to surgical site infections.

Increasing Antimicrobial Resistance Monitored in Surveillance Analysis of Blood Stream Infections in Febrile Neutropenic Pediatric Oncology Patients

  • Background: Continuous surveillance of pattern of blood stream infection is necessary in febrile neutropenia (FN)especially with the recent escalating trend in the management of pediatric cancer patients towards intensified regimens and with the increase in infections caused by resistant organisms limiting the choice of antibiotics. Aim: To monitor change in pattern of blood stream infections (BSI) in FN pediatric cancer patients. Materials and Methods: Surveillance of FN episodes with positive BSI was prospectively monitored and compared to a previous surveillance in the same pediatric oncology unit. Results: A total of 232 BSI positive episodes were documented in 192 patients during a 6 months period. The results of recent surveillance analysis showed an increase in intensified regimens of chemotherapy, antimicrobial resistance, fungal infections, and prolonged duration of episodes when compared to previous surveillance, with p value sof <0.001, 0.005, 0.021, and <0.001, respectively. There was an apparent decrease in the crude mortality but this was not statistically significant, to 6% in 2011 from 10 % in 2006. Conclusions: The pattern of BSI at our institution is still inclining towards gram positive organisms but is showing a shift towards more antibiotic resistance and fungal infections.

Intestinal parasite Infections in Stray Cats (국내서식 야외 고양이의 기생충 감염 조사)

  • This survey of endoparasite infections among stray cats in Korea has been carried out during November 1995 to October 1997. A total of 215 stray cats were collected from SeoulㆍGyeonggi (54), Gangwon (38), Chungchong (34), Jeonla (38), and Gyeoungsang (51) areas. The positive rate of endoparasites was 184 (85.6%) cats among 215. Of isolated parasites, nematodes were the most often detected parasites especially Toxocara cati (102 cats), Ancylostoma tubaeformae (9 cats). The next most often detected parasites were cestodes (60/215; 27.9%) sepecially Spirometra erinaecei (34 cats), Taenia taeniaformis (26 cats). Opisthorchis tenuicollis was found in bile duct (9 cats) and Isospora spp. were identified in 41 cats (19.1 %). In mixed parasite infection, the rate of infections of nematodes, trematodes and protozoas was 3.3%, nematodes and cestodes with 8.8%, nematodes and protozoa with 4.2%, cestodes and protozoa with 1.9%. In parasitic infection by a considering age category, age from 2 to 4 years had the highest infection (92.3%), 1-2 years were 87.6%, over 4 years was 80.0%. 79.6 percents of the stray cats in SeoulㆍKyounggi areas were ranked the 1st in parasite infection. The next was Kyoungsang 76.5%, Kangwon 65.8%, Cholla 63.2% and Chungchong 61.8% in order. Considering age categories from birth to greater than 4 years, parasite infections were most prevalent in cats 2 to 4 years old; other ages seemed to have no effect on prevalence of parasitism. Also sex seemed to have no effect on prevalence of parasitism. In endoparasite infection, nematode and cestode infections were most frequently found, trematode infections were low, coccidiosis was showed very often.

Recognition and Performance on Management for Nosocomial Infections among Nursing Students (병원감염관리에 대한 간호학생의 인지도와 수행도)

  • Purpose: This study was designed to identify recognition and the performance level for nosocomial infections amongst student nurses and to provide data for preparing on efficient policy and control program for nosocomial infections. Method: 191 senior nursing student participated in this research from 5 nursing college which allowed data collection, in the city of Seoul. The questionnaire was composed of 73 items(likert scale) about eight areas; aseptic technique, disinfection, precaution, hand washing, urinary tract infection, respiratory infection, catheter related infection, and self care about nosocomial infections. Results: The mean score of recognition and performance level for management of nosocomial infection were 4.29 and 3.41 respectively. The mean score of the recognition level was significantly lower than the performance level in the eight areas. The mean score of both recognition and performance were highest in the area of disinfection. However, the mean score of recognition was lowest in the area of aseptic technique and the mean score of performance was lowest in the area of catheter related infections. The correlation of recognition and performance level was statistically significant. Conclusions: These research findings should be useful in promoting an intensive and continuous educational program on nosocomial infection for nursing students and to establish an efficient policy for preventing nosocomial infections.

Knowledge and Behavior of Visitors in the Prevention of Respiratory Tract Infections in an Emergency Service, Hospital (응급의료센터 내원객의 호흡기 감염예방에 대한 지식과 수행도)

  • Purpose: The purpose of this study was to investigate the effects of emergency setting visitors' knowledge and behavior patterns in relation to prevention of respiratory tract infections. Methods: A descriptive survey was used. The participants were visitors to the emergency service in 'D' general hospital in 'D' city, and the data were collected from July 1 to September 1, 2016. The collected data were analyzed using frequency analysis, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficients, and multiple regression analysis. Results: Factors influencing prevention of respiratory tract infections were visitors' education level, methods of dissemination of prevention information, and participants' knowledge with regard to preventive methods. The explanatory power was found to be 35% in the regression model. Conclusion: The findings indicate that visitors' education level, knowledge of infection prevention, and the dissemination of information regarding infection prevention by the hospital play an important role in the prevention of respiratory tract infections in emergency services in the hospital. These results highlight the need for a customized education program for prevention of respiratory tract infections in emergency settings. Programs should take into consideration the educational background of visitors, and provide them with appropriate information regarding infection prevention.
